Clinical Study of Qingpeng Ointment Treatment of Shoulder-Hand Syndrome After Cerebral Hemorrhage During the Rehabilitation Period

Background: Shoulder-hand syndrome (SHS) refers to a syndrome causing sudden edema, shoulder pain and limited hand function. Qingpeng ointment, a kind of Tibetan medicine, can reduce swelling, relieve pain, tonify stagnation and clear the meridians, which is consistent with the pathological mechanism of SHS after stroke. Therefore, if clinical trials can be used to explore the effectiveness of Qingpeng ointment for treatment of poststroke SHS and promote its application in clinical medicine, this is of specific significance for the treatment of poststroke SHS. Objective: To investigate the clinical efficacy and safety of Qingpeng ointment in the treatment of poststroke SHS. To provide an objective basis for a better therapeutic treatment for poststroke SHS. Method: A prospective, randomized, controlled study was conducted. This study recruited 120 patients with poststroke SHS who met the inclusion criteria. They were randomized into the treatment group and the control group, with 60 patients allocated to each group. The treatment group received routine medical treatment and rehabilitative care after using the Qingpeng ointment, while the patients in the control group received only routine treatment without the ointment. All patients received clinical assessment with the Visual Analogue Scale (VAS), measurement of the range of motion (ROM) of the upper-limb joints, the Fugl-Meyer Assessment of Upper Extremity (FMA-U) and the Modified Barthel Index Score (MBI) before and after the whole treatment. Results: After 4 weeks of treatment, the VAS scores of both groups were decreased significantly (P<0.05), and the difference between the two groups was statistically significant (P < 0.05). There is no statistical significance for the difference between the treatment group and control group in terms of the FMA-U and MBI scores and the forward bend, backward, outstretch, external rotation and pronation angles after treatment. The increases in the values of VAS, FMA-M and MBI in the treatment group were greater than those in the control group, and the difference was statistically significant (P < 0.05). The increases in the values of the forward bend, outreach and external rotation angles in the treatment group were greater than those in the control group, and the difference was statistically significant (P < 0.05). Conclusion: The treatment group showed better results than the control group in terms of the relief of pain symptoms, the improvement of motor function and the improvement of the activities of daily living for patients with shoulder-hand syndrome after cerebral hemorrhage. Qingpeng ointment is effective and safe in treating poststroke SHS.

Aim.To determine one traditional Chinese medicine (TCM)Nao-Xue-Shuoral liquid which protects and improves secondary brain insults (SBI) in hypertensive cerebral hemorrhage (HCH).Methods.158 patients with HCH were divided into routine clinical medicine plusNao-Xue-Shuoral liquid (n=78) as treatment group, and routine clinical medicine (n=80) only served as the control group. The incidence of SBI and the classification of a favorable prognosis and a bad prognosis using the Glasgow outcome scale (GOS) were assessed to evaluate the clinical effects. The changes of IL-6 and TNF-αlevels were determined to study the mechanism of the effects for the TCM.Results.The incidence of SBI at the end of week 2 was 8.97% in the treatment group and 23.75% in the control group, and the difference was significant (P<0.001). The incidence of a favorable prognosis was 48.72% in the treatment group and 32.72% in the control group, and the difference was significant (P<0.01) at the end of week 2. These findings indicate clear differences for IL-6 and TNF-αat the end of week 1 and week 2 compared with before treatment for the treatment group and a marked difference at the end of week 2 between the two groups. It also shows a significant difference between the end of week 2 and before treatment for IL-6 and TNF-αfor the control group, although the difference was much smaller than the treatment group.Conclusion. Nao-Xue-Shuoral liquid could protect against the occurrence of SBI and improve HCH and SBI patients. It may also decrease the damage and the mass effects of the hematoma by reducing IL-6 and TNF-αto obtain the effects, and thus it is a potentially suitable drug for HCH and SBI.

