INTEGRAL ASSESSMENT OF THE BIOPSYCHOSOCIAL STATUS OF OLDER CITIZENS AND IMPLEMENTATION OF THE SYSTEMIC SUPPORT COMPONENTS

В статье рассмотрены проблемы определения нуждаемости граждан старших возрастных групп в комплексной медико-социально-психологической помощи в рамках реализации персонифицированного подхода к предоставлению услуг. Вводится понятие интегральной оценки биопсихосоциального статуса пожилых граждан и предлагается алгоритм ее проведения, основанный на ранжировании обследуемых по степени снижения функций организма вследствие старения и развития ассоциированных с возрастом заболеваний. Обоснована возможность и показаны особенности использования этого алгоритма на примере Санкт-Петербурга как модельного региона с учетом его технического, кадрового и организационного потенциала. Сформулированы требования по реализации составляющих системной поддержки оказания комплексной медико-социально-психологической помощи пожилым гражданам в процессе организационно-технического обеспечения интегральной оценки их биопсихосоциального статуса. The article discusses the problems of determining the need of citizens of older age groups in complex medical, social and psychological assistance in the framework of the implementation of a personalized approach to the service provision. The concept of an integral assessment of the biopsychosocial status of older citizens has been introduced. Based on the ranking of subjects according to the degree of decrease in body functions due to ageing and the development of age-associated diseases, an algorithm for the implementation of the assessment has been proposed. The possibility of using this algorithm has been substantiated, and its features have been shown on the example of St. Petersburg as a model region, taking into account its technical, personnel and organizational potential. The requirements for the implementation of the systemic support components for the provision of comprehensive medical, social and psychological assistance to senior citizens in the process of organizational and technical support for an integrated assessment of their biopsychosocial status have been defined.

Worldwide, active aging policy calls for greater participation of senior citizens in the social, economic, and political realms. Despite emerging evidence of initiatives engaging senior citizens in social activities, little is known about the use of participatory approaches in the design and/or implementation of policies that matter to older citizens. This article identifies initiatives facilitating the civic participation of older people in policy-making in European Union member and associate states, drawing on a review of the literature, consultation of national policy experts, and exemplary case studies. Four main patterns of senior civic participation are identified: adopting consultative or co-decisional participatory approaches in policy design or policy implementation. The four are represented to varying degrees at different geographical levels (national, regional, local), with different actor configurations (appointed, elected/nominated, corporate representation), and with varying degree of institutionalization (temporary/permanent). Case studies illustrate approaches taken to enhance the quality and effectiveness of public services for senior citizens. Future research should strengthen this line of enquiry to cast further light on conditions facilitating the civic participation of senior citizens.

Asthma is a serious health problem affecting all age groups. Melatonin or its agonists are commonly used to treat many diseases, but there are conflicting data on asthma therapy. This paper analyzes researches on the possible use of melatonin in the therapy of asthma. Melatonin is a potent antioxidant and a vasodilator, but in some experiments, it can act as a pro-oxidant and a vasoconstrictor, which may depend on the duration of use. It has been suggested that circadian rhythms should be corrected in asthmatics to optimize the desired effects of drugs and to reduce the severity of their adverse reactions. Disordered diurnal variations in the salivary levels of melatonin and cortisol are detectable in patients with asthma and may be implicated in its pathogenesis. In addition, the conflicting data on the effect of melatonin on the development of asthma are associated with an incomplete view of the factors influencing the level of melatonin. Thus, to study the effects of melatonin, it is necessary to take into account the greatest possible factors that may influence the level of melatonin and the course of asthma: a daily diet in terms of the use of caffeine, alcohol, sleep-wakefulness pattern, sleep quality proper, and drowsiness during the daytime, social burden, the level of anxiety and stress resistance, and to investigate the levels of endogenous melatonin or its derivatives, immune status, oxidative stress intensity, etc. Obviously, the use of melatonin in the therapy of asthma can be considered, by applying a personalized approach.

