O Sistema Único de Saúde garante o direito universal de acesso, de forma integral e equânime, e para tanto exige a necessidade de controle e fiscalização de seus recursos. Neste contexto, o Sistema Nacional de Auditoria torna-se crucial, através da atuação de seus componentes, com a emissão de seus relatórios que servem como lastro para a orientação gestora. Pensando nisso, questiona-se: Qual reflexo os relatórios das auditorias trazem sobre a situação da gestão em saúde no Estado do Rio Grande do Norte? O presente estudo tem como objetivo descrever as auditorias realizadas no Estado do Rio Grande do Norte, com base na análise de seus resultados através dos relatórios. Foi elaborado por meio de uma pesquisa documental com abordagem descritiva, e teve como base a análise dos relatórios das auditorias realizadas no Estado do Rio Grande do Norte, concluídas pelo Sistema Nacional de Auditoria do SUS – SNA. Para o desenvolvimento do estudo, foi elaborado um banco de dados e sua análise foi desenvolvida de modo quantitativo descritivo, com auxílio de programa estatístico. A pesquisa teve como resultado um total de 314 auditorias. Foi constatada a prevalência para as demandas do SNA,com destaque ao Componente Federal, concentração na 7ª região do estado e na Atenção Básica. O estudo evidenciou a importância da auditoria na efetivação dos propósitos impostos pelo SUS, demonstrou o valor do relatório da auditoria, cujas constatações visam apontar a melhoria de determinados fatores necessários para o funcionamento pleno do SUS.Palavras-chave: Auditoria médica. The present study aims to define as audits carried out in the state of Rio Grande do Norte based on the analysis of its results through reports. This was elaborated through a documentary research with a descriptive approach, based on an analysis of the reports of the audits carried out in the state of Rio Grande do Norte, concluded by the National Audit System of UHS-NAS; for the development of the study, a database was elaborated, and its analysis was developed in a descriptive quantitative way with the aid of the statistical program. A survey resulted in a total of 314 auditoriums. The prevalence of NAS demands was found, with emphasis on the Federal Component, concentrated in the 7th region of the state and in Primary Care. The study showed the importance of auditing in the enforcement of taxes imposed by UHS, if the value of the audit report made findings aim to improve the definition of the enforcement of taxes for the operation of UHS.Keywords: Medical audit. Treatment of phenylketonuria (PKU) includes the use of a metabolic formula which should be provided free of charge by the Unified Health System (SUS). This retrospective, observational study sought to characterize judicial channels to obtain PKU treatment in Rio Grande do Sul (RS), Brazil. Lawsuits filed between 2001- 2010 and having as beneficiaries PKU patients requesting treatment for the disease were included. Of 20 lawsuits filed, corresponding to 16.8% of RS patients with PKU, 19 were retrieved for analysis. Of these, only two sought to obtain therapies other than metabolic formula. In all the other 17 cases, prior treatment requests had been granted by the State Department of Health. Defendants included the State (n = 19), the Union (n = 1), and municipalities (n = 4). In 18/19 cases, the courts ruled in favor of the plaintiffs. Violation of the right to health and discontinuation of State-provided treatment were the main reasons for judicial recourse. Unlike other genetic diseases, patients with PKU seek legal remedy to obtain a product already covered by the national pharmaceutical assistance policy, suggesting that management failures are a driving factor for judicialization in Brazil.

Abstract The objective was to analyze the functioning of the referral and counter-referral system for integral care in the Healthcare Network. Qualitative study performed through interviews with 66 participants - managers, workers and users of a town of the state of Bahia, Brazil, and upon approval by the Ethics Committee, opinion number nº 334.737. The material was analyzed using the Content Analysis proposed by Bardin, finished in 2014. Various conceptions of referral and counter-referral were identified, as follows: referral of users, user’s broader view; non-fragmented care; and integral care. For the users, the difficulties and facilities in the flows are concentrated in the Regulation and Scheduling Center and Family Health Units. It is concluded that, to enable the establishment of the network in an integral way, it is necessary to identify important strategies provided by the Unified Health System (SUS) and strengthen these strategies, as well as to identify any drawbacks to remedy them.

