Prevalence of Vitamin-D Deficiency in Patients with Acute Coronary Syndrome in Syria

The objectives of this study were to estimate the prevalence of vitamin D deficiency in patients with acute coronary syndrome in comparison with normal individuals and study the correlation between these two conditions. We measured the plasma 25-hydroxy vitamin D (25-OH-D) levels in 60 patients with acute coronary syndromes (ACS) of both gender and in 30 age matched control individuals of both gender without any known cardiovascular or systemic diseases. The levels of 25-OH-D were measured by ELISA method and the results were statically analyzed to find out any possible correlation. We classified the cases according to their plasma 25(OH)D levels. 25(OH)D levels of ≥ 30 ng/ml were considered normal, levels < 30 and > 20 ng/ml were classified as insufficient, while levels of ≤ 20 ng/ml were classified as deficient. In the current study the prevalence of hypovitaminosis D in the patients group was much higher than it was in the control group. Vitamin D deficiency was observed in 80% and insufficiency in 13% of total patients of ACS, there by bringing the total count to 93%. Whereas only 7% of the patients had adequate vitamin D levels. Thus, these results indicate the existence of a significant correlation between the vitamin D deficiency and ACS in comparison to healthy controls

Background: Vitamin D, a fat-soluble vitamin has its receptor present in myriad of tissues and it modulates multiple cellular processes. Vitamin D deficiency is reported to be associated with coronary artery disease. Cardiovascular disease is the leading cause of mortality worldwide. Aims and Objective: The primary outcome was to investigate if there is a correlation of 25-OH levels with the percentage of luminal stenosis, as measured with coronary angiogram. The secondary outcome was to determine the differences in angiographically proven luminal stenosis across categories of 25-OH vitamin D levels. Materials and Methods: Thirty patients with acute coronary syndrome with diabetes mellitus were included in this cross-sectional descriptive study. All patients were tested for fasting vitamin D levels, fasting blood sugar, HbA1C and serum creatinine. Detailed history of the patients was recorded. Data was analyzed by the statistical software SPSS version 19 and p value <0.05 was considered significant. Statistical tests like Chi- square, independent t test and log regression was used. Results: In this study 30 patients undergoing coronary angiography for acute coronary syndrome, Vitamin D levels showed severe deficiency in 6.7% (2) cases while mild deficiency was seen in 50% of the cases. Patients with single vessel disease on the coronary angiogram had lower mean HbA1C (9.18) levels in our study. Patients with triple vessel disease had poorly controlled mean HbA1C levels (10.42). Conclusion: In this study we did not find any significant difference between the serum Vitamin D deficiency levels with patients with angiographic severity of the coronary artery disease. Patients with poorly controlled diabetes mellitus had more severe angiographic proven coronary artery disease.

Deep neck infection (DNI) refers to infections in spaces created by superficial and deep cervical fascia around the muscles and organs in the neck. Vitamin D is highly important for an effective immune system. Vitamin D receptors (VDR) have been identified in immune system cells, and particularly in T and B lymphocytes, macrophages, and dendritic cells. Vitamin D deficiency is thought to result in impaired immune response, decreased leukocyte chemotaxis, and an increased disposition to infection. The purpose of this study was to investigate whether vitamin D deficiency is an underlying occult factor in the development of DNI. Sixty-five patients aged 6 to 90, diagnosed with DNI, and 70 healthy age- and sex-compatible cases were included in the study. Serum levels of calcium, phosphorus, parathyroid hormone, and 25-hydroxy vitamin D (25(OH)D) were determined in each case. 25-hydroxy vitamin D levels above 20 ng/mL were regarded as normal, 12 to 20 ng/mL as insufficient, 5 to 12 ng/mL as deficient, and less than 5 ng/mL as severely deficient. Mean serum 25(OH)D levels were 10.4 (6.2) ng/mL in the patient group and 15.5 (6.4) ng/mL in the control group ( P < .01). This difference was statistically significant ( P < .01). Vitamin D was within normal limits in 9.2% (n = 6) of cases in the study group, insufficient in 29.2% (n = 19), deficient in 35.3% (n = 23), and severely deficient in 26.2% (n = 17). The equivalent values in the control group were 21.4% (n = 15), 48.5% (n = 34), 30% (n = 21), and 0% (n = 0). Serum 25(OH)D levels were significantly lower in patients with DNI compared to the healthy cases; 25(OH)D levels may be a factor in the development of DNI.

