Retrospective statistical study a collection of all patients with post intra vitreal injection (IVI) endophthalmitis in last 10 year period between 2010–2019 in eye department of Tobruk medical center -Tobruk and eye department of Alwahda hospital - Derna. We aim in this study to review the microbiology, clinical characteristics, diagnosis, and management strategies of post-intra vitreal injection endophthalmitis, by comparing our results with the different studies designs and treatment protocols. There were 9 patients of the after intra vitreal injections endophthalmitis cases. The age average is 70 years, 6 cases are females and 3 casees are male, all of them were practice post intravitreal Avastin injection endophthalmitis. There were 8 patients had Diabetes mellitus (DM), 2 patients had cardiovascular disease and 1 patient had hepatitis C as a systemic disease. The most common causative organism after intra vitreous injection endophthalmitis is Coagulase-negative staphylococci (CNS) 100% from the 56% positive causative organisms and 44% are no detected microorganisms, the outcome of cases which managed with pars-plana-vitrectomy (PPV) improved significantly about 60 Letters (> 3 Lines) and cases managed without PPV > 15 Letters (> 3 Lines). The cases of post intraocular endophthalmitis which diagnosed and treated early with PPV give a good result and can save the vision. The cases of acute POE in the first 3 day had significant better function (P value = 0.02).

ABSTRACT: VISUAL RESULTS OF DIABETES RETINOPATHY PATIENTS POST PARS PLANA VITRECTOMY OPERATION AT PERTAMINA BINTANG AMIN HOSPITAL Background: Diabetic retinopathy is a neovascular complication that is very specific for type 1 diabetes and type 2 diabetes. Based on data according to Basic Health Research (RISKESDAS) in 2018, diabetes mellitus in 2013 reached 1.5% of patients with diabetes, and is increasing. to 2.0% in 2018. Treatment of diabetes mellitus patients with complications of diabetic retinopathy by performing pars plana vitrectomy surgery which is anoperative management that can improve eye visual or restore visual fuction. Research Objectives: To determine the visual outcome in diabetic retinopathy patients after pars plana vitrectomy surgery at Pertamina Bintang Amin Hospital, Lampung Province in 2020.Research Methods: This type of research was cross -sectional. Sampling was done by total sampling. Bivariate data analysis using Paired T-Test.Results: Statistical analysis using Paired T-Test showed the p-value for visualoutcome analysis in diabetic retinopathy patients before and after pars plana vitrectomy surgery was 0.000 (p-value <0.05).Conclusions: There are significant changes in diabetic retinopathy patients visualoutcome before pars plana vitrectomy surgery and after pars plana vitrectomy surgery at Pertamina Hospital Amin Star Lampung Province 2020. Keywords: Diabetic retinopathy, pars plana vitrectomy operation, Visualoutcome INTISARI: VISUALOUTCOME PADA PASIEN RETINOPATI DIABETIK PASCA OPERASI PARS PLANA VITRECTOMY DI RUMAH SAKIT PERTAMINA BINTANG AMIN  Latar Belakang: Retinopati diabetik adalah komplikasi neovaskular yang sangat spesifik untuk diabetes tipe 1 dan diabetes tipe 2. Berdasarkan data menurut Riset Kesehatan Dasar (RISKESDAS) pada tahun 2018, penyakit diabetes melitus di tahun 2013 mencapai 1,5% pasien pengidap diabetes, dan meningkat menjadi 2,0% pada tahun 2018. Penanganan pada pasien diabetes melitus dengan komplikasi retinopati diabetik diantaranya adalah dengan dilakukannya operasi pars plana vitrektomi yang merupakan penatalaksanaan operatif yang dapat memperbaiki visus mata atau memulihkan fungsi penglihatan . Tujuan Penelitian: Untuk mengetahui visualoutcome pada pasien retinopati diabetik pasca operasi pars plana vitrektomi di Rumah Sakit Pertamina Bintang Amin Provinsi Lampung Tahun 2020.Metode Penelitian: Jenis penelitian ini adalah potong silang (Cross-sectioal). Pengambilan sampel dilakukan secara total sampling. Analisis data bivariat dengan Paired T-Test.Hasil Penelitian: Analisis statistik menggunakan uji Paired T-Test menunjukkan p-value untuk analisa visualoutcome pada pasien retinopati diabetic sebelum dan sesudah operasi operasi pars plana vitrektomi sebesar 0,000 (p-value < 0,05).Kesimpulan: Terdapat perubahan yang signifikan pada visualoutcome pasien retinopati diabetik sebelum operasi pars plana vitrektomi dan sesudah operasi pars plana vitrektomi di Rumah Sakit Pertamina Bintang Amin Provinsi Lampung Tahun 2020. Kata Kunci: Retinopati Diabetik, Operasi Pars Plana Vitrektomi, Visualoutcome

Background: Urinary tract infection (UTI) is a common medical condition among women. E.coli is the most common causative organism. Appropriate understanding of the development of antimicrobial resistance over the past helps to establish efficient treatment strategies in the future. The study aims to discover antimicrobial resistance trends exhibited by E.coli strains isolated from women urine cultures over the past 10 years. Methods: A total of 1874 affected urine samples over the years 2009 to 2018 were collectively reviewed and classified according to the response they showed to 24 different antimicrobial disks in the laboratory. Relations between time and resistance evolutionary profiles were calculated. Results: Gentamicin (p value =0.039), Augmentin (p value =0.017), Cefoxitin (p value =0.001), Cefixime (p value =0.026) fulfilled satisfactory figures in terms of average resistance, regression of resistance, speed of resistance evolution, steadiness of performance, side effects, spectrum range and cost with high significance. Conclusion: Drugs that showed satisfactory figures are recommended for future treatment protocols in Jordan.

