The worldwide concern about oncological morbidity is due to the steady increase in the detection rate of malignant neoplasms, which has a tendency to increase. More than 7 million people die from oncology every year in the world. The system of organizing the provision of medical care to the population with oncological diseases in the Russian Federation is currently undergoing a stage of improvement. New programs are aimed at early diagnosis and prevention of malignant neoplasms. Each region has the right to create and implement routing taking into account the territorial characteristics and the existing material and technical base and personnel. The routing system, created for a specific region, determines the order of treatment of patients and the functions of all links in the system of providing medical care to patients with oncopathology. An optimized system for routing patients according to the "Oncology" profile for patients with malignant neoplasms of the maxillofacial region should be structured in accordance with the regional healthcare system. A complex system of interaction, an information system capable of containing and processing a set of information, a trained interested staff dictate the need for making management decisions in the direction of optimizing the oncological service. Management decisions and technologies should be focused on improving the quality and speed of patient admission, providing highly qualified medical care using modern equipment and algorithms. The staff, in turn, determines the basis of the health care system, in connection with which it is necessary to continue education, identify and search for specialists that a specific organization needs, on which the functioning of the entire service for providing medical care to patients with oncopathology of the maxillofacial region depends. In general, the optimization of the system of providing medical care to patients with malignant neoplasms of the maxillofacial region dictates the need to combine organizational measures and a clinical approach. Unambiguously, the multi-component system of integration of various directions and departments in the process of organizing an oncological service predetermines the need to use modern management technologies and the use of all available resources in order to improve the quality of medical care.

As we know, neoplasms are characterized by high prevalence. The purpose of this study was to analyse the dynamics of cancers in the Nizhny Novgorod Region in 2017–2019. Materials and methods. The object of this research were the residents of the city of Nizhny Novgorod and the Nizhny Novgorod Region who sought medical help at Polyclinic No. 3 of the Volga District Medical Centre, Russia’s Federal Medical-Biological Agency (Nizhny Novgorod). Computed tomography was used to diagnose neoplasms. Results. In 2017, 50,470 diseases were identified, of which 2548 were neoplasms (599 malignant, and the rest benign). In 2018, 57,012 diseases were diagnosed, of which 2710 neoplasms, 639 being malignant. In 2019, the statistics was rather similar: a total of 55,110 diseases, of which 2705 neoplasms (661 malignant). Thus, the total number of diagnosed diseases, neoplasms included, was somewhat less in 2017, which can be explained by certain improvement in diagnostics and reporting, as well as a more active participation of the population in screening in 2018–2019. The absolute number of malignant neoplasms, although not much, still grows annually, and they are typically diagnosed after the age of 55–60 years. In general, the most often attacked is the digestive system, which is indicated by a steady increase in the number of new diagnoses by 1.5–2 % each year. Fortunately, there is also a positive trend, i.e. the number of malignant tumours of the excretory system, skin and female repr oductive system is falling. OrlovaAcute myeloid leukemia accounts for about 4.2% of all forms of cancer in children and adolescents. Significant progress has been made in the treatment of oncohematological pathology in recent years, especially in the treatment of lymphoblastic leukemia (survival rate is up to 80%). However, unsolved problems remain in many diseases, in particular in myeloid leukemia, in which the survival rate is 38-41%. In this regard, the most critical issue is the provision of care for children with an unfavorable prognosis with malignant neoplasms. У лікуванні онкогематологічної патології в останні роки досягнуто значних успіхів, особливо при лікуванні лімфобластної лейкемії (виживаємість становить до 80%). Однак невирішеними питаннями залишаються при багатьох захворюваннях, зокрема при мієлоїдній лейкемії, при яких виживаємість становить 38-41%. У зв'язку з цим особливо гостро стоїть питання з наданням допомоги дітям з несприятливим прогнозом із злоякісними новоутвореннями. Background.Analysis of medical care delivery for children with cancer in armed conflict is highly important because the high-tech treatment in this context is extraordinary difficult and challenging task. Objective. Our aim was to analyze the morbidity and mortality rates in children with malignant tumors, to assess the pediatric patient capacity and medical service density in the Donetsk People’s Republic.Methods.The ecological study was conducted where the units of analysis were represented by the aggregated data of the Republican Cancer Registry on the number of primary and secondary patients with malignant and benign tumors, the deceased patients in the DNR in 2014–2017, pediatric patient capacity, and medical service density.Results.The number of pediatric patient capacity for children with cancer was 10 (0.27 per 10,000 children aged 0–17), pediatric patient capacity for children with hematological disorders — 40 (1.37 per 10,000 children aged 0–17). The treatment of children with cancer was performed by 5 healthcare providers: 1 pediatric oncologist (0.02 per 10,000 children aged 0–17), 3 hematologists (0.08 per 10,000 pediatric population aged 0–17), and 1 practitioner who did not have a specialist certificate in oncology. Morbidity rate for malignant neoplasms from 2014 to 2017 decreased by 25% (in 2014 — 9.6 per 10,000 children aged 0–17; in 2017 — 7.2). In the morbidity structure, the incidence proportion of hemoblastoses was 68.4%, brain tumors — 2.6%, other solid tumors — 29%. The death rate due to malignant neoplasms decreased by 37% (in 2014 — 2.7; in 2017 — 1.7).Conclusion.Low levels of the incidence rate and pattern of morbidity indicate defects in the identification and recording of patients. This explains the performance of the bed: low average bed occupancy per year and low turnover. For a reliable analysis of mortality statistical data is not available: in 2014–2015 only the number of in-hospital deceased patients is presented. Limited data is due to the lack of reliable patient catamnesis which is explained by the high rate of population migration.

