Background. Primary tumoral calcinosis is an orphan disease. There are few data in the literature on the incidence of this disease, as well as clinical recommendations for treatment. Clinical case. This report presents the case of an 11.5-year-old boy with primary tumoral calcinosis and equinus deformity of the foot. The patient had multiple foci of the subcutaneal calcification, cannot walk, experienced fatigue, and had high fever and equinus deformity of the left foot. Immunological and genetic studies were performed, but any specific mutations were not found. After the diagnosis was verified and interleukin-1 inhibitor therapy was prescribed, there was a significant positive trend observed in the patient: a significant improvement in the patients general condition, a decrease in the number of calcinates, and a reduction in inflammation. Calcification of the Achilles tendon and gastrocnemius muscle was the cause of the deformity of the left foot. Discussion. Significant improvement was achieved during treatment: the boy started walking, fatigue was decreased, no new calcificates were formed, and inflammation was under the control. Using an inhibitor of interleukin-1 as a permanent therapy of primary tumoral calcification allowed performsurgical treatment without complications from an operation site, as well as a relapse of deformity. Conclusion. The clinical case presented here demonstrated the application of an interdisciplinary approach to the treatment of an extremely rare disease.

Рецензируемая книга Александра  Михайловича  Терехина «Сумасшествия в  музыкальном театре: опера, балет», изданная в  2020 году, уникальна во  многих отношениях. Во-первых, интерес вызывает личность самого автора. Профессиональная и  творческая деятельность Терехина связана с  медициной и  музыкой. Врач-психиатр с  сорокалетним стажем, Терехин более четверти века работал в Мариинском театре в качестве артиста миманса. Во-вторых, тема сумасшествия (безумия) еще не  становилась самостоятельным предметом изучения в российских музыковедческих исследованиях. В-третьих, оригинальность книги составляют медицинские заключения. В центре внимания врача — тексты оперных и балетных либретто, на основе которых автор раскрывает причины помешательства и  ставит различные диагнозы персонажам музыкального театра. Рассматривая конкретные клинические случаи, Терехин прибегает к профессиональным медицинским терминам. Среди диагнозов отметим такие, как реактивный параноид у Лючии («Лючия ди Ламмермур» Г. Доницетти), шизофрения у Мельника («Русалка» А. С. Даргомыжского, интоксикационный (гашишный) психоз у Солора («Баядерка» Л. Минкуса). Книга А. М. Терехина, не претендуя на всеохватность освещения темы сумасшествия (безумия), открывает музыковедам новые грани междисциплинарного подхода в изучении оперного и балетного репертуара. Alexander Mikhailovich Terekhin’s peer-reviewed book Madness in Musical Theater: Opera, Ballet, published in 2020, is unique in many ways. Firstly, the author’s personality is of interest. Terekhin’s professional and creative activity is connected with medicine and music. A psychiatrist with forty years of experience, Terekhin worked for more than a quarter of a century at the Mariinsky Theater as a mimance artist. Secondly, the theme of madness (insanity) has not yet become an independent subject of study in Russian musicological studies. Thirdly, the originality of the book is based on medical reports. The doctor focuses on the texts of opera and ballet librettos, on the basis of which the author reveals the causes of insanity and makes various diagnoses to the characters of the musical theater. Considering a specific clinical case, Terekhin resorts to professional medical terms. Among some diagnoses we can mention: reactive paranoid in Lucia (Lucia di Lammermoor by Donizetti), schizophrenia in Melnik (Rusalka by Dargomyzhsky, intoxicational (hashish) psychosis in Solor (La Bayadere by Minkus). Without claiming to cover the topic of insanity (insanity) comprehensively the book by Terekhin, opens up new facets of the interdisciplinary approach in studying opera and ballet repertoire to musicologists.

