Stroke patients have high foot instability and a high risk of falling down in walking and standing positions due to muscle and sensory disorders. There are many patients who complain of inconvenience in their daily lives, but independent walking is very difficult. In this work, we explore the effect on walking and dynamic balance by securing internal and external stability of the foot. Fifteen participants participated in the experiment, with 10m walk tests, timed and go tests, and functional reach tests. The walk 10m test was not statistically significant, with 0.59 ± 0.23 m/s before the device was worn and 0.66 ± 0.29 m/s after the device was worn. Timed up and Go tests were statistically significant 26.57 ± 6.12 seconds before wearing the device, 22.31 ± 4.32 seconds after wearing the device, 222.59 ± 10.31 cm before wearing the device, and 230.93 ± 11.33 cm after wearing the device. Securing internal and external stability of the foot can have a positive impact on securing a dynamic balance in which weight movement is performed.

Abstract Objective: To determine the effects of proprioception training in improving balance in patients with diabetic neuropathy. Methods: A quasi-experimental design study was conducted at the Safi Hospital Faisalabad for eight weeks from August to December 2019. Out of, 38, 19 patients were placed in exercise group (10 males, 9 females; mean age 64 ± 7.7 years; range 60 to 83 years) and 19 patients were placed in controls group (12 males, 7 females; mean age 63 ± 8.2 years) were included in this study through consecutive sampling technique. Proprioception Training was given to exercise group twice a week for eight weeks and the diabetes awareness campaign was given once a week to the control group. Static and dynamic balance were assessed by using One Leg Standing (OLS) Test with eyes open and closed, Berg Balance Scale (BBS), Functional Reach Test (FRT), Timed Up and Go Test (TUGT) and 10-M Walk Test (10-MWT). The data was collected before and after treatment and was compared using independent sample t-test. Results: The finding of the study showed that OLS score with eyes open improved significantly with p<0.05 and does not show improvement with eyes closed p =.073. The dynamic balance from the Berg Balance Scale, Functional Reach Test, Timed Up and Go Test and 10-M Walk Test revealed significant improvement after the balance exercises with p<0.05 Conclusion: It is concluded that proprioception training exercises are effective in improving balance among patients with Diabetic neuropathy. Abstract Background: The aim of the study was to conduct a long-term evaluation of whether total hip replacement permanently affects the quality of postural reactions and body balance. Material and methods: The unilateral Total Hip Replacement (THR) group consisted of 30 subjects (mean age: 69.4). The control group consisted of 30 healthy subjects (mean age: 68.8). The force platform and functional tests such as Timed Up and Go, 3m walk test, Functional Reach Test, 30s Chair Stand Test, Step Test and Berg Balance Scale were used to assess dynamic balance. Results: Subjects from the study group exhibited significantly increased time (p=0.002) and distance (p=0.012) in the tests performed on the force platform compared to the control group. We also observed worse balance and functional test scores in the THR group: Timed Up and Go test (p<0.001), 3m walk test (p<0.001), Functional Reach Test (p=0.003), 30s Chair Stand Test (p=0.002) and Step Test (operated leg: p<0.001, non-operated leg: p=0.002). The results obtained in the Berg Balance Scale tests were not significantly different between the groups (p=0.597). Conclusions: Our research shows that total hip replacement permanently impairs patients’ dynamic balance and functionality in certain lower-extremity activities. This randomised controlled and double-blinded pilot study evaluated if interactive virtual reality balance related games integrated within conventional rehabilitation sessions resulted in more superior retraining of dynamic balance compared to CR after stroke. 19 subjects diagnosed with a recent episode of stroke were recruited from a local rehabilitation hospital and randomly assigned to either a control or an experimental group. Subjects in the control groups underwent 60 minutes of conventional rehabilitation while those in the experimental groups underwent 40 minutes of convention rehabilitation and 20 minutes of self-directed virtual reality balanced rehabilitation. Functional Reach Test, Timed Up and Go, Modified Barthel Index, Berg Balance Scale, and Centre of Pressure of subjects in both groups were evaluated before and on completion of the rehabilitation sessions. Results indicate that the inclusion of interactive virtual reality balance related games within conventional rehabilitation can lead to improved functional mobility and balance after a recent episode of stroke without increasing treatment time that requires more health professional manpower.

Purpose The effects of aging on physical and mental health may be ameliorated by regular participation in physical activity (PA). There is also evidence for the benefits of various training modalities on cognition and functional ability in older adults. The aim of this study was to compare effects of a 12-week active video gaming intervention (X Box Kinect Sports) to conventional multimodal supervised exercise on fitness, functional ability and cognitive performance in older adults with memory complaints. Methods Participants (n = 45, 72±5 yrs.) were recruited from 6 retirement homes and cluster-randomized into the Interactive Video Gaming (IVG) group (N = 23) or Conventional Multimodal (CM) group (N = 22), meeting 2 x 1 hour sessions, weekly for 12 weeks. Pre-post measures included: 6 min walk, timed up and go, dynamic balance, functional reach, Mini-Mental State Examination, N-back Task and the Modified Stroop task. Results The IVG group demonstrated significant improvement in the total number correct responses on the Stroop task (P = 0.028) and for average reaction time of correct colour-words (P = 0.024), compared to the CM group. Functional ability improved significantly in the IVG group, including the 6-min walk (P = 0.017), dynamic balance (P = 0.03), timed up and go (P<0.001) and functional reach (P<0.0010). Conclusion An active interactive video gaming intervention was more effective than conventional multimodal exercise in improving executive and global cognitive performance and functional capacity in older adults with subjective memory complaints. Aim: to determine if dynamic balance is related to activities of daily living (ADL) dependence in stroke patients. Methods: a cross-sectional study was carried out. The Barthel index (BI) was used to assess ADL dependence. The Timed Up and Go Test (TUG) and the Four Square Step Test (FSST) were used to evaluate dynamic balance. A Spearman correlation was used assuming a data abnormality in the performance of TUG and FSST and considering a significance of p<0.05. Results: 62 stroke patients were evaluated. The BI showed correlations with the performance of the TUG and the FSST. These correlations were also found between performance of both balance tests and ADLs, such as bathing, moving, and going up and down stairs. In contrast to TUG, correlations were estimated between the ADL related to dressing and the performance of the FSST. Conclusion: dynamic balance evaluated by the TUG and the FSST showed correlations with the ADL dependency in stroke patients.

