Objectives: To examine the effects of Psyllium husk on HDL-Cholesterol. Design: Single blind placebo controlled study. Place and Duration of Study: Study was conducted at department of Pharmacology and therapeutics at Basic Medical Sciences Institute (BMSI), Jinnah Postgraduate Medical Centre (JPMC), Karachi, from January 2006 to July 2006. Patient and Methods: Forty hyperlipidemic patients were included, among which 20 patients were on placebo as control group, and 20 were on Psyllium husk, 3 gram daily, in divided doses for the period of three months. Patients with peptic ulcer, renal disease, hepatic disease, hypothyroidism, diabetes mellitus, and alcoholism were excluded from the study. HDL- Cholesterol was determined by using kit Cat. # 303210040 by Eli Tech Diagnostic, France. Data were expressedas the mean ± SD and “t” test was applied to determine statistical significance as the difference. A probability value of <0.05 was the limit of significance. Results: Two patients were dropped from the study due to low compliance of metallic taste of psyllium husk. Psyllium husk has increased HDL-Cholesterol, in 90 days of treatment, from 34.61±1.85 to 36.77±1.96 (mg/dl), which was highly significant statistically whenpaired “t” test was applied for results. Percentage change was +6.24. Conclusions: It is concluded from this study that Psyllium husk decreases the risk of CHD by increasing HDL-Cholesterol.

Background: Shoulder-hand syndrome (SHS) refers to a syndrome causing sudden edema, shoulder pain and limited hand function. Qingpeng ointment, a kind of Tibetan medicine, can reduce swelling, relieve pain, tonify stagnation and clear the meridians, which is consistent with the pathological mechanism of SHS after stroke. Therefore, if clinical trials can be used to explore the effectiveness of Qingpeng ointment for treatment of poststroke SHS and promote its application in clinical medicine, this is of specific significance for the treatment of poststroke SHS. Objective: To investigate the clinical efficacy and safety of Qingpeng ointment in the treatment of poststroke SHS. To provide an objective basis for a better therapeutic treatment for poststroke SHS. Method: A prospective, randomized, controlled study was conducted. This study recruited 120 patients with poststroke SHS who met the inclusion criteria. They were randomized into the treatment group and the control group, with 60 patients allocated to each group. The treatment group received routine medical treatment and rehabilitative care after using the Qingpeng ointment, while the patients in the control group received only routine treatment without the ointment. All patients received clinical assessment with the Visual Analogue Scale (VAS), measurement of the range of motion (ROM) of the upper-limb joints, the Fugl-Meyer Assessment of Upper Extremity (FMA-U) and the Modified Barthel Index Score (MBI) before and after the whole treatment. Results: After 4 weeks of treatment, the VAS scores of both groups were decreased significantly (P<0.05), and the difference between the two groups was statistically significant (P < 0.05). There is no statistical significance for the difference between the treatment group and control group in terms of the FMA-U and MBI scores and the forward bend, backward, outstretch, external rotation and pronation angles after treatment. The increases in the values of VAS, FMA-M and MBI in the treatment group were greater than those in the control group, and the difference was statistically significant (P < 0.05). The increases in the values of the forward bend, outreach and external rotation angles in the treatment group were greater than those in the control group, and the difference was statistically significant (P < 0.05). Conclusion: The treatment group showed better results than the control group in terms of the relief of pain symptoms, the improvement of motor function and the improvement of the activities of daily living for patients with shoulder-hand syndrome after cerebral hemorrhage. Qingpeng ointment is effective and safe in treating poststroke SHS.

BACKGROUND: There has been an alarming increase in the prevalence of Coronary Artery Disease (CAD) worldwide and is a leading cause of mortality in India. Yoga is emerging as a signicant tool in managing the disease. OBJECTIVES:The study was conducted to nd out the effect of yogic lifestyle modication on anginal episodes in CAD patients. METHODS: This prospective, controlled study included the CAD patients between the ages 30 to 70 years who were randomised to yoga group (group A, n=51) and control group (group B, n=51). They were assessed clinically on the basis of number of anginal episodes per week. The yoga group patients were prescribed a family based yoga programme in addition to the conventional drug treatment for CAD. The control group received conventional drug therapy alone. The patients were assessed at baseline and thereafter, at the end of 6 months. RESULTS: The number of anginal episodes per week reduced signicantly by 25.40% in the yoga group vis-à-vis the control group at the end of six months (independent t-test p-value<0.01). It was also inferred from the study that while at baseline, there was no statistically signicant difference between the yoga and control groups (independent t-test p-value>0.05), the difference became signicant at the end of six months (independent t-test p-value<0.05). CONCLUSIONS: In the present study, we witnessed a substantial and signicant effect of yogic lifestyle intervention in decreasing the number of anginal episodes per week in CAD patients thereby reducing the morbidity associated with the disease considerably

