The increasing prevalence of chronic conditions and multimorbidity poses great challenges to healthcare systems. As patients’ engagement in self-managing their chronic conditions becomes increasingly important, eHealth interventions are a promising resource for the provision of adequate and timely support. However, there is inconclusive evidence about how to design eHealth services to meet the complex needs of patients. This study applied an evidence-based and theory-informed user-centered design approach in three phases to identify the needs of older adults and healthcare professionals in the collaborative management of multimorbidity (phase 1), develop an eHealth service to address these needs (phase 2), and test the feasibility and acceptance of the eHealth service in a clinical setting (phase 3). Twenty-two user needs were identified and a web-based application—ePATH (electronic Patient Activation in Treatment at Home)—with separate user interfaces for patients and healthcare professionals was developed. The feasibility study with two nurses and five patients led to a redesign and highlighted the importance of adequately addressing not only varying user needs but also the complex nature of healthcare organizations when implementing new services and processes in chronic care management.

OBJECTIVE To apply an evidence-based and theory-informed user-centered design approach for a) identifying the needs of older adults and healthcare professionals in the collaborative management of multimorbidity, b) developing an eHealth service to address these needs, and c) testing the feasibility and acceptance of the eHealth service in a clinical setting. METHODS A theory-driven user-centered design approach was carried out in three interconnected phases: 1) exploration of user needs with patients and healthcare professionals; 2) design and development of content and software; 3) feasibility study of the software prototype and redesign. RESULTS Phase 1 resulted in the specification of twenty-two user needs, which were grouped into five themes: diagnosis-specific information, medication management support, self-management support, care coordination support, psychosocial support. A web-based application – ePATH (electronic Patient Activation in Treatment at Home) – with separate user interfaces for patients and healthcare professionals was developed to address these needs. Two nurses and five patients tested the ePATH service in clinical practice, resulting in a refined web interface and a mobile application (mPATH) to support self-tracking and make it possible to get reminders through push notifications. The feasibility study highlighted the importance of adequately addressing not only varying user needs, but also the complex nature of healthcare organizations when implementing new services and processes in chronic care management. CONCLUSIONS We conclude that user engagement in design and development should not be limited to the elicitation of user needs and creation of new services but should focus more holistically on improving current practices to shape better care, which requires adequate strategies and resources to implement changes in people’s lives and complex organizations. BACKGROUND Mobile health (mHealth) systems have been shown to be useful in supporting self-management by promoting adherence to schedules and longitudinal health interventions, especially in people with disabilities (PwD). The Interactive Mobile Health and Rehabilitation (iMHere) System was developed to empower PwD and those with chronic conditions with supports needed for self-management and independent living. Since the first iteration of the iMHere 1.0 app, several studies have evaluated accessibility and usability of the system. Potential opportunities to improve and simplify the user interface (UI) were identified, and the iMHere modules were redesigned accordingly. OBJECTIVE The aim of this study was to evaluate the usability of the redesigned modules within iMHere 1.0. METHODS This study evaluated the original and redesigned MyMeds and SkinCare modules. To assess the participants’ dexterity levels, the Purdue Pegboard Test (PPBT) was administered. Participants were then asked to perform a set of tasks using both the original and redesigned MyMeds and SkinCare modules to assess efficiency and effectiveness. Usability was measured using the Telehealth Usability Questionnaire (TUQ) to evaluate 10 new accessibility features that were added to the redesigned app. Participants were also asked which version they preferred. RESULTS Twenty-four participants with disabilities and varied degrees of dexterity impairments completed the entire study protocol. Participants displayed improved efficiency and effectiveness of use when using the redesigned modules, as compared to the original modules. Participants also reported improved usability and preferred the redesigned modules. CONCLUSIONS This study demonstrated that the iMHere System became more efficient, effective, and usable for individuals with dexterity impairments after redesigning it according to user-centered principles.

The objective of this study was to optimize a planned audit and feedback (A&F) intervention for home and community care through incorporating user-centered design principles. Phase 1 comprised the development of a paper prototype, followed by testing of the prototype using semistructured interviews and focus groups. Phase 2 involved the development of a revised and online prototype based on the results of Phase 1, followed by user testing. A total of 39 participants across Phase 1 ( n = 33) and Phase 2 ( n = 6) provided input into the design of the A&F intervention. Key changes included improving clarity of the variables, reducing cognitive load, and highlighting opportunities for action. Preliminary findings suggest potential usefulness in this approach to optimize home care management.

Anyelir Cake And Bakery is the largest cake shop in Purwakarta which is engaged in the production of various kinds of cakes.  In the process of distributing products from the central store to branch stores, they still use WhatsApp to place orders. This causes frequent discrepancies in the number of products requested and the number of products received due to an error in reading the message. In this study, a mobile-based application will be built that is able to manage order data more accurately. The development of this application uses the User Centered Design (UCD). This is a design method that focuses on user needs so that the final result of this application does not need to change user behavior when using the application. The UCD method consists of four stages, namely plan the human centered design, specify user and organizational requirements, product design solutions, and evaluate design against user requirements. In the development process, application testing was carried out to get feedback from users with good scores, namely an average of 4 (agree) and 5 (strongly agree). This shows that the application is in accordance with the needs of the user.

