Background. Obesity is associated with the risk of spontaneous preterm birth. Hexoprenaline is the effective and most widely used tocolytic agent, possessing however a significant number of side effects. The effect of hexoprenaline tocolysis on heart rate variability, lipid spectrum and glycaemia level in obese pregnant women remain unexplored.Aim of the research. To study the effect of tocolytic therapy with hexoprenaline on heart rate variability, lipid spectrum and glycemic level in obese pregnant women.Materials and methods. The study included two groups of pregnant women with threatened preterm labor who received tocolysis with hexoprenaline. One group consisted of 68 obese patients, the other – 72 non-obese pregnant women (control group). Patients underwent Holter monitoring. Fasting serum glucose and lipids spectrum were measured before starting tocolytic therapy and after 24 hours of tocolysis.Results. In obese pregnant women with hexoprenaline infusion, the heart rate, the 24-hours number of supraventricular extrasystoles and ventricular extrasystoles during the day are significantly higher. Frequency domain parameters, very low frequency during the day, low frequency at night and 24-hours high frequency were significantly decreased than in control group. After a day of tocolysis in obese pregnant women, the level of total cholesterol, low density lipoproteins, triglycerides, and glucose significantly increases when compared with the results before therapy. For patients in the control group treated with hexoprenaline, only the concentration of high-density lipoproteins is increased.Conclusion. Obesity in pregnant women receiving hexoprenaline tocolysis is associated with low heart rate variability and an increase in the number of cardiac arrhythmias, as well as lipid disorders and an increase in glucose level.

By means of daily ECG monitoring on the device “Kardiоtechnika-06” (Inkart Ltd, St. Petersburg, Russia) the authors studied the character of sympathetic and parasympathetic interaction as correlation of the percentage inputs of high frequency (HF), low frequency (LF), and very low frequency (VLF) spectrum components of the heart rate variability in 73 pregnant women aged of 27.7+6.1 years in perinatal period (before and after the delivery). The control group consisted of 30 healthy non-pregnant women aged 32.8+7.6 years on average. It was revealed that the daily dynamics of sympathetic and parasympathetic interaction in pregnant and the puerperas in perinatal period was similar to the control group of women provided that the delivery was without complications. The dynamics was characterized by the increase of parasympathetic effects at the night time and the increase of sympathetic ones at the day time. The time period between 6.00 a.m. and 9.00 a.m. is especially important for the functional condition of the vegetative nervous system in the perinatal period of both pregnant women and puerperas as far as sharp changes in the sympathetic and parasympathetic interaction were noted. Throughout 24 hours period in the perinatal period, the sympathetic and parasympathetic interaction is characterized by considerable changes in the puerperas in comparison with the pregnant women. The revealed feature of daily dynamics of heart rate variability in the pregnant women and the puerperas provide the physiological conditions to optimize the cycle of sleeping and wakeful states in the women in perinatal period.

Aim.To summarize all relevant trials and critically evaluate the effect of acupuncture on heart rate variability (HRV).Method.This was a systematic review with meta-analysis. Keyword search was conducted in 7 databases for randomized controlled trials (RCTs). Data extraction and risk of bias were done.Results.Fourteen included studies showed a decreasing effect of acupuncture on low frequency (LF) and low frequency to high frequency ratio (LF/HF ratio) of HRV for nonhealthy subjects and on normalized low frequency (LF norm) for healthy subjects. The overall effect was in favour of the sham/control group for high frequency (HF) in nonhealthy subjects and for normalized high frequency (HF norm) in healthy subjects. Significant decreasing effect on HF and LF/HF ratio of HRV when acupuncture was performed on ST36 among healthy subjects and PC6 among both healthy and nonhealthy subjects, respectively.Discussion.This study partially supports the possible effect of acupuncture in modulating the LF of HRV in both healthy and nonhealthy subjects, while previous review reported that acupuncture did not have any convincing effect on HRV in healthy subjects. More published work is needed in this area to determine if HRV can be an indicator of the therapeutic effect of acupuncture.