Background/Aim. Tibolone is a preparation that belongs to the group of steroidal substances. The effects of the use of the use of tibolone are the consequence of the activities of its metabolities, considering that their hormonal activity depends on the type of tissue in which they develop. The aim of this study was to evaluate the influence of the use of tibolone on risk factors for the development of cardiovascular diseases in postmenopausal women. Methods. A prospective observational stady included 94 patients who had the concentration of l7? estradiol < 50 pg/ml, and who was in menopause more than a year. Out of the total number of patients, 63 accepted to receive tibolone 2.5 mg daily (tibolone group), while 31 of the patients refused to take tibolone (control group). We measured the concentration of lipids (cholesterol, LDL cholesterol, HDL cholesterol, triglicerides), antitrombin III, fibrinogen, and C-reactive protein, before and after the treatment within a 6-month period. Then, we compared the difference between the values of concentrations and tested the statistical significance of the difference. We also evaluated the changes of values in the concentrations of the examinated parameters inside a 6-month period in the control group. Results. In 31 patients of the control group, from the control group there were no significant changes in the values of the defined parameters as compared to their initial values after six months. But there were changes of statistical significance (p < 0.001) in values of the concentrations of the exeminated parameters before and after the treatment in the tibolone group. In fact, we recorded decreases in the total cholesterol by 17.8%, HDL cholesterol by 27%, LDL cholesterol by 4% (without statistical significance p > 0.05) and triglicerids by 35%. There were no statistical differences in the concentrations of antitrombin III, fibrionogen, and C-reactive proteine in the tibolone group before and after the treatment. Conclusion. The use of tibolone dose decrease the concentration of the total cholesterol, triglicerides, HDL cholesterol, without a significant decrease of LDL cholesterol. Also, the use of tibolone does not have any significant effect on the concentrations of antitrombin III, fibrinogen and C-reactive proteine. The number of serum parameters measured in this study was limited, thus that was the reason to discuss only about the metabolism of lipids in the patients from the tibolone group. The final conclusion about the risk for cardiovascular diseases in the patients on tibolone, howerer, reqnires were extensive further clinical exeminations.

Objective. To evaluate the involvement of the bursa located next to the head of the 5th metatarsal bone in patients with psoriatic arthritis (PsA) in comparison with the other seronegative spondyloarthritis (SpA).Methods. All patients with PsA seen during a period of 24 months were enrolled. The control group included healthy subjects and patients with the other SpA. All subjects underwent clinical and ultrasound (US) examination of the lateral surface of the 5th metatarsal.Results. 150 PsA patients (88 M; 62 F), 172 SpA (107 M; 65 F), and 95 healthy controls (58 M; 37 F) were evaluated. Based on clinical and US evaluation, bursitis was diagnosed in 17/150 (11.3%) PsA patients but in none of the SpA (P<0.0001) and healthy (P=0.0002) controls. In detecting bursitis, US was more sensitive than clinical examination, although the difference did not reach statistical significance (P=0.09).Conclusion. The bursa of the 5th metatarsophalangeal joint appears to be involved in PsA more frequently than by chance. If confirmed by other studies, this finding could be considered as a distinctive clinical sign of PsA, useful for differential diagnosis with the other SpA. In asymptomatic patients, US proved to be more sensitive in the detection of bursitis.

Background. Although the association between human chorionic gonadotropin (hCG) and hyperandrogenism was identified more than 40 years ago, relevant questions remain unanswered.Design and Methods. We conducted a prospective, longitudinal, and controlled study in 23 women with a diagnosis of a complete hydatidiform mole (HM).Results. All participants completed the study. Before HM evacuation mean hCG was markedly higher in the cases than in the control group (P≤0.001). Free testosterone (T) and dehydroepiandrosterone sulfate (DHEA-S) were found to be higher in the cases (2.78 ± 1.24 pg/mL and 231.50 ± 127.20 μ/dL) when compared to the control group (1.50 ± 0.75 pg/mL and 133.59 ± 60.69 μ/dL) (P=0.0001and 0.001), respectively. There was a strong correlation between hCG and free T/total T/DHEA-S concentrations (r=0.78;P≤0.001,r=0.74;  P≤0.001, andr=0.71;  P≤0.001), respectively. In the cases group 48 hours after HM evacuation, hCG levels were found to be significantly lower when compared to initial levels (P=0.001) and free T and DHEA-S declined significantly (P=0.0002and 0.009).Conclusion. Before uterus evacuation, hCG, free T, and DHEA-S levels were significantly higher when compared with controls finding a strong correlation between hCG and free T/DHEA-S levels. Forty-eight hours after HM treatment hCG levels declined and the difference was lost. A novel finding of our study is that in cases, besides free T, DHEA-S was also found to be significantly higher and both the ovaries and adrenal glands appear to be the sites of this androgen overproduction.

