Background: There is evidence regarding beneficial use of Transcutaneous Electrical Nerve Stimulation (TENS) on post Lower Segment Caesarean Section (LSCS) incision pain. However, efficacy of different types of TENS following C section pain has not yet been explored adequately.Methods: 96 women who had recently undergone LSCS were included for the study. The subjects were in the age group of 20 to 40 years (25.84±3.96); having pain intensity 4 or more on Numerical Pain Rating Scale (NPRS). They were divided into three groups by random allocation method; namely Group A: Acupuncture (Low/Motor) TENS, Group B: Conventional (High/Sensory) TENS and Group C: Control group. Group A and B received specific type of TENS twice a day for 15 minutes. Control group C did not receive any TENS intervention. All subjects received standard post-operative medications and physiotherapy. Pain intensity was recorded on NPRS pre and post intervention.Results: Both Acupuncture TENS and Conventional TENS significantly decreased post-operative pain intensity as compared to control group (p value <0.0001).Conclusions: Both, acupuncture and conventional TENS are equally effective in reducing post LSCS incision pain at a strong and non-painful intensity.

ABSTRAK Pendahuluan: Sirkumsisi merupakan tindakan yang memerlukan persiapan, salah satunya tindakan anestesi. Selama sirkumsisi, rasa nyeri akan dirasakan hanya pada saat penyuntikan anestesi. Perawat sering kesulitan melakukan prosedur penyuntikan anestesi sirkumsisi karena anak sering menolak dilakukan tindakan keperawatan yang akan menimbulkan rasa nyeri. Salah satu manajemen non farmakologi untuk menurunkan nyeri pada anak adalah terapi slow deep breathing sambil bermain meniup baling-baling. Tujuan: mengidentifikasi pengaruh terapi slow deep breathing dengan bermain meniup baling-baling terhadap intensitas nyeri pada anak yang dilakukan penyuntikan anestesi sirkumsisi. Metode: Penelitian ini menggunakan quasi eksperimental dengan control group post test. Populasi penelitian ini adalah seluruh anak yang dilakukan penyuntikan anestesi sirkumsisi di Klinik Khitan Kencana Medika. Sampel yang digunakan berjumlah 36 anak, 18 anak kelompok intervensi dan 18 anak kelompok kontrol. Respon nyeri diukur menggunakan Faces Pain Rating Scale. Hasil: penelitian dianalisis menggunakan uji Mann-Whitney menunjukkan  ada perbedaan yang signifikan antara kelompok intervensi dan kontrol dengan p-value < 0,001 dan nilai signifikansi alpha £ 0,05. Kesimpulan penelitian ada pengaruh terapi slow deep breathing dengan bermain meniup baling-baling terhadap intensitas nyeri pada anak yang dilakukan penyuntikan anestesi sirkumsisi.   Kata kunci: anak, nyeri, penyuntikan anestesi sirkumsisi, slow deep breathing   ABSTRACT Introduction: Circumcision is treatment which needed preparation, one of them is anesthesia treatment. As long as circumcision treatment, pain will be feel only at the time anesthesia circumcision injection. Nurses often have difficult to make action procedure of anesthesia circumcision injection because children often refuse to procedures which will occurs pain feeling. One of non pharmacology managements to reduce pain on children by giving children breath is playing with a blow vane. Objective: The purpose of study to identify the effect of slow deep breathing therapy by playing with a blow vane to pain intensity on children with anesthesia circumcision injection. Methods: This study used a quasi experimental design by control group post test. Populations were all children who had been performed anesthesia circumcision injection in Kencana Medika Circumcision Clinic. Used 36 samples consist of 18 samples were in intervention and control group. Pain response was measured by Faces Pain Rating Scale. Result: The results were analyzed using the Mann-Whitney showed there are significant differencebetweenthe interventionandcontrolgroups with ap-value of0.000 and α £ 0.05. Background: To date, there has been no investigation addressing the effects of vibration anesthesia during carboxytherapy. Aim: Investigate the analgesic effect of different vibratory devices during carboxytherapy for the treatment of cellulite. Materials & methods: A total of 78 women between 18 and 49 years of age with cellulite in the gluteal region will be randomly allocated to three groups: Group A (carboxytherapy and vibratory device A), Group B (carboxytherapy and vibratory device B) and control group. Pain intensity will be assessed using a numerical rating scale after each puncture. Expected outcome: Vibration anesthesia is expected to be effective at diminishing the pain intensity caused by carboxytherapy comparison with the control group, with no differences between the vibratory devices. OBJECTIVE To compare the effectiveness of mobilization and self-exercises in the management of adhesive capsulitis of shoulder STUDY DESIGN Randomized Control Trial SAMPLE SELECTION 30 patients of adhesive capsulitis of shoulder from physiotherapy department of tertiary care hospitals of Karachi were selected through simple random sampling technique. PROCEDURE Treatment was continued for 5 days per week for the period of 3 weeks followed by assessment. Patients were randomly divided into two equal groups. Group A was treated with midrange mobilization while group B performed self-exercises. Both groups received TENS and hot pack prior to the exercises. Mean ± SD, frequencies and percentages were used for descriptive analysis. ROM via goniometry and pain intensity through VAS was analyzed by paired t-test within the groups and by independent t-test between the groups, using SPSS. P-value of less than 0.05 was considered significant. RESULTS 60% were females (n=18) and 40% were males (n=12) with mean age of 50.17±6.37 years. Significant improvement (p-value <0.05) in pain and shoulder ROM was observed among patients of Group A as compared to Group B. Pain intensity was decreased to 1.67 ± 0.62 in group A, whereas ROMs in these patients were also better than other group.