Objectives: To examine the effects of Psyllium husk on HDL-Cholesterol. Design: Single blind placebo controlled study. Place and Duration of Study: Study was conducted at department of Pharmacology and therapeutics at Basic Medical Sciences Institute (BMSI), Jinnah Postgraduate Medical Centre (JPMC), Karachi, from January 2006 to July 2006. Patient and Methods: Forty hyperlipidemic patients were included, among which 20 patients were on placebo as control group, and 20 were on Psyllium husk, 3 gram daily, in divided doses for the period of three months. Patients with peptic ulcer, renal disease, hepatic disease, hypothyroidism, diabetes mellitus, and alcoholism were excluded from the study. HDL- Cholesterol was determined by using kit Cat. # 303210040 by Eli Tech Diagnostic, France. Data were expressedas the mean ± SD and “t” test was applied to determine statistical significance as the difference. A probability value of <0.05 was the limit of significance. Results: Two patients were dropped from the study due to low compliance of metallic taste of psyllium husk. Psyllium husk has increased HDL-Cholesterol, in 90 days of treatment, from 34.61±1.85 to 36.77±1.96 (mg/dl), which was highly significant statistically whenpaired “t” test was applied for results. Percentage change was +6.24. Conclusions: It is concluded from this study that Psyllium husk decreases the risk of CHD by increasing HDL-Cholesterol.

Objective: To evaluate the effect of butyphthalide in the treatment of massive cerebral infarction. Methods: One hundred and twenty patients with massive cerebral infarction who were admitted to the hospital between January 2017 and December 2017 were selected and divided into a treatment group (n = 60) and a control group (n = 60) using random number table, 80 each group. Patients in the control group were given conventional cerebral infarction therapy, while patients in the treatment group were given butyphthalide injection besides the conventional treatment. The National Institutes of Health Stroke Scale (NIHSS) score, score of activity of daily living (ADL), lipoprotein-associated phospholipase A2 (LP-PLA2) and prognosis were recorded and compared between the two groups. The response rates of the two groups were recorded. Results: The total response rates of the control group and treatment group were 73.85% and 93.85% respectively at the postoperative 21st day, and the difference had statistical significance (P<0.05). The NIHSS score of the two groups obviously decreased, and the ADL score significantly increased after treatment; the differences of NIHSS score and ADL score before and after treatment in the same group had statistical significance (P<0.05). The improvement of the indexes of the treatment group was obviously superior to that of the control group, and the differences between the two groups had statistical significance (P<0.05). The level of LP-PLA2 of both groups significantly decreased at the postoperative 21st day, and the difference before and after treatment in the same group was statistically significant (P<0.05); the treatment group had a significantly lower level of LP-PLA2 than the control group, and the difference had statistical significance (P<0.05). The treatment group had significantly higher positive outcome rate and lower mortality rate than the control group at the postoperative 90th day, and the differences had statistical significance (P<0.05). The incidence of adverse events of the treatment group and control group was 8.3% (5/60) and 5.0% (3/60) respectively, suggesting no significant difference (P>0.05). Conclusion: Butyphthalide has a favourable effect in treating massive cerebral infarction. Objectives: To explore the clinical value of minimally invasive aspiration and drainage of intracranial hematoma in the treatment of cerebral hemorrhage. Methods: Seventy-eight patients with cerebral hemorrhage who were treated in the Taian City Central Hospital and the Second Affiliated Hospital of Shandong First Medical University between June 2018 and December 2019 were selected. The patients were randomly numbered and divided into two groups by drawing lots, 39 in each group. The control group was treated with the traditional internal medicine conservative therapy, and the observation group was treated with minimally invasive intracranial hematoma aspiration and drainage. The indexes of the two groups were compared. Results: The efficacy rate of the observation group was significantly higher than that of the control group, and the difference was statistically significant (P<0.05). The National Institutes of Health Stroke Scale (NIHSS) score of the observation group was lower than that of the control group after treatment, and the difference was statistically significant (P<0.05). After treatment, the good recovery rate of the observation group was higher compared to the control group, and the difference had statistical significance (P<0.05). The incidence of complications in the observation group was lower than that of the control group, with a statistically significant difference (P<0.05). Objective. To evaluate the involvement of the bursa located next to the head of the 5th metatarsal bone in patients with psoriatic arthritis (PsA) in comparison with the other seronegative spondyloarthritis (SpA).Methods. All patients with PsA seen during a period of 24 months were enrolled. The control group included healthy subjects and patients with the other SpA. All subjects underwent clinical and ultrasound (US) examination of the lateral surface of the 5th metatarsal.Results. 150 PsA patients (88 M; 62 F), 172 SpA (107 M; 65 F), and 95 healthy controls (58 M; 37 F) were evaluated. Based on clinical and US evaluation, bursitis was diagnosed in 17/150 (11.3%) PsA patients but in none of the SpA (P<0.0001) and healthy (P=0.0002) controls. In detecting bursitis, US was more sensitive than clinical examination, although the difference did not reach statistical significance (P=0.09).Conclusion. The bursa of the 5th metatarsophalangeal joint appears to be involved in PsA more frequently than by chance. If confirmed by other studies, this finding could be considered as a distinctive clinical sign of PsA, useful for differential diagnosis with the other SpA. In asymptomatic patients, US proved to be more sensitive in the detection of bursitis.