AbstractThe aim of the study was to determine the frequency and titers of anti-thyroid peroxidase (Anti-TPO), anti-thyroglobulin (Anti-TG), and anti-glutamic acid decarboxylase (Anti-GAD) antibodies in Turkish patients with type 1 diabetes mellitus (DM), and to compare the frequency of anti-TPO and anti-TG titers in the presence or absence of anti-GAD. A total of 104 patients including 56 males and 48 females with type 1 DM and their age-, gender-, and body mass index-matched control group, including 31 males and 27 females, 58 cases in total with an age range of 15-50 years, were recruited into this study. In patients with type 1 DM, positive anti-GAD was detected in 30.8% (n=32). In patients with positive anti-GAD, rate of positive anti-TPO was 37.5%; however, in patients with negative anti-GAD, the rate of positive anti-TPO was 9.7% and the difference was statistically significant (p=0.001). In patients with positive anti-GAD, the rate of positive anti-TG was 18.8%. In patients with negative anti-GAD, the rate of positive anti-TG was 2.8%, and the difference between them was statistically significant (p=0.005). In patients with positive and negative anti-GAD, rates of both positive anti-TPO and anti-TG were 15.6% and 1.4%, respectively, with the difference showing statistical significance (p=0.004). Thyroid autoimmunity in type 1 DM patients with positive anti-GAD was apparently higher; therefore, these patients should be followed more frequently and carefully.

Aim: In this work, it is aimed that examining the effects of target-oriented circular training on biomotoric features by using a tennis ball throwing machine at 12-14 age tennis performance sportsmen for ten weeks. Method: This research was implemented on sixteen active licensed athletes who played tennis for at least four years in Ankara. The athletes were randomly divided into two separate groups as the experimental group (n=8) and the control group (n=8). After measuring the height, weight, and fat rate of the athletes, biometric tests were started. Flamingo balance, sit-reach, reaction time, five meters and twenty meters sprints, T-test, and standing long jump tests were performed, respectively. Athletes were get heated for ten minutes before the tests and they were given a trying chance. The best scores were recorded by repeating each test twice. In the statistical analysis of the collected data, IBM SPSS 19 package program was used. In repeated measurements, the results were compared by two-way ANOVA with intergroup, intragroup and post-training data. Results: As a result, between the experimental group and control group data; on averages of T-Test (p<0.139), visual reaction (p<0.001), Flexibility (p<0.024), Vertical Jump (p<0.022), Flamingo Balance right foot (p<0.046) and left foot (p<0.045) statistical significance was confirmed. The present study investigates the effect of negative emotions (focusing on disappointment, sadness and regret) on cognitive schemas. The sample consist in students from the University of Bucharest (convenience groups). A univariate unifactorial experimental design was used, as the current study had one independent variable, negative emotions, and a dependent variable, cognitive schemas, which was measured with the Core Beliefs Questionnaire instrument. We applied T test for independent samples, using SPSS. The t test was statistically significant, t (45) = 2.57, p <0.05. The confidence interval (95%) for the difference between the averages is between 2.95 and 24.35. After applying the intervention, the evaluated sample (M = 43.17, SD = 29,074) showed a statistically significant difference compared to the control group (M = 29.52, SD = 11.257). The data collected support the main research hypothesis of the study, negative emotions can bring about changes in certain cognitive schemas by modifying, altering or disrupting them.

Traffic accidents are one of the public health problems that affect all sectors of life. The phenomenon of traffic accidents so far has not received much public attention as a cause of death. The increase in mortality rates that occur on the highway is very high and quite a concern and vigilance for the community. This usually happens one of them because the Indonesian people do not know how to help victims who are good and right when finding victims. As a result most of actions is wrong, so  it can add to injury and  death. This study aims to determine the effect of basic life support (BLS) training on the motivation and demeanour of class XI students in rescuing  traffic accidents in SMA Negeri 2 Tenggarong. This study uses a quasy experimental method of pre and post design with a control group. Proportional stratified random sampling technique with a sample of 78 students and data collection using a questionnaire sheet. Data that was analyzed by paired t test in each group showed a p-value of 0,000 <0.05, meaning that there was a statistically change in motivation and attitude between before and after treatment in the form of BLS training for class XI students at SMAN 2 Tenggarong.  The result is not much different from the independent t test which showed p-value 0,000 <0.05, which means there is a statistically different demeanour between the difference before and after treatment in the form of BLS training in the control and intervention groups. The results showed that there was a statistically significant change in motivation and demeanour between before and after the BLS training was given to the motivation and demeanour of class XI students at SMAN 2 Tenggarong with a p-value of 0,000 <0.05.

ABSTRACT [The Effectiveness of Using a Model Problem-based Learning (PBL) to the Results of Physics Learning on High School Students 9 Lubuklinggau]. The objective of this study was to determine the effectiveness of the use of problem-based learning model. The research method used experiments with the form of Nonequivalent Control Group Design. The sample in this research is the students of class XI A and class XI B SMA N 9 Lubuklinggau City which amounted to 50 students. The  technique of collecting data in this research is a test. The test is in the form of pretest and finally test (posttest). The purpose of holding initial and final tests to determine the effectiveness of Physics learning with PBM model and conventional learning. The test instrument that was made before the test was tested was done by panelists test using Intracorelation class coefficient (ICC). Data analysis technique used in this research is t-test. The results showed that there are differences in student learning outcomes taught with Problem-Based Learning model (PBM) with students taught with conventional learning model. The difference can be seen in the result of t-test which yields t-count> t-table (3,98> 2.01). The effectiveness of using the PBM model in the experimental class is 13.5% better than in the control class. Keywords: Effectiveness; model; PBL; learning outcomes.
