A Model for Creating a Common Health Promoting School Environment

Introduction: Deterioration of students’ health, the absence of scientific substantiation of consistent actions, key directions and indicators of work of comprehensive schools in the sphere of health protection of participants in the educational process determine the purpose of the study to give a rationale for the algorithm and model of creating a common health promoting school environment. Materials and methods: The study was carried out in four directions: 1) study of health promoting activities in modern schools; 2) analysis of indicators of socio-psychological climate of schools; 3) study of the lifestyle, work pressure and schedule, health status and psychological well-being of teachers; and 4) study of foreign instruments for assessing health promotion interventions at schools. The objects of the study included comprehensive schools, schoolchildren, teachers, and foreign instruments for assessing health promotion interventions at schools. The research materials were statistically processed by nonparametric methods using Statistica 13.3 software. Results: Most of the surveyed Russian schools are at the initial stages of developing health promoting frameworks. Based on the expert statistical analysis of health promotion interventions of Russian schools at different levels of development and having different achievements in the field of preventive activities, effective directions and specific indicators for assessing results at each stage of creating a common preventive environment in a comprehensive school were identified and substantiated. Conclusions: An algorithm and a model of a common preventive school environment consisting of seven components have been developed and a system for its monitoring by key indicators, the subjects and objects of which are students, their parents and teachers, has been substantiated. The results of health promoting activities at school include health improvement in schoolchildren and teachers, their emotional well-being, a decreased prevalence of behavioral risk factors, and improvement of knowledge and skills in relation to health and of the academic performance of students.

Introduction: The concept of the health promoting school offers a coherent approach to promoting health and well-being within a whole school community. This study sought to evaluate the health programmes and policies adopted by different Lebanese schools, and their appropriateness for this role as part of health promotion. Methods: Our research took the form of a prospective observational study involving 50 schools from different areas of Lebanon. Data were collected using a questionnaire based on the Health Assessment Tool for Schools (HATS). Data were analysed to describe school characteristics and to reveal differences in the health topics addressed. Results: Most of the schools included in the study came from Mount Lebanon (56%). Around 70% of the involved schools offered health-related courses in their curricula. Health-related seminars and workshops (60%) and health awareness campaigns took place at the studied schools, with 98% incorporating physical education into the curriculum. Dental health (74%), smoking cessation (72%) and physical activity (68%) were among other most addressed topics, while mental health was the least discussed (20%). The majority (70%) of schools had a shared vision of what promoting health involved and sought to promote the active involvement of community members in the life of the school (60%). Around two-thirds of the schools (66%) provided physical support and facilities, and had developed policies for health promotion. Less than half of these schools’ health committees, however, had developed plans to improve health promotion and review data to ensure the effectiveness of their programmes. Conclusion: The study findings suggest that despite weaknesses, the majority of the sampled schools had either implemented or were in the process of implementing a health promoting school programme to improve health education and students’ well-being.

Purpose – The propositional knowledge about the Health Promoting School (HPS) and how it privileges the health sector, and research through intervention and behaviour change rather than gaining an understanding of how social bases of health impact and influence individuals and the wider school community. The purpose of this paper is to explore how bricolage offers opportunity for understanding complexity, thick description and inter- and multi-disciplinary work. The experience of health promotion and what it looks like at the school level and provides epistemological considerations for reframing research about HPSs for purposes of social justice and equity through bricolage. Design/methodology/approach – An introduction reveals the challenges of health promotion settings, and schools in particular to achieve social justice and equity. Bricolage is discussed with reference to complexity, thick description and inter- and multi-disciplinary work. Considerations are given to bricolage as research to gain understanding and to contribute to social change. Findings – As a setting the HPS is a complex site of social interaction and where there is interplay of multiple, casual factors that influence health and well-being. The potential for social justice and equity remains latent and new approaches to investigating and researching are required. Bricolage offers substantial possibilities as it recognises the value of researching social contexts but with a deliberate intent to engage with participants. Practical implications – This paper considers how bricolage can re-focus ontological and epistemological positions to engage in health promotion as a social action. Originality/value – This paper raises questions about the ability of the HPS model to deliver on social justice under current compliance regimes.

A challenge for implementing DOHaD-defined health promotion is how to engage the at-risk population. The WHO Health Promoting School (HPS) model has proven success engaging youth and improving health behaviors. Hence, we introduced DOHaD concepts to 151 pupils aged 12–15 years in three HPS programs in rural Uganda, inquired what factors would make DOHaD-related health promotion resonate with them, and discussed how they recommended making learning about DOHaD acceptable to youth. Economic factors were judged the most compelling; with nutrition and responsive care elements next in importance. Suggested approaches included: teach how good health is beneficial, what works and why, and give tools to use to achieve it, and make information positive rather than linked to later harm. Involve youth in making DOHaD learning happen, make being a parent sound interesting, and include issues meaningful to boys. These are the first data from youth charged with addressing their engagement in the DOHaD agenda.