Alterations in the autonomic cardiovascular control have been implicated to play an important etiologic role in preeclampsia. The present study was designed to evaluate autonomic functions in preeclamptic pregnant women and compare the values with normotensive pregnant and healthy nonpregnant controls. Assessment of autonomic functions was done by cardiovascular reflex tests and by analysis of heart rate variability (HRV). Cardiovascular reflex tests included deep breathing test (DBT) and lying to standing test (LST). HRV was analyzed in both time and frequency domain for quantifying the tone of autonomic nervous system to the heart. The time domain measures included standard deviation of normal R-R intervals (SDNN) and square root of mean squared differences of successive R-R intervals (RMSSD). In the frequency domain we measured total power (TP), high frequency (HF) power, low frequency (LF) power, and LF/HF ratio. Cardiovascular reflex tests showed a significant parasympathetic deficit in preeclamptic women. Among parameters of HRV, preeclamptic group had lower values of SDNN, RMSSD, TP, HF, and LF (ms2) and higher value of LF in normalised units along with high LF/HF ratio compared to normotensive pregnant and nonpregnant controls. Furthermore, normotensive pregnant women had lower values of SDNN, TP, and LF component in both absolute power and normalised units compared to nonpregnant females. The results confirm that normal pregnancy is associated with autonomic disturbances which get exaggerated in the state of preeclampsia.

Background: Stroke, a medical condition that causes physical disability and mental health problems, impacts negatively on quality of life. Post-stroke rehabilitation is critical to restoring quality of life in these patients. Objectives: This study was designed to evaluate the effect of a mind–body interactive qigong intervention on the physical and mental aspects of quality of life, considering bio-physiological and mental covariates in subacute stroke inpatients. Methods: A randomized controlled trial with repeated measures design was used. A total of 68 participants were recruited from the medical and rehabilitation wards at a teaching hospital in northern Taiwan and then randomly assigned either to the Chan-Chuang qigong group, which received standard care plus a 10-day mind–body interactive exercise program, or to the control group, which received standard care only. Data were collected using the National Institutes of Health Stroke Scale, Hospital Anxiety and Depression Scale, Short Form-12, stroke-related neurologic deficit, muscular strength, heart rate variability and fatigue at three time points: pre-intervention, halfway through the intervention (day 5) and on the final day of the intervention (day 10). Results: The results of the mixed-effect model analysis showed that the qigong group had a significantly higher quality of life score at day 10 ( p<0.05) than the control group. Among the covariates, neurologic deficit ( p=0.04), muscle strength ( p=0.04), low frequency to high frequency ratio ( p=0.02) and anxiety ( p=0.04) were significantly associated with changes in quality of life. Conversely, heart rate, heart rate variability (standard deviation of normal-to-normal intervals, low frequency and high frequency), fatigue and depression were not significantly associated with change in quality of life ( p >0.05). Conclusions: This study supports the potential benefits of a 10-day mind–body interactive exercise (Chan-Chuang qigong) program for subacute stroke inpatients and provides information that may be useful in planning adjunctive rehabilitative care for stroke inpatients.

Sympathovagal modulation during immersion in a virtual environment is an important influence on human performance of a task. The aim of this study is to investigate sympathovagal modulation using heart rate variability and perceived exertion during exercise in a virtual reality (VR) environment. Sixteen young healthy volunteers were tested while using a stationary bicycle and maintained at an anaerobic threshold intensity for exercise sessions of approximately 10 min duration. Four randomized viewing alternatives were provided including desktop monitor, projector, head mounted device (HMD), and no simulation display. The “no simulation display” served as the control group. A quick ramp exercise test was conducted and maintained at an anaerobic threshold intensity for each session to evaluate power spectral density and rating of perceived exertion (RPE). The sampled heart rate data were rearranged by cubic spline interpolation into power spectrums spanning the ultra-low frequency (ULF) to high frequency (HF) range. A significant difference was found between the no-display and projector groups for total power (TP) and very low frequency (VLF) components. In particular, there was a significant difference when comparing HMD and no-display exercise RPE curves within 6 min of cycling and at the termination of the exercise. A significant difference was also achieved in projector vs. control group comparison at the termination of the exercise. Our results indicate that the use of HMD and the projected VR during cycling can reduce the TP and VLF power spectral density through a proposed decrease in the renin-angiotensin system, with the implication that this humoral effect may enable anaerobic exercise for longer durations through a reduction in sympathetic tone and subsequent increased blood flow to the muscles.

