Intrauterine malnutrition has a long-term effect on human health. This study aimed to evaluate the associations between exposure to famine in early life and obesity in adulthood in Chinese adults. A total of 5033 participants (22,132 observations) of the China Health and Nutrition Survey (CHNS) in 1991–2015 were classified into three famine exposure groups according to their birth year: unexposed (1963–1966), fetal-exposed (1959–1962) and childhood-exposed (1955–1958). Compared with the unexposed group, the fetal-exposed group had higher levels of body-mass-index (BMI) and waist-circumference (WC), and higher prevalence of overweight and central obesity, whereas the childhood-exposed group had lower levels of the measurements. However, the positive associations of fetal exposure with BMI, WC and prevalence of overweight and central obesity were attenuated by additionally adjusting for age at survey. Stratified analysis showed that the adverse effect of fetal exposure to famine was only observed in subjects at several specific age-groups, and in men living in rural areas and in women living in more severe famine exposed areas (p for interaction < 0.05). Our results provide evidence for the weak effect of fetal exposure to famine on body measurements in adulthood, and suggest the importance of severity of famine exposure and timing of exposure.

Abstract BackgroundThe relationship between malnutrition and ascending aorta dilatation (AAD) is still unclear. Therefore, the aim of this study was to examine the association of exposure to the Chinese famine during early life with AAD in adulthood.MethodsWe investigated data of 2598 adults born between 1952 and 1964 from Guangdong, China. All enrolled subjects were divided into five groups: no exposed, fetal exposed, early, mid and late childhood exposed. AAD was assessed by cardiac ultrasound. Multivariate logistic regression and interaction tests were used to estimate the odds ratio (OR) and confidence interval (CI) between famine exposure and AAD.ResultsThere were 2598 (943 male, mean age 59.1 ± 3.65 years) participants were enrolled, among them consist of 270 (10.4%) subjects with AAD. Compared with no exposed group, the adjusted ORs for AAD widening from fetal-exposed, early, mid to late childhood exposed were 1.374 (95% CI: 0.794, 2.364, P = 0.251), 1.976 (95% CI: 1.243, 3.181, P = 0.004), 1.929 (95% CI: 1.237, 3.058, P = 0.004) and 2.227 (95% CI: 1.433, 3.524, P < 0.001), respectively. Stratified analysis showed that the effect of famine exposure on AAD was more pronounced in female, smokers, BMI ≥ 24 kg/m2 and hypertensive patients.ConclusionsWe observed that exposure to famine during early life was an independent risk factor for AAD in adulthood; this effect was not modified by gender, BMI, smoking, hypertension and diabetes.Yu-qing HUANG and Lin LIU are equally to this paper.

BackgroundThe association between famine exposure in early life and risk of arthritis (combination of osteoarthritis and inflammatory arthritis) in adulthood is unclear. The aim of this study is to explore the association.MethodsA total of 4124 subjects were selected from the national data of the China Health and Retirement Longitudinal Study in 2011–2012. Doctor-diagnosed arthritis was self-reported in participants’ questionnaire. Birthdates were used to categorise participants into famine-exposed and non-exposed groups. Logistic regression model was used to explore the association of famine exposure in early life with the risk of arthritis in adulthood.ResultsThe prevalence of arthritis in both infant-exposed and preschool-exposed groups was significantly higher than those in the non-exposed group (35.0% and 30.6% vs 27.3%; p<0.05). Compared with the non-exposed group, the infant-exposed group showed a significantly elevated risk of arthritis in adulthood after adjusting for confounding factors (OR=1.65; 95% CI 1.29 to 2.11; p<0.001). In the stratified analysis, we found that participants who lived in severely affected areas (OR=1.91; 95% CI 1.41 to 2.59; p<0.001), who are female (OR=2.21; 95% CI 1.57 to 3.11; p<0.001) and those with a body mass index ≥24.0 kg/m2 (OR=2.46; 95% CI 1.70 to 3.55; p<0.001) in the infant-exposed group had increased risk of arthritis in adulthood. Similar results were additionally observed when age-balanced control group was used.ConclusionGreat China Famine exposure in infancy may be associated with an elevated risk of arthritis in adulthood, particularly in women and participants with adiposity. These findings suggest nutrition intervention in infancy and weight control in later life may reduce the risk of arthritis in adulthood.