Dryness of the mucous membranes (in mouth, nose, genitals in women) and skin are the most common complaints of patients with Sjogren’s disease — a multisystem immune‑mediated disorder that causes anxiety and concern associated with the difficulties of initial diagnosis. The authors present a clinical case of a patient with primary Sjogren’s syndrome, a large number of symptoms of digestive diseases, complaints of dryness of mucous membranes, in particular the oral cavity. The diagnosis was confirmed by allied specialists such as ophthalmologist, gynecologist, rheumatologist, dentist. Detailed dental examination revealed the main dental complaints (constant dry mouth, galitos, changes in taste), clinical condition (dryness of the red border of the lips, swelling of the oral mucosa, changes in the relief of the gums), decreased secretory activity and significant reduction of unstimulated salivation with a moderate level of stimulated salivation. It is noted that a significant decrease in sialometry (0.01 ml per minute) and functional activity of the small salivary glands (up to 18) confirms the presence of true xerostomia. Micro‑crystallization of saliva revealed disorganization of irregularly shaped structures, as well as a significant number of organic inclusions, which indicates a violation of homeostasis in the oral cavity. Given the results of ultrasound examination of the large salivary glands and the absence of enlargement and swelling of the glands during clinical examination, we can assume that the destructive damage to the glands is not so pronounced, evacuation function of the gland is not reduced, but the amount of secretion is significantly reduced. To provide medical aid to patients with Sjogren’s syndrome, the accessibility of the qualified consultation is important and required, with conduction of comprehensive examinations by allied specialists, including a dentist.

The article presents a clinical case of intestinal epithelial dysplasia (tufting enteropathy), a rare disease which relates to congenital diarrheas, has a particular clinical presentation and is life-threating. The diagnosis was confirmed by histological examination and electronic microscopy of mucous membrane of small intestine. Additional evidence is substantiated for the existence of prenatal markers of intestinal epithelial dysplasia represented as polyhydramnios and intrauterine dilatation of fetal intestinal loops. These symptoms are considered to be a clinical hallmark of sodium and chloride diarrheas and are described in single cases in enteropathy called a microvillus inclusion disease.

Riedel's thyroiditis is a rare disease characterized by chronic fibrosis. Clinical performance of this disease is dense stony goiter, which can poorly be displaced during palpation. The overgrowth of the goiter can lead to the development of compression syndrome. To diagnose we need to made fine needle biopsy and made the final diagnose according to its results or according to the morphological description of the postoperative material. An important step in the diagnosis of Riedel's thyroiditis is the determination of serum IgG and IgG4 to exclude an IgG4-associated disease. Treatment of this disease includes drug therapy, which is based on glucocorticosteroids administration or surgical treatment when develops compression syndrome. This article presents a clinical case of a patient with Riedel's thyroiditis; the main complaints were associated with the growth of goiter and the development of compression syndrome. In this regard, patient underwent surgery on the thyroid gland, and after this we get final diagnose. Due to feeling unwell, drug therapy with glucocorticosteroids was prescribed, against the background of which we noted a positive trend.

The Sahel, a semi-arid climatic zone with highly seasonal and erratic rainfall, experienced severe droughts in the 1970s and 1980s. Based on remote sensing vegetation indices since early 1980, a clear greening trend is found, which can be attributed to the recovery of contemporaneous precipitation. Here, we present an analysis using long-term leaf area index (LAI), precipitation, and sea surface temperature (SST) records to investigate their trends and relationships. LAI and precipitation show a significant positive trend between 1982 and 2016, at 1.72 × 10 −3 yr −1 (p < 0.01) and 4.63 mm yr−1 (p < 0.01), respectively. However, a piecewise linear regression approach indicates that the trends in both LAI and precipitation are not continuous throughout the 35 year period. In fact, both the greening and wetting of the Sahel have been leveled off (pause of rapid growth) since about 1999. The trends of LAI and precipitation between 1982 and 1999 and 1999–2016 are 4.25 × 10 − 3 yr −1 to − 0.27 × 10 −3 yr −1, and 9.72 mm yr −1 to 2.17 mm yr −1, respectively. These declines in trends are further investigated using an SST index, which is composed of the SSTs of the Mediterranean Sea, the subtropical North Atlantic, and the global tropical oceans. Causality analysis based on information flow theory affirms this precipitation stabilization between 2003 and 2014. Our results highlight that both the greening and the wetting of the Sahel have been leveled off, a feature that was previously hidden in the apparent long-lasting greening and wetting records since the extreme low values in the 1980s.