Postoperative endophthalmitis is a rare clinical occurrence. However, it remains one of the most serious complications following cataract surgery because of its poor prognosis. We investigated the epidemiologic trends in postoperative endophthalmitis following cataract surgery, particularly in Asian populations. The incidence of postcataract endophthalmitis was generally consistent with epidemiologic data reported from Caucasian populations. The most frequently occurring causative organism was coagulase-negative Staphylococci in most studies of Asian populations. However, Pseudomonas aeruginosa and Nocardia were found to be the most common microorganisms in several studies. The rates of culture positivity were slightly lower than in Caucasian studies. In the evaluation of risk factors of poor visual outcomes, initial visual acuity and virulence of the causative microorganisms were generally found to be the most important risk factors. A history of pars plana vitrectomy was found to be the major risk factors for developing endophthalmitis in several studies.

Diabetes mellitus is a systemic disease that increases the risk of infections. Exogenous endophthalmitis is an inflammatory disease to which diabetic patients are more predisposed to than nondiabetic patients undergoing any intraocular intervention. This might be because of the change in the immune and inflammatory factors that intervene in wound healing and in the bacterial flora of the ocular adnexa. We conducted a literature review to assess the risk of exogenous endophthalmitis in diabetic patients undergoing cataract extraction, pars plana vitrectomy, and intravitreal injections and to check whether its treatment differ from in non-diabetics. We found that diabetic patients are more predisposed to virulent organisms and that the incidence of ophthalmic symptoms was not substantially different in diabetic versus nondiabetic patients. Regarding treatment, all patients with light perception should receive pars plana vitrectomy, while those with hand motion and better vision should be given an intravitreal antibiotics injection. Some authors recommend vitrectomy to diabetic patients with even counting figure vision.

<b><i>Purpose:</i></b> To describe 2 cases of vitreoretinal metastases in patients treated with immunotherapy for metastatic melanoma. <b><i>Methods:</i></b> Retrospective case series. <b><i>Results:</i></b> We pre­sent 2 patients with metastatic melanoma treated with systemic immunotherapy with subsequent development of ocular vitreoretinal metastasis. The first patient was a male with metastatic melanoma from a site of unknown origin that was in complete remission following a course of ipilimumab and nivolumab therapy. He presented to an outside provider for evaluation of vitritis and a pigmented lesion in the right eye that was presumed secondary to toxoplasmosis. After failing initial management with oral antibiotics, he underwent diagnostic pars plana vitrectomy, and vitreous biopsy was consistent with metastatic melanoma to the vitreous. He was additionally found to have an elevated pigmented retinal mass consistent with a retinal metastasis from melanoma that initially failed treatment with plaque brachytherapy and ultimately required enucleation. The second case was a monocular male with metastatic melanoma from cutaneous melanoma with decreased vision 3 months after the initiation of nivolumab therapy. He presented with dense vitreous debris in his seeing eye and was thought to have nivolumab-associated inflammation. He was initially treated with difluprednate with improved vision and decrease in vitreous debris, but developed dense pigment deposition in the affected eye later in the treatment course. Diagnostic pars plana vitrectomy was performed, and vitreous biopsy was positive for malignant melanoma cells. His systemic disease was in remission at the time of diagnosis of ocular metastasis. External beam radiation was recommended given his monocular status. <b><i>Conclusion:</i></b> Vitreoretinal metastasis can develop despite favorable systemic response to immunotherapy in patients with metastatic cutaneous melanoma. Lack of ocular penetration and extension of life span with immunotherapeutic agents may be the underlying mechanism for vitreoretinal metastasis.