AbstractObjectiveThere is an increasing concern of anaemia in Japanese women, but no national trend data has existed to date. We analysed long-term national trends of anaemia in adult women.DesignSecondary analyses of 15 consecutive cross-sectional nationwide surveys conducted during the period 1989–2003. Analyses were based on 5-year intervals (1989–1993, 1994–1998, 1999–2003).SettingJapan.SubjectsThe subjects included the National Health and Nutrition Survey of Japan (NHNS-J) population. Analyses were based on 50 967 non-pregnant/non-lactating women aged 20 years and over, with complete data sets. We stratified subjects into six age groups (20–29 years, 30–39 years, 40–49 years, 50–59 years, 60–69 years and 70+ years), and three age groups (20–49 years, 50–69 years, 70+ years) for analyses on residential areas (metropolitan, cities and towns).ResultsDecreases in trends of mean haemoglobin values (intravenous) were significant in all age groups. Changes in the prevalence of anaemia were significant only in women aged 30–39 and 40–49 years. Anaemia (haemoglobin < 12.0 g dl−1) increased from 16.8% to 20.6% in women aged 30–39 years, and from 20.2% to 26.9% in women aged 40–49 years, respectively. Prevalence was highest in women aged 70 years and older in all times, but no significant change was observed.ConclusionsHaemoglobin values have declined significantly in all ages in the last 15 years. Anaemia was especially significant in women aged 30–39 years and 40–49 years. Our findings suggest that a large number of young women in Japan are at risk of anaemia, so continuous monitoring and controlling efforts of this trend are needed.

This study aims to solve the problem that the traditional method of measuring the poverty level in rural and urban areas of China from a purely monetary perspective can’t comprehensively analyze and reflect the poverty. In this study, a multidimensional poverty measurement model with non-monetary indicators is proposed, the data of families and their members provided by the China Health and Nutrition Survey (CHNS) of a certain year’s health and nutrition survey in China are used for analysis, and a fuzzy set method is adopted to analyze the poverty situation in various regions of China. First, the fuzzy function set method is used to calculate the one-dimensional poverty index. On the basis of income, the multi-dimensional poverty fuzzy index is calculated from five dimensions, including education, health, assets, and living standard. The calculation results of the single-dimensional poverty and the multi-dimensional poverty are compared to further analyze the reasons of the family poverty of rural residents. Second, the poverty rate of each dimension in each region is calculated by referring to the appropriate measurement indexes of each dimension of the message passing interface (MPI) team. The results show that the concept of measuring poverty by the fuzzy set method is more sensitive to the overall distribution of population in the poverty dimension than the poverty line method. Compared with the poverty line method, the fuzzy set method can better consider the overall distribution of population in poverty dimension. Accordingly, China should strengthen the infrastructure construction in rural areas, increase the investment in education in rural areas, and improve the overall quality of the poor population.

: This study aimed to describe the nutritional status and dietary patterns of Cambodian school-aged children compared with those in the South East Asian Nutrition Survey (SEANUTS; Indonesia, Malaysia, Thailand, and Vietnam in 2011) and to clarify the urban-rural differences using data from a nationally representative sample. The survey was conducted in 2014/2015 with a sample of 2020 children aged 6–17 years from 136 randomly selected schools. Standardized anthropometric measurements and a 1-day dietary survey by 24-hour recall method were conducted. Extended analyses in the present study revealed that the difference between rural and urban areas was similar to that of the SEANUTS; the overall prevalence of stunting remained high (33.2%). Stunting was more prevalent in children living in rural areas than in those in urban areas (total: 36.4% vs 20.4%). In contrast, the overall prevalence of overweight and obesity was not as high (3.1%), but was higher among urban children in all age groups compared with those living in rural areas (total: 6.4% vs 2.3%). Overall, the dietary intake of children did not meet the local recommended dietary allowances, which was similar to the results of the SEANUTS and differed across urban and rural areas.

Background: Previous studies have clarified the relationship between serum uric acid (SUA) and hypertension; most of previous studies suggest that elevated uric acid levels are associated with an increased risk of hypertension, while in China, there are relatively few studies to explore above association. The objective of this longitudinal study is to investigate the correlation of SUA and hypertension in Chinese adults with a nationwide large-scale sample.Methods: Data from the China Health and Nutrition Survey 2009, 2011, and 2016 were used; a total of 8,469 participants (3,973 men and 4,496 women) were involved. This study was conducted separately by gender. Clinical characteristics of the participants among different uric acid groups are compared. The binary logistic regression analysis was conducted to examine the association between SUA and hypertension. Restricted cubic spline analysis with three knots of the SUA concentration were used to characterize the dose-response relationship. Additionally, we compared the incidence of hypertension in the different baseline uric acid groups during follow-up in 2011 and 2015.Results: After the covariates were fully adjusted, we found that elevated uric acid levels were correlated with increased risk of hypertension in both males (p &lt; 0.01) and females (p &lt; 0.01). With 2-year or 6-year of follow-up, we found participants with higher baseline uric acid levels had a higher incidence of hypertension (p &lt; 0.01). In stratified analysis by obesity, above relationship remained significant in nonobesity population (males: p &lt; 0.05, females: p &lt; 0.01) and became nonsignificant in obesity people. In stratified analysis by age, above positively correlation remained significant in middle-aged men (p &lt; 0.05) and elderly women (p &lt; 0.01). Restricted cubic spline revealed the dose-response relationship between SUA and hypertension; we also found that above relationship was much stronger in females.Conclusion: This study suggests that elevated SUA levels might be positively associated with an increased risk of hypertension in general Chinese adults.

