Background: Many approach have been tried for management of vestibular fistula, most of them have post operative complication like wound infection, wound dehiscence, more hospital stay which compromise the functional out come and aesthetic appearance of the perineum. Objective: The aim of present study was to see the effectiveness of Transfistula Anorectplasty (TFARP) Which was required minimal dissection without interruption of perineal body and perineal skin. Methodology: The cross sectional study on 43 patients with vestibular fistula were held in Dhaka Shishu Hospital from September 2008 to April 2010. Patients were operated after obtaining informed consent and standard bowel preparation. Data on demographics, operation time and postoperative complications were collected as a questionnaire and plotted on Microsoft Excel and analyzed systematically by SPSS version 17. Patients were followed up for a period of 2½ months postoperatively. Results: There is no statistical deference in patient population regarding age, geographical distribution, and clinical presentation. Mean operation time was 76.5 min for TFARP and 84.34 min for ASARP. Two (02) cases had wound infection after TFARP operation and 11 patients after ASARP operation. One patient develops partial wound dehiscence after TFARP and was healed after conservative treatment within 7 days. On the other hand 05 patients developed partial wound dehiscence and 04 patients developed complete wound disruption after ASARP which were also treated conservatively need more than 2 weeks on an average. Mean hospital stay were 6.45 days after TFARP operation and mean hospital stay were 7.87 days after ASARP operation. 20 neonates and infant who were treated by TFARP operation have good bowel movement without laxative and symmetrical anal contraction after stimulation. Conclusion: TFARP is an operation produce less morbidity and is more effective procedures and superior procedures than that of ASARP operation and give aesthetic appearance of the perineum. OBJECTIVES: Nobody is refrained from the use of medications in today's times, not even kids,The objective of this study is to focus on the knowledge and awareness level, perception, of children of primary and secondary grade towards medicines , prevention of Infections and Diseases and drug compliance. This descriptive, cross sectional observational study was done METHODS: amongst the kids of North West Rajasthan ,India from August 2020 to October 2020 via an online questionnaire . A sample size of 100 kids , 50 each from Primary and Secondary grade was chosen for the study. The data was entered in Microsoft Excel and was statistically analysed. Results are outcomes of various RESULTS: questions put up to conduct detailed analysis of kids' awareness and medicinal knowledge, drug preferences, factors inuencing and attitude related to hygiene, safety and protection from Covid 19 infection. Kids are the young budding phys CONCLUSION: icians and their childhood beliefs and perceptions might affect their behaviour and decisions in adult lives. Parents and schools can play a great role through educational programs . It's well said, “Children are like wet cement, whatever falls on them, leaves an impression.”

Introduction: Acute kidney injury is a common and life-threatening event in patients with liver cirrhosis occurring in approximately 20-50% of hospitalized patients of liver cirrhosis. Pre-renal acute kidney injury, the hepatorenal syndrome type of acute kidney injury and acute tubular necrosis represent the common causes. The aim of this study was to study the profile of acute kidney injury in patients with liver cirrhosis. Methods: Consecutive patients of liver cirrhosis admitted in Liver unit of Bir Hospital were studied to see the presence of acute kidney injury in this hospital based descriptive cross-sectional study. Clinical and laboratory parameters along with various clinical outcome were compared between different groups categorized by the severity of liver disease and renal dysfunction. Results: Out of 302 liver cirrhosis patients, 56 (18.5%) had acute kidney injury among which 23 (46%) were found to have pre-renal acute kidney injury, 15 (30%) with hepatorenal syndrome– acute kidney injury and 12 (24%) with intrinsic renal disease. Patients with higher stages of acute kidney injury had longer duration of hospital stay and hepatorenal syndrome-acute kidney injury was seen in patients with higher grade of ascites and with hyponatremia. Conclusions: Acute kidney injury is a common occurrence in patients with advanced liver cirrhosis with pre-renal acute kidney injury being the commonest cause. Median hospital stay is directly affected by the severity of acute kidney injury and hepatorenal syndrome–acute kidney injury was seen in patients with higher grade of ascites and hyponatremia. Early identification of patients at high risk for acute kidney injury may help to reduce mortality and contain costs.

