Aims: Today, we have technology to break up a ureter stone in ureter as well as in renal pelvis during ureterorenoscopic procedures. In the past, when this option was not available, the surgeons improved several techniques and antiretropulsion devices in order not to let the stone migrate through renal pelvis. However, we still do not know whether it is advantageous to dust a stone in ureter where it is impacted or in a wider area such as renal pelvis. This study was carried out to clarify whether it is advantageous to breaking an upper ureter stone up where it is enclaved or in a wider area such as renal pelvis. Study Design: The data of 134 patients who underwent semirigid ureterorenoscopy (srURS) due to single and primary upper ureteral stones were included in our study and analyzed retrospectively. The patients were divided into two groups according to the development of spontaneous push-up during surgery (Group 1: non-push-up group, Group 2: push-up group). Results: Laboratory findings were changed significantly in both groups before and after surgery. However, this change was not significant between the groups. Operation times were statistically similar in both groups in contrast with the literature. Stone-free rates were significantly higher in srURS than in flexible ureterorenoscopy (fURS) (p<0,05). Complication rates were also found similar in this study. Conclusion: The application of srURS after fixing an upper ureter stone at its location using a Stone Cone® results in higher stone-free rates than pushing it back in order to dust it in renal pelvis. We recommend srURS supported by an antiretropulsion method as a treatment for upper ureteral stones.

Introduction: In this study we compare the success rates and complication rates of shock wave lithotripsy (SWL), laparoscopic, and ureteroscopic approaches for large (between 1 and 2 cm) proximal ureteral stones.Methods: In total, 151 patients with ureteral stones between 1 and 2 cm in diameter were randomized into 3 groups (52 SWL, 51 laparoscopy and 48 retrograde intrarenal surgery [RIRS]). The groups were compared for stone size, success rates, and complication rates using the modified Clavien grading system.Results: Stone burden of the groups were similar (p = 0.36). The success rates were 96%, 81% and 79%, respectively in the laparoscopy, SWL, and ureteroscopy groups. The success rate in laparoscopy group was significantly higher (p < 0.05). When these groups were compared for complication rates, RIRS seemed to bethe group with the lowest complication rates (4.11%) (p < 0.05). SWL and laparoscopy seem to have similar rates of complication (7.06% and 7.86%, respectively, p = 0.12).Interpretation: To our knowledge, this is the first study to compare the results of laparoscopy, SWL and RIRS in ureteral stones. Our results showed that in management of patients with upper ureteral stones between 1 and 2 cm, laparoscopy is the most successful method based on its stone-free rates and acceptable complication rates. However, the limitations of our study are lack of hospital stay and cost-effectiveness data. Also, studies conducted on larger populations should support our findings. When a less invasive method is the only choice, SWL and flexible ureterorenoscopy methods have similar success rates. RIRS, however, has a lower complication rate than the other approaches.

