Background: Arteriovenous fistula (AVF) for hemodialysis integrates outward remodeling with vessel wall thickening in response to drastic hemodynamic changes. Aim of this study is to determine the role of Ki67, a well-established proliferative marker, related to AVF, and its relationship with time-dependent histological morphologic changes. Materials and methods: All patients were enrolled in 1 year and stratified in two groups: (A) pre-dialysis patients submitted to first AVF and (B) patients submitted to revision of AVF. Morphological changes: neo-angiogenesis (NAG), myointimal thickening (MIT), inflammatory infiltrate (IT), and aneurysmatic fistula degeneration (AD). The time of AVF creation was recorded. A biopsy of native vein in Group A and of arterialized vein in Group B was submitted to histological and immunohistochemical (IHC) analysis. IHC for Ki67 was automatically performed in all specimens. Ki67 immunoreactivity was assessed as the mean number of positive cells on several high-power fields, counted in the hot spots. Results: A total of 138 patients were enrolled, 69 (50.0%) Group A and 69 (50.0%) Group B. No NAG or MIT were found in Group A. Seven (10.1%) Group A veins showed a mild MIT. Analyzing the Group B, a moderate-to-severe MIT was present in 35 (50.7%), IT in 19 (27.5%), NAG in 37 (53.6%); AD was present in 10 (14.5%). All AVF of Group B with the exception of one (1.4%) showed a positivity for Ki67, with a mean of 12.31 ± 13.79 positive cells/hot spot (range 0–65). Ki67-immunoreactive cells had a subendothelial localization in 23 (33.3%) cases, a myointimal localization in SMC in 35 (50.7%) cases. The number of positive cells was significantly correlated with subendothelial localization of Ki67 ( p = 0.001) and with NA ( p = 0.001). Conclusions: Native veins do not contain cycling cells. In contrast, vascular cell proliferation starts immediately after AVF creation and persists independently of the time the fistula is set up. The amount of proliferating cells is significantly associated with MIT and subendothelial localization of Ki67-immunoreactive cells, thus suggesting a role of Ki-67 index in predicting AVF failure.

The aim of this study was to investigate the agreement between cycloplegic and non-cycloplegic autorefraction with an open-field auto refractor in a school vision screening set up, and to define a threshold for myopia that agrees with the standard cycloplegic refraction threshold. The study was conducted as part of the Sankara Nethralaya Tamil Nadu Essilor Myopia (STEM) study, which investigated the prevalence, incidence, and risk factors for myopia among children in South India. Children from two schools aged 5 to 15 years, with no ocular abnormalities and whose parents gave informed consent for cycloplegic refraction were included in the study. All the children underwent visual acuity assessment (Pocket Vision Screener, Elite school of Optometry, India), followed by non-cycloplegic and cycloplegic (1% tropicamide) open-field autorefraction (Grand Seiko, WAM-5500). A total of 387 children were included in the study, of whom 201 were boys. The mean (SD) age of the children was 12.2 (±2.1) years. Overall, the mean difference between cycloplegic and non-cycloplegic spherical equivalent (SE) open-field autorefraction measures was 0.34 D (limits of agreement (LOA), 1.06 D to −0.38 D). For myopes, the mean difference between cycloplegic and non-cycloplegic SE was 0.13 D (LOA, 0.63D to −0.36D). The prevalence of myopia was 12% (95% CI, 8% to 15%) using the threshold of cycloplegic SE ≤ −0.50 D, and was 14% (95% CI, 11% to 17%) with SE ≤ −0.50 D using non-cycloplegic refraction. When myopia was defined as SE of ≤−0.75 D under non-cycloplegic conditions, there was no difference between cycloplegic and non-cycloplegic open-field autorefraction prevalence estimates (12%; 95% CI, 8% to 15%; p = 1.00). Overall, non-cycloplegic refraction underestimates hyperopia and overestimates myopia; but for subjects with myopia, this difference is minimal and not clinically significant. A threshold of SE ≤ −0.75 D agrees well for the estimation of myopia prevalence among children when using non-cycloplegic refraction and is comparable with the standard definition of cycloplegic myopic refraction of SE ≤ −0.50 D.


