AIM: To study the effect of adjuvant melatonin with clomiphene citrate on ovulution and fertility rates in infertile PCO patients. MATERIAL AND METHODS: A total of 107 cases participated in the study out of which 7 women were excluded from the study. So the results of 100 women were analyzed. Out of 100 women 50 were of group A i.e. clomiphene supplemented with melatonin group and 50 were in group B which was clomiphene only group with no supplementation. Cases were followed for three cycles and pregnancy rates in both groups were evaluated. RESULTS: Study demonstrated the potential benefit of melatonin supplementation in naturally conceived cycles where ovulation induction was given by en-clomiphene citrate. The ovulation rates were comparable in both study Vs control group (68% Vs 66%) while there was increase in pregnancy rate in melatonin group (36% Vs 24%). However difference between pregnancy rates in both the groups failed to reach statistically significant levels (p value 0.4065). CONCLUSION: The adjuvant role of melatonin with en-clomiphene citrate in cases of Infertile PCO patients is in improving pregnancy rates; which is due to strong anti-oxidant effect of melatonin which in turn reduces oxidative stress thus resulting in good quality oocyte production.

OBJECTIVES: To evaluate the efficacy of zinc supplementation in patients with diarrheal illnesses. Study Design: Quasi Experimental study. Setting: The study was conducted at the pediatrics department of Madina Teaching Hospital, Sargodha road, Faisalabad, Period: 6 months period from November 2008 to April 2009. Material and Methods: Two hundred children suffering from diarrhea with mild to moderate dehydration were included in the study and divided into two groups, A and B. Group A was given zinc supplementation along with ORS and adequate nutrition according to age. In group B 100 children were taken as control to whom only ORS and adequate nutrition for age was given. Results: Baseline clinical parameters were comparable in both groups at admission. The effect observed on stool consistency was more marked in 72 hours i.e. 71% patients were improved in group A versus 40% in group B, as compared to stool frequency in 72 hours i.e. 56% in group A versus 39% in group B. Although in 7 days, 99% patients showed improvement in stool consistency in group A,70% in group B(p value 0.037) and stool frequency showed improvement of 96% in group A as compared to 77% in group B(p value 0.037). Duration of hospital stay on the average was about 2 days less in case of zinc supplemented group as compared to control group. Conclusion: Zinc supplementation reduced the duration and severity of mild to moderate diarrheal illness and treatment was well tolerated with no significant side effects.

Background: There is evidence regarding beneficial use of Transcutaneous Electrical Nerve Stimulation (TENS) on post Lower Segment Caesarean Section (LSCS) incision pain. However, efficacy of different types of TENS following C section pain has not yet been explored adequately.Methods: 96 women who had recently undergone LSCS were included for the study. The subjects were in the age group of 20 to 40 years (25.84±3.96); having pain intensity 4 or more on Numerical Pain Rating Scale (NPRS). They were divided into three groups by random allocation method; namely Group A: Acupuncture (Low/Motor) TENS, Group B: Conventional (High/Sensory) TENS and Group C: Control group. Group A and B received specific type of TENS twice a day for 15 minutes. Control group C did not receive any TENS intervention. All subjects received standard post-operative medications and physiotherapy. Pain intensity was recorded on NPRS pre and post intervention.Results: Both Acupuncture TENS and Conventional TENS significantly decreased post-operative pain intensity as compared to control group (p value <0.0001).Conclusions: Both, acupuncture and conventional TENS are equally effective in reducing post LSCS incision pain at a strong and non-painful intensity.

Objective: To compare the efficacy of Letrozole and clomiphene citrate in Patients of Anovulation polycystic ovarian syndrome with Infertility. Study Design: Randomized Controlled Trial Setting: Department of Obstetrics and Gynecology Independent Medical College Faisalabad. Period: 30-09-2019 to 29-6-2020. Material & Methods: This study included 100 patients with 50 patients in each group. Group A received 2.5 to 5mg letrozole in each cycle from day 3 to 7. Group B received clomiphene citrate 50 to 100 mg incremental dose depending on ovarian response. Both Drugs were given for consecutive 3 to 6 cycles to see response. Main outcome Measure included optimal follicle size (> 18mm), endometrial thickness, and pregnancy rate. Epidemiological data and efficacy outcome measures were recorded on a Performa. Statistical analysis was done using SPSS version 13. Chi-square test applied and p-value <o.o5 was considered significant. Both group included primary infertility patients. Results: The mean age of patients was 28.03+ 3.02 years. Mean age of group A patients was 29.04+3.44 and 28.47+3.90 group B patients. Mono ovulation in group A patients (88.9%) and 27 in group b patients (60.0%). The mean endometrial thickness was 9.6mm + 1.6 in letrozole group and 6.9mm + 1.2 in clomiphene citrate group A. In group A 18% got pregnant from Letrozole group and 1.1% from the group B. Conclusion: Efficacy was significantly higher in group A Patients received letrozole as compared to patients echo received clomiphene citrate. The effects of Letrozole showed better outcome in terms of Ovulation Induction Endometrial Thickness and Pregnancy rate.