ABSTRACTIntroduction: The objective of a basketball game is to insert the ball into the opponent's basket and keep our basket alone so as not to concede the ball. This becomes important because it determines points to improve the ability and maximize points in the game so this needs to be trained. Free throw shots can be done using one hand or two hands. Whereas a free throw is a shot given to a player for committing an offense. This shot is done in the exact position behind the line of free shots in accordance with the rules. Purpose:This study aims to compare training model 1 (beef concept with push up and jogging) and training model 2 (beef concept with dumble and jogging) to improve the accuracy of free throw shooting.This research was conducted on 40 students of SMP Negeri 1 Kuta Selatan who were chosen in simple random which has fulfilled inclusion and exclusion criteria. The sample was divided into two groups so that each group was 20 people and given different treatment for six weeks. Group 1 was given model 1 training and Group 2 was given a model 2 training. Methods:The data obtained were analyzed by paired t- test to find the difference between before and after training in both groups and t-independent test to find the difference of shooting free throw accuracy between groups both before and after the training. The meaning limit is 0.05. The research design used is experimental design Randomized Pre and Post test Group Design. According to Pocock's (2008) theory, each group consisted of 20 groups of 1 and group 2. All groups were given preliminary tests, between treatment one and two treatment were given concurrent training, then each treatment was observed.Results:The result showed that the accuracy of free throw shooting between groups after training was 3.85 ± 1.09 in group 1 and in group 2 was 2.90 ± 1.12. The average accuracy of free throw shooting in training model 1 was higher than with model 2 training which was statistically significantly different with p = 0,010 (p <0,05).Conclution:From the results of this study it is concluded that the training of model 1 further improves the accuracy of free throw shooting rather than training model 2, Suggestion: in practice it is suggested to use training model 1 to improve the accuracy of free throw shooting. <b><i>Introduction:</i></b> The primary aim of this study is to compare the efficacy and safety of retrograde intrarenal surgery (RIRS) among the children and adults with similar stone burdens and locations. Also, the secondary aim of this study is to identify the factors affecting the stone-free rates (SFRs) of RIRS. <b><i>Methods:</i></b> We retrospectively compared perioperative results, SFRs, and complication rates (CRs) between pediatric (group 1, <i>n</i>: 55) and adult (group 2, <i>n</i>: 220) age groups diagnosed with kidney stones and treated by flexible ureterorenoscopy using the same instruments. Furthermore, multivariate analysis was performed to determine the factors affecting the SFR. <b><i>Results:</i></b> A total of 275 patients (pediatric group: 55; adult group: 220) were included in this study. The mean age of pediatric and adult groups was 7.2 ± 5.3 and 43.9 ± 15.1, respectively. The mean stone size was 13.9 ± 6.6 mm in the pediatric group compared to 14.8 ± 6.7 mm in the adult group (<i>p</i> = 0.35). Stone localizations and number were similar. JJ stenting for passive dilatation and use of UAS were higher in the pediatric group (<i>p</i> = 0.002; <i>p</i> = 0.017). However, postoperative double pigtail stenting rate was higher in the adult group (<i>p</i> &#x3c; 0.001). Total CR was 13.8% and mostly Clavien I–II, and no difference was observed between the 2 groups (<i>p</i> = 0.541). The SFRs between the groups were similar (pediatric group: 81.8%; adult group: 78.2%; <i>p</i> = 0.554). On multivariate analysis, stone size (<i>p</i> &#x3c; 0.001) and lower calyx stone (<i>p</i> &#x3c; 0.001) were the negative predictive factors for SFR. <b><i>Conclusion:</i></b> There are small technical differences between pediatric patients and adult patients in our study, but RIRS in children is as safe and efficient as it is in adults.

Background: We aimed prospectively investigate the laboratory and electrocardiographic parameters (hearth rate, QRS, QT, QTc, Tpe, Tpe/QTc, arrhythmia prevalance) in patients with graves disease before and after antithyroid therapy. Methods: 71 patients (48 female, 23 male), age between 18-50 (mean±SD: 36.48±12.20 ) with GD were included into the study. Patients treated with antithyroid therapy (thionamids and/or surgical therapy) to maintain euthyroid status. Patients were examined in terms of electrocardiographic parameters before and after the treatment. Results: Mean TSH, free thyroxin (fT4) and tri-iodothyrionine (fT3) levels of all patients were 0.005±0.21, 3.27± 1.81, 11.42±7.44, respectively. While 9 patients (group 2) underwent surgical therapy, had suspicious of malignant nodule or large goiter and unresponsiveness to medical treatment; the other patients (n=62, group 1) were treated with medical therapy. Patients with surgical therapy had more increased serum fT4 (p=0.045), anti-thyroglobulin value (p=0.018) and more severe graves orbitopathy (n=0.051) before treatment when compared to medical therapy group. Baseline Tpe duration and baseline Tpe/QTc ratio and frequency of supraventricular ectopic beats were found to be significantly higher in group 2 when compared to group 1 (p=0.00, p=0.005). Otherwise baseline mean heart rate, QRS duration, QTc values of both groups were similar. Although the patients became their euthyroid status, group 2 patients had still suffered from more sustained supraventricular ectopics beats than group 1. Conclusion: Distinct from medical treatment group, surgical treatment group with euthyroidism at least 3 months had still suffered from an arrhythmia (Tpe, Tpe/QTc, supraventricular and ventricular ectopic beats).

