Quality control is an effort to increase customer satisfaction and minimize damage. Super Absorbent Polymer which is a product as a raw material for making baby diapers / elderly and sanitary napkins. In Super Absorbent Polymer, defects usually found in the colour contamination that there is a dark colour on the product, print labels mistakes and packaging defects. This study aims to determine the value of DPMO and sigma level, identify the efforts which is taken to reduce disability, identify the types of disability, and find the main factors causing disability. Six Sigma method is used to analyze data with define, measure, analyze, improve and control. The quantitative data obtained by direct observation of quality problems. By using the method of six sigma can be seen that the quality of the resulting product is quite good that is 3.07 sigma with a damage rate of 58,624 for a million production (DPMO). The three highest defect product causes are color contamination of 93.34%, misprint of 3.55%, and packaging damage as much as 3.11%. The main factor causing defects is the engine factor for the type of colour contamination defect,. The type of improper defects in the packaging labels and main factor for the packaging defects is the human factor.kecacatan rusaknya kemasan faktor utama ialah faktor manusia.

<em>CV Jaya Reksa Manggala (JRM) is one of clothing company. With increasing competition in this clothing company, the companies are required to keep improving the quality of clothing products. One of the important things in quality is reduction of product defects. One of the proven methods of  quality improvement is the Six Sigma DMAIC method, which is a continous improvement method that aims to achieve the best quality. One of the products of CV JRM which is still have a lot of defects is 514 products. From the initial data obtained, DPMO value for process I and II for 514 products is 10,768.52 and 27,341.42 with sigma level 3.79 and 3.43. The steps undertaken in this Six Sigma DMAIC method include define, measure, analyze, implementation and control. Several proposed improvements were then made to reduce the problem of these defects and from the data at the control stage, the DPMO values of processes I and II became 2,941.76 and 1,812.69 with sigma levels of 4.25 and 4.41. From this result it can be said that with Six Sigma DMAIC method, there is improvement of quality for product 514 at CV JRM. </em>

UD. BJ Mendit on Mendit Barat, Kec. Pakis is one of centrale of home industry in Malang with it's roof production process. From the observation that the researcher do into UD. BJ Mendit, finded that the production process had been doing often with technical and non technical problems that make some failed roof product with the criterion are keropos, break, barst, gopel, sandbank. Way of this roof production planning repair is important to do so that can make this production unit produce an unfailed product and reduce loss. Purposes of this research are identifying types of roof failed product, analysing main cause of roof failed product with FMEA, and giving recommended action for Mendit's roof production process repair. This research doing the same steps as same as Six Sigma cycle: DMAIC, but not doing the last step, Control. On Define step, are doing product identifying and make the Six Sigma team. On Measure step, are determining priority failed product identifying into CTQ (Critical To Quality) and measurement of present performance with counting the DPMO (Defect Per Million Opportunities) dan Sigma level. On Analyze step, is detecting main atributs that influencing the failed product on CTQ. On Improve step, is doing process improvement with developing alternative to reduce failed product atributs. From the steps had done, finded that the biggest failed criterion is keropos. Keropos is caused by lack of skill and correctness of worker on processing the raw material, can also caused by process execution which not fully corret so that produce critical process to be repaired first to increase the quality of Mendit's roof.

Purpose The purpose of this paper is to achieve cost reduction and quality improvement in SME by implementing define, measure, analyze, improve and control (DMAIC) stages of Six Sigma. Design/methodology/approach The application of DMAIC stages of Six Sigma was carried out in an SME. The cause of the wear out of the guide wheel was discerned in the define stage. In the measure stage, details from the field gathered and sigma level was determined. Using the paired comparison test and FMEA, various causes of the wear out of wheel was analyzed. The alternate material as a solution obtained was compared with present material in the improvement stage. The alternate material was implemented and ensured with proper documents, process change sheet and adoption. Findings With the application of easy to use tools a cost reduction of INR10,98,096 per annum and the quality improved from 2.9 to 4.4 sigma. Originality/value Easy tools were attempted in DMAIC model vs multiple and complex tools usage in large-sized organizations, which was not that much wide spread in SMEs. The study was carried out by changing the raw material of the outer layer of the guide wheel, by applying DMAIC model. A better grade rubber was used for solving the higher wear of the outer layer of the guide wheel. Significant cost reduction and quality improvement can be obtained in SMES by DMAIC model which leads to better brand image.

Abstrak. Dunia industri yang semakin berkembang akan mengakibatkan banyaknya persaingan. Perus-ahaan yang dapat bersaing adalah perusahaan yang dapat menjaga  kualitas produknya dengan baik sehingga dapat memenuhi kepuasan konsumen. Maka dari itu diperlukan pengendalian kualitas. Penelitian ini difokuskan pada penurunan cacat pada produk batu bata merah di CV. Ghatan Fatahillah dengan metode Six Sigma-DMAIC (Define, Measure, Analyze, Improve, dan Control). Dari hasil penelitian didapatkan DPMO sebesar 36.212 dengan nilai sigma 3,29. Jenis cacat yang paling sering terjadi pada batu bata merah yaitu pecah/patah yaitu sebesar 4.327 atau 59,81% dari total keseluruhan produk yang cacat.  Hasil dari tahap analyze dengan fishbone diagram, ditetapkan penyebab dari pecah/cacat, yaitu: pekerja kurang paham standar kualitas, tidak adanya pemeriksaan produk, pekerja kurang paham prosedur kerja, kinerja mesin tidak stabil, pekerja kurang teliti,  jumlah penggilingan tidak menentu, dan komposisi batu bata tidak menentu. Maka perlu dilakukan usulan perbaikan agar dapat mengurangi jumlah produk yang cacat pada batu bata merah. Kata kunci: DMAIC, DPMO, Pengendalian Kualitas, Six Sigma Abstract. The growing industrial world will result in a lot of competition. Companies that can compete are companies that can maintain the quality of their products well so that they can meet customer satisfaction. Therefore, quality control is needed. This research is focused on reducing defects in red brick products at CV. Ghatan Fatahillah with the Six Sigma-DMAIC method (Define, Measure, Analyze, Improve, and Control). From the research results obtained DPMO of 36,212 with a sigma value of 3.29. The type of defect that most often occurs in red bricks is broken / broken, which is 4,327 or 59.81% of the total defective products. The results of the Analyze stage with the fishbone diagram showed that the causes of breakage / defects were determined, namely: workers do not understand quality standards, lack of product inspection, workers do not understand work procedures, unstable machine performance, workers are not careful, the number of mills is erratic, and composition erratic bricks. So it is necessary to make improvement proposals in order to reduce the number of defective products in red bricks. Keywords: DMAIC, DPMO, Quality Control, Six Sigma

