  Objectives: To create a radiological profile of fungal sinusitis and determine the radiological differences between fungal and nonfungal sinusitis based on the presence of hyperattenuation, bony erosion, neo-osteogenesis, air-fluid level, and extrasinus extension.Methods: This is a retrospective, single-blind, case-control study involving the analysis of 119 computed tomography (CT) scans of the paranasal sinuses. Based on the histopathology, they were divided into cases comprising fungal sinusitis and controls of nonfungal sinusitis. Benign and malignant tumors and previously operated cases of fungal sinusitis were excluded from the study. The principal investigators were blinded to the diagnosis. The comparison parameters were hyperattenuation, the presence of air-fluid level, bone erosion, neo-osteogenesis, and extrasinus extension. Data was analyzed by Chi-square and Fischer exact t-test using SPSS 14.0 software and a p < 0.05 was considered significant.Results: Our study showed the presence of hyperattenuation, neo-osteogenesis, bone erosion, air-fluid level, extrasinus extension in 75.2%, 48.3%, 25.9%, 36.2%, and 6.9% of the cases and 13.1%, 16.4%, 6.6%, 9.8%, and 0 controls, respectively. All the parameters were statistically significant in cases when compared to controls.Conclusion: Hyperattenuation, neo-osteogenesis, air-fluid level, bone erosion, and extrasinus extension are the parameters on CT imaging that will help routinely assess and differentiate fungal sinusitis from nonfungal sinusitis with considerable accuracy, although, there is an overlap with malignancy when the parameter of bone erosion is considered as a differential diagnosis of chronic invasive fungal sinusitis. It reiterates the fact that history, clinical examination, and laboratory evaluation hold an important role in provisional diagnosis.

AbstractObjective:We report a case of allergic fungal sinusitis causing bone erosion and diplopia.Case report:A 43-year-old man presented with a four-month history of increased nasal congestion and progressive diplopia. Clinical examination revealed bilateral nasal polyposis and a right lateral gaze deficit, consistent with a VIth cranial nerve palsy. Computed tomography of the paranasal sinuses demonstrated a large sellar mass with extensive bony erosion and both supra- and infra-sellar extension. An endoscopic approach to the sphenoid sinus, clivus and posterior cranial fossa with image guidance was performed, enabling surgical treatment involving nasal polypectomy, wide marsupialisation of the sphenoid sinus and removal of the extensive allergic fungal mucin. The patient awoke from anaesthesia with complete resolution of his diplopia.Conclusion:Otolaryngologists should be aware that approximately 20 per cent of patients with allergic fungal sinusitis demonstrate paranasal sinus expansion and bone erosion involving surrounding anatomical structures. Such patients may have clinical findings involving the orbit and cranial vault.

Prolap organ panggul merupakan kondisi yang mempengaruhi kualitas hidup wanita. Prolaps organ panggul ini dapat disebabkan oleh perlukaan sewaktu proses persalinan, proses penuaan, komposisi jaringan pada seorang wanita, batuk- batuk kronis, atau sering melakukan pekerjaan berat. Pengenalan dini prolaps terkait dengan prognosis pemulihan anatomik dan fungsional organ panggul. Hingga kini, penerapannya dalam dunia klinis belum banyak sehingga pelatihan dan pembelajaran lebih lanjut tentang pelvic organ prolapse quantification (POPQ) jelas diperlukan. Penelitian ini dilakukan dengan metode case control study di polikilinik Obgin RSUP. Dr. M. Djamil Padang mulai bulan September 2013 sampai jumlah sampel terpenuhi sebanyak 98 orang. Dengan 49 orang kelompok kontrol dan 49 orang kelompok kasus .Analisis dilakukan untuk menilai hubungan usia, paritas, pekerjaan dan indek massa tubuh dengan kejadian prolap organ panggul berdasarkan skor POPQ. Data disajikan dalam bentuk tabel. Data diuji dengan t test dan chi square test. Jika p<0,05 menunjukan hasil yang bermakna. Terdapat hubungan yang bermakna antara usia dengan kejadian prolap organ panggul dengan (p<0,05) dan OR 27,871.terdapat hubungan yang bermakna antara paritas dengan kejadian prolap organ panggul dengan (p<0,05) dan OR 52,970.Dari analisa statistik pekerjaan tidak bisa di uji secara statistik.indek massa tubuh tidak terdapat hubungan yang bermakna terhadap kejadian prolap organ panggul.(p>0,05)

