Introduction. The problem of high mortality from cardiovascular diseases is caused, among other things, by asymptomatic atherosclerosis, which proceeds latently for a long time and manifests itself as a serious vascular catastrophe, which is of particular importance for people working at production facilities with difficult and dangerous working conditions.The aim of the study was to assess the risk factors for atherosclerosis and the state of the vascular wall in the workers of the main professions of coal enterprises in the South of Kuzbass.Material and methods. In total, the study included 384 people (men), of whom 266 were the workers in coal mines in the South of Kuzbass and 118 people who were not employed in the coal industry, aged 40 to 55 years. The diagnosis of atherosclerosis was carried out on the ultrasound system “Vivid E9” of the manufacturing company GE using a linear sensor for measuring the thickness of the intima-media complex and visualization of atherosclerotic plaques. To identify significant risk factors, anthropometric, anamnestic data, indices of lipid and carbohydrate metabolism, and the presence of arterial hypertension were studied.Results. Significant differences in the frequency of asymptomatic atherosclerosis in the form of an increase in intima-media thickness by more than 1 mm and / or the presence of atherosclerotic plaques in both groups were not found: 60.2% among the miners and 62.3% among non-coal mining workers (p=0.703); at the same time, the percentage of detection of atherosclerotic plaques in arteries was significantly lower among coal workers — 46.9% versus 60.5% among people in the comparison group (p=0.016). The common risk factors for all examined subjects were arterial hypertension and increased level of glycated hemoglobin; in turn, in coal miners additional risk factors were increased values of low density lipoproteins and waist-hip index, as well as burdened heredity for cardiovascular diseases, and among the individuals not employed in the coal industry it was smoking.Conclusions. The most significant risk factors for atherosclerosis in coal industry workers were arterial hypertension, raised values of glycated hemoglobin, low density lipoproteins and waist-hip index, as well as burdened heredity for cardiovascular diseases. Significant differences in the frequency of asymptomatic atherosclerosis in the form of an increase in the thickness of the intima-media complex of the main arteries and the presence of atherosclerotic plaques in both groups were not established, but the percentage of the detection of atherosclerotic plaques was lower among coal miners.The authors declare no conflict of interests.

Introduction. Due to the impact of adverse working conditions and climate, workers in coal-mining enterprises in the Arctic are at increased risk of occupational diseases (OD).The aim of the study was to study the working conditions, causes, structure and prevalence of occupational diseases in miners of coal mines in the Arctic.Materials and methods. Th e data of social and hygienic monitoring “Working conditions and occupational morbidity” of the population of Vorkuta and Chukotka Autonomous District in 2007–2017 are studied.Results. It was established that in 2007–2017 years, 2,296 ODs were diagnosed for the first time in 1851 coal mines, mainly in the drifters, clearing face miners, repairmen and machinists of mining excavating machines. Most often, the ODs occurred when exposed to the severity of labor, fibrogenic aerosols and hand-arm vibration. The development of professional pathology in 98% of cases was due to design flaws of machines and mechanisms, as well as imperfections of workplaces and technological processes. Diseases of the musculoskeletal system (36.2%), respiratory organs (28.9%) and nervous system (22.5%) prevailed in the structure of professional pathology of miners of coal mines. Among the three most common nosological forms of OD were radiculopathy (32.1%), chronic bronchitis (27.7%) and mono-polyneuropathy (15.4%). In 2017, coal miners in the Arctic had a professional morbidity rate of 2.82 times higher than the national rates for coal mining.Conclusions. To preserve the health of miners of coal mining enterprises, technical measures to improve working conditions and medical interventions aimed at increasing the body’s resistance to the effects of harmful production and climatic factors are necessary.

Aims. The purpose of this study was to identify current epidemiological features of meningococcal infection in Moscow.Materials and methods. Cases of invasive meningococcal disease in Moscow from 2014 to 2018 and the biomaterial from patients with an invasive meningococcal disease were analyzed.Results. The features of the epidemic process of meningococcal disease in Moscow were revealed: increasing in the incidence rate involving teenagers and young adults into the epidemic process; meningococcal strains of serogroups W and A increased in the etiology of the invasive meningococcal disease; high mortality rate.Conclusions. It seems reasonable to recommend vaccination against meningococcal disease by including adolescents, young adults and persons over 65 years old.