Objective: The objective of this study is to determine the 1998 rates, types, regional variation and Medicare expenditure of private psychiatry services for older people in Australia, as compared with younger adults and with 1985–1986 data. Method: Medicare Benefits Schedule Item Statistics for the psychiatric item numbers 300–352 and item 14224 were obtained from the Health Insurance Commission for each State and Territory. The items were examined in the age groups 15–64 years, 65 years and over and 75 years and over. Main outcome measures were per capita service provision by age group, State and Territory and Medicare expenditure by age group. Results: During 1998, 6.4% (5765.6 per 100 000) of private psychiatric services were to patients aged > 64 years. Patients aged 15–64 received 2.7 times the number of psychiatric services per capita than patients > 64 and 3.6 times that of patients aged > 74 years. Patients aged > 64 received more hospital and nursing home consultations, home visits and electroconvulsive therapy per capita, while younger adults used more office-based consultations, longer consultations, and group therapy. Victoria had the highest per capita rate (7659.2 per 100 000) and the Northern Territory the lowest (540.4 per 100 000), although the highest proportion of services to older patients was in Western Australia. Per capita the proportion of Medicare expenditure allocated to adults aged less than 65 years was 4.1 times that for adults over 64 years. Conclusions: Private psychiatric service provision to older people is inequitable when compared with younger adults. The proportion of Medicare private psychiatry expenditure on older adults has declined since 1985–1986.

ABSTRACTElderly people have a relatively low likelihood of becoming a victim of crime, compared with other age groups. Theft rather than violence constitutes the bulk of offences against them. Criminological research indicates that it is not vulnerability but lifestyle and the amount of time spent out of the home which are pre-disposing factors. However, when offences occur they have a disproportionately high personal impact. Ways of reducing both exposure and impact are discussed. These include (1) Hardening the targets (2) Creating more protective environments (3) Encouraging self-help (4) Entrancing awareness of crime and prevention techniques (5) Risk avoidance.

The objective is to improve the results of treatment of locally advanced colorectal cancer in patients of older age groups due to the personalized approach to choosing surgical tactics with a comprehensive assessment of the functional operability and molecular biological properties of the tumor.Material and methods. A comparative evaluation of the results of surgical treatment of locally advanced colorectal cancer in patients of different age groups was conducted on representative clinical material. We studied the molecular profile of tumors, taking into account the age factor, and the clinical significance of specific markers in predicting the results of treatment.Results. The most significant indicators of functional operability of patients that must be considered when choosing a surgical tactic were identified. . The features of the immunophenotype of tumors, taking into account the age factor, were determined; and the prognostic significance of specific immunohistochemical indicators was estimated.Conclusion. The use of a personalized approach to choosing surgical tactics allows to achieve encouraging results of treatment in geriatric patients with locally advanced colorectal cancer.

This report focuses on the concept of the silver economy, which has emerged as a response to population ageing in Europe in recent years. The silver economy refers to all economic activities linked to older age groups. The concept is based on the notion that many older people continue to make valuable economic and societal contributions after retirement, and that older citizens can provide significant economic and societal benefits, particularly if they are healthy and active. This report examines policies and initiatives to promote the silver economy and the closely related concepts of healthy ageing, active ageing and age-friendliness. The report seeks to uncover what are the preconditions for expanding the Nordic silver economy, and how cross-border collaboration can help enhance the potential of the silver economy in border regions.

