TB paru merupakan penyakit menular yang disebabkan Mycobacterium tuberculosis. Penularan terjadi ketika penderita TB paru BTA positif batuk atau bersn dan tanpa disengaja penderita menyebarkan kuman ke udara dalam bentuk percikan dahak.Seorang penderita TB paru BTA Positif dapat mengifeksi 10-15 orang disekitarnya.Ketidakpatuhan penderita untuk berobat ini dipengaruhi oleh banyak faktor. Beberapa faktor diantaranya yaitu faktor tingkat pengetahuan,dukungan keluarga,dan pengawas menelan obat (PMO).Penelitian ini bertujuan untuk mengetahui adanya determinan social kesehatan terhadap kepatuhan pengobatan dots penderita Tb paru diwilayah kerja  puskesmas manuju kabupaten Gowa. Penelitian ini menggunakan metode deskriftive analitik dengan menggunakan metode pendekatan studi “Cross Sectional”dengan mengambil sampel dari suatu populasi dan menggunakan kuesioner sebagai alat pengumpul data. Populasi dalam Penelitian ini adalah jumlah sampel 33 orang. Data yang diperoleh diolah dala bentuk analisis univariat bivariate dan multivariate dengan α=0,05 dengan menggunakan SPSS.Hasil penelitian ini menunjukan bahwa hubungan pengetahuan p=0,02 lebih kecil dari α=0,05 artinya ada hubungan antara tingkat pengetahuan dengan ketidakpatuhan pengobatan, dukungan keluarga (p=0,01) lebih kecil dari α=0,05 artinya ada hubungan antara dukungan keluarga dengan ketidakpatuhan pengobatan, dan pengawasan menelan obat (p=0,00) lebih kecil dari α=0,05 artinya ada hubungan antara pengawas menelan obat dengan ketidakpatuhan berobat. Kesimpulan penelitian ini adalah Ada Hubungan Pengetahuan, dukungan keluarga dan pengawasan menelan obat Terhadap kepatuhan pengobatan DOTs penderita TB paru di wilayah kerja puskesmas manuju kabupaten Gowa

Tuberkulosis Paru merupakan penyakit menular yang disebabkan oleh Mycobacterium tuberculosis. Dampak TB Paru adalah penurunan daya tahan tubuh, kelemahan fisik, merugikan secara ekonomis dan dapat mengakibatkan isolasi sosial. Keadaan tersebut dapat mempengaruhi harga diri penderita TB Paru. Perubahan harga diri pada penderita TB Paru dapat mempengaruhi keberhasilan pengobatan, sehingga dibutuhkan adanya dukungan keluarga. Tujuan Penelitian ini adalah untuk mengidentifikasi pengaruh dukungan keluarga terhadap harga diri penderita TB Paru di Wilayah Puskesmas Eks Kawedanan Indramayu tahun 2015. Jenis penelitian ini adalah deskriptif analitik dengan rancangan penelitian cross sectional study. Pengambilan sampel dilakukan dengan tekhnik total sampling sebayak 45 responden. Hasil penelitian menunjukkan bahwa penderita TB Paru yang memiliki harga diri tinggi sebanyak 23 responden (51,1%) dan yang memiliki harga diri rendah sebanyak 22 responden (48,9%), penderita TB Paru yang mendapatkan dukungan keluarga sebanyak 26 responden (57,8 %) dan yang tidak mendapatkandukungan keluarga sebanyak 19 (42,2 %). Responden yang mendapatkan dukungan keluarga lebih banyak yang memiliki harga diri tinggi dibandingkan dengan responden yang tidak mendapatkan dukungan keluarga yaitu 69,6% dengan nilai p value = 0,047 (< 0,05). Simpulan dari penelitian ini adalah terdapat hubungandukungan keluarga denganharga diri penderita TB Paru. Hasil penelitian ini diharapkan dapat menjadi masukan bagi pemegang program TB untuk memberikan konseling terkait pentingnya dukungan keluarga bagi penderita TB Paru.

