Unique technologies for organizing medical care for COVID-19 patients with concomitant diseases of the cardiovascular system and surgical pathology

The article describes some of the key elements of unique technologies for organizing medical care for patients with COVID-19 with concomitant diseases of the cardiovascular system and surgical pathology, which, along with ensuring infectious safety, allow to optimize the stream of value creation for these patients. Innovative approaches to managing the production activities of a medical organization are proposed. The goal of the work done by the hospital administration was to create a management model for a state medical organization based on modern methods, including a process-oriented approach and lean healthcare technologies. The relevance of the chosen topic is due to the fact that the activities of healthcare institutions in the context of the global coronavirus pandemic require more flexibility and mobility; therefore, the primary task is to master new management tools for a medical organization, taking into account industry characteristics and consumer needs. Analytical and descriptive methods, sociological survey were used. The article describes our own experience in carrying out structural changes in a medical organization. The use of modern management approaches allows the formation of positive interaction within the workforce and provides active feedback with patients.

Medical services implementation schemes, based on the optimization of the flow of patient created values, are proposed. Changes appearing in certain performance indicators of the medical organization during the process of new management model introduction are analyzed. Aim. Creation of a management model for a state medical organization in the basis of which are the modern methods based on a process-oriented approach to the separation of management functions. Relevance. The work of healthcare organizations in a highly competitive environment requires more flexibility and mobility, thus the primary task is to create new management models of the organization, taking into account industry characteristics and consumer needs. Materials and Methods. The work uses the published results of the analysis of structural changes in organizational activity by other authors. The study was carried out on the basis of internal documents of the FSFI FSRCC FMBA of Russia on the creation of a new management structure focused on the development of an organization in the conditions of the market of medical services. Analytical and descriptive methods and sociological survey were used. Results and Discussion. Changes in the indicators for service provision for compulsory medical insurance and the volume of medical care provided as a result of the reforms carried out by the management of the healthcare organization of the FSRCC for four years have been analyzed. There was an increase in the number of patients seeking medical services at the FSRCC, who want to receive information/assistance quickly, in accordance with the adopted concept of a client-oriented marketing strategy, which required the development of “short” routes for patients, especially in conditions of increased service. By the joint efforts of marketers and specialist doctors, using design technologies, self-supporting Centres are being formed, which are virtual subdivisions that bring together highly qualified doctors to address the specific needs of patients. The “short” path scheme is based on the adoption of the definition of “medical service” as a unit of measurement of the complexity of the process of “medical care” provision. Conclusions. An increase in the number of visits at the outpatient-polyclinic stage of medical care provision (diagnostics, consultations) over five years doubles the achievement of the goals set for the medical organization, considered as an economic entity of a market economy.

Relevance. Today, in preventive medicine, climatic conditions that have a pathological effect on the functional state of a person are increasingly being updated. the occurrence of exacerbations of many diseases can be causally associated with various weather conditions. Aim: to develop the main tasks for improving the organization of medical care for weather-dependent patients with diseases of the cardiovascular system. Material and methods. The assessment of personnel, material and technical support and the main performance indicators of an outpatient clinic was carried out on the example of the Voronezh city polyclinic No. 18 to develop the main tasks for improving the organization of medical care for weather-dependent patients with diseases of the cardiovascular system. Results. The main personnel problem is the low staffing of district therapists and specialists of a narrow service. One of the priorities for reducing the burden on medical hospitals is the organization of inpatient replacement medical care on the basis of outpatient clinics. The indicators for the implementation of state guarantees for the outpatient network for 2018, which were fully implemented, are given. The analysis of the planned load performance by polyclinic specialists is presented. Cardiological and neurological services carry out measures to reduce the risk of exacerbations of diseases with cerebral atherosclerosis, hypertension, and major neurological nosologies. Conclusion. Improving the organization of medical care for weather-dependent patients with cardiovascular diseases are: informing patients about the sources of specialized medical weather forecasts in the region, organizing the work of the medical prevention office, implementing an interdepartmental approach to providing health care to the most vulnerable groups of the population.

