Background: Unwanted pregnancies were one of the serious threats to human development around the world. This analysis identifies factors associated with unwanted pregnancies, including intrapersonal, interpersonal, and institutional health services.Objective: We analyzed the extent of unwanted pregnancies and how they predict factors related to Indonesia's unwanted pregnancies.Methods: We analyzed data from the Indonesian Demographic and Health Survey (IDHS) in 2017. Descriptive statistics on the prevalence of unwanted pregnancies and correlated factors. Multiple logistic regression was used to examine the association between intrapersonal factors, interpersonal factors, and community factors with unwanted pregnancies.Results: Participants reported that their most recent was 83.8% indicated a desired pregnancy and 16.2% were unwanted. Multivariate analysis shows the age of the woman (OR=1.37), economic status (OR=1.05), number of children (OR=1.47), contraception failure (OR=0.78), and health worker intervention (OR=1.27) was found to significantly increase the risk of unwanted pregnancy (p<0.05). The number of children is the most substantial factor in unwanted pregnancy among women childbearing in Indonesia.Conclusions: Nearly a fifth of pregnancies among vulnerable women in Indonesia are unwanted pregnancies. The national family planning program in East Kalimantan needs to achieve a larger target on modern contraceptives. Despite the fertility decline, this program still facing increasing discontinuation rates. A better understanding of the factors behind the discontinuation of a modern method would help in improving programs. This study aims to analyze the determinants of discontinuation of modern contraceptive use. This was a cross-sectional study, the dataset came from Indonesia Demographic and Health Survey 2017 of East Kalimantan Province. The sample is all couples of childbearing age between 10–49 years with marital status who have used and are still using contraception (408 samples). Descriptive analyses were used to assess the reasons for discontinuation. Multiple logistic regression was used to estimate the likelihood of discontinuation by demographic characteristics and others. The proportion of respondents who continue using modern contraceptives was 51%, against 49% discontinuation. The reasons for discontinuation were the husband’s disapproval (31%) and health problems related to side effects (26.5%). In the multivariate analysis showed maternal age, women who live in urban areas, and women with birth planning near the future will have an opportunity to discontinue in modern contraceptives. It concluded that there is still high modern contraceptive discontinuation in East Kalimantan, therefore it needed for disseminating information through entertainment-education in social media, health workers better counseling services from also better tools, and include the male participation in family planning counseling. DETERMINAN YANG BERHUBUNGAN DENGAN PUTUS PAKAI KONTRASEPSI MODERN DI KALIMANTAN TIMUR: ANALISIS LANJUT SURVEI DEMOGRAFI DAN KESEHATAN INDONESIA 2017Program keluarga berencana nasional di Kalimantan Timur perlu mencapai target yang lebih baik dalam penggunaan kontrasepsi modern. Meskipun terdapat penurunan fertilitas, namun program keluarga berencana masih menghadapi peningkatan angka putus pakai. Pemahaman yang lebih baik tentang faktor-faktor di balik putus pakai metode kontrasepsi modern akan membantu meningkatkan program. Penelitian ini bertujuan menganalisis faktor-faktor penentu putus pakai penggunaan kontrasepsi modern. Desain penelitian ini adalah cross-sectional, set data berasal dari Survei Demografi Kesehatan Indonesia 2017 untuk Provinsi Kalimantan Timur. Sampel adalah semua pasangan usia subur berusia 10–49 tahun dengan status perkawinan baik bagi yang pernah menggunakan dan masih menggunakan kontrasepsi, yaitu 408 sampel. Analisis deskriptif digunakan untuk menilai alasan putus pakai. Regresi logistik berganda digunakan untuk memperkirakan kemungkinan putus pakai berdasar atas karakteristik demografis dan lainnya. Proporsi responden yang masih terus menggunakan kontrasepsi modern adalah 51% dibanding dengan 49% putus pakai. Alasan penghentian adalah ketidaksetujuan suami (31%) dan masalah kesehatan yang berkaitan dengan efek samping (26,5%). Dalam analisis multivariat menunjukkan usia ibu, wanita yang tinggal di daerah perkotaan, dan wanita dengan perencanaan kelahiran dalam waktu dekat akan memiliki kesempatan untuk berhenti menggunakan kontrasepsi modern. Dapat disimpulkan bahwa kejadian putus pakai kontrasepsi modern masih tinggi di Kalimantan Timur, oleh karena itu diperlukan diseminasi informasi melalui entertainment-education dalam sosial media, layanan konseling dari petugas kesehatan, serta alat bantu konseling yang lebih baik dan juga keikutsertaan pria dalam proses konseling.