Abstract: Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis (M. tuberculosis). In Indonesia, there are about 430.000 new cases, of which 61.000 cases ended in death. This disease has many clinical varieties, therefore, a gold standard for the right and exact diagnosis is needed. The examination of sputum by using Ziehl-Neelsen staining must be more improved for public health service. This study aimed to determine the profile of Mycobacterium tuberculosis (acid-fast bacteria) among patients with coughing ≥2 weeks at Ranotana, Wenang and Sario Primary Health Cares (PHCs) by using Ziehl-Neelsen staining. This was a descriptive study with a cross sectional design. Samples were obtained by using total sampling method during the period of September 2015 - December 2015. The results showed that there were 38 cases of coughing ≥2 weeks as follows: 15 cases at Wenang PHC, 13 cases at Ranotana PHC, and 10 cases at Sario PHC. The examination of acid-fast bacteria from the 38 cases of three PHCs showed that 1 case (2.7%) had acid-fast bacteria (++). Conclusion: In this study, there was only one case (2,7%) with positive Mycobacterium tuberculosis. Indonesia memiliki sekitar 430.000 kasus baru dimana 61.000 kasus berakhir dengan kematian. Penyakit ini memiliki gejala klinis yang bervariasi sehingga perlu ditetapkan standar baku untuk menegakkan diagnosis lebih cepat dan akurat. Pemeriksaan sputum dengan pewarnaan Ziehl-Neelsen harus lebih ditingkatkan pada pelayanan kesehatan masyarakat. Penelitian ini bertujuan untuk mendapatkan gambaran Mycobacterium tuberculosis (basil tahan asam, BTA) dengan pewarnaan Ziehl-Neelsen pada pasien batuk ≥2 minggu di Puskesmas Wenang, Puskesmas Ranotana, dan Puskesmas Sario Kota Manado. Jenis penelitian ini deskriptif dengan desain potong lintang. Pengambilan sampel menggunakan cara total sampling pada kurun waktu September 2015 - Desember 2015. Hasil penelitian mendapatkan 38 kasus batuk ≥2 minggu yaitu 15 kasus di Puskesmas Wenang, 13 kasus di Puskesmas Ranotana dan 10 kasus di Puskesmas Sario. Pada pemeriksaan (BTA) di Puskesmas Wenang, Puskesmas Ranotana dan Puskesmas Sario didapatkan BTA (++) 2,7% sedangkan BTA (-) 97,3%.Simpulan: Pada penelitian ini didapatkan 1 kasus (2,7%) Mycobacterium tuberculosis positif

Tuberkulosis adalah penyakit infeksi paru yang disebabkan oleh Mycobacterium Tuberculosis. Di Indonesia kasus tuberkulosis setiap tahunnya semakin meningkat. Indonesia menepati urutan ke-6 penderita tuberkulosis tertinggi di dunia, sedangkan Jawa Barat menepati urutan pertama. Tujuan penelitian ini adalah untuk melihat adanya hubungan anatar efikasi diri dengan kepatuhan minum obat OAT. Metode penelitian menggunakan desain deskriptif korelasional dengan pendekatan cross sectional dengan melibatkan 23 responden yang sedang menjalani pengobatan di puskesmas parongpong, responden dipilih dengan menggunakan Purposive sampling. Variabel independen adalah efikasi diri, variabel dependent adalah kepatuhan minum obat. Instrumen dalam penelitian ini adalah kuesioner yang dianalisis menggunakan pearson’s r . Hasil dan analisis Efikasi Diri dengan Kepatuhan Minum obat mempunyai hubungan  dengan nilai  p-value 0,030 atau (p ≤ 0,05) dengan tingkat hubungan sedang (0,454). Efikasi merupakan salah satu faktor yang mempengaruhi kepatuhan minum obat pasien tuberkulosis di wilayah kerja Puskesmas Parongpong. Saran untuk penelitian selanjutnya perlu mengetahui peran kader kesehatan dalam kepatuhan minum obat.

Objective To compare the diagnostic performance of the manual BACTEC™ Mycobacteria Growth Indicator Tube (MGIT™) system (M-MGIT) with the automated BACTEC™ MGIT™ 960 system (A-MGIT) and Löwenstein-Jensen (L-J) culture method in detecting mycobacteria in sputum specimens from patients with suspected pulmonary tuberculosis (TB). Methods For this cross-sectional study, sputum samples were taken from patients aged ≥18 years attending a TB clinic in Beijing, China between July 2015 and October 2016. Processed sputum samples were inoculated into the MGIT systems and L-J medium for up to 6 and 8 weeks, respectively. Results The M-MGIT and A-MGIT methods detected significantly more Mycobacterium tuberculosis complex (MTC) isolates than L-J culture from the 565 sputum samples (39%, 40% and 32%, respectively). Using a positive result from any of the three culture systems as reference, the sensitivity of M-MGIT, A-MGIT and L-J methods were 92%, 94%, and 74%, respectively. The time-to-detection of mycobacteria was 12.9±4.2 days for M-MGIT, 11.8±5.2 days for A-MGIT and 24.2±8.7 days for L-J. Conclusions M-MGIT has a similar diagnostic performance to A-MGIT, and is a fast and reliable alternative to conventional culture methods in the diagnosis of pulmonary TB in a developing country.