RESUMO Objetivo: descrever os diagnósticos de Enfermagem segundo a taxonomia da American Nursing Diagnosis Association II.  Método: trata-se de estudo quantitativo, descritivo, transversal, com 51 produtores de tabaco expostos a agroquímicos que viviam na comunidade rural. Coletaram-se os dados por meio das consultas de Enfermagem. Tabularam-se os dados n Programa Microsoft Excel 2010, posteriormente inseridos no banco de dados utilizando o programa SPSS, versão 22.0, for Windows, e submetidos à revisão e análise estatística. Apresentaram-se os resultados em tabelas. Resultados: verificou-se, por meio das consultas de Enfermagem, que a maioria era do sexo feminino, tinha ensino fundamental incompleto e não praticava nenhum tipo de exercício físico. Acrescenta-se, além disso, que eles desconheciam os riscos para a saúde colocados pelos pesticidas e negligenciaram os padrões de segurança recomendados para o manuseio desses produtos. Conclusão: pode-se observar, a partir deste estudo, que o enfermeiro tem um papel importante no cuidado e na educação, nos diversos espaços de promoção, prevenção e manutenção do cuidado em saúde. Method: this is a quantitative, descriptive, cross-sectional study conducted with co plantations. The research was carried out with 51 tobacco growers exposed to agrochemicals living in the rural community. The data were collected through nursing consultations. The data collected were tabulated in the Microsoft Excel 2010 software and later entered into a database using the SPSS software version 22.0, for Windows, and subjected to statistical review and analysis. The results were presented in tables. Results: the Nursing consultations revealed that the majority of the farm workers were females, had incomplete elementary education, and did not practice any type of physical exercise. In addition, they were unaware of the health risks posed by pesticides and neglected the recommended safety standards for the handling of these products. Conclusion: it was observed in this study that nurses have an important role in care and education, in the various spaces of promotion, prevention and maintenance of health care. Objective: To compare the efficacy of LigaSure Vessel Sealer in Near Total Thyroidectomy versus Conventional Clamp Knot Tie Technique in terms of bleeding, operative time and postoperative drainage. Methodology: This comparative cross sectional study was conducted at Department of Surgery, Liaquat University of Medical and Health Sciences, Jamshoro. Study duration was one year from November 2019 to October 2020. All patients of any age with benign multinodular goiter and either of gender were included. The study subjects were grouped into two categories by randomization (odd / even). The odd numbers were given to patients operated for ligasure and even numbers were given to patients operated with conventional clamp knot tie technique.  Outcomes were observed with respect to post-operative calcium level, intra-operative bleeding, operative time, post-operative pain & post-operative hospital stay. All the data was recorded via study proforma. Data was analyzed by using SPSS version 20. Results: Total 55 patients were observed. Mean age was 33.25±10.60 years in clamp knot tie procedure group and 35.16±07.96 years in ligasure technique group; without significant difference (p-0.448). Pre and post-operative calcium levels were statistically insignificant among both groups (p-0.358 and 0.163), while loss of blood, hospital stay, post-operative pain and operative duration were significantly greater in clamp knot tie technique group in comparison to ligasure technique group (p-<0.001). Conclusion: LigaSure Vessel Sealer is a feasible and reliable surgical technique and significantly more effective as compared to conventional clamp knot tie technique in terms of post-operative bleeding, operative time, post-operative pain and post-operative hospital stay. However, calcium level was statistically insignificant.

A cross-sectional study of secondary data/information obtained from the Hospital Information System (HIS) spanning the years 2008 - 2009 was performed. The distribution of the main hospital admissions by gender, age, color/race, region and federal unit of residence, average expenditure and average length of hospital stay, year of hospitalization and mortality rates (MR) were studied. The data collected were tabulated by TabNet and keyed into Microsoft Excel 2007. It was verified that elderly males (54.3%), from 60 to 69 years old (50.6%), nonwhites (36.3%) and residents of Southeast and North regions of the country had the highest rates of hospitalization. Seniors were hospitalized for an average of 4.8 days, and the major causes were exposure to alcohol (43.7%) and to drugs (33.9%). Expenses related to hospital admissions were, on average, R$ 529,817.70. The highest mortality rates were recorded among females (MR = 4.34), in elderly, 80 years or older (MR = 10.16) and Caucasians (MR = 3.95), where pharmacological substances with action on the Autonomic Nervous System were the leading cause of death. There are demographic differences in morbi-mortality of these elderly since, although men and younger elderly were the main victims, women and elderly of advanced age have greater mortality. The leading causes of hospitalization were alcohol and drugs.

