Abstract: Along with the development of medical technology, endoscopy is mostly used in determination of the diagnosis and examination of gastrointestinal diseases. This study was aimed to determine the indications of gastrointestinal endoscopy, diagnoses of pre and post endoscopy, sex and age of patients undergoing endoscopy, and the prevalence of endoscopic patients from January 2018 to August 2019 at Prof. Dr. R. D. Kandou Hospital Manado. This was a descriptive and retrospective study using data of Medical Record Installation at Prof. Dr. R. D. Kandou Hospital. The results obtained 495 patients who were endoscopy performed on them. Males were predominant (59.8%) as well as age group of 50-59 years old (22.8%). The most frequent indication of endoscopy was dyspepsia/epigastric pain (68.5%). Moreover, EGD plus colonoscopy was the most common endoscopy performed. GERD (20.45%) had the highest percentage of pre endoscopy diagnosis meanwhile esophagitis Los Angeles Classification Grade A (28.8%) had the highest percentage of post endoscopy diagnosis. In conclusion, gastrointestinal endoscopy was performed more common on males, age group of 50-59 years, with indication of dyspepsia (epigastric pain), and in EGD plus colonoscopy. Abstract: Gastrointestinal Endoscopy (GIE) is a technique in gastroenterology – hepatology to directly see condition of the gastrointestinal tract by using a tool called endoscope. For endoscopy in the upper gastrointestinal tract is called esophagogastroduodenoscopy (EGD). Besides Esophagogastroduodenoscopy there is also colonoscopy, is used to evaluate the condition of lower gastrointestinal tract, in the area of rectum, sigmoid colon, descending colon, transverse colon, ascending colon, cecum, and also ileum.This research is aimed to understanding profile of gastrointestinal endoscopy in patients in RSUP Prof. Dr. R. D. Kandou Manado period January 2016 – August 2016. This was a descriptive retrospective study using taking secondary data of patients from Medical Record Department of Prof. Dr. R. D. Kandou Hospital Manado. The results showed that there 59 patients with endoscopy examination performed on them. The majority of patients were 30 males (51%), age group 50-59 years old (30%), with dyspepsia (epigastric pain) as indication (57%), EGD (80%), and chronic gastritis plus Helicobacter pylori infection as diagnosis after biopsy (38%).Keywords: endoscopy, indication, diagnosis Abstrak: Endoskopi gastrointestinal (EGI) merupakan salah satu teknik dalam ilmu gastroenterology-hepatologi untuk melihat secara langsung keadaan di dalam saluran cerna dengan menggunakan alat yang bernama endoskop. Pemeriksaan endoskopi pada saluran cerna bagian atas disebut esofagogastrodudenoskopi (EGD) sedangkan kolonoskopi digunakan untuk mengevaluasi serta memeriksa lumen pada saluran cerna bagian bawah, yaitu pada daerah rektum, kolon sigmoid, kolon desenden, kolon transversum, kolon asenden, sekum, serta ileum. Penelitian ini bertujuan unutk mengetahui profil EGI pada pasien di RSUP Prof. DR. R. D. Kandou periode Januari 2016-Agustus 2016. Jenis penelitian ialah deskriptif retrospektif dengan menggunakan data sekunder pasien di Instalasi Rekam Medik RSUP Prof. Dr. R. D. Kando Manado. Dari hasil penelitian diperoleh pasien yang melakukan pemeriksaan endoskopi sebanyak 59 orang. Mayoritas pasien ialah pasien jenis kelamin laki-laki sebanyak 30 orang (51%), kelompok usia 50-59 tahun (30%), dengan indikasi dispepsia (nyeri epigastrium) (57%), tindakan endoskopi jenis EGD (80%), dan diagnosis dibiopsi gastritis kronik dengan infeksi Helicobacter pylori (38%). BACKGROUND: Esomeprazole, an S-isomer of omeprazole, is the first proton pump inhibitor developed as an optical isomer, and it has shown high healing rates in erosive esophagitis. AIM: To evaluate the efficacy and tolerability of esomeprazole in subjects with erosive esophagitis, according to the Los Angeles classification study design: an open, multi-center clinical study. MATERIAL AND METHODS: Two hundred and eighteen subjects with reflux esophagitis confirmed by endoscopy were included in an open, multi-center study in Brazil. All of them received esomeprazole 40 mg, once daily, for a 4-week period. Subjects who had unhealed esophagitis by week 4 continued the treatment for another 4 weeks. The primary efficacy endpoint was the healing rates by weeks 4 and 8. The secondary endpoints were the number of patients with symptom resolution by week 4, the number of days to sustained symptom resolution, number of symptom-free days and nights and safety and tolerability of the drug. RESULTS: Healing rates by weeks 4 and 8 were 82% (confidence interval: 77.4%-87.6%) and 96.1% (confidence interval: 93.5% - 98.8%), respectively. Ninety-nine (99%) of the patients had heartburn resolution by week 2. The most common adverse events were headache (4%), diarrhea (2.6%) and epigastric pain (2.2%). CONCLUSION: For the studied period, esomeprazole was shown to be a safe and well-tolerated drug, providing significant healing rates of mucosal breaks, regardless of LA classification, in patients with erosive esophagitis. Esomeprazole was also shown to be effective in quickly relieving symptoms.