The purpose of the study was to present the process and results of developing standards and self-evaluation procedures for health promoting special schools (HPSS) for children with intellectual disabilities. The work was undertaken in order to address the needs of special schools interested in establishing a health promoting school (HPS). We used following methods: 1) Preparation: interviews with principals and teachers at 8 special schools, as well as regional HPS network coordinators, visits to 4 schools; 2) Development of HPSS project standards, self-evaluation procedures and tools: consultations with representatives of 8 schools and with regional coordinators; 3) Pilot study of HPSS the self-evaluation procedure and tools at 8 schools and development of a finalised version: direct observation, document analysis, interviews with selected participants, survey studies of school staff and students’ parents, testing students using one of the following methods: “Draw and write”, “Draw and tell”, conversation or written response. The study included people supporting HPS on a national and regional level, school principals and health promotion coordinators from special schools for students with intellectual disabilities. The pilot study was conducted on a group of 341 teaching staff, 148 non-teaching staff, 468 parents of students, and 435 students. The analysis of the study’s reports and consultations with school principals and health-promotion coordinators from participating schools guided the development of the final version of standards, in addition to the associated model of HPSS and self-evaluation procedure along with a set of tools to measure the accomplishment of the standards. The HPSS standards and self-evaluation procedure were approved by the Ministry of National Education and officially disseminated, with special schools gaining the opportunity to apply for the Health Promoting School National Certificate. The self-evaluation element of HPSS makes it possible to improve the activities of special schools with respect to health promotion while encouraging collaboration and exchange of ideas with regular schools.

Work environment factors are highly correlated with employees’ health and well-being. Our aim was to sum up current evidence of health promotion interventions in the workplace, focusing on interventions for the prevention of musculoskeletal disorders, psychological and behavioral disorders as well as interventions for older employees and economic evaluations. We conducted a comprehensive literature search including systematic reviews published from April 2012 to October 2017 in electronic databases and search engines, websites of relevant organizations and institutions. It consisted of simple and specific terms and word combinations related to workplace health promotion based on the search strategy of a previous review. After full-text screening, 74 references met the eligibility criteria. Using the same search strategy, there was a higher proportion of relevant high-quality studies as compared with the earlier review. The heterogeneity of health promotion interventions regarding intervention components, settings and study populations still limits the comparability of studies. Future studies should also address the societal and insurer perspective, including costs to the worker such as lost income and lost time at work of family members due to caregiving activities. To this end, more high-quality evidence is needed.

Abstract Background Sports clubs offer a unique position to increase performance and physical activity but may also provide additional health promoting opportunities. Research is limited on support clubs need to increase health promotion efforts. This study took a participative approach to gather French stakeholder ideas on perceived assistance sports clubs need to increase health promotion efforts and prioritized them based on ratings of importance and feasibility. Methods This concept mapping study had 4-steps: 1) drafting a focus prompt to a key issue, 2) brainstorming ideas in response to the focus prompt, 3) sorting ideas into themed piles and 4) rating ideas (1-6) based on two indicators. French stakeholders (45) in sports and health organizations were invited to respond using the groupwisdom™ platform. Researchers produced visual cluster maps of themed piles and Go-Zone graphs displaying ideas perceived as important and feasible. Results Participants generated 62 ideas responding to the focus prompt: 'What assistance would benefit sports clubs to become health-promoting settings?'. Once researchers edited ideas, 78 were available to sort. Final sorting formed 9 clusters: Tools for health promotion, Communication tools, Stakeholder training courses, Diagnostic & Financing, Awareness & Mobilization, Advocacy, Policies & Methods, Sharing & Networking, Communication & Dissemination. Importance and feasibility ratings produced Go-Zones with 34 ideas above the mean for both indicators. Top focus areas include: increasing awareness of health promotion benefits, mobilizing actors, advocating for support and educating club actors. Conclusions Understanding support stakeholders need to increase health promotion efforts in sports clubs is a crucial step to plan and implement policies. Including stakeholders' perceptions helps establish effective interventions by increasing the possibility of integration into current or emerging policies and acceptance from those working in clubs. Key messages Generating and organizing stakeholder ideas gives insight into perceptions of what support is needed to develop and implement health promotion interventions in the sports club context. Based on importance and feasibility, sports clubs can increase health promotion efforts by focusing on: increasing awareness of health promotion benefits, mobilizing actors, advocacy and education.