The reclamation complex of the Nonchernozem zone of Russia has been under the influence of a combination of unfavourable factors for a long time. The problems of scientific and staff support today limit the development of the complex and create risks for the successful implementation of the Federal Target Program "Development of Land Reclamation". In order to analyze the situation and find the right solutions, a study was carried out using the method of expert assessments. Twenty-four highly qualified specialists took part in it: 2 Academicians of the Russian Academy of Sciences, 4 Corresponding Members of the Russian Academy of Sciences, 10 Doctors of Sciences and 9 Candidates of Sciences. To preserve and build up the agro-resource potential within the framework of the Federal Target Program "Development of Land Reclamation" in the next decade, it is necessary to develop 300-400 thousand hectares of shrubby fallow, to carry out major repairs of drainage systems on an area of up to 300 thousand hectares and their reconstruction on an area of up to 100 thousand hectares, carry out chemical reclamation on an area of up to 5 million hectares on average per year. The need for qualified personnel to solve these problems will amount to 18–20 thousand professional workers and up to 5 thousand specialists, including 1.0 thousand people in research and development, 1.5 thousand people in exploration and design sphere, 2.5 thousand people in the construction and operational sphere. The fundamental importance belongs to the scientific support of the innovative reclamation complex, the renewal of which should be carried out on the principles of resource and energy conservation, nature likeness, informational support and digitalization of management processes.

The issues of financing in a medical organization are one of the most relevant areas of healthcare in the Russian Federation. Providing departments with the necessary financial resources to provide effective and accessible medical care to the population is important for both the administration and doctors and nurses. In some regions of the Volga Federal District, the departments are combined (Penza, Saransk), that is, all types of medical care are provided to both adults and children in one place, which leads to a number of problems associated with the irrational use of medicines. One of these is the department of Maxillofacial Surgery of the N.N. Burdenko National Clinical Hospital, on the basis of which the disadvantages of using drugs for parenteral use are considered. The purpose of this work is an economic analysis of the effectiveness of the use of solutions for parenteral use in children on the basis of an adult medical organization. The materials of the study were data from patients ' medical histories, prescribing lists, invoices from the pharmacy organization, and data on the cost of medicines. The results of the work are an analysis of the existing problems of combined type departments related to drug provision, a number of medicines for parenteral use that are not available to order, but are included in the standards of treatment of children with injuries of the maxillofacial region. The ABC - analysis of expenses under the item "medicines" is calculated, the data of the VEN – analysis is presented, which includes vital, necessary and secondary medicines for the treatment of children with injuries of the maxillofacial region. An algorithm for the selection of effective and rational therapy has been developed, using the example of antibacterial agents for parenteral administration.