Masalah kesehatan masyarakat yang cukup seriuspada kelompok usia balita sampai saat ini adalah kejadian malnutrisi, hal iniberdampak pada gangguan pertumbuhan dan perkembangan fisik balita. Malnutrisi juga dapat menyebabkan balita menjadi rentan terhadap penyakit infeksi. Beberapa faktor penyebab malnutrisi diantaranya faktor makanan dan penyakit infeksi yang mungkin diderita anak, faktor ketahanan pangan dikeluarga, pola pengasuhan anak, pelayanan kesehatan dan kesehatan lingkungan. Tujuan penelitian ini adalah untuk mengetahui apakah sindrom penyakit tropis merupakan prediktor terjadinya gizi kurang pada balita. Penelitian ini termasuk penelitian observasional analitik dengan rancangan Case Control Study. Populasi dalam penelitian ini adalah seluruh pasangan ibu dan balita yang ada di daerah pesisir kecamatan Nambo Kota Kendari denganteknik simple randam sampling di dapatkan jumlah sampel  sebanyak 164 orang yang terdiri dari dua kelompok yaitu kelompok kasus dan kelompok kontrol dengan perbandingan 1:1. Data dianalisis secara statistik dengan uji Chi square pada tingkat kepercayaan 95% (α=0,05) dengan mempertimbangkan nilai Odd Ratio dan nilai Confidence Interval. Hasil penelitian ini menunjukan bahwa sindrom penyakit tropis (diare dengan р-value = 0,001 dan OR = 4,162, batuk – batuk dengan р-value = 0,001 dan OR = 3,552,ISPAnon pneumoni dengan р-value = 0,004 dan OR = 3,003) merupakan faktor prediktor terjadinya malnutrisi pada balita sedangkan infeksi cacing dengan р-value = 0,056 dan OR= 1,773 bukan merupakan faktor prediktor terjadinya malnutrisi pada balita di daerah pesisir Kecamatan Nambo Kota Kendari

Background: Infusion reactions (IRs) are the most common adverse events (AEs) of infliximab (IFX) treatment in patients with inflammatory bowel disease (IBD). Prophylactic premedication (PM) with corticosteroids or antihistamines prior to IFX infusions has been used in clinical practice, but its efficacy is not known. The aim of this study was to assess the influence of steroid PM on IR incidence in pediatric patients with IBD receiving IFX. Methods: We performed a case–control study that included pediatric patients with IBD receiving IFX. Patients were divided into four subgroups according to the agent and PM they received: Remicade (original drug) + PM, and two biosimilars—Reshma +/− PM, and Flixabi—PM. At our site, until 2018, PM with steroids was used as a part of standard IFX infusion (PM+); however, since then, this method has no longer been administered (PM−). IRs were divided into mild/severe reactions. Differences between subgroups were assessed with the appropriate chi-square test. Multivariate logistic regression was used to assess associations between PM and IR incidence, correcting for co-medication usage. Results: There were 105 children (55 PM+, 44 male, mean age 15 years) included in the study who received 1276 infusions. There was no difference between the PM+ and PM− subgroups, either in incidence of IR (18.2% vs. 16.0% of patients, p > 0.05) or in percentage of infusions followed by IR (2.02% vs. 1.02% of infusions, p > 0.5). The OR of developing IR when using PM was 0.34, and the difference in IRs ratio in PM+ and PM− patients was not statistically significant (95% CI, 0.034–1.9). There were 11/18 (61.1%) severe IRs (anaphylactic shock) reported in all patients (both PM+ and PM−). Conclusion: At our site, the incidence of IR was low, and PM did not decrease the incidence of IR in pediatric patients with IBD receiving IFX. These results indicate that PM with steroids should not be a standard part of IFX infusion to prevent IR.

Although the diagnosis of chronic invasive fungal sinusitis relies chiefly on identification of invasive fungi on histology, the insidious nature of the disease can preclude detection of fungal organisms. Here, we present a case of chronic invasive fungal sinusitis with negative histopathologic findings and a definitive diagnosis made through fungal DNA detection. Clinicians should consider polymerase chain reaction an important complement to histology and culture in the diagnosis of chronic invasive fungal sinusitis.

