Background and Objectives Self-reported hearing levels determined by self-report questionnaires are not always correlated with the audiometric hearing level. It is necessary to ascertain the subjectively perceived normal hearing threshold to determine the need for hearing aids at the appropriate time. The objective is to identify the level of correlation between the self-reported hearing level and the audiometric hearing level and ascertain the subjectively perceived normal hearing threshold for each age group.Subjects and Method This cross-sectional study, using the data from the Korea National Health and Nutrition Examination Survey V, looked at participants with hearing loss who were aged ≥40 years. Pure tone audiometry was conducted bilaterally. Self-reported hearing level was ascertained via survey enquiring about hearing loss. The level of correlation between self-reported hearing level and audiometric hearing level, age-specific patterns of hear-ing self-evaluation and subjectively perceived normal hearing values at which participants complained of hearing loss were main outcomes.Results Mean audiometric thresholds (MATs) increased with the age and degree of self-reported hearing level. The accuracy of self-reported hearing level compared to the audiometric threshold decreased with age both in the better-ear and worse-ear groups. Younger participants tended to overestimate their hearing impairment, while older participants tended to underestimate it. Subjectively perceived normal hearing thresholds increased with age and they were very close to the MATs for each age group.Conclusion Results can be useful for devising different age-group specific pure tone audi-ometry-based diagnostic criteria for self-reported hearing loss, which can be used to determine the need for hearing aid in a timely manner.

A cross sectional study was done in the department of otolaryngology of Central Police Hospital. Rajarbagh, Dhaka., during the period of January 2005 to December 2007 to find out noise induced hearing loss of traffic polices and to raise awareness for early diagnosis and treatment and also preventive measures of noise induced hearing loss. 'Thirty patients who were diagnosed as a case of hearing impairment by detailed history, clinical examination and related investigations were included in the study who were exposed to noise 0-20 years. Common chief complaints of the patients were impairment of hearing (100%), Minims (50%), dizziness (20%), aural discharge (6.66)%. Medical history of 11w patients were diabetes (6.66%) and hypertension (26.66 %). 40 % patients were smoker and 60 % patients were non smoker. A ssoc jar ed other disease like chronic supporative otitis media (CSOM) t vas present in 13.33 %. Pure tone audiometry revealed normal hearing 3.3 %, conductive hearing loss 10 %, sensorineural hearing loss 73.33 %, mixed hearing loss 13.33 % and normal hearing 6.66 %, mild hearing loss 50 To, moderate type of hearing loss 36.66 % and moderately severe 3.33 %, severe hearing loss were 3.33 %. Treatment of the patients were given by vitamin 13,, Lli, B6, cochlear vasodilators such as vinpocetine and hearing aids. Patients were advised to Hume to other department of police service. those traffic police persons are vulnerable of noise induced hearing loss determined by patients complain, clinical examination and investigations like PTA, were advised to be changed to other department of police service.

