Reducing the burden of pain via greenspace exposure is a rising research topic. However, insufficient evidence has been found in relation to the environmental effect itself. Residential greenspace, as a convenient but limited natural environment for urban dwellers, has benefits and services yet to be discovered. Therefore, the current study recruited 24 young adults to evaluate the effects of physical visit to, or image viewing of, residential greenspace on pain perception and related psychophysiological outcomes, via simulated pain. Pain threshold and tolerance were recorded via the level of pain stimuli, and pain intensity was evaluated using the Visual Analog Scale (VAS). The state scale of the State–Trait Anxiety Inventory (STAI-S) and two adjective pairs were employed to measure the state anxiety and subjective stress, respectively. Meanwhile, heart rate (HR), heart rate variability (HRV), and blood pressure (BP) were measured to investigate physiological responses. Besides, Scenic Beauty Estimation (SBE) was also employed to assess participants’ preference regarding the experimental environments. The results revealed that visiting the greenspace significantly increased the pain threshold and tolerance, while no significant effect was observed for image viewing. On the other hand, no significant difference was observed in pain-related psychophysiological indices between the experimental settings, but significantly negative associations were found between the scores of SBE and subjective stress and state anxiety. In conclusion, the current study brings experimental evidence of improving pain experience via residential greenspace exposure, while the related psychophysiological benefits require further investigation.

Recent studies have revealed evidence for increased pain sensitivity in individuals with chronically low blood pressure. The present trial explored whether pain sensitivity can be reduced by pharmacological elevation of blood pressure. Effects of the sympathomimetic midodrine on threshold and tolerance to heat pain were examined in 52 hypotensive persons (mean blood pressure 96/61 mmHg) based on a randomized, placebo-controlled, double-blind design. Heat stimuli were applied to the forearm via a contact thermode. Confounding of drug effects on pain perception with changes in skin temperature, temperature sensitivity, and mood were statistically controlled for. Compared to placebo, higher pain threshold and tolerance, increased blood pressure, as well as reduced heart rate were observed under the sympathomimetic condition. Increases in systolic blood pressure between points of measurement correlated positively with increases in pain threshold and tolerance, and decreases in heart rate were associated with increases in pain threshold. The findings underline the causal role of hypotension in the augmented pain sensitivity related to this condition. Pain reduction as a function of heart rate decrease suggests involvement of a baroreceptor-related mechanism in the pain attrition. The increased proneness of persons with chronic hypotension toward clinical pain is discussed.

Background: This pre-registered study extends previous findings that swearing alleviates pain tolerance by assessing the effects of a conventional swear word (“fuck”) and two new “swear” words, “fouch” and “twizpipe”.Method: A mixed sex group of participants (N = 92) completed a repeated measures experimental design augmented by mediation analysis. The independent variable was Word with the levels, “fuck” v. “fouch” v. “twizpipe” v. a neutral word. The dependent variables were emotion rating, humour rating, distraction rating, cold pressor pain threshold, cold pressor pain tolerance, pain perception score and change from resting heart rate. Possible mediation effects were assessed for emotion, humour and distraction ratings. Results: For conventional swearing (“fuck”), confirmatory analyses found a 32% increase in pain threshold and a 33% increase in pain tolerance, accompanied by increased ratings for emotion, humour and distraction, relative to the neutral word condition. The new “swear” words, “fouch” and “twizpipe” were rated higher than the neutral word for emotion and humour although these words did not affect pain threshold or tolerance. Changes in heart rate, pain perception and were absent, as were mediation effects.Conclusions: Our data replicate previous findings that repeating a swear word at a steady pace and volume benefits pain tolerance, extending this finding to pain threshold. Our data cannot explain how such effects are manifest, although distraction appears to be of little importance, and emotion is worthy of future study. The new “swear” words did not alleviate pain even though participants rated them as emotion evoking and humorous.