ABSTRACT Objective: To analyze the influences of the Vivências e Estágios na Realidade do Sistema Único de Saúde (Brazilian Unified Health System), in the training and performance of nurses. Method: A qualitative study was carried out with 14 nurses who participated in the VER-SUS project in Rio Grande do Sul State. The data were collected through a semi-structured interview technique. The data were analyzed, interpreted and discussed, through the technique of Content Analysis. Results: The categories “VER-SUS Influences on Nursing Training for SUS” and “VER-SUS Contributions for Nurses Performance in the SUS” were highlighted. Final considerations: It was possible to identify the influences of the VER-SUS project in the training and performance of the nursing professionals, pointing out the revision of the Pedagogical Projects of Course and the preparation of the teachers, as a way to carry out the training with emphasis in the Brazilian Unified Health System.

AbstractThe outbreak of COVID 19 has been provoking several problems to the health system around the world. One of the concerning is the crash of the health system due to the increasing demand suddenly. To avoid it, knowing the total number and daily new cases is crucial. In this study, we fitted curves growth models using a Bayesian approach. We extracted information obtained from some countries to build the prior distribution of the model. The total number of cases of the COVID 19 in the state of Goias was analyzed. Results from analysis indicated that the date of the outbreak peak is between 51 and 68 days after the beginning. Moreover, the total number of cases is around 3180 cases. The analysis did not take into consideration possibles changes in government control measures. We hope this study can provide some valuable information to public health management.

Introduction: to describe the epidemiological profile of patients cared for by the pediatric cardiac surgery service at a referral center in the State of Santa Catarina, Brazil, during five years. Methods: retrospective, longitudinal cohort-type study of cardiovascular surgeries performed in children aged less than 18 years in a Unified Health System-affiliated hospital, from 3rd August 2013 to 2nd August 2018. Patients' demographic, clinical, and healthcare characteristics were assessed. The complexity and severity of the surgeries were assessed using the RACHS-1 method. Negative outcomes were deaths up to thirty days after surgery and complications (mediastinitis, renal failure, and systemic infection). Results: 1,191 procedures were performed, with a predominance of male (53.5%) infants (52.8%) from more near regions of the state (33.6%) and prevalence of RACHS-1 category 3 (36.5%). Most patients had undergone cardiopulmonary bypass (53.7%), mechanical ventilation (94.8%), and only one surgical procedure (56.3%). The most frequent pathology was ventricular septal defect. (15.7%). The median length of hospital stay was eighteen days, six days at intensive care units, and three days of preoperative waiting time. Lethality up to 30 days after surgery was 6%, and complications affected 22.4% of patients. The main factor associated with negative outcomes was represented by the RACHS-1 score with higher categories. Conclusion: This Unified Health System pediatric cardiac surgery service was relevant due to the magnitude of the procedures performed, and the healthcare complexity involved. The set of outcomes presented in this study indicated good healthcare quality in comparison to national standards.