The knee-ankle-foot orthosis (KAFO) tool is commonly described for patients with a history of poliomyelitis and serves to improve gait and prevent degeneration of the knee joint. However, at present the effect of using knee ankle foot orthosis in monoplegia polio patients when walking has never been evaluated. The purpose of this study was to analyze biomechanical adaptation especially in spatiotemporal parameters and stability when the subjects walked with and without KAFO aids. Methods: Fifteen monoplegia subjects with poliomyelitis were tested with two gait analyzes (i.e. with and without KAFO aids). Spatiotemporal parameters were tested using the 10 Meter Walk Test (10MWT) instrument, while stability when running was tested with the Timed up and Go (TUG) test instrument. Data from the spatiotemporal parameters obtained were analyzed using paired T tests, and the McNemar test was used to analyze the stability variables while walking. Both analyzes will analyze differences in parameters tested when subjects are in line with and without KAFO tools. Significant level used is p <0.05. Results: The results showed that there were significant differences in spatiotemporal parameters and stability when subjects walked with and without KAFO aids. This tool increases spatiotemporal parameters and stability when the subject runs using KAFO compared to when they did not use orthosis at all. When the stance phase in the leg is paralysis, hyperextension in the knee and excessive flexion of the knee is reduced, while flexion in the hip joint increases. Walking stability increased in 60% of subjects when they used KAFO when walking. This study found that gait compensation was also reduced when subjects walked using KAFO, and through direct feedback the subject stated that walking using KAFO made them not easily tired and could go further.

BACKGROUND Lower limb spasticity, as is common following a cerebrovascular attack (CVA) or stroke, can affect the balance and gait of patients. This then not only affects independence, and quality of life, but also increases the risk for other concerns, such as falling and an increased sedentariness, which could further affect health outcomes. OBJECTIVE We aimed to evaluate the effect of ankle plantar flexor spasticity severity on balance and to determine the relationship between the spasticity severity with ankle proprioception, passive range of motion (ROM), and balance confidence in post-stroke patients. METHODS Twenty-eight post-stroke patients were divided into two groups based on the level of ankle plantar flexor spasticity according to the Modified Modified Ashworth Scale (MMAS) as a High Spasticity Group (HSG) (MMAS>2) (n=14) or a Low Spasticity Group (LSG) (MMAS≤2) (n=14). The MMAS scores, Activities-Specific Balance Confidence Questionnaire, postural sway in the open and closed eyes conditions, timed up and go (TUG) test, ankle dorsiflexion passive range of motion (ROM), and ankle joint proprioception were measured. RESULTS No significant (p>0.05) differences were found between the LSG and HSG in terms of balance confidence, dynamic balance, and ankle dorsiflexion ROM. In addition, postural sway in the open and closed eye conditions was not significantly different in both the LSG and HSG for both the less affected and affected limbs. Similarly, posturography indicators in the open and closed eye conditions were not significantly different in both the LSG and HSG for both the less affected and affected limbs. However, ankle joint proprioception in terms of repositioning error angle was significantly (p≤0.05) better in the LSG compared to the HSG (p=0.01). There was also a significant relationship between TUG scores and balance confidence in the HSG(r=-0.55, p=0.04) CONCLUSIONS Our data suggests that several aspects of balance needs to be considered in the assessment and rehabilitation of post-stroke patients and there is a need to monitor entire patterns of activities to support wider engagement in rehabilitation activities. One of the main criteria for economic development is the size of the public debt and its dynamics. The article considers the impact of public debt on the financial security of Ukraine. The views of scientists on the essence of public debt and financial security of the state are substantiated. An analysis of the dynamics and structure of public debt of Ukraine for 2014-2019. It is proved that one of the main criteria for economic development is the size of public debt and its dynamics. State budget deficit, attracting and using loans to cover it have led to the formation and significant growth of public debt in Ukraine. The volume of public debt indicates an increase in the debt security of the state, which is a component of financial security. Therefore, the issue of the impact of public debt on the financial security of Ukraine is becoming increasingly relevant. The constant growth and large amounts of debt make it necessary to study it, which will have a positive impact on economic processes that will ensure the stability of the financial system and enhance its security.

Today, regional integration and globalization have added new dimensions to the problems of violence, religious extremism and terrorism that attract a lot of attention in the academic community of many counties. A polyconfessional and polyethnic state, Kazakhstan, where various trends of world religions are inevitably present, is especially aware of the problem of religious extremism. In these conditions, interconfessional relations as a guarantor of internal and external stability in our republic is one of its most important problems. This article presents the aspects related to the religious environment and threats of religious extremism in Kazakhstan and outlines feasible solutions.