BACKGROUND: The inability of athletes to train or the decrease in the intensity and frequency of training may cause athletes to lose performance. Particularly in view of the current COVID-19 pandemic, maintaining strength outside the normal framework provides an advantage to athletes for the next competitions. OBJECTIVE: To compare the CrossFit Barbara which can be applied easily at home during the off-season or some situations such as the epidemic limitation to classic resistance training methods used to maintain the strength performance of national kickboxers. METHODS: Forty-three national kickboxers, CrossFit (CF, n= 22), and resistance training (RT, n= 21), participated in this study. While CF performed 20 pull-ups, 30 push-ups, 40 sit-ups, and 50 squat exercises, RT performed bench press, lat pull down, leg press, biceps curl, and triceps extension exercises twice per week for six weeks. Before and after the six weeks, the following variables were measured; body mass (BM) and body fat percentage (FP), VO2max, bench press (BP), squat (SQ), leg strength (LS), hand grip strength (HGS), pull-up, push-up and counter movement jump (CMJ). RESULTS: BP (p< 0.001, F= 41.125, ηp2= 0.501), SQ (p< 0.001, F= 26.604, ηp2= 0.394), LS (p< 0.001, F= 15.234, ηp2= 0.271), push-up (p< 0.001, F= 31.978, ηp2= 0.438) and pull-up (p< 0.001, F= 24.410, ηp2= 0.373) values changed significantly in group-time interaction between CF and RT groups, while there was no significant difference for the BM (p= 0.198, F= 1.715, ηp2= 0.040), Fat (p= 0.265, F= 1.279, ηp2= 0.030), HGS (p= 0.665, F= 0.190, ηp2= 0.005, CMJ (p= 0.054, F= 3.946, ηp2= 0.088) and VO2max (p=0.747, F= 0.106, ηp2= 0.003). Furthermore, according to the before and after study values, BP, SQ, LS, and CMJ decreased significantly (p< 0.05) while BM, FP, HGS, VO2max, pull-up and push-up variables did not in the CF (p> 0.05). In the RT, the pull-up and push-up variables decreased significantly (p< 0.05) while there was no significant difference for BP, SQ, LS, HGS, VO2max, body mass, body fat percentage and CMJ (p> 0.05). CONCLUSION: CF Barbara workout was more effective in maintaining strength endurance performances, and RT in maintaining maximum strength performances. According to the individual performance needs of athletes, reasonable training method can be used to prevent performance decrement in the strength domain.

Abstract Background In this research the biomechanical properties of a bone model was examined. Porcine ribs are used as experimental model. The objective of this research was to investigate and compare the biomechanical properties of the bone model before and after implant placement. Methods The bone samples were divided in three groups, Group 1 where ALL-ON-FOUR protocol was used during pre-drilling and placing the implants, Group 2 where ALL-ON-FOUR protocol was used during pre-drilling, and implants were not placed, and Group 3 consisting of intact bones served as a control group. Static and dynamic loading was applied for examining the model samples. Kruskal–Wallis statistical test and as a post-hoc test Mann–Whitney U test was performed to analyze experimental results. Results According to the results of the static loading, there was no significant difference between the implanted and original ribs, however, the toughness values of the bones decreased largely on account of predrilling the bones. The analysis of dynamic fatigue measurements by Kruskal–Wallis test showed significant differences between the intact and predrilled bones. Conclusion The pre-drilled bone was much weaker in both static and dynamic tests than the natural or implanted specimens. According to the results of the dynamic tests and after a certain loading cycle the implanted samples behaved the same way as the control samples, which suggests that implantation have stabilized the skeletal bone structure.