Background: Preemptive analgesia, involves the introduction of an analgesic regimen before the onset of noxious stimuli, with the goal of preventing sensitization of the nervous system to subsequent stimuli that could amplify pain.Methods: To determine the efficacy and safety of Lornoxicam when administered preemptively by using Wong-Baker FACES Pain Rating scale. The patients undergoing abdominal surgery were randomly categorized into group A and B of 25 each. Group A- Received Lornoxicam 8mg (1ml) one hour before surgery. Group B- Not received any analgesic before surgery. Primary measurement of the efficacy was done by using Wong-Baker Faces Pain Rating Scale at 2, 4, 8, 12 and 24 hour. All parameters were analyzed by using student t test.Results: Surgeries which were included in the study are hernia repair, open appendectomy, laparoscopic (appendectomy, cholecystectomy). Reduction in pain scores at 12th hourly and 24th hourly pain scores (<0.05) was significant. Tramadol usage decreased significantly with laparoscopic surgeries.Conclusions: In this study we could demonstrate that lornoxicam when used preemptively reduces the pain score slightly and reduces the requirement of post-operative analgesics significantly.

The effect of aqueous extract of Basella alba (puishak) leaves on blood cell count of rats was studied. Forty rats of both sexes weighing between 100-150 gm were used. The rats were divided into four groups (7 rats in each group), with group A as the control group and experimental groups were denoted as B, C and D. With all aseptic precautions, aqueous extract of Basella alba (Puishak) leaves was administered into the three different doses (For group B: 60 mg/kg bw, group C: 80 mg/kg bw and group D: 100 mg/kg bw). Control group A also received distilled water as a placebo at the dose of 10 mg/kg bw for 14 days. At day 15, blood samples were collected and sent for haematological analysis. Data analysis of blood count profile of 28 rats revealed that there is an increased number of RBC, WBC and platelet count in experimental groups than in control group. ANOVA test revealed that increased blood cell counts following administration of aqueous leaves extract of keeves of B. alba were statistically significant (p value for each case was .0001<.05) than control group. Paired samples t test was performed to compare between baseline hematological parameters and parameters after 14 days of intervention. Then comparison between Group A & Group B, between Group A & Group C and between Group A & Group D were done. In all cases, probability (p) value < 0.05 was considered as statistically significant.This it is clearly evident that aqueous extract of B.alba has positive stimulant effect on blood cells count of rats. Moreover, it was found that increment of doses also increases the cell count that is positively correlated with the hypothesis. Bangladesh Pharmaceutical Journal 22(1): 73-78, 2019