Purpose:  To compare the frequency of post-operative hypotony between 23G PPV and 25G PPV in advanced diabetic eye disease. Study Design:  Quasi experimental study. Place and Duration of Study:  Study was conducted at department of Ophthalmology, Lahore General Hospital, Lahore from 7th April 2016 to 6th October 2016. Methods:  Total 100 cases of advanced diabetic eye disease with age ranging from 25 – 65 years and either gender were selected. Patients with nystagmus and claustrophobia, lamellar macular holes, epiretinal membrane and neovascular glaucoma were excluded. Patients were divided by lottery method into 2 groups. Data of the patient i.e. name, age, sex, patient’s registration number and address was recorded. Every patient had detailed preoperative work-up; including best corrected visual acuity by Snellen’s chart, intraocular pressure by applanation tonometer, indirect ophthalmoscopy and B-scan for retinal status. Group A underwent 23G PPV and group B underwent 25G PPV. Patients were followed after 24 hours of surgery to measure intraocular pressure to access hypotony. Results:  Mean age of patients in group A was 50.16 ± 10.40 years and in group B was 50.26 ± 9.91 years. Out of 100 patients 57 (57.0%) were females and 43 (43.0%) were males, with female to male ratio of 1.1:1. Post-operative hypotony was seen in 24 (48.0%) patients with 23G PPV and 02 (4.0%) patients with 25G PPV (p-value = 0.0001). Conclusion:  This study concluded that the frequency of post-operative hypotony in 23G Pars Plana Vitrectomy was higher as compared to 25G Pars Plana Vitrectomy in advanced diabetic eye disease. Abstract Aim To evaluate the primary anatomical success and visual outcomes in patients with rhegmatogenous retinal detachment (RRD) undergoing 25-gauge pars plana vitrectomy (25g PPV) surgery in Pakistan. Method This is a five-year retrospective, interventional cohort study of 418 consecutive patients with RRD who underwent 25g PPV. Surgeries were performed by two experienced surgeons at tertiary care hospitals in Pakistan. Consecutive patients, who underwent 25 g PPV surgery as treatment for RRD between October 2013-October 2018 were included. We excluded patients who had a history of previous retinal surgery or who did not complete the 4-8 weeks primary outcome visit. Data was collected from the patient’s files by using a pro forma. SPSS version 23.0 (IBM SPSS Statistics, Armonk, NY) was used. A p-value of &lt; 0.5 was considered significant. Results We identified 452 patients through coding system of hospitals who underwent 25g PPV surgery between October 2013-October 2018. Secondary retinal surgery and incomplete follow up cases were excluded. A total of 441 patients’ files were reviewed for the study, out of which 418 patients met the criteria for final analysis. Males were in a higher number 284 (67.9 %). The average age was 49±15.8. In our study, 186 (44.4%) patients were phakic at the time of presentation. At 2 months follow up, the primary anatomical success rate was 87.1 %. Conclusions Surgical outcomes of RRD with 25g PPV surgery in our study were similar to the outcomes reported in the developed world. We propose a prospective multicenter national study to prospectively evaluate the risk factors for RRD surgical failure in the Pakistani population.

Purpose: To determine the visual outcomes in patients undergoing pars plana-vitrectomy (PPV) for acute postoperative endophthalmitis after cataract surgery.Study Design: Quasi experimental study.Place and Duration of Study: Al- Ibrahim Eye Hospital, Malir, Karachi for a period of 2 years.Material and Methods: Thirty-three patients were selected using non-probability convenient sampling technique.Patients with acute postoperative endophthalmitis after cataract surgery diagnosed clinically and on B-scan, withvisual acuity of only perception of light were included while patients with better visual acuity and any other typesof endophthalmitis were excluded. PPV was performed in all cases and the patients were examined postoperativelyon 1st day, 3rd day, 1, 2 and 3 weeks after surgery. During these follow ups, visual acuity wasrecorded, detailed slit lamp and fundus examination was performed. Using SPSS version 20.0, chi-square testwas applied to test for significance keeping p-value of < 0.05 as significant.Results: Out of 33 patients 19 (57.6%) were males and 14 (42.4%) were females. Mean age of the patients was54.50 ± 14.14 years. On 1st post-operative day 8 patients (24.2%) showed improvement of visual acuity fromperception of light to finger counting while 02 (6.1%) patients reported visual acuity of 6/60. On final visit at 3rdweek postoperatively, 19 patients (57.6%) had visual acuity of finger counting, 08 patients (24.2%) had handmovement and 06 (18.2%) patients reported a visual acuity of 6/60.Conclusion: PPV for acute post-cataract endophthalmitis results in favourable outcomes in terms of visual acuity.

AbstractTo evaluate corneal endothelium damage with silicone oil (SO) presence in the anterior chamber after pars plana vitrectomy. We investigated the medical records of consecutive 54 eyes of 53 patients undergoing SO removal after pars plana vitrectomy with SO tamponade at Saitama Medical Center, Jichi Medical University, Japan. We recorded SO tamponade retention period, anterior chamber SO with gonioscope, area of SO attachment to the corneal endothelium before SO removal surgery, and the lens status. We then retrospectively investigated the correlation between SO presence in the anterior chamber and the decrease rate of corneal endothelial cell (CEC) density during SO tamponade. The average decrease rate of CEC density was 7.6 (0–38.1) %. The correlation between SO tamponade retention period and decrease rate of CEC density was high (p = 0.0001). However, there was no correlation between anterior chamber SO under gonioscope, SO attaching area, and lens status with the decrease rate of CEC density (p = 0.11, p = 0.93, p = 0.16). No correlation was observed between CEC loss and the existence of anterior chamber SO, although CEC decrease rate was relatively high after a long SO tamponade period. These findings suggest that SO presence in the anterior chamber may not directly injure CEC.