With the current high maternal mortality rates prevailing in Zambia, it is imperative to analyze the effect that the economic status of women has on maternal health outcomes. It is hypothesized that wealth is positively linked to better health outcomes. As a result, maternal health outcomes will improve when the economic status of women improve. Using data for women from the 2013-14 Zambia Demographic and Health Survey, bivariate and multivariate analysis was used to ascertain the impact of wealth as a proxy for economic status and other variables based on five independent outcomes (Number of antenatal care visits, timing of first antenatal care visit, tetanus toxoid vaccination, iron tablet/syrup uptake, and place of delivery) and a composite measure of maternal health. The results revealed that women’s wealth and region were the only significant factor associated with all the five independent indicators and the composite measure of maternal health. However, for the composite measure, literacy and age of the mother were also found to be associated with maternal health. The study therefore concluded that economic status is an important factor in improving maternal health outcomes in Zambia. As a result, policy should be aimed at providing support to women within the lowest wealth quintiles so as to enable them access maternal health services.

ABSTRACTInfant mortality is still a major health problem in Indonesia as 63% of deaths occur in the neonatal period. The trend of the neonatal mortality rate in Indonesia has decreased from 15 (2017) to 13 deaths per 1,000 live births (2018), but this does not meet the Sustainable Development Goals (SDGs) target. Evidence-based health interventions are based on determinants that most influences neonatal mortality. The purpose of this study was to determine the most influential determinants of neonatal mortality in Indonesia. This research is an analytic observational study that uses the 2017 Indonesian Demographic and Health Survey (IDHS) data, with a cross-sectional design. This study’s population was the last baby of married women of childbearing age (15-49 years) and was recorded in the 2017 IDHS with a sample of 15,102 babies. The outcome variable in this study was neonatal mortality. The analysis was conducted in stages, namely univariate analysis, bivariate analysis using the chi-square statistical test (X2), and multivariate analysis using logistic regression. The results showed that 85.71% of deaths occurred in the early neonatal period. The most influential determinants of neonatal mortality were male sex (adj OR = 2.91: 95% CI 1.78-4.76), low birth weight (adj OR = 12.08: 95% CI 7.38-18.86) and parity status ≥ 4 (adj OR = 2.11: 95% CI 1.22-3.61). Public health interventions aimed at reducing neonatal mortality should be linked to newborn and maternal characteristics that significantly influence Indonesia's neonatal mortality. Therefore, health services must consider adequate postpartum neonatal care that is fully integrated with medical care. Moreover, further prevention of potential factors that cause low birth weight before conception should be implemented and further control of births would optimize the family planning program. Keywords: gender, low birth weight, parity status, determinant, neonatal mortality, Indonesia

Fertility rate is on gradual decline in Pakistan like other developing countries with increased acceptance of family planning methods. Pakistan Demographic and Health Surveys revealed a slow decline from 4.1 in 2007 to 3.8 children per woman in 2013. Despite availability of nationally representative primary data, empirical researches on demographic and health determinants of fertility and fertility preferences in Pakistan are rare. The aim of this study is to assess the major differences in factors determining fertility and fertility preferences in Pakistan Demographic and Health Survey 2006- 2007 and 2012-2013. Using two data sets Pakistan Demographic and Health Survey 2006-2007 and 2012-2013, researchers conducted a comparison of findings generated in secondary analyses. Researchers studied the association of social demographics, reproductive history and infant mortality with fertility and fertility preferences among ever-married women of reproductive age (15-49). Results are indicated using binary logistic regression and multivariate analysis. Findings support the association of social demographics of women, living sons, child mortality and reproductive history with ideal number of children; desire to reproduce more and number of living children. Injection induced labour pains, C-section deliveries, fistula problem, abortions and spotting during gestation are associated significantly with lower ideal number of children. Living sons are positively associated with ideal number of children and negatively associated with desire to reproduce more. The desire to reproduce more in association with women’s age 15-29, rural place of residence and C-section deliveries has reduced over time from 2007 to 2013 in country. Complications during pregnancies and delivery are important determinants of fertility and fertility preferences among women. Further research is needed to assess the association of pregnancy and delivery problems with fertility and fertility preferences in Pakistan.