A series of experiments has been performed to study the steady flow of heat in liquid helium in tubes of diameter 0.05 to 1.0 cm at temperatures between 0.25 and 0.7 °K. The results are interpreted in terms of the flow of a gas of phonons, in which the mean free path λ varies with temperature, and may be either greater or less than the diameter of the tube d . When λ ≫ d the flow is limited by the scattering of the phonons at the walls, and the effect of the surface has been studied, but when λ ≪ d viscous flow is set up in which the measured thermal conductivity is increased above that for wall scattering. This behaviour is very similar to that observed in the flow of gases at low pressures, and by applying kinetic theory to the problem it can be shown that the mean free path of the phonons characterizing viscosity can be expressed by the empirical relation λ = 3.8 x 10 -3 T -4.3 cm. This result is inconsistent with the temperature dependence of λ as T -9 predicted theoretically by Landau & Khalatnikov (1949).

The effects of frictional tangential traction combined with regelation on the basal sliding of a temperate glacier down an inclined wavy bed are examined. Two friction models are treated. First, a Coulomb law model having the assumptions that sliding occurs everywhere and that the tangential traction is proportional to the normal pressure. Secondly, a velocity power law in which the tangential traction is proportional to a power of the slip velocity. The ice motion is approximated by steady slow Newtonian flow and the bed undulation about a mean bed-line has a maximum slope ∊ ≪I. Flow solutions are constructed as perturbations (in powers off ∈) of the plane laminar flow corresponding to non-slip on the mean bed-line assuming that the ice remains everywhere in contact with the bed; that is, no cavitation takes place. If the normal traction is predicted to be tensile over part of the bed, implying that cavitation has occurred, then a new solution is needed in which the ice base over cavities is traction-free. Since the cavity sections and profile of the free ice base are then part of the overall solution, an intricate mixed boundary-value problem is set up for the flow and the present analysis is inadequate.For a sinusoidal bed the perfect-slip (zero tangential traction) solution predicts compressive normal traction everywhere on the bed provided that the mean bed-line inclination α (to the horizontal) is less than a critical value αe which is of order ε. For greater values of α including a range of order ∊, the normal traction is tensile on some parts of the bed, and a solution with cavitation is needed. If the tensile sections are relatively small it is expected that the resulting cavitation will not change the overall solution significantly. However, the Coulomb friction solution has extensive zones of tensile traction for all values of α, so that extensive cavitation would occur. In contrast, the velocity-power friction solution has compressive traction everywhere on the bed for α ⩽αe=0(I) provided that the ice depth is not too large, and also for deep glaciers for α ⩽αe=O∊ Furthermore, the predicted basal sliding velocity varies much less with the length scale of the bed undulation than in the perfect-slip solution, and is smaller.

Abstract BACKGROUND: Choledocholithiasis with cholelithiasis is a common problem in the middle and older age group people . Aim of this study is to assess the usefulness of ultrasonography in the diagnosis of choledocholithiasis in resource poor set up.METHODS: After matching the inclusion and exclusion criteria, all cases of diagnosed Choledocholithiasis were taken for this prospective, single center, observational study.RESULTS: Out of total 130 subjects, 41 were male and 89 were female. Increased incidence of choledocholithiasis was found in females (M: F= 1: 2.08). The mean age of presentation was 49.27 ± 10.60 years. Ultrasonography was able to diagnose CBD stones in 57patients (43.84%) and ductal dilatation in 89(68.46%). Whereas, MRCP detected CBD stones in 112 (86.15%).Cholelithiasis was associated with choledocholithiasis in 108 patients (83.07%). CONCLUSIONS: In the evaluation of choledocholithiasis, in our study particularly trans-abdominal USG seemed to have a lower yield than expected for accuracy in determining choledocholithiasis when compared to MRCP and Intra-operative findings. So a negative USG in a particular may not exclude choledocholithiasis. So,MRCP is the ‘investigation of choice’ among the suspicious patients for CBD stone disease. That is why prior to cholecystectomy in symptomatic patients with deranged LFT, MRCP always to be done, to detect the missed stone in CBD.