Fetal exposure to famine may have long-term consequences in adulthood. The purpose of the present study was to explore the association between famine exposure in fetal life (Chinese famine in 1959–1961) and obesity risk in adulthood. A total of 8054 subjects (3422 male, 4632 female) were recruited from the cross-sectional 2010–2012 China National Nutrition and Health Survey (CNNHS). The subjects born in 1960 and 1961 were selected as the exposed group, while the subjects born in 1963 were selected as the unexposed group. Multiple linear or logistic regression was performed to examine the association between fetal exposure to famine and risk of obesity (body mass index (BMI), waist circumference (WC), obesity, central obesity) adjusting for gender, education level, economic status, physical exercise, sedentary time, smoking, drinking, the intake of livestock and poultry and the intake of cereal and beans. Compared with the unexposed group, WC increased by 0.52 cm after adjusting the covariates (p = 0.021) and females in the exposed group had a significantly higher prevalence of central obesity with an odds ratio (OR) of 1.15 (1.01,1.31) after adjusting the confounders (p = 0.030). WC increased by 0.71 cm, 1.21 cm after adjusting the covariates compared with the unexposed group among the total subjects and the female subjects in urban areas (p = 0.021, p = 0.001). The female subjects had a significantly higher prevalence of obesity and central obesity, with ORs of 1.34 (1.04,1.71) (p = 0.022), 1.28 (1.07,1.53) (p = 0.008) respectively. Our results suggest that fetal exposure to the Chinese famine increased obesity risk in adulthood, and the association was stronger in female and urban subjects.

Objective: To determine the prevalence of anaemia in children of 6 to 59 months old in Narayanganj, Bangladesh to help policy makers to formulate health and nutrition policies in national level. Methods: In 2009, a representative sample of 767 young children (age ranging from 6 to 59 months) had their haemoglobin concentration measured. The sampling process was in three stages: at first, 5 Upazila hospitals, 1 District hospital and a 200 bedded specialized hospital were randomly selected to represent the whole district and its 5 geographic urban and rural areas. Next, using census lists, 15 census sectors were randomly chosen. Finally, 767 children of 6-59 months were selected. Blood was collected by vein puncture and haemoglobin concentration was measured with a haemoglobin meter. Data were analysed to determine prevalence of anaemia. Results: The prevalence of anemia among the children of 5-59 months old was 40.9% for the district as a whole. Prevalence in the municipal region of Narayanganj was 40.9%. The rural areas had the highest prevalence of 66.9%. Prevalence was almost two times higher in children of 6-23 months in comparison to children of 24-59 months i.e. 61.8% and 31.0% respectively. The mean haemoglobin concentrations in the younger and older age groups were 10.4(±1.5) g/ dl and 11.4(±1.4) g/dl respectively. There is no difference found between the sexes. Conclusion: This is the first assessment of anaemia prevalence among young children in Narayangaj, Bangladesh. As there is very high prevalence of anaemia among the children studied in Fatulla upazila, especially those in the age group 6-23 months, public health interventions are needed here most. Using the China Health and Nutrition Survey (CHNS), this paper analyses that height for age (HAZ) of children from urban area is higher than that of those from rural areas on average and the height varies among rural children. Gender also has similar impact on children’s height, namely, HAZ of girls is lower than that of boys. In addition, only children has better nutrition health than others and living with parents or not has great impact on children’s nutrition health as well.

AbstractObjectiveTo evaluate patterns of water consumption from plain water, beverages and foods among Mexican children and adolescents and to compare actual patterns of total daily water intake with the Dietary Reference Intakes (DRI).DesignWe analysed one 24 h dietary recall from Mexican children and adolescents. We calculated intakes of total daily water and water from foods and from beverages. Actual total water intake per capita was subtracted from the DRI for water to calculate the shortfall.SettingMexican National Health and Nutrition Survey in 2012.SubjectsMexican children and adolescents (n 6867) aged 1–18 years.ResultsApproximately 73 % of children and adolescents aged 1–18 years reported drinking plain water. Beverages and plain water represented 65·5 % and 26·5 % of total daily water intake, respectively. Among 1–3-year-olds, the top three main sources of water were from foods, plain water and water from plain milk. Among 4–8- and 9–13-year-olds, the main sources were from foods, plain water and agua fresca (fruit water). Among 14–18-year-olds, the main sources of water were plain water, water from foods and soda. A higher proportion of 1–3-year-olds and 4–8-year-olds met the DRI for water (38 % and 29 %, respectively). Among 9–13-year-olds and 14–18-year-olds, 13–19 % of children met the DRI for water.ConclusionsTotal daily water intakes remain below DRI levels in all age groups. Although plain water still contributes the greatest proportion to daily water intake among fluids, caloric beverages are currently major sources of water especially among older children and adolescents.