The issue of return of fertility after discontinuation of contraceptive use is one of the important studies related to the efficacy and safety of various contraceptive methods, both hormonal and non-hormonal. Indonesian Demographic and Health Survey Data from 2002-2003 revealed that 34 percent of women in childbearing age who discontinued using the contraceptive was due to pretension to have the next pregnancy. The mechanisms of action for a variety of contraceptives would be related to the varying duration of post-use fertility return. This study was aimed to see how long it takes for women in childbearing age to stop using the contraceptive until the next pregnancy occurs. This study was an observational study using secondary data from the Indonesian Demographic and Health Survey (SDKI) data from 2002-2003. The samples of this study were 5,223 women in childbearing age who discontinued using the contraceptive. Every interest in this study was the occurrence of pregnancy after stopping using the contraceptive. Determination of the samples was limited to ever pregnant women who stopped using hormonal contraceptive and IUD, which could be observed during the survey period.The results showed that the probability of  reaching 50 percent pregnancy required 7 and 6 months after the discontinuation of injection and implant use as well as 4 months after pill and IUD use. Keywords: Return of fertility, Contraceptive, Childbearing Age

Background. Appropriate complementary feeding practice is essential for growth and development of children. This study aimed to assess dietary diversity and meal frequency practice of infants and young children in Ethiopia.Methods. Data collected in the Ethiopian Demographic and Health Survey (EDHS) from December 2010 to June 2011 were used for this study. Data collected were extracted, arranged, recoded, and analyzed by using SPSS version 17. A total of 2836 children aged 6–23 months were used for final analysis. Both bivariate and multivariate analysis were done to identify predictors of feeding practices.Result. Children with adequate dietary diversity score and meal frequency were 10.8% and 44.7%, respectively. Children born from the richest households showed better dietary diversity score (OR = 0.256). Number of children whose age less than five years was important predictor of dietary diversity (OR = 0.690). Mothers who had exposure to media were more likely to give adequate meal frequency to their children (OR = 0.707).Conclusion. Dietary diversity and meal frequency practices were inadequate in Ethiopia. Wealth quintile, exposure to media, and number of children were affecting feeding practices. Improving economic status, a habit of eating together, and exposure to media are important to improve infant feeding practices in Ethiopia.

Nationally, the results of family planning services during the COVID-19 pandemic, there was a decrease in the use of contraceptives reaching. This can cause various problems including an increase in the number of unwanted pregnancies (unwanted pregnancy) and baby booms or an increase in the birth rate. This study aims to determine how the behavior of contraceptive use among couples of childbearing age during the COVID-19 pandemic at the Primary Health Care North Kuta. This research is a survey research with cross sectional analytic research design. The population in this study were all fertile age couples (PUS) in Denpasar City and the number of samples used was 365 respondents with a purposive sampling technique. The results of this study found that most respondents are currently using contraception. Analysis of bivariate data with chi square obtained variables of age, parity, family income, perceived benefits, perceived barriers, availability of service places, and husband's support. Multivariate analysis with logistic regression found that husband's support variable has the most dominant influence on contraceptive use behavior.