The effect of aqueous extract of Basella alba (puishak) leaves on blood cell count of rats was studied. Forty rats of both sexes weighing between 100-150 gm were used. The rats were divided into four groups (7 rats in each group), with group A as the control group and experimental groups were denoted as B, C and D. With all aseptic precautions, aqueous extract of Basella alba (Puishak) leaves was administered into the three different doses (For group B: 60 mg/kg bw, group C: 80 mg/kg bw and group D: 100 mg/kg bw). Control group A also received distilled water as a placebo at the dose of 10 mg/kg bw for 14 days. At day 15, blood samples were collected and sent for haematological analysis. Data analysis of blood count profile of 28 rats revealed that there is an increased number of RBC, WBC and platelet count in experimental groups than in control group. ANOVA test revealed that increased blood cell counts following administration of aqueous leaves extract of keeves of B. alba were statistically significant (p value for each case was .0001<.05) than control group. Paired samples t test was performed to compare between baseline hematological parameters and parameters after 14 days of intervention. Then comparison between Group A & Group B, between Group A & Group C and between Group A & Group D were done. In all cases, probability (p) value < 0.05 was considered as statistically significant.This it is clearly evident that aqueous extract of B.alba has positive stimulant effect on blood cells count of rats. Abstract Small cell neuroendocrine carcinomas (SCNC) are composed of round to spindle-shaped cells with features of both neuroendocrine and epithelial neoplasms. SCNC can be found in basically all epitheloid organs; however, the vast majority arises in the lung, while extrapulmonary (EP) localization is rare. Small cell lung cancer (SCLC) and EP SCNC are considered one histological entity and are treated similarly. Despite the high initial response to chemo- and radiotherapy, most patients relapse after short remission, and overall prognosis is dismal. Clinical trials employing high-dose chemotherapy (HDCT) followed by autologous stem cell transplantation (auto-SCT) neither demonstrated a clear benefit nor clarified its significance in SCLC. The role of intensified treatment in EP SCNC has not been specifically addressed in detail. Conversely to SCLC, some subgroup analyses indicated that patients with EP SCNC might benefit from intensive treatment. We analyzed a total of 22 patients: 8 with EP SCNC (group A; m:f 3:5; median age 36 years (y) [range 22–56]) and 14 patients with SCLC (group B; m:f 10:4; median age 55 y [40–63]), all undergoing HDCT with auto-SCT at our single center from 07/90–01/05. A control group C consisted of 30 patients with EP SCNC (m:f 21:9, median age 66 years [32–81], who received different standard treatments without auto-SCT. All group A patients had stage IV disease (liver n=5, lymph nodes n=4, pancreas n=1, orbita n=1, bone n=1, kidney n=1). Two of these patients received additional local radiotherapy. In group B, 7 patients had limited and 7 patients extensive disease, (stage I (n=1), II (n=2), III (n=10), IV (n=1). HDCT with VIC (etoposide, ifosfamide, carboplatin; n=21), or CCT (carboplatin, cyclophosphamide, thiotepa; n=1) was followed by infusion of a median 3.2x10^6 CD34+ cells. Prophylactic radiotherapy was performed in 12 patients (mediastinum n=11; brain n=10). With a median follow-up (FU) of 48 months (7–152) for group A and 85 months (0–170) for group B, 5/8 (63%) of patients with EP SCNC (group A) are alive and in complete remission (CR), compared to 5/14 (36%) SCLC patients (group B). Best response ever achieved after HDCT was a CR in 5/8 (63%), a partial remission (PR) in 2/8 (25%) and stable disease (SD) in 1/8 (12%) in group A. In group B, a CR was attained in 11/14 (79%), a PR and a SD in 1/14 (7%) patients, respectively. In the conventionally treated control group C, a transient PR was achieved in 5/30 (16%), and after a median FU of 9.7 months, only 2/30 (6.7%) EP SCNC patients are alive. Our analysis suggests that selected SCNC patients may benefit from HDCT, particularly when integrated into multimodal treatment concepts. The remarkably favorable outcome in patients with EP primary site, even when HDCT was implemented as salvage treatment warrants further studies on the role of HDCT in SCNC. Careful attention will have to be paid to prognostic clinical features, such as primary site and/or histological parameters including neuroendocrine marker profiles and mitotic indices. These may help to predict which patients will benefit from intensified treatment. In addition, further histological studies should address the identification of markers specific for lung- vs. extrapulmonary primary localization. For this purpose all available tumor tissue from our study is currently under histological re-analysis, assessing the expression of the novel tumor testis antigens.

Purpose. To investigate the relationships between angle of deviation, fusional convergence and divergence amplitude, AC/A ratio, near point of convergence (NPC), and myopia-phoria in intermittent exotropia (IXT). Methods. 93 patients with IXT, divided into two groups, were recruited in the study. In Group A (73 patients), fusional convergence and divergence amplitudes, NPC, and AC/A ratio were studied and compared with a control group. In Group B (in 20 patients), myopia-phoria while switching from monocular to binocular view was studied with an infrared video retinoscopy and compared with a control group. Results. In Group A, positive fusional amplitudes, NPC, and AC/A ratio of IXT patients did not differ from those of normals. Negative fusional amplitudes were significantly higher in the patient group than in the control group. In Group B, myopic shift was statistically significantly higher in the patient group and there was a statistically significant positive correlation between myopic shift and angle of strabismus. Conclusions. Apart from the role of fusional convergence which accounts for myopia-phoria, that of the other binocular functions in the control at near of IXT and conversely their specific role in the pathogenesis of IXT remain unclear and the cause of divergent infantile strabismus is still unknown.