ABSTRACT BACKGROUND: The gastroesophageal reflux disease (GERD) is the most common esophageal disease in medical practice, and it is suspected according to patients’ symptoms. GERD can be classified in erosive esophagitis (EE) according to the presence of upper gastrointestinal endoscopy findings. OBJECTIVE: To evaluate endoscopic findings in patients with symptoms suggestive of GERD comparing epicemiological and risk factors. METHODS: Upper endoscopy reports were examined retrospectively from patients with symptoms of GERD such as heartburn, regurgitation, cough, throat clearing, globus and chest pain. EE was determined based on Los Angeles classification. Comparisons between risk factors in EE and non-EE groups were done with statistical analysis. RESULTS: A total of 984 endoscopic reports were examined and 676 selected for analysis (281 with EE and 395 with non-EE form). Most were female 381 (56.36%) with a mean age of 44.01±15.40 years. Hiatal hernia was present in 47(6.96%) and smoking in 41(6.07%). Univariate logistic regression showed that male (OR=2.24, CI 95%, 1.63-3.06) and hiatal hernia (OR=4.52, CI 95%, 2.30-8.89) were independent predictors of erosions in the EE group. The presence of hiatal hernia (OR=12.04, CI 95%, 3.57-40.62), smoking (OR=8.46, CI 95%, 3.28-31.32) and aged patients (OR=8.01, CI 95%, 2.42-26.49) were also indicated as a risk factor for severe EE (grades C and D of Los Angeles). CONCLUSION Male gender and hiatal hernia were associated with EE. Aged patients, smoking and hiatal hernia were related to severe EE. It is suggested that the risk factors for EE and non-EE types are different. Cohort studies are necessary to identify the exact mechanisms involved in each disease form.

Malaria is one of the health problems in Indonesia, especially West Sumatra. Determination of the description of Malaria disease in an area is needed to determine the spread and severity of the disease. This study aims to determine the frequency distribution according to age, sex and place of residence, description of the types of Plasmodium causes of Malaria and hematological features in Malaria patients at Siti Rahmah Padang Hospital in 2018. This type of research is a descriptive observational study with an approach or design cross section (cross sectional). The frequency distribution of Malaria sufferers in Siti Rahmah Padang Hospital in 2018 according to the highest age was in the age group 21-30 years as many as 28 cases (36.8%), the highest sex among men was 46 (60.5%) and the highest number of residences was found in Koto Tangah sub-district there were 31 cases (40.8%). The type of Plasmodium found in Malaria cases in Siti Rahmah Padang Hospital in 2018 was P. vivax (73 cases (96.05%)) and P. falciparum (3 cases (3.95%)). The results of laboratory tests on Hb, hematocrit, platelet and leukocyte levels in Malaria positive patients in Siti Rahmah Padang Hospital in 2018 were in the normal range.