Provinsi Nusa Tenggara Timur (NTT) memiliki Total Fertility Rate (TFR) tertinggi (4,2 anak) dan yang terendah adalah Provinsi Yogyakarta (1,8 anak). Tujuan penelitian ini adalah mengidentifikasi faktor-faktor yang berhubungan dengan angka fertilitas total di kedua provinsi tersebut. Penelitian ini menggunakan metode kuantitatif berdasarkan sumber data sekunder berbagai survei meliputi Survei Demografi Kesehatan Indonesia (1991-2007); Survei Kesehatan Nasional dan Survei Kesehatan Rumah Tangga tahun 2003; dan Mini Survei tahun 2007 dan 2008. Metode kualitatif dilakukan dengan diskusi kelompok terarah pada wanita pasangan usia subur (PUS) dan wawancara mendalam dengan pengelola program di kabupaten/ kota hingga ke desa/kelurahan. Ditemukan TFR di Yogyakarta sangat rendah dan sebaliknya di NTT sangat tinggi. Perbedaan TFR tersebut disebabkan oleh latar belakang demografi dan nondemografi seperti respons terhadap berbagai program penurunan fertilitas. Faktor budaya terhadap nilai anak berpengaruh besar terhadap jumlah anak yang ingin dimiliki. Tingkat pendidikan yang rendah berhubungan dengan faktor yang berpengaruh langsung terhadap pemakaian kontrasepsi, termasuk jenis kontrasepsi. Pelayanan Keluarga Berencana (KB) yang rendah memicu tingginya kebutuhan kontrasepsi yang tidak terpenuhi. Kesertaan ber-KB diNTT dan di Yogyakarta adalah 42,2% dan 66,9%. Wanita PUS yang keinginan ber-KB tidak terpenuhi masih tinggi di NTT (17,4%) dibandingkan dengan Yogyakarta (6,8%).Kata kunci: Angka fertilitas total, pasangan usia subur, keluarga berencanaAbstractProvince of Nusa Tenggara Timur (NTT) have a total fertility rate (TFR) to the highest (4,2 children) and the lowest is the Province of Yogyakarta (1,8 children). This research is to identify factors that associated with total fertility rate in both provinces. This study uses qualitative method from the Indonesia Demography and Health Surveys (from 1991 to 2007), National Health Survey and Household Health Survey (2003), and Family Planning Mini Surveys (2007 and 2008). The qualitative method were collected using focus group discussions with fertile couple and in-depth interviews withfamily planning fieldworkers in district to village . The research reveals that TFR of Yogyakarta is very low and NTT is very high. The differentiation of the TFR is due to the demographic and nondemographic background as well as the respond againts the program to decrease the fertility. Cultural factor is the important one againts the value of children that will be influenced to the number of children desired in one family. Low education will bedirectly related to the use of contraceptive including mix contraceptive. Low family planning services which triggers the high unmet need oc contraceprive. Family Planning participation in NTT is 42,2% and 66,9% in Yogyakarta. Women on childbearing age who wishes to use contraceptive but unmet need in NTT were still high (17,4%) compared with Yogyakarta(6,8%).Key words: Total fertility rate, fertile couple, family planning

Background: Despite constant decrease in rate of neonatal mortality, the rate is still higher than that of other under-five children. One of the first steps towards reduction of neonatal mortality is to identify its determinants using health production function. The aim of the present study was to estimate neonatal health production function for Iran. Methods: In this cross-sectional study, Iranian Multiple Indicator Demographic and Health Survey (IrMIDHS) 2010 was used. Four categories of socioeconomic, mother, neonatal demographic and healthcare system factors were entered into the Binomial Logistic Regression model to estimate neonate health production function. Households’ economic status was constructed using principal component analysis. Results: History of abortion/stillbirth had the highest significant positive impact on odds of neonatal mortality (odds ratio=1.98; 95 % CI=1.55-2.75), indicating that neonates of mothers with such a history had 1.98 times higher chance of death compared to other neonates. Moreover, odds ratio of neonatal death for the poorest quintiles was 1.70 (95 % CI=1.08-2.74), indicating that by moving from the poorest quintile to the richest one, the odds of being alive for neonates increased up to 70%. However, skilled birth attendant decreased the chance of death up to 58% (odds ratio=0.58; 95 % CI=0.36-0.93). Conclusion: Considering the most significant inputs of neonatal health production function in Iran, improvement of economic status of households, provision of appropriate care services for mothers, and improvement of delivery care provided by trained personnel, could be priorities for health policymakers to act and reduce neonatal mortality in Iran.