Background. Although the association between human chorionic gonadotropin (hCG) and hyperandrogenism was identified more than 40 years ago, relevant questions remain unanswered.Design and Methods. We conducted a prospective, longitudinal, and controlled study in 23 women with a diagnosis of a complete hydatidiform mole (HM).Results. All participants completed the study. Before HM evacuation mean hCG was markedly higher in the cases than in the control group (P≤0.001). Free testosterone (T) and dehydroepiandrosterone sulfate (DHEA-S) were found to be higher in the cases (2.78 ± 1.24 pg/mL and 231.50 ± 127.20 μ/dL) when compared to the control group (1.50 ± 0.75 pg/mL and 133.59 ± 60.69 μ/dL) (P=0.0001and 0.001), respectively. There was a strong correlation between hCG and free T/total T/DHEA-S concentrations (r=0.78;P≤0.001,r=0.74;  P≤0.001, andr=0.71;  P≤0.001), respectively. In the cases group 48 hours after HM evacuation, hCG levels were found to be significantly lower when compared to initial levels (P=0.001) and free T and DHEA-S declined significantly (P=0.0002and 0.009).Conclusion. Before uterus evacuation, hCG, free T, and DHEA-S levels were significantly higher when compared with controls finding a strong correlation between hCG and free T/DHEA-S levels. Forty-eight hours after HM treatment hCG levels declined and the difference was lost. A novel finding of our study is that in cases, besides free T, DHEA-S was also found to be significantly higher and both the ovaries and adrenal glands appear to be the sites of this androgen overproduction.

AbstractThe aim of the study was to determine the frequency and titers of anti-thyroid peroxidase (Anti-TPO), anti-thyroglobulin (Anti-TG), and anti-glutamic acid decarboxylase (Anti-GAD) antibodies in Turkish patients with type 1 diabetes mellitus (DM), and to compare the frequency of anti-TPO and anti-TG titers in the presence or absence of anti-GAD. A total of 104 patients including 56 males and 48 females with type 1 DM and their age-, gender-, and body mass index-matched control group, including 31 males and 27 females, 58 cases in total with an age range of 15-50 years, were recruited into this study. In patients with type 1 DM, positive anti-GAD was detected in 30.8% (n=32). In patients with positive anti-GAD, rate of positive anti-TPO was 37.5%; however, in patients with negative anti-GAD, the rate of positive anti-TPO was 9.7% and the difference was statistically significant (p=0.001). In patients with positive anti-GAD, the rate of positive anti-TG was 18.8%. In patients with negative anti-GAD, the rate of positive anti-TG was 2.8%, and the difference between them was statistically significant (p=0.005). In patients with positive and negative anti-GAD, rates of both positive anti-TPO and anti-TG were 15.6% and 1.4%, respectively, with the difference showing statistical significance (p=0.004). Thyroid autoimmunity in type 1 DM patients with positive anti-GAD was apparently higher; therefore, these patients should be followed more frequently and carefully.