BACKGROUND Issues of geriatric otolaryngologic emergency have not been widely applied despite increase in geriatric population. OBJECTIVE This study aimed at determining prevalence, sociodemographic features, aetiology, clinical features, Complications and sources ofreferral of geriatric otorhinolaryngological, head and neck emergency in our center. METHODS This was a prospective hospital based study of geriatric otorhinolaryngology emergency in the Ear, Nose and Throat Department of Ekiti State University Teaching Hospital. The study was carried out between October 2016 and September 2018. Data were obtained by using pretested interviewers questionnaire.All data were collated and analyzed using SPSS version 18.0. The data were expressed by frequency table, percentage, bar charts and pie charts. RESULTS Geriatric otorhinolaryngology, head and neck emergency accounted for 5.3%. Major prevalence age group was 43.9% in the age group (60-64). There were 38.6% males with male to female ratio of 1:1.5. The main aetiology of geriatric otorhinolaryngology emergency was 29.5% trauma/road traffic accident/foreign body impaction and 25.8% tumour. Main anatomical distribution of geriatric otorhinolaryngology emergency were 38.6% throat diseases and 31.1% ear diseases. The most frequent clinical features were pain in 27.3%, hearing loss in 21.2%, tinnitus in 15.9%, bleeding in 14.4%, difficulty breathing in 12.9% and discharge in 11.4%. Common diagnosis in this study were 15.9% sinonasal tumour, 14.4% upper aerodigestive foreign body impaction, 10.6% earwax impaction and 19.8% otitis externa. Acute presentation (<13 weeks) occurred in 1 week in 74.2% and 2-13 weeks In 19.7%. Commonest time of presentation was daytime in 65.9%. Major sources of referral were 43.2% general practitioner and 31.1% casualty officers. Presentation of geriatric otorhinolaryngology emergency were mainly ear, nose and throat clinic in 59.8% with accident and emergency in 28.8%. Commonest associated comorbid illnesses among the geriatric patients were 18.2% hypertension, 14.4% arthritis and 9.8% diabetes mellitus. CONCLUSIONS Geriatric otorhinolaryngological emergency are common pathology with associated with comorbid illnesses. Detailed clinical assessment are mandatory for effective management outcome.

Longobardi and colleagues examined the effect of inflammatory bowel disease (IBD) on employment, using data from 10,891 respondents aged 20 to 64 years from the 1998 cycle of the Canadian National Population Health Survey (NPHS) (1). This sample included 187 (1.7%) subjects who self-reported IBD or a similar bowel disorder. A significantly greater proportion of IBD than non-IBD respondents reported that they were not in the labour force (28.9% versus 18.5%). Even after adjusting for other factors (age group, level of pain, etc), subjects with IBD had a 2.9% higher nonparticipation rate (21.4%). For example, among people not hospitalized within the past year and with no limitation of activities due to pain, IBD subjects were 1.2 times more likely to be unemployed than those without IBD. Subjects who reported high levels of pain had a very high probability of being out of the labour force. Based on Canadian annual compensation data for all employed persons in Canada, and age- and sex-specific prevalence, and incidence rates for IBD, the authors estimated that there are 119,980 IBD patients between the ages of 20 and 64 years in Canada and that this group includes 3479 people who are not in the labour force. This translates into lost wages of $104.2 million, or $868 per IBD patient

Background:Nausea and vomiting are common during the first half of pregnancy and usually require only supportive measures. When symptoms are progressive and weight loss occurs, treatable causes should be sought by means of upper gastrointestinal endoscopy. We report a case of an immunocompetent gravida with invasiveCandida albicansesophagitis.Case:The immunocompetent primigravida developed progressive nausea, vomiting, epigastric pain, and a 4.1 kg weight loss during the second trimester of pregnancy. Treatment with metoclopramide and cimetidine for presumed gastroesophageal reflux was not effective. The patient had normal T-cell CD4 and CD8 subsets and was human immunodeficiency virus (HIV) antibody negative. Upper gastrointestinal endoscopy revealedC. albicansesophagitis which was treated with oral nystatin. The esophagitis had resolved completely when reassessed postpartum. The use of histamine2blockers is associated with an increased risk for fungal esophagitis and may have been a contributing cause in this case.Conclusion:Pregnant patients with persistent nausea, vomiting, and weight loss should be evaluated by endoscopy for fungal esophagitis.