Background: India is currently experiencing rapid epidemiological transition with rising prevalence of obesity which may be due to sedentary lifestyle and changing dietary pattern. Certain occupations like the job of bank employees are sedentary which predispose individuals to obesity. Hence the present study was carried out to study the prevalence of obesity in bank employees in Latur city of Maharashtra.Methods: A cross-sectional study was carried out amongst 400 bank employees of Latur city. Banks were listed according to sectors i.e. government, co-operative and private. The study subjects were interviewed by predesigned and pretested proforma including bio-social characteristics like age, gender, height, weight etc. The anthropometric measurements were taken like height, weight etc. Data was entered in Microsoft Excel and analysed by using appropriate test whenever necessary.Results: According to body mass index (BMI) classification by WHO classification 34.5% subjects were overweight and obese and 25.75% of bank employees had obesity by waist-hip ratio as per WHO classification and 18.5% of bank employees had isolated abdominal obesity.Conclusions: The prevalence of generalized as well as abdominal obesity by WHO classification was more in bank employees. The associated risk factors like age, gender and designation was found to be significant.

Background: Laparoscopy surgery trials are small and unconvincing at present and are limited to higher centers. The objective of the study is to determine the clinical features, prevalence of site of hydatid cyst and complications of this modality of this treatment.Methods: A cross sectional study was carried out in all patients with one or two hepatic hydatid cyst who underwent laparoscopic management in KMCTH from January 2013 to March 2015 were included in the study. Aspiration, deroofing and evacuation of the hydatid cyst were done.Results: Twenty six patients underwent laparoscopic management for liver hydatid cysts. Males were seven (65.38%) and females were 9(34.61%).The mean age was 35.5±13.1 years (range 21-55years.) The commonest complaint was pain and discomfort in 13(50%) patients and lump in 6(13.06%) patients. Twenty four (92.3%) patients were successfully treated with laparoscopic approach. Two (7.69%) patients had to be converted to laparotomy because of dense adhesions and bleeding. Mean operation time was 43.6±10.6 minutes. Two (7.69%) patients had port site infection. One (3.84%) patient had bile leak and no recurrence and mortality in our series.Conclusions: Laparoscopic management of liver hydatid cyst was safe and effective in selective group of patients in equipped hospital.

Background: Emergency caesarean section is one of the commonly performed operation in the department of gynae and obstetrics.Objectives: The aim and objective of the study was to evaluate the organisms responsible for post emergency caesarean wound infection and their antibiotics sensitivity.Materials & Methods: The study is a prospective type of cross sectional study where 100 patient of post emergency caesarean wound infection was evaluated during July 2012 to December 2012 (6 Month Duration) at Rajshahi Medical College Hospital (RMCH) Rajshahi. The wound swab was sent for culture and sensitivity test.Results: The peak incidence of wound infection to between 6th - 8th post operative day. In wound discharge shows various organism but 16% showed no growth. The most of the infected wound showed (48%) thick creamy pus and the organisms isolated is Staphylococcus. antibiotics mostly used were Ciprofloxacin, Cephradine and Metronidazole. This study revealed that maximum wound infection is due to S. aureus. Regarding antibiotic sensitivity of the cultured organisms, all of them are sensitive to Imepenem (100%). Staphylococcus aureus mostly sensitive to Flucloxacillin (79.16%). Ciprofloxacin sensitivity is found in 58.33% cases. E coli mostly sensitive to Imepenem (100%) followed by Ceftriaxone (53.84%). Klebsiella pneumonia found mostly sensitive to Imepenem (100%) followed by Ceftriaxone (75%) and Cephradine (50%).Conclusion: Selection of appropriate antibiotic is important to reduce post caesarean wound infection.KYAMC Journal Vol. 9, No.-2, July 2018, Page 61-64
