The article is a description of the effectiveness of treatment of patients of reproductive age with chronic nonspecific exo-, endocervicitis and vaginitis in the presence of benign and precancerous diseases of the cervix of the uterus of mild degree. Comparative characteristics of the use of the combined preparation for intravaginal administration Elzhina and intravaginal administration of metronidazole with oral fluconazole were carried out. The advantages of using the drug Elzhina in terms of preoperative preparation over the use of radio wave excisional and surgical methods of treatment are noted. The efficacy, tolerability, and safety of the therapy were also evaluated in both treatment options. It is noted that the drug containing ornidazole 500 mg, prednisolone 3 mg, econazole 150 mg, neomycin 65 000 IU, will reduce the severity of inflammatory processes, which has a beneficial effect on the results of radio wave and surgical methods for the treatment of benign and precancerous diseases of the cervix. Disc degeneration and associated disorders are among the most debated topics in the orthopedic literature over the past few decades. These may be attributed to interrelated mechanical, biochemical, and environmental factors. The treatment options vary from conservative approaches to surgery, depending on the severity of degeneration and response to conservative therapies. Spinal fusion is considered to be the “gold standard” in surgical methods till date. However, the association of adjacent level degeneration has led to the evolution of motion preservation technologies like spinal arthroplasty and posterior dynamic stabilization systems. These new technologies are aimed to address pain and preserve motion while maintaining a proper load sharing among various spinal elements. This paper provides an elaborative biomechanical review of the technologies aimed to address the disc degeneration and reiterates the point that biomechanical efficacy followed by long-term clinical success will allow these nonfusion technologies as alternatives to fusion, at least in certain patient population.

Polycystic ovarian syndrome (PCOS) is the most prevalent endocrine disorder affecting females. It is a common cause of menstrual irregularities and infertility during reproductive age. Genetic and hormonal factors play crucial role in the pathogenesis of PCOS. Low level of progesterone in PCOS causes overstimulation of immune system that produces more estrogen which leads to various autoantibodies. Different autoantibodies have been documented in PCOS, for example, anti-nuclear (ANA), anti-thyroid, anti-spermatic, anti-SM, anti-histone, anti-carbonic anhydrase, anti-ovarian, and anti-islet cell antibodies. There is an association between PCOS and autoimmune diseases such as ANA and anti-TPO that have been documented in systemic lupus erythematosus and Hashimoto thyroiditis, respectively, and it is suspected that there are autoantibodies that might affect the long term clinical management of these patients. Therefore fluctuating levels of autoantibodies in different PCOS patients give us the way to open new chapter for future research on molecular level. This may lead to discovery of better treatment options for PCOS in near future.

Leiomyomas, also known as uterine fibroids, are a common benign tumor in women of reproductive age. These lesions disrupt the function of the uterus causing menorrhagia and pelvic pressure as well as reproductive disorders. These women pose a true challenge for clinicians in the attempt of choosing the suitable treatment for each patient. Patient’s age, interest in fertility preservation, and leiomyoma location and size are all factors to be taken into account when deciding upon the preferable therapeutic option. For the past few decades, surgical treatment was the only reliable long-term treatment available. A variety of surgical approaches have been developed over the years but these developments have come at the expense of other treatment options. The classical medical treatment includes gonadotropin-releasing hormone (GnRH) agonists and antagonists. These agents are well known for their limited clinical effect as well as their broad spectrum of side effects, inspiring a need for new pharmacological treatments. In recent years, promising results have been reported with the use of selective progesterone receptor modulators (SPRM). Long-term clinical trials have shown a reduction in bleeding and shrinkage of leiomyoma mass. These results instill hope for women suffering from symptomatic leiomyomas seeking an effective, long-term medical option for their condition.

Subject: Salivary stone disease is a disease that occurs as a result of inflammatory processes occurring in the salivary glands. More recently, salivary stone disease was considered a rare disease. Thanks to the available modern methods of diagnosis, the detection of salivary stone disease is significantly progressing. Purpose: In our work we aimed to figure out the most effective methods of diagnosis of salivary stone disease . Among the private methods of radiation diagnosis of salivary stone disease, we have identified ultrasound and CT examination of the salivary glands and ducts. Our task was also to determine how conservative treatment is competitive with the surgical method of treatment of this disease. Results: As a result of our studies , we concluded that the ultrasound method in the diagnosis of inflammation of the salivary glands and the presence of concretions in the glands and ducts is highly informative , safe and accessible to patients . Conservative methods of treatment used in patients who were diagnosed with the presence of concretions at the outlet of the salivary gland ducts are no less effective than surgical methods. Surgical treatment of salivary stone disease is advisable only in cases of block of the salivary gland and ducts concretions more than 0.5 cm in diameter. Conclusion: Salivary Stone disease in our medical institution is diagnosed quite often. The method of ultrasound diagnosis is the main method of examination of patients who came to us with symptoms of salivary stone disease. CT method is advisable to carry out patients in need of surgical treatment of the disease. The method of CT diagnosis allows to establish the exact coordinates and sizes of concretions located in the salivary glands and deep in the ducts.