To observe the difference in clinical effects of scalp-point cluster acupuncture combined with rehabilitation training in treating spastic paralysis of upper limbs after stroke. Using a randomized controlled design, 96 patients with upper limb spastic paralysis after stroke were randomly divided into two groups: treatment group (scalp acupuncture plus rehabilitation training group 48 cases), control group (rehabilitation training group 48 cases). After 2 courses of treatment, it was judged by observing clinical efficacy evaluation, Ashworth classification, and Fugl-Meyer (FMA) score. The total effective rate was 91.7% in the treatment group and 68.7% in the control group; the difference between the two groups was statistically significant (P <0.05). Before treatment, the difference in Ashworth classification between the two groups was not statistically significant (P> 0.05); after treatment, the difference between the two groups was statistically significant (P <0.01). After treatment, the difference between the two groups was statistically significant (P<0.05). The Fugl-Meyer (FMA) scores of the two groups of patients before treatment were comparable (P>0.05), and there were significant differences between the two groups after treatment (P <0.05). The difference of Fugl-Meyer (FMA) scores between the two groups was statistically significant (P < 0.01). Scalp cluster acupuncture therapy is more effective than traditional acupuncture therapy alone in treating vertebral artery type cervical spondylosis. Scalp cluster acupuncture combined with Bobath technique is effective in treating spastic paralysis of upper limbs after stroke, and it is worthy of clinical application.

Dental healthcare is very important since dental and oral hygiene should be maintained. Statistics showed that more than 80% of children in developed and developing countries suffer from dental disease. This study aimed to see the safety of OHIS (Oral Hygiene Index Simplified) in primary school students who got and did not get little dentist cadre training in Bangli Regency in 2019. The study was done in an experimental design: pre and post-test with control design, which was conducted in August-September 2019. The sample in this study is 366 students. The difference in OHIS scores before and after treatment in the control group and treatment group was carried out by the bivariate Mann Whitney U Test. The results of the study showed that before dental health training was conducted, there were 54.3% of primary school students in the treatment group with good OHIS score criteria, and after the training was carried out as many as 98.4% of the students in the treatment group had a good OHIS. Additionally, prior to the training, 57.5% of the control group had fair OHIS criteria score. Then, after the training was carried out, 73.7% of the control group had good OHIS score. Hence, the study concluded that there was a significant difference in the OHIS of primary school students who got little doctor training prior to and after the training. Also, there was a significant difference in the OHIS of the students who did not get such treatment before and after the training.

This study aims to look at the histopathological picture of the dorsal arteries of the penis of the hiperlipidemic wistar rats (Rattus novergicus) induction by high-fat diet that seen in terms of lumen diameter and thickness of the arterial endotelium wall. Hyperlipidemia is a risk factor for ateriosclerosis in the penile arteries causing erectile dysfunction in men. This study is an experimental study with a randomized posttest only control goup design. The sample are  10 individuals adult male wistar rats aged 3-4 months with a range of body weight 150-200 grams. Before treatment, adaptation was carried out for 7 days. After that the sample rats in the treatment group were made hyperlidemic by feeding lard for 50 days. Then surgery is performed for histopathological preparations at the posttest. To determine the differences in endotelium thickness and arterial lumen diameter in the two groups, an independent t-test was used. Thick diameter data of the endotelium and dorsal arteries of the penis of the wistar rat between the lower treatment group and the control group. The difference that occurred was statistically significant (p <0.05). So it can be concluded that the provision of high-fat diet (hyperlipidemia) decreases the lumen diameter and endotelium thickness of dorsal arteries penis.