Introduction: Urinary tract calculi are the most frequent condition in urology clinics worldwide. The overall prevalence is about 5 % and lower urinary tract stones account for 70% among them. Medical expulsive therapy has been used to treat distal ureteric stone as it reduces symptoms and facilitates stone expulsion. The aim of this study is to evaluate and compare the efficacy of tamsulosin and alfuzosin as medical therapy in ureteric stones. Materials and Methods: A total of 87 patients with distal ureteral stones of size ≤10 mm were randomly divided into 3 groups. Group A patients (n-30) received 0.4 mg of tamsulosin daily, group B patients (n-29) received 10 mg of alfuzosin daily and group C patients (n-28) received 75 mg of diclofenac sodium. Patients in all groups received diclofenac sodium for one week and then as required. Follow-up was done on a weekly basis for 4 weeks. The stone expulsion rate, time for stone expulsion, and side-effects were recorded in each group. Results: The mean stone size (5.66, 5.79, 5.67) mm and age (29.1, 30.31, 29.4) were comparable in each group. The stone expulsion rate was 83.3%, 79.3%, and 50% in groups A, B, and C respectively.  It showed that both the study groups (Group A and Group B) were effective than the control group (p-value 0.006 and 0.02 respectively) but there was no difference between tamsulosin and alfuzosin (p-value 0.69). The duration of stone expulsion was 11.5 days, 11.8 days, and 17.3 days for Group A, B, and C respectively. The drugs related side effects reported by patients were mild and transient. Conclusion:  The use of tamsulosin and alfuzosin for the medical treatment of ureteric stones proved to be safe and effective and neither did have any significant benefits over the other.

 Latar Belakang : Kehamilan melibatkan berbagai perubahan fisiologis antara lain perubahan fisik, perubahan sistem pencernaan, sistem respirasi, sistem traktus urinarius,muskuloskeletal dan sirkulasi. Dalam proses adaptasi tidak jarang ibu akan mengalami ketidaknyamanan salah satunya adalah nyeri punggung, meskipun nyeri punggung adalah hal fisiologis namun tetap perlu diberikan suatu penanganan, seperti yoga prenatal. Tujuan : untuk mengetahui pengaruh yoga prenatal terhadap nyeri punggung pada ibu hamil trimester II dan III. Metode penelitrian dengan quasi experiment Desain penelitian Pretest and Postest Without Control Group Design. Jumlah sampel dalam penelitian ini adalah 30 ibu hamil Trimester II dan III, alat ukur kuesioner Numeric Pain Rating Scale (NPRS). Penelitian dilakukan bulan April tahun 2019. Data dianalisis dengan Wilcoxon (Wilcoxon Signed Rank Test). Hasil penelitian menunjukkan adanya pengaruh yoga prenatal terhadap penurunan intensitas nyeri punggung pada ibu hamil trimenster II dan III karena nilai Asym. Sig sebesar 0,000(p value < 0,05).

Objectives: Is to compare outcomes in terms of mean post-operative pain andhospital stay between stapled versus conventional hemorrhoidectomy. Hypothesis: There isa difference in mean post-operative pain and hospital stay between stapled and conventionalhemorrhoidectomy, stapled technique is better than conventional technique. Study Design:Randomized control trial. Setting: Department of General Surgery Bakhtawar Amin MemorialHospital Multan. Period: February 2016 February 2017. Methodology: A total number of 60patients enrolled in the study, both genders. Statistical software SPSS ver.23 was used toanalyze the data. Mean and SD were calculated and presented for numerical variables likeduration of hemorrhoids, age and pain score while frequencies and percentage were calculatedand presented for categorical variables like ender and grade of hemorrhoids. Independent t-testand chi square test were applied to see effect modification. P value ≤0.05 was considered assignificant. Results: Total number of 60 (100%) patients in the study, 32 (53.3%) were maleand 28 (46.7 %) were female. Mean hospital stay in group A was 1.63 ± 0.71 and in groupB means duration of hospital stay was 1.73 ± 0.74. A P value was 0.001. On stratification ofdata it is concluded that in group A 9 patients have no pain 6 have mild and 6 have moderatepain and 5 patients have severe pain, similarly in group B, 3 patients have no pain 2 have mildpain 1 have moderate pain and no patient have severe pain. P value for male patients was2.65. Conclusion: This study confirms that stapled hemorrhoidectomy is associated with lesspostoperative pain with no effect of age and gender on outcome.