BACKGROUND Overtreatment and overtesting expose patients to unnecessary, wasteful, and potentially harmful care. Reducing overtreatment or overtesting that has become ingrained in current clinical practices and is being delivered on a routine basis will require solutions that incorporate a deep understanding of multiple perspectives, particularly those on the front lines of clinical care: patients and their clinicians. Design approaches are a promising and innovative way to incorporate stakeholder needs, desires, and challenges to develop solutions to complex problems. OBJECTIVE This study aimed (1) to engage patients in a design process to develop high-level deintensification strategies for primary care (ie, strategies for scaling back or stopping routine medical services that more recent evidence reveals are not beneficial) and (2) to engage both patients and primary care providers in further co-design to develop and refine the broad deintensification strategies identified in phase 1. METHODS We engaged stakeholders in design charrettes—intensive workshops in which key stakeholders are brought together to develop creative solutions to a specific problem—focused on deintensification of routine overuse in primary care. We conducted the study in 2 phases: a 6.5-hour design charrette with 2 different groups of patients (phase 1) and a subsequent 4-hour charrette with clinicians and a subgroup of phase 1 patients (phase 2). Both phases included surveys and educational presentations related to deintensification. Phase 1 involved several design activities (mind mapping, business origami, and empathy mapping) to help patients gain a deeper understanding of the individuals involved in deintensification. Following that, we asked participants to review hypothetical scenarios where patients, clinicians, or the broader health system context posed a barrier to deintensification and then to brainstorm solutions. The deintensification themes identified in phase 1 were used to guide phase 2. This second phase primarily involved 1 design activity (<italic>WhoDo</italic>). In this activity, patients and clinicians worked together to develop concrete actions that specific stakeholders could take to support deintensification efforts. This activity included identifying barriers to the actions and approaches to overcoming those barriers. RESULTS A total of 35 patients participated in phase 1, and 9 patients and 7 clinicians participated in phase 2. The analysis of the deintensification strategies and survey data is currently underway. The results are expected to be submitted for publication in early 2020. CONCLUSIONS Health care interventions are frequently developed without input from the people who are most affected. The exclusion of these stakeholders in the design process often influences and limits the impact of the intervention. This study employed design charrettes, guided by a flexible user-centered design model, to bring clinicians and patients with differing backgrounds and with different expectations together to cocreate real-world solutions to the complex issue of deintensifying medical services.

Lamandau Store is a place for marketing and selling products that produced by Micro, Small and Medium Entrepreneurs named Usaha Mikro Kecil Menengah (UMKM) and Small Medium Industry products or Industri Kecil Menengah (IKM) in Lamandau Regency. The marketing and sales system at Lamandau Store has been manual which is costumers come to the store. This has not attracted enough consumers, resulting in lower levels of marketing and sales. This study aims to produce an Android-based mobile commerce application that can be used to support the marketing and sales process at the Lamandau Store. The research was conducted in several stages, namely the Data Collection Stage, the System Design Stage and the Report Writing Stage. The system design stage was developed based on the User Centered Design (UCD) method. The results show that a mobile commerce application is in accordance with user needs and has been presented as a new innovation at the Lamandau Store.

Abstract Cognitive impairments (CIs) result in difficulties with a wide range of daily activities. Older adults are especially at risk for CIs, and as the older adult population increases, so does the importance of understanding and supporting the needs of those with CIs. The Enhancing Neurocognitive Health, Abilities, Networks, and Community Engagement (ENHANCE) Center was established with a focus on developing technology-based support for socialization, transportation, and prospective memory needs of older adults with CIs due to mild cognitive impairment (MCI), traumatic brain injury (TBI), and stroke. The extent to which relevant literature in these domains existed was unknown. We conducted a scoping review to identify existing research meeting the following criteria: participants aged 50+ years classified as having a CI due to MCI, TBI, or stroke; and a focus on technology-based support for socialization, transportation, and/or prospective memory needs. Using PRISMA guidelines, we searched three electronic databases, and reviewers screened citations for inclusion and completed data charting. Following screening, only 11 studies met our inclusion criteria. Qualitative and quantitative data are reported for each study. In addition to few studies available, it was common for studies to include 20 or fewer participants. Most assessed technology interactions at one time and few studies examined longitudinal use and benefits. While each paper examined one aspect of user-centered design, no technologies were reported that underwent all stages of the user-centered design process, from needs assessment to iterative design and usability testing, to efficacy trial. Such gaps highlight the important role ENHANCE can play.

Hemodialysis is a complicated process that comes with many risks to both the user and the patient. The Tablo Hemodialysis System demonstrates how applying user-centered approaches to design could greatly improve device usability, and in turn, user and patient safety. Even with the Covid-19 pandemic creating unexpected hurdles to this approach, Tablo’s sensor-based technology offers a unique solution to ensure that user needs continue to be met.