According to modern data, the cardiovascular system is a kind of indicator of the organism's adaptive activity, and the structure of the heart rhythm carries information about the development of adaptive reactions in response to irritating factors of the external and internal environment.The aim of the study – to investigate the functional state of the autonomic nervous system in pregnant women who have harmful habits by evaluating and analyzing the parameters of heart rate variability in order to prevent complications of non-delivery of gestational process.Materials and Methods. The study involved 73 pregnant women, 53 of them were pregnant (the main group) who had malignant habits in history. Before studying adaptive reactions in pregnant women, a study of cardiac rhythm variability was performed in practically healthy non-pregnant women of reproductive age (25.3±2.2) years). The control group consisted of 20 pregnant women without any harmful habits. Determination of heart rate variability was performed on the basis of peripheral heart rate recording, which included measuring the sequence of RR intervals for 5 minutes, followed by mathematical analysis using the PlsMntr software product.Results and Discussion. It was established that in the main group, in the complicated failure to pass the gestational process, violations of cardiac rhythm variability were detected in 59.7 % of cases. There is an increase in the frequency of very low frequency (VL F) waves (43.5 % relative to control, p<0.05), indicating a predominance of humoral and metabolic rate of regulation of the cardiac rhythm. At the threat of premature births, a decrease in the particle (LF) is observed at 36.6 %, a decrease in the share of high-frequency waves (НF) by 30.8 %. Taking into account the results of the study, one can speak of a decrease in the tone of the sympathetic and parasympathetic nervous system, reducing the influence of the reflexive vegetative level of regulation, which, in turn, indicates the depletion of regulatory mechanisms and the lack of an adaptive protective effect of n.vagus on the heart.Conclusions. Our innovative low-invasive method of evaluating the adaptive reactions of the organism through the determination and analysis of indicators of cardiac rhythm variability in pregnant women with the existing harmful habits offers us the opportunity to observe manifestations of functional stress of regulatory systems of pregnant women, preceding the development of complication of non-delivery of gestational process.

This study compares the effects of the uptake or inhalation of 50uL Mentha piperita (MP) essential oil for 10 days on heart rate variability (HRV) and cardiopulmonary regulation during various exercise intensities. Forty-eight healthy male subjects were randomly assigned to MP uptake (MPU; n=16), MP inhalation (MPI; n=16), and control group (C; n=16). All participants were measured resting HRV, respiratory, cardiovascular, and metabolic parameters during aerobic, anaero- bic, and graded exercise tests (GXT) before and after treatment. There were significant increases in the low-frequency area (LFa; 1.8±0.1 vs 2.2±0.2 ms²), the ratio of low frequency to respiration frequency area (LFa/RFa; 0.9±0.1 vs 1.3±0.1) at resting and carbon dioxide production (VCO 2 ; 41.2±4.0 vs 49.2±6.8 mL/min -1 /kg -1 ), ventilation per minute (V E ; 80.2±4.3 vs 97.5±5.5 L/min -1 ), and respiratory rate (RR; 38.2±1.9 to 44.3±2.1 breath/min -1 ) in an anaerobic test following MPU inter- vention. In GXT, maximal carbon dioxide production (VCO 2max; 51.9±3.5 to 59.1±6.4 mL/min -1 /kg -1 ), maximal ventilation per minute (V Emax ; 126.4±6.5 to 138.4±5.4 L/min -1 ) and maximal respiratory rate (RR max ; 52.7±3.6 to 60.1±2.3 breath/min -1 ) significantly increased in MPU. The correlations of ΔLFa with ΔVCO 2max , ΔV Emax , and ΔRR max in the MPU group were signifi- cant. Continuous uptake or inhalation of 50uL MP oil for 10 days does not improve aerobic capacity and maximal exercise performance, but 10 days’ uptake of MP essential oil increased sympathetic activity at rest and may relate to respiratory regulation under high-intensity exercise.