Abstract Background Researchers and policymakers have acknowledged sports clubs (SCs) as health promoting settings. Limited research links the health promoting sports club (HPSC) concept with evidence-driven strategies to offer SCs guidance to develop health promotion interventions. As implementation science insists on theoretically grounded interventions, this work's objective was to provide SCs an evidence-driven intervention framework for planning health promotions. Methods A 4-step process was undertaken: 1) investigate indicators for SCs to be considered health promoting, 2) adapt the theoretical HPSC concept to create a HPSC model, 3) reformulate published evidence-driven guidelines into implementable intervention components (ICs) and 4) merge the model with the ICs to provide an intervention planning framework for SCs. During 3 workshops, researchers defined the model elements and ICs. Workshop participants classified ICs into the HSPC model. Each IC could be classified multiple times within the model. Results Researchers drafted 5 HPSC indicators: 1) an approach embracing all SC actions, 2) involve all SC levels in actions and decisions, 3) involve external partners, 4) promoting health is continuous and iterative and 5) base actions on needs. To create the HPSC model, elements were defined: 3 SC levels (club, management, coaches) and 4 health determinants (organizational, social, environmental, economic) per level based on the indicators. Published guidelines from literature reviews aided in developing 14 strategies with 55 ICs. Workshop classification of ICs into the model included: club (n = 79), management (n = 67) and coaches (n = 48). Conclusions The theoretical HPSC model and intervention planning framework act as starting points to develop and implement interventions to increase HP efforts by stakeholders in several ways: 1) SCs can apply strategies based on goals, 2) SCs can target specific levels with corresponding ICs or 3) ICs can target specific health determinants. Key messages A Health Promoting Sports Club model defines 4 health determinants at 3 levels (coach, management operational) of sports clubs to plan, develop and implement targeted health promotion activities. This HPSC intervention planning framework has 14 strategies with 55 intervention components targeting multiple sports club levels giving stakeholders a path to become a health promoting sports club.

Considering the population aging and the failure of biologistic and hospital-based treatment model, health promotion programs based on scientific evidence are necessary. A comprehensive review of literature was performed, aiming to identify and analyze health promotion and education experiments focused on the aging process. Papers published in eight databases, together with the database of the Pan-American Health Organization, were selected based on review of titles and abstracts, followed by a full text review conducted by two independent reviewers. A total of 22 studies were included, the majority of which adopted a quantitative approach, with a sample larger than 100 elderly or pre-retirement individuals. The majority of studies reported positive results in terms of health promotion and education. One study obtained minimum improvement and one reported that no statistically significant improvement had occurred. The positive effects most indicated by authors were: general or self-perceived improvement in physical health, improvement in psychosocial aspects and in relation to the aging process, improvement in adherence to preventative actions and in healthy conduct and lifestyle, increase in level of physical activity, improvement in quality of life and/or physical well-being, and improvement in activities of daily living or reduction of the risk of developing disabilities.

The complex program of health promotion for university students with disorders of adaptation processes was developed. The program includes application of dynamic, isometric and breathing exercises at working table during micro-pauses for amelioration of cerebral blood circulation. Increasing of resistance of organism to shortage of oxygen and normalization of vegetative functions. The application of massage of biologically active points applied during scheduled breaks is addressed to optimizing functional conditions of students. The implementation of complex of common developing exercises, application of health promoting running and rainy shower in the end of training exercise is oriented on increasing physical readiness and stimulating students' own adaptive possibilities.The approbation of originally developed program demonstrated its good efficiency in adjustment of deadaptation disorders occurring in the process of education. The results of study demonstrated that in boys and girls after three-months health promoting training neuro-psychic tension decreased, resistance to stress impact increased and anxiety lessened. Simultaneously, increasing of number of students with high level of adaptation to actual social conditions was observed. The amelioration of vegetative resistance of organisms of students was noted. The testing of physical readiness demonstrated that in students, after correcting exercises, muscular strength of wrists increased, velocity force capacities and indices of force endurance in control exercise "jump-rope" ameliorated and suppleness and static resistance of organism. The application of proposed means of health promotion promoted mobilization of workability of nervous cells and nervous system that is substantiated by the results of tapping-test.The application of health promotion program resulted in increasing of academic progress in studies of boys and girls up to 0.4-0.7 points. The obtained data was used to recommend for implementing in educational institutions the developed program of health promoting student youth and minimizing deadaptation disorders occurring in student social environment.

There are profound health inequities for agricultural workers. We sought to assess the literature on migrant and seasonal farmworker health with an eye toward health promotion interventions, special populations, use of community health workers (CHWs), and technology. We conducted a systematic mapping review by searching five databases in March 2021. Using quantitative content analysis after establishing interrater reliability, we coded titles and abstracts to assess 13 topics and six characteristics of the research such as its focus on health promotion, use of technology, and inclusion of CHWs. We identified 1,083 records. Just 8.2% of records described or evaluated a health promotion effort to intervene in farmworker well-being and even fewer (4.2%) examined unique populations of farmworkers such as indigenous farmworkers ( n = 11) or sexual minority farmworkers ( n = 1). A small body of literature focused on the role of CHWs or promotores most frequently described their role in implementing health interventions. The literature on farmworker health has gaps regarding health promotion interventions, special populations, and integration of CHWs into research projects. We offer suggestions to fill in identified gaps in the literature.