This study compared the exergame beach volleyball’s acute effects on state anxiety level in single vs. multiplayer mode in adult men. Sixty adult men (age: 21.98 [4.58] years, body mass: 75.40 [15.70] kg, height: 1.77 [0.09] m, and body mass index: 24.19 [5.44] kg/m2; data are expressed as median [interquartile range]) were assigned to play exergame of beach volleyball in single- or multiplayer mode for approximately 30 min using the Xbox 360 Kinect®. The state anxiety level was evaluated before and after the intervention. There was no significant difference in the state anxiety levels after an exergame session between the single and multiplayer modes (p-value = 0.407, effect size (rB) = −0.12, defined as small). Furthermore, there was no significant difference in the state anxiety levels before and after an exergame session in single-player mode (p-value = 0.516, effect size (d) = 0.14, defined as trivial) and multiplayer mode (p-value = 0.053, rB = 0.43, defined as medium). In conclusion, state anxiety level after exergame beach volleyball did not differ between the single and multiplayer modes in adult men.

Background/aims:To test whether patients with Parkinson’s disease (PD) have an altered sensory perception in response to painful and nonpainful stimuli and if the PD medications can influence the responses.Methods:PD patients (12) and healthy subjects (12) were recruited. Sensory perception was examined in forearms, low back, and hands by light brush, pinprick, cold pressure test (CPT), and pressure algometry.Results:A significant difference was found in PD patients in response to brush (p<0.05) and pinprick (p<0.001). Lower back and forearms were found to be more sensitive. Tolerance time in CPT was shorter in PD patients (p=0.016). Pressure pain threshold (PPT) before (p=0.011) and after (p=0.050) the CPT showed a higher sensitivity in the patients regardless of the site. No association was found between the sensory perception and the medications.Conclusion:PD patients demonstrated an altered perception to touch and pain stimuli with a general increased in pain intensity and independent of the PD medications.

The aim of this study is to describe the short-term effects of manual lymph drainage (MLD) isolated in supraclavicular area in healthy subjects. A 4-week cross-sectional, double-blinded randomized clinical trial was conducted. Participants: 24 healthy participants between 18 and 30 years old were recruited from Universidad Europea de Madrid from December 2018 to September 2019. A total of four groups were studied: control, placebo, Vodder, and Godoy. The order of the interventions was randomized. Resting Heart Rate and Oxygen Saturation, blood pressure, pressure pain threshold of trapezius muscle, respiratory rate, range of active cervical movements were measured before and after every intervention. All the participants fulfilled four different interventions with a one-week-wash-out period. No statistically significant differences were found between groups in descriptive data; neither in saturation of oxygen, diastolic blood pressure and cervical range of motion. Significant differences were found in favor of Vodder (p = 0.026) in heart rate diminution and in cardiac-rate-reduction. A significant difference in respiratory rate diminution is found in favor of the Godoy group in comparison with the control group (p = 0.020). A significant difference is found in favor of the Godoy group in systolic blood pressure decrease (p = 0.015) even in pressure pain threshold (p < 0.05). MLD decreases systolic blood pressure in healthy participants. However, it does not produce any changes in other physiologic outcomes maintaining physiologic values, which may suggest the safety of the technique in patients suffering from other pathologies.

The question of whether the perception of externally applied pain stimuli and the perception of interoceptive stimuli are based upon similar neuronal or behavioral processes is still open to debate. If such a similarity exists, one would expect that persons who are highly sensitive to visceral stimuli would also exhibit a higher sensitivity to pain stimuli. The present study investigated the sensitivity to heat pain of individuals with high vs. low cardiac interoceptive sensitivity. Pain threshold and pain tolerance were assessed using a testing-the-limits procedure. Furthermore, participants rated the subjective intensity and unpleasantness of tonic heat stimuli on visual analog scales and in a questionnaire. The results show that pain experience did not differ between participants with high and low interoceptive sensitivity, nor did correlation analyses reveal significant relationships between pain experience and cardiac interoceptive sensitivity. These findings suggest that the pathway of peripheral pain processing is at least partially independent of the pathway of interoceptive sensations.