The article substantiates of the clinical relevance problem of HPV-associated chronic cervicitis in reproductive age women. The study used modern diagnostic methods for this disease. The article based an analysis of the literature and our own observations, we studied some methods of treatment of HPV-associated cervicitis and made a clinical comparison of the use of suppositories and proteflazide alcohol solution, combined therapy of inozine pranobex with simultaneous local ectocervix destruction (laser vaporization) as part of complex treatment and relapse prevention of this pathology in reproductive age women. The effectiveness of therapy of 80 reproductive age patients with verified chronic cervicitis against the background of high oncogenic risk papillomavirus infection and the results of complex treatment were analyzed.Key words: Human papillomavirus infection, cervicitis, diagnosis, antiviral therapy, complex treatment.

Dentists are always in quest of new methods for the treatment of periodontal diseases, and photodynamic therapy (PDT) is considered one of the most effective methods. A significant improvement in the clinical pic-ture was obtained in all patients throughout the control period of observation of the patients as a result of the treatment with the use of PDT. According to the data from research study it can be said that photodynamic therapy is an effective and gentle method of antibacterial treatment of inflammatory periodontal diseases, which can be effectively used to prevent exacerbations of chronic inflammatory periodontal diseases, to achieve a long-term positive effect and to refuse, in some cases, from surgical methods of treatment. There is a signifi-cant increase in the duration of remission of the disease, improvement of microcirculatory blood circulation in per-iodontal tissues and a small microbial contamination of periodontal pockets of patients treated with photodynam-ic therapy in the long term compared with groups of pa-tients treated with only antiseptic and antibacterial drugs.

Glaucoma is a socially sensitive disease, being one of the leading causes of irreversible visual impairment and blindness. Refractory glaucoma is one of the most severe forms of the disease as it is resistant to conservative and surgical methods of treatment. Because of pronounced postoperative fibroblastic activity of eye tissues, leading to gross scarring and obliteration of outflow paths, modern materials and methods of drainage surgery are needed. The review presents a variety of drains used today and considers in detail their main characteristics and the biocompatibility of the material used with eye tissues. Various groups of implants are presented, whose effectiveness and safety are compared and assessed. The statistics of postoperative complications and long-term results of surgical treatment are given.

4,000 patients with discogenic lumbosacral radiculitis were treated. 2,000 patients were managed conservatively, 1,000 patients underwent microdiscectomy and 1,000 patients underwent endoscopic discectomy. The authors of the paper compared indications for different types of treatment, effectiveness of treatment, and duration of rehabilitation. The age of patients in both groups ranged 18 to 78 years. Treatment outcomes were evaluated using the Macnab scale, including the need for additional treatment, duration of rehabilitation treatment, and terms of return to work. Currently, the results of conservative treatment of lumbar hernias are being discussed in the literature. In general, according to authors, the result of conservative treatment is inversely proportional to the size of hernias: the smaller the hernia, the more successful the treatment results. Moreover, there is no clear correlation with the conservative treatment methods. At the same time, the results of surgical intervention directly correlate with the size of hernias or sequesters: the larger the hernia, the better the outcome. Alternatively, microsurgical and endoscopic interventions were used with similar outcomes and relapse rates. The long-term outcomes were tracked using the Macnab scale. Relapses of hernias in the endoscopic discectomy group were found in 18 patients, which was 1.8%. Relapses of hernias in the microdiscectomy group were found in 11 patients, which was 1.1%. Modern conservative methods of treatment allow good outcomes for treatment of hernias even up to 8 mm. In patients with hernias 8 mm to 10 mm the results of conservative treatment were generally not satisfactory and relapses occurred quickly. In patients with hernias of more than 10 mm, conservative treatment is not recommended; positive outcomes are possible only with the use of surgical methods.

Objective: a comparative analysis of the results of the use of different surgical methods of treatment of patients with formed pancreatic cysts.Material and methods. 108 patients with formed pancreatic cysts were treated and analysis of shortand long-term results of their surgical treatment was performed. Patients were divided into three groups depending on the type of surgical intervention: I – external drainage – 44 (40.7%), II – internal drainage –33 (30.6%), III – resection operations – 31 (28.7%).Results and discussions. Marsupialization of cyst by laparotomy incision was performed in patients of I group (n = 44). 18 (40.9%) complications, 9 (20.5%) lethal cases were after operation. Anastomoses of cysts with the small intestine were mostly performed in II group (n = 33) – 21 (63.6%). 7 (21.2%) complications, 1 (3.0%) lethal case were after operation. Distal resections were performed in patients of III group (n = 31) in 16 (51.6%) cases. Duodenum-preserving resections were introduced for treatment of cyst of pancreas head – 12 (38.7%). When performing this type of operations we proposed nikelid titanium stents for the prevention of anastomosis stenosis and preoperative retrograde stenting of the common bile duct for the prevention of damage. 10 (32.3%) complications and no lethal cases were after operation. Immediate results were worse in patients of I group. 47 (43.5%) patients were analyzed in long-term period. The number of recurrences of the disease (13.3%) and long-term mortality (33.3%) prevailed in the group of patients undergoing internal drainage of cysts. Quality of life, level of mental and physical health, that was assessed using SF-36, were higher in group of patients with reactionary treatment.Conclusion. The best immediate and long-term results were noted after resection operations, that enables to recommend their as the most optimal and radical method for treatment of patients with pancreatic cysts. Introducing of duodenum-preserving resections in case of pancreas head cyst improves the quality of patients life.
