Background: Ascites could be caused by many underlying diseases, mainly portal hypertension, malignancy, and heart failure. Other etiologies include tuberculosis and pancreatitis. Difficulties in confirming the cause of ascites have been seen in many clinical settings. Ambulatory laparoscopy is one of powerful tools to rule out many etiologies of ascites despite being invasive in its nature. This case report would like to show one case of peritoneal TB presenting with ascites. Patient has undergone many laboratory workups in search of ascites origin. Literature study is done to look for evidence about timing and role of laparoscopy in ascites work-up. Case illustration: A-26-year-old P2 woman came to hospital with a history of vaginal delivery 22 days before admission presenting with massive ascites. No obstetric complication was found. Transvaginal ultrasound revealed normal postpartum uterus and ovaries, surrounded with ascites. Both the liver and kidneys were found normal on ultrasound. Abdominal CT scan with contrast showed massive ascites with thickened omentum. ADA was 36 IU/L. We decided to perform diagnostic laparoscopy and biopsy for histopathology. We found massive yellowish ascites and hyperaemic tubal enlargement with thickening of the peritoneum. Pathology examination proved the appearance of chronic salpingitis and granulomatous peritonitis consistent with tuberculosis peritonitis. Conclusion: Laparoscopy as a minimal invasive tool can diagnose ascites with unknown etiology patients after failure to prove diagnosis from clinical laboratory and radiological examination. It is one best alternative to diagnose peritoneal TB presenting with ascites with its superiority in visualizing abdominal cavity and obtaining specimens for histology with lower risk of morbidity.

This article begins with the assumption that one can gather some picture of the history of man's attitude toward death by examining the literature he has written for children. And so, it proceeds to survey death in children's literature from the early, anonymous works such as Mother Goose, to contemporary picture books like The Dead Bird. This literature is basically divided into four periods: Anonymous, Puritan, Intermediate and Contemporary. At least one representative example and further references follow the explanation of the values and principles of each period. The conclusion points out that, aside from the merits of this survey as a literature study and an indication of man's attitudes towards death through history, it can serve the important role of challenging the reader to examine his own death attitudes.

Background: Abdominal pain was the third most common complaint of individuals often recurrent and needs immediate care. The ability of the method to access and inspect the entire abdominal cavity and retro-peritoneum without harming the structures of it places laparoscopy as the procedure of choice in the diagnosis of abdominal pain. Methods: The study is a retrospective study conducted in Tagore Medical College and Hospital, Chennai during the period of 1st January 2013 to 31st December 2017. Patients of age above 18 years with history of abdominal pain for 6 months or more were included in the study. Basic investigations were also done for the patient. Based on the clinical examinations, patients were subjected to diagnostic laparoscopy. All cases were done as elective surgeries. Results: A total of 48 patients were found in the record, in which the diagnosis remained uncertain despite of all important investigations. The majority of the patients 19 (40%) were in the age group of 21-40 years followed by 17 (36%) in 41-65 years. The duration of pain ranged between 6 months to 1 years. On laparoscopic examination, majority of patients (46%) were diagnosed with chronic appendicitis. Conclusions: Laparoscopy is not only safe, but also quick and effective investigation tool for chronic abdominal pain.

<em>This research is a qualitative study using a library approach, the object is the epistemology of Islamic education, conservative-modernist-neo-modernist and burhani-bayani-irfani. The data collection method in this research uses literature study. In this stage, the researcher tries to select data (books) in the epistemology of Islamic education, conservative-modernist-neo-modernist and burhani-bayani-irfani. In this writing, the analysis used with hermeneutics, namely the author explains that behind the text there is context meaning or behind the explicit meaning there is an explicit meaning if interpreted literally can be interpreted as an interpretation or interpretation of the epistemology of Islamic education, conservative-modernist-neo-modernist and burhani-bayani- irfani. The author also uses content analysis, namely research conducted on information documented in recordings, both images, sounds, writing and others. With this method, data analysis and scientific processing will be carried out on the epistemological content of Islamic education, the conservative-modernist-neo-modernist and burhani-bayani-irfani schools. The results showed that science continues to develop throughout the history of thought such as rational flow of viewing educational activities as actualizing the potentials of individuals so that according to this flow the essence of education is the transformation of various potential potentials into actual abilities. The success of the transformation of the various potentials that exist in this rational flow, is largely determined by how much the optimization of the functions of sense powers and ratios. Baby epistemology is an epistemology that emphasizes the authority of the text as an indicator of truth. Reason in this epistemology is only as a support text and does not have a good space or in other words lower when compared to the text. In object Bayani tradition can be seen in fiqh. Second, Burhani epistemology is knowledge gained from demonstrative analogies which in this case gives the role of reason and senses in human beings. The review of literature presents a brief history of study of adhesive process from the beginning of XX century to the present, the modern concepts of the pathogenesis of adhesive process in the abdominal cavity after surgery on the organs of the small pelvis, taking into account the development of molecular biology, biochemistry, immunology. Problems of study of adhesive process are relevant at this stage of development of surgery of high reproductive technologies and the use of surgical energies, careful approach to the tissues. The processes that occur in the peritoneum after surgery are described in detail. The mechanism of neoangiogenesis in the newly formed growths is presented in detail, conditionally and schematically. The role of growth factors of cytokines, fibrinolysis in the process of formation of adhesions is reflected. The sequence of processes leading to the formation of adhesions is presented in the time interval: from the moment of peritoneal damage during operation up to 12 days of the postoperative period, the time of formation of fibrous adhesions. The structure of connective tissue fibres by components fibres, i.e. the structure of collagen fibers of different types is examined. The review presents conditional classification of anti-adhesive drugs with description of the properties and characteristics by the method of their application, the composition of constituent substances, the forms of release.

Objective To evaluate the role of transvaginal ultrasound triage in women with a history of tamoxifen treatment who present with post-menopausal bleeding. Study design A retrospective review was undertaken of patients who presented with symptoms of post-menopausal bleeding and underwent ultrasound triage. Endometrial thickness and ultrasonographic features were then correlated with hysteroscopic and histopathological outcome data. The findings and outcomes for women with a history of tamoxifen use (tamoxifen group) were compared to those who had not taken tamoxifen (non-tamoxifen group). Results A total of 614 women with post-menopausal bleeding underwent transvaginal ultrasound triage, of whom 53 had a history of current or previous tamoxifen treatment. An endometrial thickness of ≥5 mm or the presence of other abnormal features was used to triage women to further investigation by hysteroscopy and biopsy. Endometrial thickness was significantly greater in the tamoxifen group (11 mm vs. 6 mm). Nearly all of the tamoxifen group were triaged to further investigation (98.1%), compared with significantly fewer in the non-tamoxifen group (68.3%) Overall, the incidence of endometrial pathology was also significantly higher in tamoxifen patients (43.4% vs. 31.7%). Conclusion For women presenting with post-menopausal bleeding, the use of transvaginal ultrasound as a triage tool is rarely helpful in evaluating women who have a history as tamoxifen use, as most will require further investigation with hysteroscopy and biopsy. A post-menopausal bleeding protocol that omits transvaginal ultrasound for women with a history of tamoxifen use may be an appropriate and effective pathway for managing these patients.