Abstract: Diabetes mellitus is a group of chronic metabolic disease which can affect nearly every organ system in the body. Complications of this disease are diverse and include retinopathy, nepropathy and neuropathy. It has a high prevalence and continued to increase. The relationship between diabetes mellitus and hearing loss have been studied. This study aimed to obtain the average of hearing threshold in patients with diabetes mellitus. This was a descriptive observational study with a cross sectional design. Total 38 diabetes mellitus patients were included in the study. Hearing threshold obtained based on air conduction pure tone audiometry average at 500, 1000, 2000, and 4000 Hz. The results showed that subjects were 65.8% females and 32.4% males. There were 9 subjects (23.6%) with normal hearing, 24 subjects (63.2%) with bilateral hearing loss, and 5 subjects (13.2%) with unilateral hearing loss. Of the 29 subjects with hearing loss, the levels were mild and moderate. None of the subjects had moderately severe, severe, or profound. Conclusion: The majority of subjects in this study had hearing loss.Keywords: diabetes mellitus, hearing threshold, pure tone audiometryAbstrak: Diabetes melitus merupakan suatu kelompok penyakit metabolik kronik yang dapat mempengaruhi hampir setiap sistem organ dalam tubuh. Komplikasi penyakit ini beragam, termasuk retinopati, nefropati dan neuropati. Prevalensinya cukup tinggi dan diperkirakan akan terus meningkat. Terdapat beberapa penelitian yang menghubungkan diabetes melitus dan gangguan pendengaran. Penelitian ini bertujuan untuk mengetahui ambang pendengaran rata – rata pada penderita diabetes melitus. Penelitian ini merupakan penelitian deskriptif observasional dengan desain penelitian potong lintang. Sampel total ialah 38 pasien diabetes melitus. Ambang pendengaran rata – rata diperoleh berdasarkan hantaran udara audiometri nada murni rata-rata pada frekuensi 500, 1000, 2000, dan 4000 Hz. Hasil penelitian memperlihatkan bahwa subjek penelitian 65,8% perempuan dan 34,2% laki – laki. Terdapat 9 orang (23,6%) yang mempunyai pendengaran normal, 24 orang (63,2 %) mengalami gangguan pendengaran bilateral dan 5 orang (13,2%) dengan gangguan pendengaran unilateral. Dari 29 subjek penelitian dengan gangguan pendengaran mengalami gangguan pendengaran kategori ringan dan sedang. Tidak ditemukan subjek penelitian dengan gangguan pendengaran kategori sedang berat, berat atau sangat berat. Simpulan: Mayoritas subjek penelitian mengalami gangguan pendengar.Kata kunci: ambang pendengaran, audiometri nada murni, diabetes melitus

Background: Prisoners, due to confinement are isolated from contact with the society and access to many of the facilities, including medical care. This study aimed to evaluate the hearing threshold of inmates of Kaduna convict prison. Method: It was a cross-sectional study of prison inmates at the Kaduna convict prison between April 2017 and February 2019. Ethical approvals were obtained from relevant bodies and all consented inmates aged 18 – 55years in the Kaduna convict prison were enrolled. Equal number of control matched for age and gender were enrolled from the communities in Kaduna North Local Government Area. Data were collated using a structured questionnaire. A diagnostic Pure Tone Audiometry was performed to assess their hearing threshold. Statistical Package for Social Sciences (SPSS) version 20.0 was used for analysis. Results: Four hundred and thirty inmates and equal number of control group were enrolled. The mean age for the inmates and controls were 30.2±7.5 and 30.4±8.02 years respectively. There were 383 males and 47 females in both groups. Among the 860 ears of the inmates, 238(27.7%) ears had hearing loss while in the control group, 95/ (11.1%) ears had impaired hearing. Conductive hearing loss was the commonest among the inmates 111(46.6%) while sensorineural was commoner among the controls 57(60.0%). The mean pure tone average among the inmates was 25.6±11.3dBHL and 26.1±11.2dBHL on the right and left ears respectively while in the control group, it was 18.4±7.8dBHL on the right and 17.9±7.9dBHL on the left. Conclusion: This study revealed that hearing loss was more prevalent among prison inmates than the general population. In majority of the inmates, the hearing loss was mild, conductive and mostly affecting all the frequencies.

Introduction: Noise-induced hearing loss is a significant, often unrecognized health problem among the military pilots. Advanced aircraft are powerful and more efficient but generate high noise causing noise-induced hearing loss (NIHL) among military pilots. Aim: To determine the prevalence of noise-induced hearing loss among pilots of the Bangladesh Air Force (BAF) and its association with aircraft type and flying hours. Methods: This cross-sectional study was carried out at Central Medical Board (CMB), Bangladesh Air Force (BAF) Dhaka from April 2017 to March 2018. The audiometry was done in the ENT department of CMB, BAF. Pure-tone audiometry was used to measure hearing thresholds and following pure tone frequencies were used for the test: 0.25, 0.5, 1, 2, 3, 4, 6 and 8 kHz. The association between flight hours and the development of noise-induced hearing loss was analyzed in the different age groups. Results: The study revealed NIHL in the aircrew was 16.5% with a higher prevalence among the helicopter pilots (18.6%) as compared with fighter (16.7%) and transport (11.5%) pilots. The left ear was affected in most of the cases than the right ear. Pilots of more than 40 years old and ≥1500 flying hours had a significantly higher hearing threshold compared with other categories. Conclusion: Exposure to noise in aircrew is unavoidable. By avoiding prolonged exposure to loud noise environments and utilization of personal hearing safety devices can prevent noise-induced hearing loss. JAFMC Bangladesh. The etiology of hearing misfortune is multifactorial which makes the analysis a difficult exercise and frequently an unsolved puzzle. While unadulterated tone audiometry is an obligatory examination, others, for example, research facility and radiology depend on clinical assessment. This current report's on the examination of connection between self-detailed commotion presentation amid relaxation activities and audiological indicators including measured hearing threshold levels (HTL) and tinnitus. The research was conducted by cross sectional survey of 1432 individuals from 11 to 35 years old. Methodology included of a full audiometric assessment including otoscopy, pure tone audiometry (PTA), otoacoustic emission (OAE) and tympanometry, hearing loss investigation, diagnosis, evoked response audiometry radiology, deafness.