The article is devoted to the development of integrated assessment and normative scales for assessing the indicators of special physical fitness of basketball players at the stage of preliminary basic training for use in the process of stage control. The purpose of the study is to develop an integrated assessment and indicative regulatory scales for determining the level of special physical fitness with the definition of ranges of levels of readiness for the stage control of basketball players aged 13-14. The formula for calculating the integrated assessment of special physical fitness is presented, which allows to calculate the level of fitness regardless of the number of tests used in the control. Based on the sigma deviation, normative scales for assessing special physical fitness indicators for ten basketball-specific tests have been developed, defining ranges of five levels of training - low, below average, medium, above average and high for stage control of basketball players aged 13-14. With the help of an integrated assessment, the coach can determine the level of various aspects of players' training both individually and on average as a team, as well as differentiated by groups of basketball players with the same characteristics (role, height, experience of basketball, etc.). This approach allows the results of testing the coach to compare the results of different athletes, the principle of expression in conventional units remains the same.

The article presents the results of an integrated assessment of the landscape and recreational potential (hereinafter referred to as LPR) of the territory of the Central Caucasus using GIS technologies. Landscapes located within the Central Caucasus are of particular interest for the development of recreation and tourism. In the territory of the Central Caucasus, there has not been an assessment of the LPR of landscapes before. The process of assessing the PSR was performed using the Quantum GIS, zoning for certain types of tourism was also performed in Quantum GIS. The borders of the Central Caucasus were drawn and clarified. The displayed boundaries of groups of landscapes, which are located within the Central Caucasus. The method of assessment of landscape LPR is justified, which is based on a ball approach that allows taking into account natural resources for the development of certain types of recreation. To calculate each indicator and visualize the results of the study, the method of cartograms was used. When analyzing individual landscape components, the method of 5 km-side hexagonal operators was used. After calculating the main indicators for each polygon, we classified each component (layer) into 5 classes (points). The Jenkens natural boundary algorithm was used for classification. All evaluation factors were calculated for each operating cell. Individual components of the landscape have different significance for the development of certain types of tourism. We introduced a weighting coefficient for each landscape component and estimated its impact on a specific type of tourism. The weighting coefficient is calculated based on a survey of experts in the field of tourism and recreation. An integral assessment of the landscape and recreational potential of groups of landscapes in the Central Caucasus has been carried out, which allows us to identify zones of various degrees of favorability in the landscapes for certain types of tourism (mountaineering, trekking, rafting, and skiing). The result of the component-by-component assessment of landscapes was a combined map of landscape and recreational potential of landscape groups in the Central Caucasus. The zoning of landscapes allows us to determine the vectors of tourism development in municipalities located within the Central Caucasus, as well as depressed territories. According to the obtained data of the integrated assessment, the conclusion is made about the most promising landscapes in which it is necessary to develop various tourist destinations. The received data must be taken into account in the territorial planning documents. The selected zones in the landscapes of potential development of various types of tourism, with appropriate solutions, will create a more competitive environment in the republics of the North Caucasus.

We all know that technology has become a dominant force in today’s society for people of all ages. However, certain elements of society have less access to technology than others. In the literature, discussions and research on these discrepancies tend to focus on factors such as gender, sex, socioeconomic status, race, education, and employment. Occasionally, age is taken into consideration. In reviewing online articles related to the digital divide, it appears that there are many more articles, reports, and projects that focus on factors other than age. Few looked at the impact of the digital divide on senior citizens. One article, “The Internet and Older Adults” (U.S. Administration on Aging, 2004), reports that: Senior citizens comprise 13% of the U.S. population, but just 4% of the U.S. Internet population. Since their numbers are so small, there has not been much research about what these ‘wired seniors’ are doing online and how they feel about the Internet. It turns out that seniors who have Internet access benefit greatly from the resources available online—communicating with family, researching health information, tracking their investments—all from the comfort of their home or senior center. (paragraph 2) The term “digital divide” is often heard and freely used, but what is it? It has been defined by Carvin (2000) as: “…the gap between those people and communities with access to information technology and those without it. Yet, the fact is there are many divides, characterized by community, ethnic, economic, and age groups.” He goes on to add that “households earning incomes over $75,000 are over 20 times more likely to have home Internet access than those at the lowest income levels” (Carvin, 2000, paragraph 1).
