Prevalence of metabolic syndrome in employees of various professional groups

A review of 47 studies evaluating the occurrence of metabolic syndrome among employees of different professional groups around the world was made. The influence of various factors of the professional environment (physical activity in the workplace, exposure to occupational stress, exposure to harmful toxicants) on the frequency of the syndrome and its components, considering gender, age and work experience. Analysis of the literature has shown that the characteristics of work activity affect the formation of the syndrome along with well - known risk factors-lifestyle, gender and age. Thus, in most cases, the frequency of metabolic syndrome in people who have mostly "sedentary" work is much higher than in other professional groups. It is proved that the risk of the syndrome is increased when exposed to certain chemicals that are in the air of the work area, even in small concentrations, as well as under unfavorable conditions of activity - shift work, night duty, professional stress. To improve the quality of life of employees and reduce economic and social costs, it is necessary to develop programs for the prevention and rehabilitation of people with this pathology, taking into account specific working conditions, while making adjustments for gender, age, social factors (smoking status, alcohol consumption, eating behavior, physical activity during and outside of work).

The association between immunity and metabolic syndrome (MetS) has been studied, but its interaction with lifestyles remains unclear. We studied their association and interactions with lifestyles in 40,768 adults aged over 40 years from a large-scale, hospital-based cohort study collected during 2010–2013. White blood cell counts (WBC) and serum C-reactive protein concentrations (CRP) were used as indexes of immune status. The participants were categorized into four groups by the cutoff points of 6.2 × 109/L WBC(L-WBC) and <0.5 mg/dL CRP(L-CRP): L-WBC+L-CRP(n = 25,604), H-WBC+L-CRP(n = 13,880), L-WBC+H-CRP(n = 464), and H-WBC+H-CRP(n = 820). The participants in the H-WBC+L-CRP were younger and had higher numbers of males than the L-WBC+L-CRP. MetS risk was higher by 1.75- and 1.86-fold in the H-WBC+L-CRP and H-WBC+H-CRP, respectively, than the L-WBC+L-CRP. MetS components, including plasma glucose and triglyceride concentrations, and SBP were elevated in H-WBC+L-CRP and H-WBC+H-CRP compared with L-WBC+L-CR+P. The risk of hyperglycemia and high HbA1c was the highest in the H-WBC+H-CRP among all groups. Areas of WBC counts and serum CRP concentrations were 0.637 and 0.672, respectively, in the receiver operating characteristic curve. Daily intake of energy, carbohydrate, protein, and fat was not significantly different in the groups based on WBC counts and CRP. However, a plant-based diet (PBD), physical activity, and non-smoking were related to lowering WBC counts and CRP, but a Western-style diet was linked to elevating CRP. A high PBD intake and smoking status interacted with immunity to influence MetS risk: a low PBD and current smoking were associated with a higher MetS risk in the H-WBC+H-CRP. In conclusion, overactivated immunity determined by CRP and WBC was associated with MetS risk. Behavior modification with PBD and physical activity might be related to immunity regulation.

In the present study, we examined (i) whether C3 (complement C3) was an independent marker of prevalent CHD (coronary heart disease), and (ii) which preferential associations existed between C3 and some cardiovascular risk factors when jointly analysed with CRP (C-reactive protein) and fibrinogen. In a cohort of 756 unselected adults, 39% of whom had the metabolic syndrome, C3 and other risk variables were evaluated in a cross-sectional manner. In a logistic regression model for the likelihood of CHD, a significant OR (odds ratio) of 3.5 [95% CI (confidence intervals), 1.27 and 9.62)] for C3 was obtained after adjustment for smoking status, TC (total cholesterol) and usage of statins. A similar model, also comprising systolic blood pressure, with a cut-off point of ≥1.6 g/l C3 exhibited a 1.9-fold risk (95% CI, 1.01 and 3.58) compared with individuals below the cut-off point. Both analyses displayed an adjusted OR of 1.37 for each S.D. increment in C3. The significant relationship of C3 with a likelihood of CHD also proved to be independent of CRP. In multiple linear regression models, associations were tested for each acute-phase protein with measures of obesity, fasting insulin, triacylglycerols (triglycerides), TC, HDL (high-density lipoprotein)-cholesterol, physical activity, smoking status, diagnosis of metabolic syndrome and family income. When both genders were combined, C3 was independently associated with serum triacylglycerols, waist circumference, BMI (body mass index) and TC. CRP was independently associated with waist circumference, TC, family income (inversely) and physical activity, and fibrinogen with BMI, TC, smoking status and metabolic syndrome. In summary, elevated levels of complement C3 are associated with an increased likelihood of CHD independent of standard risk factors and regardless of the presence of acute coronary events, suggesting that C3 might be actively involved in coronary atherothrombosis. Unlike CRP and fibrinogen, C3 was preferentially associated with waist girth and serum triacylglycerols.

