High risk groups experience anemia, namely adolescents, especially young women, this is due to the need for iron absorption peaking at the age of 14-15 years while young men, experiencing peak absorption of iron one or two years later. The main risk factors for iron deficiency anemia are low iron intake, poor absorption of iron, and periods of life when the need for iron is high. Food fortification is the most effective way to prevent iron deficiency. One of the foods that can prevent iron is green beans. Consuming 2 cups of green beans every day means consuming 50% of the daily requirement of iron, 18 mg and can increase hemoglobin levels for 2 weeks. The purpose of this PKM is to provide an alternative picture of food in increasing hemoglobin levels in adolescents. The method used in this PKM is screening anemia through examination of hemoglobin and health education in young women in SMK 1 Tegal City. PKM results show that there is an increase in adolescent knowledge about anemia, the number of adolescent girls experiencing anemia is 35% of the total respondents. Iron is the main nutrient that plays an important role in the synthesis of hemoglobin so that the lack of iron intake obtained from food causes hemoglobin levels to decrease.

Increasing obesity is a major global health concern while at the same time iron-deficiency anemia remains common worldwide. Although these two conditions represent opposite ends of the spectrum of over- and under-nutrition, they appear to be linked: overweight individuals are at higher risk of iron deficiency than normal-weight individuals. Potential explanations for this association include dilutional hypoferremia, poor dietary iron intake, increased iron requirements, and/or impaired iron absorption in obese individuals. Recent evidence suggests obesity-related inflammation may play a central role through its regulation of hepcidin. Hepcidin levels are higher in obese individuals and are linked to subclinical inflammation; this may reduce iron absorption and blunt the effects of iron fortification. Thus, low iron status in overweight individuals may result from a combination of nutritional (reduced absorption) and functional (increased sequestration) iron deficiency. In this review, we focus on subclinical inflammation in obesity, and its effect on hepcidin levels, as the most plausible explanation for the link between iron deficiency and obesity.

Iron deficiency anemia is the most common type of Anemia in Indonesia . The iron needs of adolescent girls aged 13-29 years are much higher than boys of the same age. Women iron intake is not only used to support growth, but is also used to replace the iron that is lost through the blood when menstruation every month. Therefore women are at risk for anemia. Anemia in adolescents will have a long impact on himself and the baby who will be born later. This activity is an effort to reduce the incidence of anemia in adolescents. In addition to knowing hemoglobin levels, this activity was also given information about anemia in adolescents and the consumption of iron supplements. The results of examination of hemoglobin levels in 16 female students (11.2%) out of a total of 142 female students were detected as having anemia. The results of this activity also showed an increase in the average hemoglobin level for these students from 10, 9 gr / dl to 11.7 gr / dl. In addition, the results of the examination after the intervention also showed that 8 female students who had anemia at the beginning then within one month had normal Hemoglobin levels (> 12 gr / dl).Keyword: adolescent anemia, early detection, iron supplementation

Background: Anemia in adolescent girls possess greatest burden on their health as it leads to poor cognitive development, poor reproductive health and if not treated may lead to intrauterine growth retardation in their developing fetus. The prevalence of anemia among adolescent is 27% in developing country and 6% in developed country. The Government of India has launched National Anemia Control Programme, to prevent and control anaemia in children and adolescents. But the problem is still persisting because of many issues like - poor palatability, intolerance to the drug used, poor absorption of the drugs used, constipation, discoloration of stool, unpleasant odor and taste etc. result in poor compliance and therefore non adherence to the therapy which finally results in poor improvement. By looking at the magnitude of the problem, an Ayurveda drug Vajravatakmandura has been selected for the study and the present study was undertaken to evaluate the efficacy of the trial drug in iron deficiency anemia in adolescent girls. Material and methods: 100 subjects satisfying inclusion and exclusion criteria were selected and were randomly divided in two groups. In group A, the trial drug Vajravatakmaņdura was administered and in group B, IFA tablets were given for two month of duration with follow up at every fortnight. Result: Both the trial and control groups showed extremely significant result over subjective parameters. Trial drug Vajravatakmaņdura in group A was found more effective over the subjective parameters - weakness, palpitation, pallor and loss of appetite with % gain of 39.77%, 39.39%, 39.17%, 38.09% respectively. Statistical analysis between before and after treatment findings of objective parameters, extremely significant (P less than 0.0001) improvement was found in both groups in all parameters, except for RBC count in group A, which was significant. No adverse effects were reported during entire period of study by any of the patients in trial group treated with trial drug. Conclusion: The trial drug “Vajravatakmandura” is effective, safe and palatable for the management of iron deficiency anemia in adolescent girls.

The effects in rats, of age, iron-deficiency anemia, and ascorbic acid, citrate, fluoride, and ethylenediaminetetraacetate (EDTA) on enteric radioiron transport were studied in vitro by an everted gut-sac technique. Sacs from young animals transported more than those from older ones. Proximal jejunal sacs from anemic animals transported more than similar sacs from nonanemic rats, but the reverse effect appeared in sacs formed from proximal duodenum. When added to media containing ascorbic acid or citrate, fluoride depressed transport as did anaerobic incubation in the presence of ascorbic acid. Anaerobic incubation in the presence of EDTA appeared to permit elevated transport. Ascorbic acid, citrate, and EDTA all enhanced the level of Fe59 appearing in serosal media. These results appear to agree with previously established in vivo phenomena and tend to validate the in vitro method as one of promise for further studies of factors affecting iron absorption and of the mechanism of iron absorption.