Abstract Background: To explore the use of an automated needle targeting (ANT) device as an assistive intraoperative navigation modality during PCNL for the treatment of large renal stones, with the aim of reducing surgical durations and radiation exposure.Methods: This open-label, single-surgeon clinical trial included patients with a diagnosis of renal stones for whom PCNL using the ANT device via the percutaneous access technique was indicated. Ethical approval was obtained from the UMMC ethics review board (Ref. 20118105-6740). The ANT was assembled after an initial motor calibration, and the image calibration was performed using the patient’s fluoroscopic images. Subsequently, the ANT software calculated a bullseye alignment before percutaneous puncture. Accurate renal access was confirmed by the efflux of urine in the Chiba biopsy needle, as well as by imaging with the C-arm intensifier at different angles. The primary endpoints were the time to successful renal access (from ANT set-up to urine efflux) and adverse events.Results: In all cases, successful renal access was achieved with a single attempt. The mean time to renal access was 6 minutes, 8 seconds. The mean fluoroscopy duration was 101 seconds, with a mean radiation dose of 23.46 mGy. No adverse events were documented.Conclusion: The ANT device enabled successful, safe and efficient renal access for PCNL in this study. Further research is needed to justify the effectiveness of this device in terms of enabling accurate renal access while reducing the surgical duration and radiation exposure to both surgeons and patients.

Abstract This paper presents results of the development process of low-background, digital gamma-rays spectrometer equipped with Broad Energy Germanium detector (CANBERRA BE5030), multi-layer passive shield and cosmic veto system that consists of five plastic scintillators (SCIONIX EJ-200). Data acquisition is performed using digitizer CAEN DT5725 with CoMPASS software. Output data analysis is carried out with purposely written and developed code VETO. On the basis of conducted tests, acquisition parameters were set up and tuned as well as time delays between all detectors were established. As a result of the configuration process, the mean background counts reduction of 64% in the whole spectrum and 65% in annihilation peak were achieved. This procedure allowed diminishing detection limits of selected isotopes 37% on average.

Abstract Background Determination of isolated prostate-specific antigen (PSA) in asymptomatic individuals has not demonstrated sufficient sensitivity and specificity to be useful in the routine evaluation of prostate disease. To enhance the accuracy of serum PSA we have used a proportion of serum PSA and prostate volume, which we refer to as prostate-specific antigen density (PSAD). Prostate volume in this study was calculated using transrectal ultrasonography (TRUS). Materials and Methods A total of 106 patients with prostatic disease clinically confined to the prostate glands were evaluated. Results and Observation The mean PSAD for prostate cancer was 0.15 ± 0.01 while that for benign hypertrophy of the prostate (BPH) was 0.11 ± 0.02 (p < 0.05). Significant difference (p < 0.05) was noted in the prostate volume in these two groups with the mean prostate volume measured by TRUS in the BPH to be 53.85 ± 9.71 mL compared with 58.14 ± 7.48 mL in the carcinoma. PSA density of 0.13 ng/mL can be used as a cutoff for the individual in our set-up who should go for prostate biopsy with sensitivity and specificity of over 90%. Conclusion These results suggest that PSAD may be useful in distinguishing BPH and prostate cancer.

The evolution of crossing sea states and the emergence of rogue waves in such systems are studied via numerical simulations performed using a higher order spectral (HOS) method to solve the free surface Euler equations with a flat bottom. Two classes of crossing sea states are analyzed: one using directional spectra from the Draupner wave crossing at different angles, another considering a Draupner-like spectra crossed with a narrowband JONSWAP state to model spectral growth between wind sea and swell. These two classes of crossing sea states are constructed using the spectral output of a WAVEWATCH III hindcast on the Draupner rogue wave event. We measure ensemble statistical moments as functions of time, finding that although the crossing angle influences the statistical evolution to some degree, there are no significant third-order effects present. Additionally, we pay particular attention to the mean sea level measured beneath extreme crest heights, the elevation of which (set up or set down) is shown to be related to the spectral content in the low wavenumber region of the corresponding spectrum.

The main purpose of this paper is to advocate a rule-based forecasting technique for anticipating stock index volatility. This paper intends to set up a stock index indicators projection prototype by using a multiple criteria decision making model consisting of the cluster analysis (CA) technique and Rough Set Theory (RST) to select the important attributes and forecast TSEC Capitalization Weighted Stock Index. The projection prototype was then released to forecast the stock index in the first half of 2009 with an accuracy of 66.67%. The results point out that the decision rules were authenticated to employ in forecasting the stock index volatility appropriately.
