Abstract Context.—Good communication of pathologic characteristics of a malignancy is crucial to therapy choices and accurate prognostication. The information must be easily retrieved from a surgical pathology report. Objectives.—To evaluate, first in 1999, the quality of surgical pathology reports for segmental breast resections for cancer in Quebec hospitals. Subsequently, to reevaluate, in 2003, the same indicators to determine if the first surveillance, with feedback, was associated with an improvement in the quality of the reports. Design.—All Quebec hospitals performing the preset number of 20 or more segmental breast resections for cancer in 1999 and 2003 participated. A committee of pathologists, after review of the literature, chose 7 diagnostic elements deemed vital to a surgical pathology report for conservative breast cancer surgery. Medical archivists in each institution were instructed on how to retrieve the data. The main outcome measure was the presence or absence of the diagnostic information clearly presented on the surgical pathology report. Results.—Fifty-one hospitals participated in 1999 and 50 in 2003. Overall, conformity improved from 85.0% in 1999 for the first evaluation to 92.5% in 2003 for the second evaluation (P < .001). Six of the 7 indicators showed an improvement in the level of conformity between the first and second evaluations. Conformity was weakest for recording the distance between the tumor and the resection margin (68.2%) and vascular/lymphatic invasion (61.4%) in 1999. Conclusions.—Surveillance of quality of surgical pathology reports, with feedback, is significantly associated with an improvement in the quality of reports.

The Cost

The debate on deliberative democracy could open a fruitful perspective for research on social movement conceptions and practices of democracy. This article reports a pilot study of the values and norms that guide the global justice movement's organizational choices based upon focus groups and in-depth interviews with participants in various Italian social forums. Deliberative democracy, which emphasizes participation and the quality of communication, is particularly relevant for a multifaceted, heterogeneous movement that incorporates many social, generational, and ideological groups as well as movement organizations from different countries. The global justice movement—a "movement of movements" according to some activists—comprises a dense network of movement organizations, often the product of previous protest cycles. It builds upon past experiences of organizational institutionalization, but also upon reflexive criticisms of it. These networks of networks provide important resources, but also pose challenges for participation and internal communication. The activists in our study addressed these challenges by building an organizational culture that stressed diversity rather than homogeneity; subjectivity, rather than obedience to organizational demands; transparency, even at the cost of effectiveness; open confrontations oriented to consensus building over efficient decision making; and "ideological contamination" rather than dogmatism. Traditional participatory models of democracy are bridged with concerns for good communication and deliberation.

UNSTRUCTURED The recent drive to include virtual care in surgical practice has been accelerated due to the COVID-19 pandemic. Many physicians feel that communicating via telehealth is unlike traditional methods of providing health care, and thus guidance on maintaining excellence in communication is necessary, especially as academic literature on virtual care in surgery is nonexistent. Challenges faced in transitioning to virtual care include the inability to utilize body language, barriers to traditional physical examination, exacerbation of existing vulnerabilities and inequities in patient groups, the declining quality of medical education, and the fragmentation of the multidisciplinary health care team. This paper seeks to resolve these challenges by focusing on the pillars of good communication, including preparation, professionalism, empathy, respect, and the virtual physical examination.

National character and civilization building cannot be separated from the education institution. As a place for producing intellectuals, education institutions might be regarded as the “second homes” in which students study to develop their competencies. They are the places in which the collaboration from the students’ parents is needed in order to create a good learning atmosphere. So far the Hindu education institutions receive lack of this kind of collaboration. As a solution, it is necessary to provide more space for the students’ parents to contribute to the Hindu higher education improvement. Such a place is necessary for a good communication as in a particular situation students’ parents should be involved in improving the quality of the education and avoiding opposition.