Introduction Leiomyomas are rare benign tumors that can occur in the male urinary tract. We present a unique case of leiomyoma of the vas deferens. Case Description We present the clinical case of a 69-year-old patient with a suspected bulk close to the right epididymis, which turned out to be a leiomyoma of the vas deferens. To our knowledge, it is the fourth case in literature. Conclusions A proper identification and the knowledge of this pathology, even if it is a very unusual event, is necessary to avoid a surgical over treatment and preserve the testicle, by removing only the tumor.

&lt;p&gt;Since November 1995, we have monitored the volume transport of Faroe Bank Channel overflow (FBC-overflow) and since 2001, the bottom temperature at the sill of the channel. The FBC-overflow is the coldest and densest overflow component and contributes approximately one third of the total overflow. Together with water that it entrains en route, it is therefore one of the main sources for North Atlantic Deep Water and the lower limb of the Atlantic Meridional Overturning Circulation (AMOC). In spite of reported AMOC weakening, the FBC-overflow has shown no signs of reduced volume transport. In contrast, a linear trend analysis indicated a weak (but non-significant) positive trend in volume transport of +5% from 1996 to 2018. The bottom water at the sill of the channel is the coldest component of the FBC-overflow and the densest overflow component overall. Since high-quality monitoring of the bottom water temperature began in summer 2001, the bottom water has warmed by approximately 0.2 &amp;#176;C with most of the warming occurring in two periods: 2004-2007 and 2015-2019. During the period, salinity has also been changing and the combined temperature/salinity effect on the density of the FBC-overflow will be discussed.&lt;/p&gt;

Abstract Background: Clinical trial evidence has demonstrated greater anemia-related fatigue at lower hemoglobin (Hb) levels in ESA-treated patients with chemotherapy-induced anemia (CIA). To understand these patient-reported outcomes (PROs), during ESA treatment in a real-world setting, data were analyzed from the Dosing and Outcomes Study of Erythropoiesis-Stimulating Therapies (D.O.S.E.) Registry, an ongoing, prospective, observational study. Methods: Patients included for analysis were required to have a baseline Hb prior to ESA administration and at least two ESA administrations. PROs were measured using the Functional Assessment of Cancer Therapy: Fatigue subscale (FACT-F), a 13-question measure of anemia-associated fatigue with lower scores indicating greater fatigue in patients with CIA. Patients were to have completed up to three FACT-F questionnaires within seven days of a post-baseline Hb level. If multiple Hb values were identified, the Hb value closest to the FACT-F assessment was reported. The association between FACT-F scores and Hb levels was tested in a repeated-measures analysis. Mean FACT-F scores were calculated for specified Hb ranges. Results: 740 CIA patients from 63 sites were identified between 12/03 and 7/08. 1088 concurrent FACT-F and Hb level assessments were available during ESA treatment. Mean FACT-F scores were lower when patients had lower Hb levels (table). The association between FACT-F scores and Hb levels showed a significant positive trend (p&lt;.0001). 25 Background: Cancer cachexia affects 60-80% of advanced cancer patients and leads to weight loss, worsening functional status, and increased mortality. Thus, specialized clinics are needed to measure nutritional and functional trajectories over time in this population. Recently, the McGill University Health Centre has developed a systematic, standardized and interdisciplinary approach for the profiling and management of cancer cachexia. Methods: Patients with advanced cancer were recruited and categorized as cachetic. The following information was collected at baseline and at three follow-up visits: hand grip strength measured by Jamar dynamometer, the Edmonton Symptom Assessment System (ESAS) and the abridged Patient Generated Subjective Global Assessment (aPG-SGA) questionnaires. Results: Fifty-nine patients were eligible for this study yet only 24 completed 3 follow-ups and were included. There were 15 men and 9 women, mean age 65.7 years. Five patients had locally advanced disease and 19 had metastatic disease. The table contains results for handgrip strength, ESAS and aPG-SGA scores. Conclusions: Following baseline assessment and treatment, patients were noted to have a significant improvement in the aPG-SGA total score. Handgrip strength was maintained throughout the follow up period. ESAS appetite and fatigue exhibited a positive trend but did not achieve significance over time. This work suggests the benefits of an interdisciplinary cachexia clinic for the maintenance of nutritional and functional status in patients with cancer cachexia as well as possible improvements in quality of life.