Abstract Background Achondroplasia is the most common form of disproportionate skeletal dysplasia. The condition is caused by a mutation in the FGFR3 gene, affecting endochondral bone growth, including the craniofacial anatomy. Recurrent otitis media infections, chronic middle ear effusion, and hearing loss are common in children with achondroplasia, but few studies have investigated hearing loss in adults with this condition. Objectives This population-based study investigated the prevalence, severity, and type of hearing loss in Norwegian adults with achondroplasia. Methods We collected data on 45 adults with genetically confirmed achondroplasia: 23 men and 22 women, aged 16–70 years. All participants underwent a comprehensive audiologic assessment, including medical history, pure-tone audiometry, speech audiometry, and impedance audiometry. According to the Global Burden of Disease classification, pure-tone average ≥ 20 decibel hearing level (dB HL) was considered clinically significant hearing loss. Results Insertion of ventilation tubes had been performed in 44% (20/45) of the participants, 49% (22/45) had a history of adenoidectomy, while 20% (9/45) used hearing aids. Hearing loss in at least one ear was found in 53% (24/45) of the participants; in 57% (13/23) of the men and 50% (11/22) of the women. In the youngest age group (age 16–44 years), 50% (14/28) had hearing loss, although predominantly mild (20–34 dB HL). An abnormal tympanometry (Type B or C) was found in 71% (32/45) of the participants. The majority (15/24) had conductive hearing loss, or a combination of conductive and sensorineural hearing loss (8/24). Conclusions Adults with achondroplasia are at increased risk of early hearing loss. Our findings underline the importance of a regular hearing assessment being part of standard care in achondroplasia, including adolescents and young adults. In adult patients diagnosed with hearing loss, an evaluation by an otolaryngologist should be considered, and the need for hearing aids, assistive listening devices, and workplace and educational accommodations should be discussed. Background Sudden sensorineural hearing loss (SSNHL) is an otologic emergency that warrants urgent management. Pure-tone audiometry remains the gold standard for definitively diagnosing SSNHL. However, in clinical settings such as primary care practices and urgent care facilities, conventional pure-tone audiometry is often unavailable. Objective This study aimed to determine the correlation between hearing outcomes measured by conventional pure-tone audiometry and those measured by the proposed smartphone-based Ear Scale app and determine the diagnostic validity of the hearing scale differences between the two ears as obtained by the Ear Scale app for SSNHL. Methods This cross-sectional study included a cohort of 88 participants with possible SSNHL who were referred to an otolaryngology clinic or emergency department at a tertiary medical center in Taipei, Taiwan, between January 2018 and June 2019. All participants underwent hearing assessments with conventional pure-tone audiometry and the proposed smartphone-based Ear Scale app consecutively. The gold standard for diagnosing SSNHL was defined as the pure-tone average (PTA) difference between the two ears being ≥30 dB HL. The hearing results measured by the Ear Scale app were presented as 20 stratified hearing scales. The hearing scale difference between the two ears was estimated to detect SSNHL. Results The study sample comprised 88 adults with a mean age of 46 years, and 50% (44/88) were females. PTA measured by conventional pure-tone audiometry was strongly correlated with the hearing scale assessed by the Ear Scale app, with a Pearson correlation coefficient of .88 (95% CI .82-.92). The sensitivity of the 5–hearing scale difference (25 dB HL difference) between the impaired ear and the contralateral ear in diagnosing SSNHL was 95.5% (95% CI 87.5%-99.1%), with a specificity of 66.7% (95% CI 43.0%-85.4%). Conclusions Our findings suggest that the proposed smartphone-based Ear Scale app can be useful in the evaluation of SSNHL in clinical settings where conventional pure-tone audiometry is not available.

