Introduction: A life threatening arrhythmia is a medical condition that requires immediate intervention, or it can cost a patient’s life. However, there is limited understanding of nurses’ knowledge and skills in identification and care provided to patients with life threatening arrhythmias in Tanzania.  The objective of this study was to assess knowledge and skills of nurses in identifying life threatening arrhythmias and the required patient care. Methods: A descriptive cross-sectional study was conducted to assess the knowledge and skill level of nurses in identification of life threatening arrhythmias and the required patient care, as well as exploring the barriers in acquiring and implementing particular knowledge and skills. A convenient sample of 141 nurses working in critical care areas at Muhimbili National Hospital, were recruited. Results: The majority (44%) of the participants were 31-40 years old and more than three quarters were females. The majority (60%) scored highly when tested about their knowledge of life threatening arrhythmia identification. However, the majority of nurses (84.4%) scored poorly when being observed on their skills. A total of 116 (82.3%) nurses were able to identify asystole on an electrocardiogram strip, and demonstrated a high level of knowledge (95%) in the nursing care of patients in asystole. Although they demonstrated poor skills in general, nurses were competent (97.9%) in electrode placement on the patient’s chest before connecting the patient to the cardiac monitor. The overwhelming workload was identified by many (68.8%) as the major barrier in acquiring and implementing knowledge and skills. Conclusion: Although the majority of the participants scored highly in their level of knowledge regarding life threatening arrhythmias, they scored poorly in most of the observed skills when identifying and treating this patient group. It is important that hospital administration take into consideration the identified areas of deficiency and work to improve the skills among nurses and enhance optimal care of patients.

However, there is a limited knowledge and skills in identifying and caring for patients with life-threatening arrhythmias among medical students. The aim of this study was to compare 6th year MD program students' pre-post-training knowledge and the ability to detect life-threatening arrhythmias. 140 6th year MD program students took part in this study. Evaluations were performed before and after training in cardiac arrhythmias. Results: The majority of the participants received a high level of knowledge in identifying life-threatening arrhythmias on the ECG strip. Modern teaching methods improve the learning achievements among students.

Summary. Iliac vein compression syndrome (May-Thurner syndrome – MTS) is an anatomically variable clinical condition in which the left common iliac vein is compressed between the right common iliac artery and the underlying spine. This anatomic variant results in an increased incidence of left iliac or iliofemoral vein thrombosis. It predominantly affects young women in the second or third decades of life with preponderance during pregnancy or oral contraceptive use. Although MTS is rare, its true prevalence is underestimated but it can be a life-threatening condition due to development of pulmonary embolism (PE). In this case based review the authors present three cases of MTS. All patients had been previously confirmed with PE, but despite they were admitted to hospital, diagnosed and correctly treated for PE and investigated for thrombophilia, the iliac vein compression syndrome was not suspected or investigated. With this presentation the authors would like to emphasize that MTS is mostly underdiagnosed, and it needs to be ruled out in left iliofemoral vein thrombosis in young individuals.

Background: Alveolar hemorrhage is a potentially life-threatening condition which is usually managed by the pulmonologist. When considering its etiology, there is a rare association that sets the disease into the hands of the gastroenterologist. Case presentation: We report the case of a 48 year-old female who was admitted to the intensive care unit for severe anemia and hemoptysis. On imaging, diffuse pulmonary infiltrates suggestive of alveolar hemorrhage were detected and a diagnosis of pulmonary hemosiderosis was made. She received cortisone therapy and hematologic correction of anemia, with slow recovery. In search of an etiology for the pulmonary hemosiderosis, an extensive workup was done, and celiac disease specific serology was found positive. After confirmation of celiac disease by biopsy, a diagnosis of Lane-Hamilton syndrome was established. The patient was recommended a gluten-free diet and at 6 months follow-up, resolution of anemia and pulmonary infiltrates were observed. Conclusion: Although the association is rare, celiac disease should be considered in a patient with idiopathic pulmonary hemosiderosis. In our case, severe anemia and alveolar infiltrates markedly improved with glucocorticoids and gluten-free diet. Abbreviations: APTT: activated partial thromboplastin time; BAL: bronchoalveolar lavage; CD: celiac disease; Cd: crypt depth; GFD: gluten-free diet; GI: gastrointestinal; IEL: intraepithelial lymphocyte; INR: international normalized ratio; IPH: idiopathic pu

Knowledge and skills have become the most crucial resource capital which enables organizations to survive in the ever changing business environment. One of the common strategies for organizations to increase their performance and productivity is through training and the main role of human resource development is to fulfil the needs of the organizations by providing employees with up to date expertise, information, knowledge and skills. Since huge financial investments and enormous time are allocated for training, organizations hope that the training will lead to the desired workoutcomes. However, this does not always happen. There is only a small percentage of training programs which had successfully shown lasting transferability to the workplace and this indicates thatunderstanding and improving the training transfer process is still a major concern for training researchers and practitioners. As training transfer is influenced by several variables at different levels of analysis, this study attempts to investigate the relationship between training transfer determinants, the involvement of different stakeholders and training activities in the training process. Specifically, this study investigates the influence of training transfer determinants on goal setting amongst small businesswomen. In addition, it will emphasize the roles of primary stakeholders in the social networkat different times during the training process towards achieving training transfer. Therefore, the framework postulates social networks as a moderating variable in enhancing training transfer andgoal setting amongst small businesswomen.