Background and Purpose: Hyperglycemia is reported to be associated with poor outcome in patients with spontaneous Intracerebral Hemorrhage (ICH), but the association between blood glucose level and outcomes in Primary Intraventricular Hemorrhage (PIVH) remains unclear. We sought to identify the parameters associated with admission hyperglycemia and analyze the impact of hyperglycemia on clinical outcome in patients with PIVH. Methods: Patients admitted to Department of Neurosurgery, West China Hospital with PIVH between 2010 and 2016 were retrospectively included in our study. Clinical, radiographic, and laboratory data were collected. Univariate and multivariate logistic regression analyses were used to identify independent predictors of poor outcomes. Results: One hundred and seventy patients were included in the analysis. Mean admission blood glucose level was 7.78±2.73 mmol/L and 10 patients (5.9%) had a history of diabetes mellitus. History of diabetes mellitus (P = 0.01; Odds Ratio [OR], 9.10; 95% Confidence Interval [CI], 1.64 to 50.54) was independent predictor of admission critical hyperglycemia defined at 8.17 mmol/L. Patients with admission critical hyperglycemia poorer outcome at discharge (P < 0.001) and 90 days (P < 0.001). After adjustment, admission blood glucose was significantly associated with discharge (P = 0.01; OR, 1.30; 95% CI, 1.06 to 1.59) and 90-day poor outcomes (P = 0.03; OR, 1.27; 95% CI, 1.03 to 1.58), as well as mortality at 90 days (P = 0.005; OR, 1.41; 95% CI, 1.11 to 1.78). In addition, admission critical hyperglycemia showed significantly increased the incidence rate of pneumonia in PIVH (P = 0.02; OR, 6.04; 95% CI 1.27 to 28.80) even after adjusting for the confounders. Conclusion: Admission blood glucose after PIVH is associated with discharge and 90-day poor outcomes, as well as mortality at 90 days. Admission hyperglycemia significantly increases the incidence rate of pneumonia in PIVH.

Vicharchika (Eczema) is a skin disorder with predominance of Pitta Kapha Dosha, with clinical features like Kandu, Srava, Pidaka, Shyavata, Rookshata, Raji, Ruja and Daha mainly in the extremities. It is the second commonest skin disease affecting all age groups, with incidence rate of 2-3% and high rate of recurrence. Ayurveda emphasizes Shodhana therapy as the main line of treatment in skin disorders. Raktamokshana is indicated as Rakta is mainly involved in Vicharchika. In the present study, two treatment modalities were selected to find out which is more appropriate.

BACKGROUND On March 18, 2020, the Malaysian government implemented Movement Control Order (MCO) to limit the contact rates among the population and infected individuals. OBJECTIVE The objective of this study was to forecast the trend of the COVID-19 epidemic in Malaysia in terms of its magnitude and duration. METHODS Data for this analysis was obtained from publicly available databases, from March 17 until March 27, 2020. By applying the Susceptible, Exposed, Infectious and Removed (SEIR) mathematical model and several predetermined assumptions, two analyses were carried out: without and with MCO implementation. RESULTS Without MCO, it is forecasted that it would take 18 days to reach the peak of infection incidence. The incidence rate would plateau at day 80 and end by day 94, with 43% of the exposed population infected. With the implementation of the MCO, it is forecasted that new cases of infection would peak at day 25, plateau at day 90 and end by day 100. At its peak, the infection could affect up to about 40% of the exposed population. CONCLUSIONS It is forecasted that the COVID-19 epidemic in Malaysia will subside soon after the mid-year of 2020. Although the implementation of MCO can flatten the epidemiological curve, it also prolongs the duration of the epidemic. The MCO can result in several unfavorable consequences in economic and psychosocial aspects. A future work of an exit plan for the MCO should also be devised and implemented gradually. The exit plan raises several timely issues of re-infection resurgence after MCO are lifted.