To assess temporal variations in the perception of “phasic” heat pain stimuli a psychophysical tracking procedure was developed that enables repeated assessment of the pain threshold at short intervals. This “double-tracking” procedure produces two tracking curves simultaneously, one that approaches the pain threshold gradually from above, the other from below. The threshold for phasic heat pain was measured in 80 tracking trials with stimuli at temperatures near the pain threshold. Concurrently, the threshold for “tonic” heat pain was determined after every 20 tracking trials with a stimulus adjustment procedure. Eleven healthy subjects (age: 26.4 yr. ± 6.0) participated in 2 sessions each. Phasic stimulation near the pain threshold did not produce any trends in either of the two threshold measures. Hence there was no long-term adaptation or sensitization. However, there were random variations (random walks) in the tracking curves, which we interpret as resulting from a stochastic relationship between stimulus and sensation. In agreement with other reports, discrimination seemed to be better at painful than at nonpainful temperatures.

In the presented work, we have assessed the features of peripheral oxygenation in children with congenital malformations of the surgical profile under various types of combined anesthesia. The aim of the study. To assess the state of peripheral oxygenation in newborns and infants with congenital malformations with various types of anesthetic support. Materials and research methods. A retrospective study included 150 newborns and infants with congenital malformations of the surgical profile, depending on the anesthesia (inhalation + regional anesthesia; inhalation + intravenous anesthesia and total intravenous). The parameters of pulse oximetry were analyzed: peripheral oxygenation, heart rate. Additionally, the concentration of oxygen in the respiratory mixture of children was taken into account. Research results. Peripheral saturation did not critically decrease at all stages of observation, with the exception of a decrease in the indicator in children of group I compared with group III at the stage of induction into anesthesia (97.79±2.45 versus 98.79±1.63, at p˂0.05, respectively) and at the most painful moment of the operation (96.29±3.47 versus 98.10±2.47, with p˂0.05). At the painful moment of the operation, it was in children of group I that a drop in heart rate was noted compared to group III (127.98±13.77 and 136.10±15.37, respectively, with p˂0.05) and group II (134.02±18.43, at p>0.05) against the background of a decrease in SpO2. Newborns and group I infants required higher oxygen concentrations in the breathing mixture. A significant difference in the indicator is noted between groups I and III at the traumatic stage – 0.47±0.29 and 0.33±0.2, with p˂0.05, respectively, and immediately after the operation – 0.34±0,19 and 0.26±0.13, with p˂0.05, respectively. Conclusions. The expediency and effectiveness of pulse oximetry for children with congenital malformations at all stages of anesthetic support during surgical treatment was confirmed. The risk group for the formation of oxygen deficiency in organs and tissues, the occurrence of pain during the operation were newborns and infants, for whom combined anesthesia was chosen in the form of an inhalation method and regional anesthesia. In the case of the appointment of this type of anesthetic support, it is necessary to more closely monitor the indicators and promptly correct the deterioration of the child's condition

Abstract Purpose: This study explored the effects of condition notification based on virtual reality technology on the anxiety levels of parents of children with simple CHD. Methods: The subjects of the study were the parents of 60 children with simple CHD who were treated in our centre. The state anxiety scale was used to assess the anxiety status of the parents of the children before and after the condition notification in different ways (based on virtual reality explanations and written explanations). Results: In this study, a total of 55 parents of children with CHD successfully completed the survey. At the time of admission, there was no significant difference in the state anxiety scale scores of the parents between the two groups. The anxiety status of the virtual reality group was relieved after the condition notification assisted by virtual reality technology, and there was a significant difference compared with the control group. Conclusion: Condition notification based on virtual reality technology can effectively alleviate the anxiety of parents of children with simple CHD, which is worthy of clinical application.