Abstract Objective: To evaluate Gene Xpert MTB/RIF and Multiplex PCRfor detection of Mycobacterium tuberculosis and Rifampicin resistance comparing with gold standard MGIT 960. It was cross sectional validation study. Methods: This study had been carried out at Department of Microbiology Armed Forces Institute of Pathology (AFIP), Rawalpindi Pakistan from March 2018 to October 2018.MGIT 960 culture system MTB positive Rifampicin (RIF) resistant and RIF susceptible (negative control) samples were taken. Gene Xpert MTB / RIF assay and Multiplex PCR were applied simultaneously and compared with gold standard MGIT 960. Results: Out of 192 samples evaluated, 84(44%) were culture positive RIF resistant and 108(56%) were culture positive RIF susceptible as negative control. Out of total culture positive RIF resistant, all 84 were found positive for MTB by Gene Xpert MTB /RIF assay and Multiplex PCR method. Gene Xpert MTB/RIF assay detected all 84 RIF culture resistant samples for Rif resistance. Multiplex PCR detected only 44 RIF culture resistant, while remaining 40 did not showed resistance to rpoB gene codon 531 region. Sensitivity, Specificity, PPV and NPV of Gene Xpert MTB/RIF were 100% each respectively. Sensitivity, Specificity, PPV and NPV of Multiplex PCR for detection of RIF resistance were 52%, 100%, 100%, 72% respectively. Conclusion: Molecular detection of MTB and RIF resistant by Gene Xpert MTB/ RIF and Multiplex PCR simultaneously is rapid and cost effective method. Both methods can help clinician to initiate early empirical therapy to patient in resource limited region. Keywords: RIF, Gene Xpert MTB/ RIF, Multiplex PCR.

The incidence of pulmonary tuberculosis (PTB) can be reduced by preventing transmission with rapid and precise case detection and early treatment. The Gene-Xpert MTB/RIF assay is a useful tool for detecting Mycobacterium tuberculosis (MTB) with rifampicin resistance within approximately two hours by using a nucleic acid amplification technique. This study was designed to reduce the underdiagnosis of smear-negative pulmonary TB and to assess the clinical and radiological characteristics of PTB patients. This cross-sectional study included 235 participants who went to the Luyang primary health care clinic from September 2016 to June 2017. The demographic data were analyzed to investigate the association of patient gender, age group, and ethnicity by chi-square test. To assess the efficacy of the diagnostic test, the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy were calculated. The area under the curve for sputum for both AFB and gene-Xpert was analyzed to compare their accuracy in diagnosing TB. In this study, TB was more common in males than in females. The majority (50.71%) of the cases belonged to the 25–44-year-old age group and the Bajau ethnicity (57.74%). Out of 50 pulmonary TB cases (smear-positive with AFB staining), 49 samples were positive according to the Gene-Xpert MTB/RIF assay and was confirmed by MTB culture. However, out of 185 smear-negative presumptive cases, 21 cases were positive by Gene-Xpert MTB/RIF assay in that a sample showed drug resistance, and these results were confirmed by MTB culture, showing resistance to isoniazid. In comparison to sputum for AFB, Gene-Xpert showed more sensitivity and specificity with almost complete accuracy. The additional 21 PTB cases detection from the presumptive cases by GeneXpert had significant impact compared to initial observation by the routine tests which overcame the diagnostic challenges and ambiguities.

Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis. The diagnosis of pulmonary TB in general still relies on microscopic examination with a sputum smear for Acid Fast Basil (AFB). Microscopic examination to detect M. tuberculosis has high specificity, but the sensitivity is 35-70%. The sputum culture method is the gold standard for the diagnosis of pulmonary TB but requires a long time, which is 6-8 weeks. In recent years ICT (Immuno Chromatography Test) has been introduced to identify potential and useful TB antigens to help diagnose pulmonary TB. Antigens detected were ESAT-6, CFP-10, CFP-21, and MPT-64. This study aims to compare the results of the examination of specimens of suspect TB sputum with the ICT method and AFB. This study based on observational with analytic design cross-sectional on 56 samples of Stored Biological Material suspects TB that were examined by the AFB method and the ICT method (Cocktail Antigen). Statistical test results Fisher Exact (α = 0.05) showed no difference between microscopic examination of smear sputum and ICT (Cocktail Antigen) TB (p = 1,000). Nevertheless, the use of ICT reagents for the diagnosis of pulmonary TB still needs to be further investigated, especially to find out the exact causes of false positive and false negative reactions.