The article describes the activities of the Pokrovsky monastery-hospital in Kiev, created by the efforts of the Grand Duchess Alexandra Petrovna (1838-1900), who took the monastic vows in 1889 under the name Anastasia. The monastery became widely known in the late XIX - early XX century for the highly skilled medical aid provided to the population since its inception up to the events of 1917. It was a unique complex of medical settings located in the same area and equipped with modern medical equipment comparable with the many World’s clinics of that time. In this monastery-hospital, pilgrims with different diseases who came at Pokrovsky monastery in Kiev were provided the required qualified, affordable medical care. Thanks to the Grand Duchess Alexandra Petrovna, her attentive attitude to all the latest innovations, novel treatment methods were developed and introduced to the work of monastery medical settings. In addition, she was able to create a special atmosphere and goodwill towards the sick. But, being engaged in devising new methods of providing medical care to patients, she was able to create a special and inviting atmosphere with respect to the patient. With the ever-increasing circle of her organizational and economic activities, being in charge of the monastery-hospital problems of great importance, Grand Duchess reserved the right to nurse her patients. She worked by vocation, at the behest of the soul. Her example of selfless labor in the monastery medical settings where monastic sisters held the medical staff responsibilities for the patients care was important for all of the staff. Her assistants - Sisters of Charity - she fostered by her own example of selfless aid to a sick person. This favorable work atmosphere was very effective helping cure the sick.

The most common cause of neurological disability in childhood is cerebral palsy. The objectives of the treatment of children with cerebral palsy are the acquisition and maintenance of new motor skills; prevention of the development of contractures and other secondary orthopedic complications; decrease in the level of pain; improving the level of childcare. At the same time, the provision of treatment and rehabilitation assistance to children with cerebral palsy requires an integrated, systematic approach. The article describes the route of rehabilitation of children with cerebral palsy in the Chuvash Republic from the moment of diagnosis until reaching the age of 18. The study was carried out on the basis of an analysis of data from the regional register of children with cerebral palsy at all stages of treatment and rehabilitation (habilitation). The first stage includes the identification of risk groups for cerebral palsy, conservative, surgical specialized medical care. The register of children with cerebral palsy made it possible to keep records of patients at the stages of treatment and rehabilitation with an assessment of the results. At the second stage, high-tech conservative and surgical treatment is carried out. After operations, patients are sent for rehabilitation to a 24-hour hospital or to a Rehabilitation Center for Children (over 3 years, 1.5 thousand children were treated, 25.7% after operations). The third stage is organized at the outpatient clinic level and in specialized sanatoriums. For 20142018 the coverage of children with cerebral palsy with conservative treatment increased 1.9 times, with botulinum therapy 2.6 times, the proportion of those who received surgery decreased to 22.4%. 92.7% of patients of the Register are covered by conservative treatment. 33.3% of operated children were referred for sanatorium-resort treatment. The control section of the rehabilitation results showed an increase in the proportion of children with improved gait quality, the ability to stand independently, walk (with support), and an increase in the average level of physical activity. A multi-level system of interagency interaction in the provision of treatment and rehabilitation assistance to children with cerebral palsy in Chuvash Republic made it possible to ensure a sufficient amount of basic and availability of highly qualified medical care, effective management and control of the stages of rehabilitation.