The paper presents frequency methods for estimating the variability of intervals between individual heart beats in Electrocardiogram. This parameter is known in the scientific literature as the Heart Rate Variability and with this method it is possible to make predictions about human health. Three frequency ranges have been studied: Very Low Frequency, Low Frequency, and High Frequency. The study in this paper was based on real cardiological data obtained from 33 patients suffering from heart fibrillations and 29 healthy individuals. The investigated records are obtained through a Holter monitoring of studied individuals in real life conditions. The obtained results show significantly lower values ​​of the tested spectral parameters in the diseased individuals compared to the healthy controls. The accomplished study shows the effective applicability of the spectral methods of Heart Rate Variability analysis and the possibility of differentiation by the spectral parameters of the patients from healthy individuals.

Introduction Autonomic dysfunction has been reported as one of non-motor manifestations of both pre-symptomatic and manifest Huntington’s Disease (HD). The aim of our study was to evaluate heart rate variability (HRV) during wake and sleep in a cohort of patients with manifest HD. Methods Thirty consecutive patients with manifest HD were enrolled, 14 men and 16 women, mean age 57.3±12.2 years. All patients underwent full-night attended video-polysomnography. HRV was analyzed during wake, NREM and REM sleep, in time and frequency domain. Results were compared with a control group of healthy volunteers matched for age and sex. Results During wake HD patients presented significantly higher mean heart rate than controls (72.4±9.6 vs 58.1±7.3 bpm; p<0.001). During NREM sleep, HD patients showed higher mean heart rate (65.6±11.1 vs 48.8±4.6 bpm; p<0.001) and greater Low Frequency (LF) component of HRV (52.9±22.6 vs 35.5±17.3 n.u.; p=0.004). During REM sleep, we observed lower standard deviation of the R-R interval (SDNN) in HD subjects (3.4±2.2 vs 3.7±1.3 ms; p=0.015). Conclusion Our results showed that HD patients have higher heart rate than controls, during wake and NREM, but not during REM sleep. Among HRV variability parameters, the most relevant difference regarded the LF component, which reflects, at least partially, the ortho-sympathetic output. Our results confirm the involvement of autonomic nervous system in HD and demonstrate that it is evident during both wake and sleep.

AbstractPurpose: Previous studies have shown that the underlying pathophysiologic mechanism in children with breath holding may be generalised autonomic dysregulation. Thus, we performed cardiac rhythm and heart rate variability analyses using 24-hour Holter monitoring to evaluate the cardiac effects of autonomic dysregulation in children with breath-holding spells. Methods: We performed cardiac rhythm and heart rate analyses using 24-hour Holter monitors to evaluate the cardiac effects of autonomic dysregulation in children during a breath-holding spell. Our study group consisted of 68 children with breath-holding spells – 56 cyanotic type and 12 pallid type – and 39 healthy controls. Results: Clinical and heart rate variability results were compared between each spell type – cyanotic or pallid – and the control group; significant differences (p<0.05) in standard deviation of all NN intervals, mean of the standard deviations of all NN intervals for all 5-minute segments, percentage of differences between adjacent RR intervals >50 ms, and square root of the mean of the sum of squares of the differences between adjacent NN intervals values were found between the pallid and cyanotic groups. Conclusions: Holter monitoring for 24 hours and heart rate variability parameters, particularly in children with pallid spells, are crucial for evaluation of cardiac rhythm changes.
