Despite the fact that significant success has been achieved in the treatment of patients with infective endocarditis (IE), difficulties in clinical practice arise in the diagnosis of this disease. The article analyzes a clinical case of successful treatment of a patient with acute ischemia of the lower extremity due to thromboembolic syndrome in IE, the diagnosis of which was initially regarded as COVID-19. The clinical case occurred during the coronavirus infection (COVID-19) pandemic, and all patients who initially complained of fever were regarded as potentially infected, which, with insufficient examination, masked the clinical picture of infective endocarditis.

The analysis of literature and exceptional clinical case of combination of inverse acne and infiltrative-productive stage of rosacea are presented. The historical facts, ethiopathogenesis, comorbidity, treatment, clinical picture and diagnostics with use of both standard and invasive methods of research (histological method) are described. Successful treatment of combination of inverse acne and an infiltrative-productive stage of rosacea with antibiotics and actinolyzat is reported.

The article presents a clinical case of a patient of the otorhinolaryngology department of the Samara Clinical Hospital No. 8 who had a new coronavirus infection. Complications were manifested by primary thrombosis of the orbital veins, facial veins, followed by the spread of the process into the pterygoid plexus, veins of the nasal cavity and paranasal sinuses. The study analyzes the dynamics of the clinical picture and compares it with the results of the postmortem examination. Vascular thrombosis of the nasal cavity and paranasal sinuses during X-ray examination can manifest itself as a decrease in pneumatization of the sinuses with possible minor exudation due to ischemic changes in the mucous membrane. Thrombosis developing with underlying COVID-19-associated coagulopathy can have different initial localizations and are a formidable complication with a high mortality rate. Patients who have undergone COVID-19 need to control the coagulogram not only during the illness, but also during the period of convalescence and early periods after recovery.

Listeriosis has a variety of clinical manifestations from febrile gastroenteritis to bacteremia, sepsis, meningoencephalitis and infective endocarditis with valvular heart disease. In this report, we present a rare case of successful treatment of infective endocarditis of the mitral valve due to Listeria monocytogenes after previous open-heart surgery.

The aim of the study was to determine the frequency and clinical and laboratory features of acute gastroenteritis (OGE) in adult residents of Novosibirsk hospitalized with a new coronavirus infection (COVID-19) in 2020.Materials and methods. We examined 3581 patients aged 18 to 96 years with a diagnosis of “New coronavirus infection”, confirmed by the detection of SARS-CoV-2 RNA in the mucus from the oropharynx by PCR, hospitalized in the State Medical Institution NSO GICB No. 1 in 2020. Along with the generally accepted diagnostic methods, feces were examined by PCR in 50 patients with gastrointestinal syndrome to detect SARS-CoV-2 RNA using a set of original specific primers.Results. Of the 3,581 hospitalized adult patients with COVID-19, 74% of cases were diagnosed with bilateral polysegmental pneumonia, and 26% of cases were diagnosed with acute respiratory infection. Manifestations of OGE were observed in 6.8% (n=243) of patients. In the group of 50 COVID-19 patients, in whom the disease began with symptoms of OGE, it came to the fore in the clinical picture of the disease in 20% of cases, and SARS-Co- V2 RNA in the feces was detected in 62% of patients.Conclusion The established frequency of OGE in COVID-19 in adults shows the need to develop and implement in clinical practice test systems (PCR of SARS-CoV-2 RNA in feces) for the diagnosis of the intestinal form of a new coronavirus infection.

The peculiarities of the clinical picture are given, which allows differentiating them between themselves and with dental pain syndromes. The main causes of facial pain are revealed, and the place of dental problems and diseases among them is shown. The main directions of prosopalgia treatment are given. A clinical case of facial pain in a patient with complicated caries is presented.

Clinical case of skleroderma, associated with borrelia infection is presented. 64 year old patient complained about lesions on the skin of the trunk, in the form of consolidation with change of colour and shine. 5 years ago on a skin in the left calcaneal area stuck tick that has been independently removed, was found. First occurrences of dark stains on a trunk was noticed a year ago. On the basis of the anamnesis, clinical picture and histologic research the diagnosis of localized skleroderma was established. Successful treatment was performed.

Today, the problem of pancreonecrosis remains in the leading positions in urgent surgery. The incidence of this pathology increases every year. This article presents the results of the analysis of the use of the drug "Roncoleukin" in clinical practice for pancreonecrosis. The study was conducted to evaluate the effectiveness of this drug.