A Constituição Federal de 1.988 traz em seu conteúdo princípios constitucionais de extrema importância, tais como, o princípio da isonomia ou da igualdade e o princípio da dignidade da pessoa humana.  O primeiro especifica, segundo a máxima Aristotélica, o tratamento igualitário a todos em situação de igualdade e desigual ao desiguais na medida de suas desigualdades. Já o segundo, tem caráter subjetivo e pressupõe as condições mínimas que garantam a um indivíduo uma vida digna. Com caráter protetivo, a Constituição Federal visa assegurar aos cidadãos um rol de direitos fundamentais, entre eles a saúde. Tratam-se de prestações positivas atribuídas ao Estado que, em suma, deve assegurar o acesso à saúde a todos os cidadãos de maneira universal, integral e igualitária em todo o território nacional. Assim, para dar atendimento ao disposto no texto constitucional políticas públicas são desenvolvidas seguindo os parâmetros definidos pela Lei Orgânica da Saúde (LOS), Lei n° 8080/90, pela Lei n° 8.142/90. Temos também a Lei n° 12.401/11 e a Lei nº 9.782/99, sendo que esta criou a Agência Nacional de Vigilância Sanitária e aquela criou a Comissão Nacional de Incorporação de Tecnologias no Sistema Único de Saúde. Inobstante a todo aparato legal criado para dar cumprimento ao dever do Estado em disponibilizar serviços de saúde à população, evidencia-se que na prática tal prestação por parte deste é ineficiente levando à crescente judicialização da saúde, atribuindo-se ao Poder Judiciário o dever de, mesmo que indiretamente, interferir em questões administrativas e orçamentárias de atribuição do Poder Executivo, no que diz respeito às questões de saúde.Palavras-chave: JUDICIALIZATION OF HEALTH: THE OBLIGATION OF THE STATE TO PROVIDE MEDICATION / TREATMENT WITHOUT FORECASTING IN THE PUBLIC, HIGH COST OR PROHIBITED EFFECTIVENESS SYSTEM ABSTRACT: The Federal Constitution of 1988 brings in its content constitutional principles of extreme importance, such as the principle of equality or equality and the principle of the dignity of the human person. The first specifies, according to the Aristotelian maxim, equal treatment to all in a situation of equality and unequal to the unequal in the measure of their inequalities. The second is subjective and presupposes the minimum conditions that guarantee an individual a dignified life. With a protective character, the Federal Constitution aims to assure citizens a list of fundamental rights, among them health. These are positive benefits attributed to the State, which, in short, must ensure access to health for all citizens in a universal, integral and egalitarian manner throughout the national territory. Thus, in order to comply with the provisions of the constitutional text, public policies are developed following the parameters defined by the Organic Law of Health (LOS), Law n 8080/90, by Law n. 8.142 / 90. We also have Law No. 12,401 / 11 and Law No. 9,782 / 99, which created the National Health Surveillance Agency and created the National Commission for the Incorporation of Technologies into the Single Health System. compliance with the State's duty to provide health services to the population, it is evident that in practice this provision on the part of the latter is inefficient leading to a growing judicialization of health, being attributed to the Judiciary the duty, even indirectly, to interfere in administrative and budgetary matters of attribution of the Executive Power, with regard to health issues.Keywords: Constitutional principles. Abstract Background: The judicialization of healthcare for acquiring medication is not an isolated occurrence in Brazil, it may also be observed in other Latin American countries, regardless of the existence of a universal health system, in Brazil, the Unified Health System. The search for justice, in terms of medications, has existed ever since the high demand for treating the Acquired Immunodeficiency Syndrome, while currently, the protagonist is cancer. Since pharmaceutical assistance is the area within Unified Health System, which is responsible for maintaining access to medications, the aim of this article is to describe the profile of judicialization in the pharmaceutical assistance of a state in Northeastern Brazil of economic significance, according to the characteristics of the lawsuits, regarding: the plaintiff filing the lawsuit; the medical and health information; the expenditure on acquiring the requested medications; and the relevance of spending on antineoplastic drugs. Methods: This was a cross-sectional, descriptive study based on a set of lawsuits filed between 2016-2018 at the Litigation Center in the State Health Department of Pernambuco.Results: A total of 2,947 lawsuits were analyzed that contained at least one requested medication, with a predominance of males (51.7%); and in which 49.8% of the requests originated in the Unified Health System, and were primarily from patients in the Metropolitan region of the state capital. The most frequent ICDs for cancer were C61, C71 and C50. The median overall expense of the lawsuits was U$1,734.94. Considering only antineoplastic drugs, expenditure exceeded U$7,500 per lawsuit over the three years, given that the median unit price of antineoplastic drugs is approximately U$65.00 compared to U$4.00 for non-antineoplastic drugs. Conclusion: The present study is of relevance as a matter of public health, and how the profile of judicialization behaves as a tool for managing and improving decision-making in times of economic austerity.