Aim of the study. Analysis of medical tourism’s organizational features based on the example of the large medical organizations in the United Kingdom, South Korea, Italy and China. Materials and methods. The data were collected by the authors by interviewing the heads of medical organizations and their deputies in the United Kingdom, South Korea, Italy and China (3–4 respondents per medical organization) using the developed questionnaire to identify the main mechanisms and tools for organizing the export of medical services. SWOT-analysis (Strengths; Weaknesses; Opportunities; Threats) was performed in order to comprehensively evaluate the received information. Results. Along with weaknesses and threats that slow down the development of medical services exports, strengths (internal factors) and opportunities ( external factors) that contribute to the development of medical tourism were also identified: the widespread popularity of the brand of medical organizations abroad which is associated with the provision of premium medical services; versatility and ability to conduct high-tech surgical operations; the presence of a separate premium class building and an international department for working with foreign patients and promoting a medical organization in the world market; well-established business relationships with assistance companies; foreign medical personnel who speak foreign languages and possess necessary skills to treat foreign patients; developed electronic medical care system; developed system of quality control of medical care; the presence of branches in other countries; the presence of a medical visa in the system of legislation; established cooperation with many countries at the embassy level; state licensing and accreditation for the provision of medical services to foreign citzens; the availability of a state website on the provision of medical assistance to foreign citizens; the possibility of the age of value added tax. Conclusion. We identified main patterns in the organization of export of medical services that can be applied to develop this direction in medical organizations of the Russian Federation during the analysis the strengths and weaknesses of four large medical organizations abroad, as well as external factors that affect the work of these medical organizations.

Article is devoted to the analysis of the Order of the Russian Ministry of Health of 19.03.2020 № 198n (an edition of 29.04.2020) “About a temporary order of the organization of work of the medical organizations for implementation of measures for prevention and reduction of risk of spread of a new koronavirusny infection of COVID19” regarding formation of the temporary staff list of the division of the medical organization providing medical care to citizens with a koronavirusny infection of COVID-19 and also the requirement with the staff of this division. The criteria recommended by the Ministry of Health of the Russian Federation for determining the number of rates of medical workers in these divisions, requirements for medical workers of the division have been considered, a practical example of calculation of rates has been given.

The article is devoted to the problems of providing medical care in outpatient settings to patients who have undergone organ transplantation. The authors analyzed the economic efficiency of the outpatient transplantation center and concluded that a new payment mechanism for this type of medical services is needed. In addition, the article describes the system of social measures necessary for recipients of organs that are performed in a medical organization due to the lack of a rehabilitation program for such patients.

Modern social and cultural reality forces the heads of medical institutions to pay increased attention to the human factor, rethinking such basic concepts as «management» and «organization». It is important to cultivate the most comfortable person-centered work environment in medical teams. The main goal of this article was to demonstrate the feasibility of using robust management to maintain the constant attention of the medical administration on the interpersonal relations of employees. The article contains a brief description of robust management tools, a description of the person-centered approach, as well as examples of the use of robust regulators and socionic profiling estimators. The potential of robust management is not revealed when the management simply declares its merits, but when the entire medical team wants and can use it, actively involved in achieving the desired result.

In the context of the Moscow healthcare reform that has been carried out in recent years, the issues of evaluating the effectiveness of financing the healthcare system and the performance of medical organizations in providing medical care to the population are particularly relevant. Given the limited public resources allocated to the health sector, the quality of management of available financial, human and material resources is becoming more important. The article considers the application of the method of assessing the quality of resource management, introduced in Moscow since 2016, and its results in terms of evaluating the effectiveness of financial resources. It is revealed that the effectiveness of financial resources management is influenced by certain indicators that characterize the organization of management of the main activities of a medical organization.

The issues of financing in a medical organization are one of the most relevant areas of healthcare in the Russian Federation. Providing departments with the necessary financial resources to provide effective and accessible medical care to the population is important for both the administration and doctors and nurses. In some regions of the Volga Federal District, the departments are combined (Penza, Saransk), that is, all types of medical care are provided to both adults and children in one place, which leads to a number of problems associated with the irrational use of medicines. One of these is the department of Maxillofacial Surgery of the N.N. Burdenko National Clinical Hospital, on the basis of which the disadvantages of using drugs for parenteral use are considered. The purpose of this work is an economic analysis of the effectiveness of the use of solutions for parenteral use in children on the basis of an adult medical organization. The materials of the study were data from patients ' medical histories, prescribing lists, invoices from the pharmacy organization, and data on the cost of medicines. The results of the work are an analysis of the existing problems of combined type departments related to drug provision, a number of medicines for parenteral use that are not available to order, but are included in the standards of treatment of children with injuries of the maxillofacial region. The ABC - analysis of expenses under the item "medicines" is calculated, the data of the VEN – analysis is presented, which includes vital, necessary and secondary medicines for the treatment of children with injuries of the maxillofacial region. An algorithm for the selection of effective and rational therapy has been developed, using the example of antibacterial agents for parenteral administration.