Abstract Age-related hearing loss (HL), defined by a pure-tone average (PTA) >25 decibels (dB) has been associated with depressive symptoms. We aimed to assess whether this association is present when hearing is better than the arbitrary, but widely-used, 25 dB threshold. The sampled population was the multicentered Hispanic Community Health Study (n=5,165). Cross-sectional data from 2008-2011 were available. Hearing was measured with pure tone audiometry. Clinically-significant depressive symptoms (CSDS) were defined by a score ≥10 on the 10-item Center for Epidemiologic Studies Depression Scale (CESD-10). Participants’ mean age was 58.3 years (SD=6.2, range=50-76). Among those with classically-defined normal hearing (PTA ≤25 dB), a 10 dB increase in HL was associated with 1.26 times the odds (95% CI=1.11, 1.42) of CSDS, adjusting for age, gender, education, vascular disease, and hearing aid use (p25 dB; p<0.001). Results held even for a stricter HL cutpoint of 15 dB. Among subjects with strictly normal hearing (PTA ≤15 dB), a 10 dB increase in HL was associated with 1.47 (1.14, 1.90) times the odds of CSDS, adjusting for confounders (p<0.01). Results also held when defining CSDS by an alternative CESD-10 score ≥16. In conclusion, increasing hearing thresholds were independently associated with CSDS among adults with subclinical HL (PTA ≤25 dB). Studies investigating whether treating HL can prevent late life depression should consider a lower threshold for defining HL.

AbstractHearing loss is often associated with the phantom sound of tinnitus. However, the degree of the association between severity of hearing loss and tinnitus loudness taking into account the impact of other variables (e.g., emotional disturbances) is not fully understood. This is an important question for audiologists who are specialized in tinnitus rehabilitation as patients often ask whether the loudness of their tinnitus will increase if their hearing gets worse.To explore the relationship between tinnitus loudness and pure tone hearing thresholds.This was a retrospective cross-sectional study.445 consecutive patients who attended a Tinnitus and Hyperacusis Therapy Specialist Clinic in UK were included.The results of audiological tests and self-report questionnaires were gathered retrospectively from the records of the patients. Multiple-regression analysis was used to assess the relationship between tinnitus loudness, hearing loss and other variables.The regression model showed a significant relationship between the pure tone average (PTA) at the frequencies 0.25, 0.5, 1, 2, and 4 kHz of the better ear and the tinnitus loudness as measured via visual analogue scale (VAS), r (regression coefficient) = 0.022 (p < 0.001). Other variables significantly associated with tinnitus loudness were tinnitus annoyance (r = 0.49, p < 0.001) and the effect of tinnitus on life (r = 0.09, p = 0.006). The regression model explained 52% of the variance of tinnitus loudness.Although increased tinnitus loudness was associated with worse PTA, the relationship was very weak. Tinnitus annoyance and impact of tinnitus on life were more strongly correlated with tinnitus loudness than PTA.

Background.Keratitis-ichthyosis-deafness (KID) syndrome is a syndrome which presents with hearing loss and visual and keratinization disorders. In such patients, hearing aids cannot be effectively used in the rehabilitation of hearing loss because of the frequent blockage of the external ear canal with epithelial debris and due to dry and tense skin of the external ear canal. Moreover, severe or profound hearing loss also limits the benefits gained from the conventional hearing aids. On the other hand, cochlear implantation is a method that has been used in limited cases in the literature.Case Report.This study presents the results of cochlear implantation applied in our clinic to two children who had been diagnosed with KID. Audiological assessments before and after the cochlear implant operation were performed using pure-tone audiometry, immittance audiometry, and auditory brainstem response (ABR), and the postoperative follow-up was conducted using pure-tone audiometry.Conclusion.Skin problems, visual disturbances, and other additional problems complicate the short-term and long-term rehabilitation after implantation in individuals with KID syndrome. Close monitoring should be exercised due to possible skin complications that may develop during the postoperative period. The families and rehabilitation teams should be warned about the possible visual disturbances and skin complications.