Introduction: (1) The female genital tract is constituted by the ovaries, fallopian tubes, uterus (body/corpus and cervix), vagina, and vulva. Female genital tract lesions can be benign or malignant. Our study was carried out to nd out the frequency of various histopathological lesions including neoplasms of female genital tract. Materials and Methods: Our study is retrospective conducted over 2 years period. Data was collected from case records of patients presenting to Goa Medical College and histopathology reports obtained from Dept of pathology. Data was represented in form of charts and tables. The analysis of statistical data of variables was done using SPSS software version 22. Signicance was calculated using chi square test. Value of p <0.05 was considered signicant. Results: Out of total 270 subjects studied, 229 were having benign lesions and 41 had malignant lesions.The mean age was 49.5 years. Leiomyoma was diagnosed in (40.58%) cases. In abnormalities of endometrium proliferative endometrium was seen in 37.6%. Chronic cervicitis was seen 78.74% cases. Among the ovarian lesions, 67.74% cases were simple follicular cysts. Cervical cancer was found in 73.17% cases followed by ovarian malignancies in 19.51 % cases, endometrial carcinoma in 4.87% cases and vulvar cancer in 2.44% cases. Among the malignant tumors (58.54%) cases were postmenopausal women. Conclusion: Microscopic assessment and clinico-pathological correlation of lesions is necessary as grossly identiable benign lesion may harbour a focus of malignancy. It aids to appropriate management in the postoperative period. A concerted effort should be done towards prevention of cancers, by creating awareness through health education in addition to implementation of screening methods.

Latar Belakang: Pneumonia merupakan pembunuh utama balita di seluruh dunia. Berdasarkan Laporan Dinas Kesehatan Banda Aceh tahun 2018, jumlah balita penderita Pneumonia meningkat setiap tahunnya. Kasus Pneumonia balita yang paling banyak terdapat di UPTD Puskesmas Banda Raya. Penelitian ini bertujuan untuk mengetahui faktor risiko yang berhubungan dengan kejadian Pneumonia pada Balita di wilayah kerja UPTD Puskesmas Banda Raya Kota Banda Aceh tahun 2019. Metode: Penelitian ini menggunakan desain Case Control Study atau Retrospective Study. Penelitian ini menggunakan total populasi dengan jumlah sampel adalah 142 anak balita berusia 12–59 bulan. Data dianalisis secara Univariat dan Bivariat. Analisis Bivariat menggunakan Uji Chi-Square dengan derajat kepercayaan 95% (p value0.05). Hasil: Hasil penelitian menunjukkan bahwa faktor risiko yang berhubungan dengan kejadian pneumonia adalah luas ventilasi rumah (OR=15.81; CI 95%=4.70-53.12; p value=0.0001); sedangkan umur balita (OR=1.15; CI 95%=0.54-2.43; p value=0.705); jenis kelamin (OR=1.11; CI 95%=0.57-2.16; p value=0.737); pengetahuan ibu (OR=0.38; CI 95%=0.12-1.24; p value=0.112); dan kepadatan hunian (OR=1.80; CI 95%=0.78-4.13; p value=0.163), tidak terbukti secara signifikan sebagai faktor risiko pneumonia balita di UPTD Puskesmas Banda Raya Kota Banda Aceh. Kesimpulan: Faktor risiko yang berhubungan dengan kejadian Pneumonia balita adalah luas ventilasi rumah. Oleh karena itu diperlukannya sanitasi lingkungan yang sehat sebagai upaya preventif terhadap kejadian Pneumonia, serta memperbaiki pola perilaku hidup bersih dan sehat.

Abstract BackgroundThe sequence of establishing proximal stability or function before facilitation of the distal body part has long been recognized in stroke rehabilitation practice but lacks scientific evidence. This study plans to examine the effects of proximal priority robotic priming and impairment-oriented training (PRI) and distal priority robotic priming and impairment-oriented training (DRI). MethodsThis single-blind, randomized, comparative efficacy study will involve 40 participants with chronic stroke. Participants will be randomized into PRI or DRI groups and receive 18 intervention sessions (90 min/d, 3 d/wk for 6 weeks). The Fugl-Meyer Assessment Upper Extremity subscale, Medical Research Council Scale, Revised Nottingham Sensory Assessment, and Wolf Motor Function Test will be administered at baseline, after treatment, and at the 3-month follow-up. Two-way repeated-measures analysis of variance and the chi-square automatic interaction detector method will be used to examine the comparative efficacy and predictors of outcome, respectively, after PRI and DRI. DiscussionThrough manipulating the sequence of applying wrist and forearm robots in therapy, this study will attempt to examine empirically the priming effect of proximal or distal priority robotic therapy in upper extremity impairment-oriented training for people with stroke. The findings will provide directions for further studies and empirical implications for clinical practice in upper extremity rehabilitation after stroke.Trial RegistrationThis trial was registered on June 23, 2020, at www.clinicaltrials.gov (NCT04446273).