Abstract Introduction Sickle cell anemia (SCA) is the most prevalent Mendelian disease in the U.S. and is characterized by HbS polymerization, hemolytic anemia and vaso-occlusion. The hallmark clinical manifestation of SCA is acute vaso-occlusive pain crisis (VOC) with severe episodes requiring hospitalization for pain management. Vaso-occlusion resulting in pain is believed to occur with hypoxia, intra-vascular erythrocyte sickling and ischemia reperfusion injury. Recurring episodes of VOC often have no clear causation and may have effects upon physiological pain perception and intrinsic psychological characteristics like catastrophizing. It is not presently known if physiological pain processing or psychological factors influence pain in SCA compared to normal volunteers (NV). We hypothesized that adults with SCA would experience hypersensitivity to painful stimuli compared to NV as seen in other chronic painful conditions. Methods 18 subjects with SCA and 19 age and sex matched NV underwent quantitative sensory testing (QST) for thermal, pressure and ischemic experimental pain phenotypes. Thermal testing (cold and heat) was performed at 4 sites on the volar forearm for temperature at first detection, threshold for sensing pain, and pain tolerance. Pressure pain threshold and tolerance were measured bilaterally at the thumb, forearm, and trapezius with an algometer. Ischemic pain testing was performed on the non-dominant arm inflating a blood pressure cuff of 220 mmHg and determining time to threshold and tolerance. Subjects were also administered the Pain Catastrophizing Scale (PCS-E) which is composed of 13 questions that address 3 dimensions: rumination, magnification, and helplessness. SCA and NV subjects with chronic pain unrelated to complications of SCA were excluded. All SCA subjects tested at baseline pain levels at least 2 weeks since their last painful event requiring intravenous narcotics. Non-parametric t-tests were used to compare results between SCA subjects and NV. Results The mean age of SCA subjects was 33.4 (range 21-47) and NV was 32.1 (range 18-44). The only significant difference for thermal QST was temperature at first cold detection (SCA mean 24.5±6.3 ¢ªC vs. NV mean of 26.6±4.7 ¢ªC, P=0.05). There were no other significant thermal differences between SCA subjects and NV for cold pain threshold (P=0.84), cold pain tolerance (P=0.82), first heat detection (P=0.21), heat pain threshold (P=0.68) or heat pain tolerance (P=0.38). Furthermore, there were no differences in pressure between SCA and NV for pain thresholds at the thumb (P=0.31), forearm (P=0.44) or trapezius (P=0.10), nor were there pain tolerance differences at the thumb (P=0.63), forearm (P0.98) or trapezius (P=0.44). Surprisingly, ischemic pain QST showed no differences for initial pain threshold (468.3±263.1 seconds for SCA vs. 569.6±395.8 seconds for NV, P=0.69) or pain tolerance (642.5±245.4 seconds for SCA vs. 774.4±453.5 seconds for NV). Compared to NV, SCA subjects had significantly higher total catastrophizing (SCA median 30 vs. NV median 7, P=0.0005), rumination (SCA median 12 vs. NV median 3, P=0.0038), magnification (SCA median 8 vs. NV median 3, P=0.0006), and helplessness scores (SCA median 13 vs. NV median 3, P=0.0004). Conclusions Thermal detection of cold temperature change was the only observed difference, despite our hypothesis that recurrent painful episodes from SCA would alter sensitivity to experimental stimuli. Of particular interest are the results of ischemic pain QST, where SCA subjects appear to tolerate ischemia as well as NV despite the expectation this test would promote earlier forearm hypoxia, erythrocyte sickling and potentially vaso-occlusion. No significant difference in onset of ischemic pain between SCA and NV subjects appears to contradict the predominant pain vaso-occlusion paradigm for SCA. In addition, SCA subjects report significant levels of pain castastrophizing. Catastrophizing in other pain conditions may contribute to a psychological state that increases the severity of pain perception and a fear that pain control is impossible. Thus, catastrophizing may enhance chronic pain, pain impact, and its refractory response to pharmacotherapy. Further studies are necessary to determine if experimental pain or catastrophizing are associated with acute pain or chronic pain in SCA and to further elucidate the pathophysiology of pain in SCA.