Background:This study was performed to determine the relationship between leisure-time physical activity (LTPA) and the metabolic syndrome (MS) in 16,681 adults (43 ± 0.44 y) enrolled in NHANES III.Methods:LTPA was classified as regularly active (≥ 5 d/wk moderate and/or ≥ 3 d/wk vigorous), irregularly active (some LTPA), or inactive (no LTPA). The MS was positive with three or more conditions: 1) abdominal obesity, 2) low HDL-C, 3) hypertriglyceridemia, 4) elevated blood pressure, or 5) elevated glucose. Logistic regression examined the relationship between LTPA and the MS, adjusting for age, race, smoking status, and educational attainment stratified by gender.Results:In men only, irregular activity and inactivity was related to an increase in the MS (irregular: OR = 1.52 95% CI 1.11, 1.23; inactive: OR = 1.60, 95% CI 1.18, 1.98; test for trend P = 0.004). Inactivity increased the odds for abdominal obesity (P < 0.05).Conclusions:LTPA levels might influence the development of MS and abdominal obesity.

AbstractObjectiveTo investigate the sociodemographic risk factors associated with metabolic syndrome (MetS) in the Mediterranean population of Catalonia, Spain.Design and settingData from the cross-sectional, population-based 2002–2003 Health Survey of Catalonia were analysed. The survey used a structured questionnaire to collect information on demographics, lifestyle and medical history. In a sub-sample of the original survey population anthropometrics and blood pressure were measured and blood samples were taken to determine HDL cholesterol, TAG and fasting glucose.SubjectsThe analysis included the 1104 individuals aged 18–74 years from this sub-sample who had complete information on all variables necessary to define MetS using the National Cholesterol Education Program’s Adult Treatment Panel III (ATP III) and the International Diabetes Federation (IDF) criteria.ResultsMetS prevalence was 28·5 % and 24·8 % according to IDF and ATP III criteria, respectively. MetS was significantly (P = 0·05) more common in males than females. MetS prevalence increased significantly (P<0·001) with age and degree of adiposity and as social class decreased. In general, MetS prevalence decreased as physical activity increased, which was significant (P = 0·0253) when applying ATP III criteria. After taking into account important confounders, MetS prevalence was significantly positively associated with male gender, age, BMI, physical inactivity and lower social class. Smoking status, marital status and working situation were not independently associated with MetS.ConclusionsAge, sex, degree of adiposity, physical activity and social class are the sociodemographic risk factors independently associated with MetS in this Mediterranean population. Understanding which factors predict MetS is important considering likely increasing MetS trends, and is useful for determining public health strategies.

Background and objective: Managing the decrease in physical function in the elderly is a major task in aging societies globally. Here, we aimed to compare the physical function and metabolic syndrome (MetS) risk factors according to levels of physical activity (PA). Material and methods: We measured PA in 77 elderly Korean men (74.21 ± 6.26 years old) with an accelerometer and recorded body composition, physical function, and MetS-related risk factors. Participants were divided into three groups based on daily moderate-vigorous physical activity (MVPA): low (under 60 min), middle (60-120 min), and high (over 120 min). The groups were compared using a one-way analysis of variance and the Scheffe post hoc test. Odds ratios (OR) were calculated by logistic regression analysis. Results: Significant differences were found between the groups for sedentary behavior time (P < 0.001), light PA (P < 0.05), moderate PA (P < 0.001), vigorous PA (P < 0.05), and total energy expenditure (P < 0.001). The high PA group showed a significantly lower percentage of body fat and fat mass and higher muscle mass than did the low and middle PA groups (P < 0.05). The 6-min walk test was significantly better in the high PA group than in the low and middle PA groups (P < 0.05). Grip strength and the Berg balance scale were also significantly better in the high PA group (P < 0.05). Bone mineral density (BMD) and high-density lipoprotein cholesterol (HDL-C) were significantly higher in the high PA group than in the low PA group (P < 0.05). Systolic blood pressure (SBP) was significantly higher in the middle PA group than in the low PA group (P < 0.05). Participants with more than three MetS criteria showed an OR of 0.09 (95% confidence interval 0.01-0.82) in the high PA group as compared with the low PA group (P < 0.05). Conclusions: Moderate-vigorous physical activity of more than 120 min daily showed better physical function and lower OR of MetS than did lower MVPA levels in elderly Korean men.

Introduction. Development of methodical base of information hygiene, systematization of data on levels of information loadings and degree of their influence on working conditions of workers is actual.The purpose of the study was to measure and evaluate the levels of information mental load of office workers, to study the patterns of development of adverse functional States in this professional group.Materials and methods. The pilot study included 25 employees of scientific and educational institutions aged 25 to 65 years. Measurement of information mental load of workers was carried out on the basis of a point assessment. Psychodiagnostics of performance of participants of research was carried out by means of a package of standard techniques. Heart rate telemetry was carried out in employees in the conditions of professional activity.Results. The integral indicator of information mental load of the studied professional group was 37.3±7.2 points and was in direct connection with the coefficient of comfort of the workplace (r=0.5, p=0.01). A reliable dependence of the speed of information processing on the length of work experience (r=0.71, p=0.0001) was also established. In the low-stress group, high indices of stress, fatigue, monotony were recorded in the workers, in the senior group — signs of chronic fatigue. The employees during the shift registered an increase in sympathetic activation.Conclusions. The results of the psychophysiological survey of employees indicate the need to regulate the information load, the development and implementation of preventive measures to reduce professional stress.