Patients exposed to pollutants are more likely to suffer from allergic rhinitis and may benefit from antioxidant treatment. Our study determined if patients diagnosed with grass-induced allergic rhinitis could benefit from broccoli sprout extract (BSE) supplementation. In total, 47 patients were confirmed with grass-induced allergic rhinitis and randomized to one of four groups: group 1 (nasal steroid spray + BSE), group 2 (nasal steroid spray + placebo tablet), group 3 (saline nasal spray + BSE) and group 4 (saline nasal spray + placebo tablet). Peak Nasal Inspiratory Flow (PNIF), Total Nasal Symptoms Scores (TNSS) and nasal mucus cytokine levels were analyzed in samples collected before and after the 3-week intervention. Comparing before and after the intervention, PNIF improved significantly when comparing Groups 1 and 2, vs. placebo, at various time points (p ≤ 0.05 at 5, 15, 60 and 240 min) following nasal challenge, while TNSS was only statistically significant at 5 (p = 0.03), 15 (p = 0.057) and 30 (p = 0.05) minutes. There were no statistically significant differences in various cytokine markers before and after the intervention. Combining nasal corticosteroid with BSE led to the most significant improvement in objective measures.

In commercial bovine in vitro fertilization (IVF) companies, there is a continuous need to improve results. Efforts to maximize in vitro embryo production have included modifications in the use of sperm separation gradients. The development of commercially available sperm centrifugation gradients represents a new possibility of increasing the number of viable sperm that can be obtained from low concentration (fresh or frozen, sexed or unsexed) semen samples in order to improve the efficiency of the IVF system to make embryo production as efficient as possible. The objective of this study was to compare two different separation gradients, as follows: Group 1: Percoll (Sigma, St. Louis, MO, USA), in 45% and 90% gradients; Group 2: EquiPure (Nidacon, Gathenburg, Sweden), in top and bottom layers. Before and after separation, sperm were evaluated at 200× magnification for total motility, and then stained to assess viability at 400× with fast-green/eosin stain (Sigma). Sperm separation was performed using frozen/thawed semen from one bull. Semen was separated by centrifugation at 200g for 30 min in both density gradients. Results obtained from Groups 1 and 2 were compared by chi-square test. Sperm separation with Percoll yielded lower numbers of sperm (average sperm concentration after separation of 92 × 106, vs. 159 × 106 sperm/mL for EquiPure; P < 0.05) but resulted in higher motility (60% vs. 39%, respectively; P < 0.05) of separated sperm. Rates of live sperm cells were not significantly different between groups (69.5% vs. 70%, respectively; P > 0.1). These results indicate that the commercial separation medium EquiPure may be associated with higher sperm concentration levels but with lowered sperm motility when compared to Percoll for bovine sperm separation. However, Equipure provided similar percentages of live sperm when compared to Percoll, which is currently used in our laboratory.

Purpose.We retrospectively reviewed medical records of glaucoma patients to investigate how switching medications may affect intraocular pressure (IOP) management. Three concomitant medications were changed to two medications: one combination drop and one single-action drop. Associated adverse effects were also examined.Subjects and Methods.A total of 112 patients with primary open-angle glaucoma or ocular hypertension were examined. All patients were concomitantly using a prostaglandin (PG) analog, aβ-blocker, and a carbonic anhydrate inhibitor (CAI). Fifty-five patients began using latanoprost (PG analog)/timolol (β-blocker) fixed-combination (LTFC) drops and a CAI (group 1), and 57 patients began using dorzolamide (CAI)/timolol fixed-combination (DTFC) drops and a PG analog (group 2). The IOP was measured every 6 months for 2 years following medication changes. Changes in visual field mean deviation (MD) and medication discontinuations were also examined.Results.There were no significant differences in IOP or MD values before and after medication changes in either group. The proportion of medication discontinuations, uncontrolled IOP, and adverse reactions was similar in both groups.Conclusion.Switching patients from multiple single-action medications to combination medications was not associated with changes in IOP, visual field testing results, or adverse event frequency.