Indonesia has several creative economy sectors that rapidly developed and potentially can be improved to 7% annually. One of them is textile industry specifically bag industry. This industry’s development could enhance the production of commodities and emerge other SMEs to initiate the similar field. Therefore, to raise its profit, customer’s satisfaction must be elevated by minimizing defected products. In order to support it, an improvement on production process is a must. This research employs Six Sigma methods integration with Fuzzy Analytical Hierarchy Process (Fuzzy AHP)-Failure Mode and Effect Anlysis (FMEA). Six Sigma is designated to analyze the level of products’ defect through Define, Measure, Analyze, Improve, and Control (DMAIC) stages and assess the factors that cause the defects by using Fuzzy AHP decision support system and FMEA in risk analysis on defect. The result of the research shows the shortage of defects that demonstrated by DPMO indicator and Sigma level. It is identified that the number of DPMO was being reduced from 20003.75 to 11185.73. While Six Sigma level demonstrated the enhancement from 3.61 to 3.86. Those suggest that the improvement solution provides has significant effect on the defects decreasing at the bag convection industry.

This study focused on quality improvement on ceramic tiles production process at PT Arwana Citramulia. This study used data defects for 12 months (May 2016 – April 2017) and only focus on one type of defect and it is chop corner. Six sigma with DMAIC (define, measure, analyze, improve, and control) approach was used to improve the process. Each step of DMAIC was conducted to carefully analyze and keep the process precisely. The ceramic tiles production process contains a number of 4375 products defects in million opportunities (DPMO), with sigma level of 4.13. In the improve step of DMAIC, FMEA form was used to propose some recommendations in order to improve the process, some of that that are provision of lubricant periodically by the operator, polishing on the surface of the liner to clean and clear, examination and maintenance periodically. Abstract This study aims to determine and identify the causes of defects in the production process of PT. Triteguh Manunggal Sejati and know the level of sigma level. This research uses the six sigma method with the DMAIC approach as a quality control tool, which includes the Define, Measure, Analyze, Improve and Control stages. Based on this study the results obtained are the level of sigma level at PT.Triteguh Manunggal Sejati is 4.96, which means that in the stage of sigma level the company has not reached the level of six sigma levels because in the production process at PT.Triteguh Manunggal Sejati still has product defects in the production process not yet achieved zero defect. The causes of product defects are based on cause and effect diagrams namely lid / seal defects are leaky lid, broken lid, and tilted lid. The main factor to achieve business success in the era of globalization is quality. Quality control is key in maintaining customer loyalty. In the company's production process is still found the resulting product does not comply with the specified specifications. This study aims to determine the value of sigma and factors causing damage to the production process of bagging section For companies with quality control is expected to achieve corporate goals. In this study, the methods used is Six Sigma with define, measure, analyze, improve. Six Sigma result is measurement of company performance baseline at measurement stage that is company at condition 5,1 sigma with DPMO equal to 162,4532. The factors causing the disability of sugar packing are lack of research and skill of the operator, instability of conveyor speeds and jet engine position, machine hygiene condition, lack of weighing machine, and ineffective treatment, and control method.

High gloss plastics part in injection molding industries were widely used in Malaysia. However the high rejection rate in this industries were major problem affecting the economic aspects. Therefore this paper presents an approach of implementing six sigma method to reduce the rejection rate in a plastic injection molding process for high gloss plastics part. Define, Measure, Analyze Improve and Control (DMAIC) methodology was applied as basis of the study. By using current process, the average of rejection is 40.6% and the aim of this study is to reduce the rejection rate to less than 10 % . All potential factors were taken into account to identify the significant factors. The improvement process was made base on the analysis output. This study was successful with increment in sigma level from 1.74 σ to 3.00 σ. .

Porous structure of the super absorbent polymer (SAP) was synthesized through inverse suspension method by adding acrylic acid (AA) and acrylic amide (AM) as monomer,N,N'-Methylene-bis-acrylamide (MBA) as cross-linking agent, the mixed solution of methanol and ethanol as pore-foaming agent. The factors that influenced the bibulous performance of the super absorbent polymer were studied including the dosage of cross-linking agent, the dosage of pore-foaming agent, monomer ratio, revolving speed. The Internal structure and surface morphology of SAP was characterized by scanning electron microscopy (SEM). It indicated that when the monomer ratio wasm(AM):m(AA)=1:2; the weight rate of cross-linking agent and pore-foaming agent were 0.06%(compared with the quality of the monomer) and 9%, the product attained the best water absorbency of 2909 g/g in distilled water, the saline absorbency was 124.5 g/g; the water absorption rate was 10. Meanwhile, SEM showed that the structure of the copolymer was full of holes.