Introduction: Urinary tract calculi are the most frequent condition in urology clinics worldwide. The overall prevalence is about 5 % and lower urinary tract stones account for 70% among them. Medical expulsive therapy has been used to treat distal ureteric stone as it reduces symptoms and facilitates stone expulsion. The aim of this study is to evaluate and compare the efficacy of tamsulosin and alfuzosin as medical therapy in ureteric stones. Materials and Methods: A total of 87 patients with distal ureteral stones of size ≤10 mm were randomly divided into 3 groups. Group A patients (n-30) received 0.4 mg of tamsulosin daily, group B patients (n-29) received 10 mg of alfuzosin daily and group C patients (n-28) received 75 mg of diclofenac sodium. Patients in all groups received diclofenac sodium for one week and then as required. Follow-up was done on a weekly basis for 4 weeks. The stone expulsion rate, time for stone expulsion, and side-effects were recorded in each group. Results: The mean stone size (5.66, 5.79, 5.67) mm and age (29.1, 30.31, 29.4) were comparable in each group. The stone expulsion rate was 83.3%, 79.3%, and 50% in groups A, B, and C respectively.  It showed that both the study groups (Group A and Group B) were effective than the control group (p-value 0.006 and 0.02 respectively) but there was no difference between tamsulosin and alfuzosin (p-value 0.69). The duration of stone expulsion was 11.5 days, 11.8 days, and 17.3 days for Group A, B, and C respectively. The drugs related side effects reported by patients were mild and transient. Conclusion:  The use of tamsulosin and alfuzosin for the medical treatment of ureteric stones proved to be safe and effective and neither did have any significant benefits over the other.

Objectives: To assess the role of lingual flap in encountering lingual nerve injury during the surgical elimination of lower wisdom tooth. Study Design: Cross Sectional Case Control study. Setting: Department of Oral & Maxillofacial Surgery, Institute of Dentistry, Liaquat University of Medical & Health Sciences Jamshoro / Hyderabad. Period: January 2016 to September 2016. Material & Methods: Subjects were categorized into two categories: Group-A (lingual flap) & group-B (control group) each having 52 patients by Lottery method. In group A an envelope mucoperiosteal flap followed by lingual flap elevation carried out and in group B only envelope flap was carried out. All patients were reviewed on the first postoperative day and again 1 and 3rd week after surgery. At each postoperative visit, patient was examined for sensory nerve impairment of the lingual nerve by same observer. Results: Total 104 cases were studied, all the cases categorized among two groups 52 in each group. In group-A 34 were males and 18 were females, while in group-B 44 were male and 8 were females. There was no significant difference among both groups according to the pre-operative assessment. According to objective findings, lingual nerve paresthesia was found among 2 cases of group A on 1st visit, while no any case was found with nerve injury in group B. Out of 2 cases, one case was improved and only one had presented with complain at 2nd visit and 3rd visit, no significant difference among both groups, p-values were quite insignificant. Conclusion: It was concluded that lingual nerve injury (LNI) occurred among few cases of lingual flap group which was insignificantly higher as compare to control group, but the nature of injury was temporary.

Introduction: Cyclophosphamide is one of the alkylating chemotherapeutic drug used in cancer patients that has antifertility effects on female gonads. Ocimum basilicum is a natural herb rich in polyphenols and is known to improve fertility. Aims & Objectives: The study was designed to evaluate the role of natural herb, Ocimum basilicum extract, as a preventive agent against ovarian follicular toxicity induced by cyclophosphamide. Place and duration of study: This experimental study was performed in the Department of Anatomy, Shaikh Zayed Postgraduate Medical Institute, Lahore. The duration of study was 8 months. Material & Methods: 45 female albino rats were divided equally in control group A, experimental group B and group C each contained 15 rats. Group A rats received single dose of 150 mg/kg normal saline intraperitoneally on 8th day of experiment, while group B was given single intraperitoneal dose of 150 mg/kg cyclophosphamide at day 8 of experiment. Group C rats were pretreated with methanolic basil (Ocimum basilicum) seeds extract for 7 days followed by single intraperitoneal dose of 150 mg/kg cyclophosphamide at day 8 of experiment. All the rats were dissected 48 hours after the last dose. Results: Graafian follicles were atrophied showing atretic granulosa cells in group B when compared with control group A with p value <0.001. However, significant improvement in status of Graafian follicles was observed in group C, when compared with group B with p value <0.025. Conclusion: This study depicts that basil seeds extract can prevent the cellular toxicity in Graafian follicles caused by cyclophosphamide treatment. So the use of basil seeds during chemotherapy can significantly limit its toxic effects on Graafian follicles.