One of the nutritional problems that often occurs among adolescents is iron deficiency anemia. Especially for adolescent girls who have a higher risk of anemia than adolescent boys. Based on data from Riskesdas 2018, the prevalence of anemia in adolescents aged 15-24 years is 32%, meaning that there are still anemia problems in Indonesia that have not been resolved. The purpose of this community dedication activity is to provide education about iron deficiency anemia for adolescent girls using leaflet media. The target of this community dedication activity is adolescent girls aged 12-19 years in the JABODETABEK area. The community dedication activity is carried out online through the WhatsApp group due to the COVID-19 pandemic. There are 31 adolescent girls who participated in this community dedication activity. This community dedication activity consists of three stages including pretest, education with leaflets, and posttest. The amount and types of pretest and posttest questions are the same. The amount of questions given is 15 questions. The correct answer is given a score of 10 and the wrong one is given a score of 0. The level of knowledge of adolescent girls is categorized as 3 groups, which is less if the correct answer is <60%, sufficient if the correct answer is 60-80%, and good if the correct answer is >80%. Most of the adolescent girls are in the age range of 17-19 years (54.8%). The results showed that there was an increase in knowledge about iron deficiency anemia in adolescent girls. The results of the Wilcoxon signed rank test showed significantly different pretest and posttest results (p-value = 0.000), so it can be concluded that providing education using leaflets can help increase knowledge of iron deficiency anemia for adolescent girls.Salah satu masalah gizi yang sering terjadi pada kalangan remaja yaitu anemia defisiensi besi. Khususnya bagi remaja putri yang memiliki risiko lebih tinggi mengalami anemia dibandingkan remaja putra. Berdasarkan data Riskesdas 2018, prevalensi anemia pada remaja berusia 15-24 tahun sebesar 32%, artinya masih terdapat permasalahan anemia di Indonesia yang belum teratasi. Tujuan dari pengabdian masyarakat ini adalah untuk memberikan edukasi mengenai anemia defisiensi besi bagi remaja putri dengan media leaflet. Sasaran dari kegiatan pengabdian masyarakat ini adalah remaja putri berusia 12-19 tahun di wilayah JABODETABEK. Kegiatan pengabdian masyarakat dilakukan secara online melalui grup whatsappkarena kondisi pandemi covid-19. Total remaja putri yang mengikuti kegiatan pengabdian masyarakat berjumlah 31 orang. Kegiatan pengabdian masyarakat ini terdiri dari tiga tahapan diantaranya pretest, edukasi dengan leaflet, dan posttest.  Jumlah dan jenis pertanyaan pretest dan posttest sama. Jumlah pertanyaan yang diberikan adalah 15 soal. Jawaban yang benar diberikan nilai 10 dan yang salah diberikan nilai 0. Tingkat pengetahuan remaja putri dikelompokkan menjadi 3 diantaranya kurang jika jawaban yang benar < 60%, cukup jika jawaban yang benar 60-80%, dan baik jika jawaban yang benar > 80%. Sebagian besar remaja putri berada pada rentang usia 17-19 tahun (54,8%). Hasil menunjukkan bahwa terdapat peningkatan pengetahuan mengenai anemia defisiensi besi pada remaja putri. Hasil uji Wilcoxon signed rank test menunjukkan hasil pretest dan posttest yang berbeda nyata (p-value = 0,000), sehingga dapat disimpulkan bahwa pemberian edukasi menggunakan leaflet dapat membantu meningkatkan pengetahuan anemia defisiensi besi bagi remaja putri.

In its recent statement on iron,1 the Committee on Nutrition emphasized the value of iron-fortified, proprietary milk formulas for the prevention of iron-deficiency anemia of infancy. Despite this recommendation, the most recent marketing information available to the Committee shows that more than 70% of the proprietary formulas currently prescribed by physicians do not contain added iron. The reasons for continuing routine use of formulas not fortified with iron are not entirely clear. One reason may be that some physicians still believe iron additives increase the incidence of feeding problems or gastrointestinal disturbances. There is no documented evidence that this is a significant problem. The Committee strongly recommends when proprietary formulas are prescribed that iron-supplemented formulas be used routinely as the standard–that is, that this be the rule rather than the exception. There seems to be little justification for continued general use of proprietary formulas not fortified with iron. The Committee is fully aware that only a small percentage of American infants are fed proprietary formulas after 6 months of age. Fluid whole milk (available in bottle or carton ) or evaporated milk, both of which contain only trace amounts of iron, are substituted at the time of greatest iron need and highest prevalence of iron-deficiency anemia. The infant's diet is usually deficient in iron, unless other foods are carefully selected to insure adequate iron intake. Since the major dietary component during infancy is milk, two courses of action should be taken: (1) Pediatricians and other health professionals should engage in a program of public education to convince American mothers to provide their infants with a source of dietary iron.