Abstract PT PLN (Persero) bring the Smart Power products. But the implementation, apparently still met the news that affects the reputation of the PT. PLN (Persero) is allegations of mafia token of electricity in the regions of South Tangerang in 2015. The aim of this study was to determine the reputation of PT PLN (Persero) allegations of mafia-related news for token electricity in Sub Cipadu Jaya South Tangerang 2015. In this study the authors used the concept of Crisis Public Relations consisting of Reputation with dimensions of Communication, Professional Attitudes, public trust, product Innovation, Social Responsibility, Quality of Public Consumer Services, Responsive Against Public Feedback. This study uses a quantitative approach and a descriptive study. This study uses a survey by distributing questionnaires. The sample in this study as many as 80 heads of families using purposive sampling technique. Using the mean central tendency. Based on the results of research dimensions to get the lowest mean value lies in the third dimension, namely, Public Trust which has three indicators. Dimensions that have the highest mean value lies in the seventh dimension, namely, Responsive Against Public Feedback has two indicators. Key words : Crisis, Public Confidence, Reputation Abstrak PT PLN (Persero) menghadirkan produk Listrik Pintar. Tetapi pada pelaksanaannya, ditemui pemberitaan yang berpengaruh pada reputasi PT. PLN (Persero), mengenai tudingan mafia token listrik di daerah Tangerang Selatan tahun 2015. Tujuan penelitian ini adalah untuk mengetahui reputasi PT PLN (Persero) terkait pemberitaan adanya tudingan mafia token listrik di Kelurahan Cipadu Jaya Tangerang Selatan tahun 2015. Dalam penelitian ini penulis menggunakan konsep Crisis Public Relations yang terdiri dari Reputasi dengan dimensi Komunikasi, Sikap Profesional, Kepercayaan publik, Inovasi produk, Tanggung Jawab Sosial, Kualitas Layanan Konsumen Publik, Responsive Terhadap Feedback Publik. Penilitian ini menggunakan pendekatan kuantitatif dan merupakan jenis penelitian deskriptif. Penelitian ini menggunakan metode survei dengan cara menyebarkan kuesioner. Sampel dalam penelitian ini sebanyak 80 kepala keluarga dengan menggunakan teknik penarikan sampel purposif. Menggunakan tendensi sentral mean. Berdasarkan hasil penelitian dimensi yang mendaptkan nilai mean terendah terletak pada dimensi ketiga yaitu, Kepercayaan Publik yang memiliki tiga indikator. Dimensi yang memiliki nilai mean tertinggi terletak pada dimensi ketujuh yaitu, Responsive Terhadap Feedback Publik yang memiliki dua indikator. The aim of the study is to present a new and efficient way of measuring the quality of life among older populations, with special attention to morale, positive outlook on life and ageing. The measure is based on the Philadelphia Geriatric Center Morale Scale (PGCM), originally consisting of 22 items. The growing numbers and proportions of older people among European populations has increased the need to obtain more reliable data on their needs, values, life experiences and overall quality of life. Using data from six surveys conducted in the Czech Republic, we have formulated a three-item positive outlook on life and ageing (POLA) scale. Our analyses are divided into three steps: (1) constructing the scale and testing its internal consistency; (2) testing the scale’s external validity using mean comparisons and correlation coefficients; and (3) determining the factors affecting a positive outlook on later life, such as gender and education. We have confirmed that the three-item approach to measuring positive outlook on ageing as a part of morale is highly applicable to surveys, producing compelling results in assessing important quality-of-life sub-concepts, such as subjective health, subjective age, and loneliness.

Abstract Background There is a high global burden of neonatal mortality, with many newborn babies dying of preventable and treatable conditions, particularly in low and middle-income countries. Improving quality of newborn care could save the lives of many thousands of babies. Quality of care (QoC) is a complex and multifaceted construct that is difficult to measure, but patients’ experiences of care are an important component in any measurement of QoC. We report the findings of a qualitative study exploring observations and experiences of health workers (HWs) and mothers of babies in neonatal units in South Africa. Methods A qualitative case study approach was adopted to explore care of newborn babies admitted to neonatal units in district hospitals. Observation data were collected by a registered nurse during working hours over a continuous five-day period. Doctors and nurses working in the neonatal unit and mothers of babies admitted during the observation period were interviewed using a semi-structured interview guide. All interviews were audio recorded. Observation data were transcribed from hand written notes. Audiotapes of interviews were transcribed verbatim and, where necessary, translated into English. A thematic content analysis was used to analyse the data. Results Observations and interviews were conducted in seven participating hospitals between November 2015 and May 2016. Our findings highlight the importance of information sharing between HWs and mothers of babies, contrasting the positive communication reported by many mothers which led to them feeling empowered and participating actively in the care of their babies, with incidents of poor communication. Poor communication, rudeness and disrespectful behaviour of HWs was frequently described by mothers, and led to mothers feeling anxious, unwilling to ask questions and excluded from their baby’s care. In some cases poor communication and misunderstandings led to serious mismanagement of babies with HWs delaying or withholding care, or to mothers putting their babies at risk by not following instructions. Conclusion Good communication between mothers and HWs is critical for building mothers’ confidence, promoting bonding and participation of mothers in the care of their baby and may have long term benefits for the health and well-being of the mother and her baby.

197 Background: Advances in targeted therapy have led to an increase in the median overall survival in patients with HER2+ metastatic breast cancer (MBC). However, patient attributions related to living longer have not been studied. Methods: A one-time, web-based survey was administered through multiple US-based breast cancer organizations. The inclusion criteria were being female, age 18+, having HER2+ MBC, and living in the US. The surveys included health related quality of life (HRQL), symptom burden, activities of daily living, satisfaction with medical care, work productivity, and attributes of living longer. To assess beliefs about length of survival with HER2+ MBC, participants were asked to rate the importance of 16 items. Descriptive results were stratified by time since metastatic diagnosis and HRQL. Results: Of 560 attempted surveys, 365 (65%) participants met inclusion criteria. Mean (SD) age was 53.6 (10.5) years, 93% were white, and 27% reported having participated in a clinical trial. 59% of respondents reported living ≥4 years since HER2+ MBC diagnosis. Common reasons patients gave for living longer included: choosing the right doctor/medical team (91%), having a positive outlook on life (79%), and learning about the disease or treatments (79%). Women with <4 years since HER2+ MBC diagnosis were more likely to report having a positive outlook on life (85% vs 76%), using complementary and alternative medicine (45% vs 36%) and exercising regularly (74% vs 66%) compared with those having ≥4 years since diagnosis. Women reporting a better compared with a poorer HRQL reported more frequent regular exercise (73% vs 49%) and more often had a positive outlook on life (86% vs 64%). Conclusions: To our knowledge, this is the first study of women with HER2+ MBC and their attributions about length of survival. Having the right medical team, remaining positive, and being educated about the disease and its treatments were most important to participants. Women reporting better quality of life were more likely to report multiple attributions of living longer. HRQL and patient perceptions about their survival are important factors that should be considered in providing patient-centered care.