Background: Kidney stones has got high prevalence worldwide especially in our part of world and per-cutaneous nephrolithotomy (PCNL) is gold standard operative modality for renal stone management. Pain control after PCNL is the most important task for treating surgeon as effective pain control causes smooth patient recovery in post-operative period. For analgesia, many drugs can be used, out of which tramadol and nalbuphine always been in comparison for effective analgesic activity. Objective: To compare efficacy of tramadol and nalbuphine as potent analgesic in patients after PCNL. Study Design: A prospective randomized study. Settings: Department of Urology and Kidney Transplantation, Allied Hospital/Faisalabad Medical University, Faisalabad and Department of Urology, DHQ Hospital/Faisalabad Medical University, Faisalabad Pakistan. Duration: From September 2016 to September 2020 (4 Years). Methodology: 286 patients undergoing PCNL for either gender and sides enrolled in study according to inclusion and exclusion criteria. Patients divided into Group A (Administered with tramadol) and Group B (administered with nalbuphine) and patients evaluated for pain up to 48 hours after surgery using 11-point numerical pain rating scale rating from0-10 where 0 is no pain, 1-3 mild pain, 4-6 moderate pain, 7-9 severe pain and 10 as worst pain and use of rescue analgesia observed in either groups. Data obtained and statistical evaluation done. Results: 286 patients enrolled from age 15-75 years with mean age of 40.2 years. Among 286 patients, 161 were male and 125 were female patients. Both groups A and B having 143 patients each. Analysis showed that tramadol has superiority over nalbuphine and need of analgesia is more in patient given nalbuphine (118 vs 31) as maintenance analgesic agent in post-operative period after PCNL. Conclusion: This study showed that tramadol has superiority over nalbuphine for analgesia after PCNL.

Introduction: Cyclophosphamide is one of the alkylating chemotherapeutic drug used in cancer patients that has antifertility effects on female gonads. Ocimum basilicum is a natural herb rich in polyphenols and is known to improve fertility. Aims & Objectives: The study was designed to evaluate the role of natural herb, Ocimum basilicum extract, as a preventive agent against ovarian follicular toxicity induced by cyclophosphamide. Place and duration of study: This experimental study was performed in the Department of Anatomy, Shaikh Zayed Postgraduate Medical Institute, Lahore. The duration of study was 8 months. Material & Methods: 45 female albino rats were divided equally in control group A, experimental group B and group C each contained 15 rats. Group A rats received single dose of 150 mg/kg normal saline intraperitoneally on 8th day of experiment, while group B was given single intraperitoneal dose of 150 mg/kg cyclophosphamide at day 8 of experiment. Group C rats were pretreated with methanolic basil (Ocimum basilicum) seeds extract for 7 days followed by single intraperitoneal dose of 150 mg/kg cyclophosphamide at day 8 of experiment. All the rats were dissected 48 hours after the last dose. Results: Graafian follicles were atrophied showing atretic granulosa cells in group B when compared with control group A with p value <0.001. However, significant improvement in status of Graafian follicles was observed in group C, when compared with group B with p value <0.025. Conclusion: This study depicts that basil seeds extract can prevent the cellular toxicity in Graafian follicles caused by cyclophosphamide treatment. So the use of basil seeds during chemotherapy can significantly limit its toxic effects on Graafian follicles.