Abstract Background Preeclampsia is a pregnancy-specific disorder that affects 3–5% of pregnant women worldwide and is one of the most frequently encountered medical complications of pregnancy. Recent research has pointed towards some role of vitamin D deficiency in pathogenesis of preeclampsia. Vitamin D deficiency in pregnant women and their children is a major health problem, with potential adverse consequences for overall health. Aim of the Study The aim of this study is to investigate the serum vitamin D levels in preeclampsia and healthy normotensive pregnant women. Patients and Methods This case control study was conducted at Ain Shams University Maternity Hospital from November 2017. The study included pregnant women 20-35years this study will be carried out on pregnant women recruited at pre labour room (preeclamptic group) and (non preeclamptic group) at Ain Shams University Maternity Hospital, 50 women in each group. Results All samples were screened by DRG® 25-OH Vitamin D (total) ELISA (EIA-5396) and it was found that mean 25 hydroxy vitamin D levels were lower in preeclamptic group than normotensive control group. Mean level of 25 hydroxy vitamin D between preeclamptic cases was 13.98 ±4.98 ng/ml and between normotensive controls was15.62± 3.51 ng/ml. Conclusion This study has shown no association between vitamin D deficiency and pre-eclampsia, supporting no role for vitamin D as a preventative agent against preeclampsia.

Aim. To assess vitamin D level in patients with acute coronary syndrome and to determine the relationships of vitamin D level with specific patterns of cardiovascular disease in this group of patients.Methods. 50 patients (35 (70%) males and 15 (30%) females) urgently admitted to the emergency cardiology department of Kaliningrad Regional Hospital were enrolled in the cross-sectional observational study. Patients with diabetes mellitus, autoimmune diseases and cancer were excluded from the study. The mean age of patients was 60 (55; 66) years. Cardiovascular risk factors (obesity, smoking, physical activity, alcohol consumption, genetic predisposition) were assessed in all patients. Total serum cholesterol levels, GFR (CKD-EPI) and troponin levels were measured in all participants. Echocardiography included the measurement of the left ventricular mass index. Coronary angiography with omnipaque contrast was performed in all cases. 25-OH vitamin D was measured with enzyme-linked immunosorbent assay (ELISA).Results. The majority of patients with acute coronary syndrome had vitamin D deficiency. 3 (6%) patients had severe vitamin D deficiency. The subgroup of patients with marked vitamin D deficiency (n = 39) more often had elevated blood pressure probably related to the left ventricular hypertrophy and left atrial enlargement. Left ventricular systolic dysfunction and occlusive coronary artery disease were more common in patients with lower vitamin D levels. Statistically significant correlations had not been found between vitamin D levels and cardiovascular risk factors as well as laboratory data.Conclusion. Vitamin D level below normal range was detected in the vast majority of patients with acute coronary syndrome, of them 6 % of patients had critically low vitamin D levels.

Background: Rheumatoid Arthritis (RA) is a progressive, chronic type of autoimmune disease and the role of vitamin D in the pathogenesis of RA is under investigation. Objective: The objective of this study was to determine the vitamin D deficiency in patients with RA as compared to healthy controls and to assess the relationship between serum vitamin D and anti-cyclic citrullinated peptide (anti-CCP) antibody levels in patients with newly diagnosed rheumatoid arthritis. Methods: The study was conducted between January 2017 to February 2018 at Fishtail Hospital and Research centre. A total of 63 patients with early RA diagnosed and a control group of 56 healthy participants, not on vitamin D supplements were recruited from Department of Internal Medicine. Five ml of blood samples were drawn from cubital veins. Blood glucose, creatinine, uric acid, calcium, RA factor, 25-hydroxy vitamin D, anti-CCP antibody, and erythrocyte sedimentation rates were measured in a centralized laboratory of Fishtail Hospital and Research centre. Results: The level of serum 25-hydroxy vitamin D were significantly lower in RA group (20.03 ±9.97 ng/mL) in comparison to the control group (24.46 ±8.45 ng/mL) (p<0.003). Our result indicates the prevalence of vitamin D deficiency was more in RA group compared with control group (47.61% vs. 33.16%, p <0.002). The level of Anti-CCP is significantly high in RA group than control group. In RA patients serum 25-hydroxy vitamin D levels were negatively correlated to anti-CCP antibody levels (rs = 0.72, p <0.001), and erythrocyte sedimentation rate (rs = 3.95, p <0.005). Conclusion: In RA patient vitamin D deficiency is quite common and serum 25-hydroxy vitamin D level was negatively correlated to anti-CCP antibody level. Our results suggest that vitamin D level is a motivation factor rather than a consequence of RA activity.

Objective: This study aimed to investigate the relation between vitamin D plasma concentrations and prevalence of prespecified coronary risk factors, and to assess the role of vitamin D deficiency as an independent risk factor for the acute coronary syndrome (ACS).Methods: In this study, plasma 25-hydroxyvitamin D [25(OH)D] levels were measured in 60 consecutive ACS patients at hospital presentation, and patient data including socio-demographics and clinical variables were recorded at the time of admission. We used the Independent samples T-test and the chi-square test to compare differences in the continuous and categorical variables, respectively. The partial correlation coefficient was used to measure association between plasma vitamin D levels and acute coronary syndrome while controlling for traditional cardiovascular risk factors.Results: This study found significant associations between low plasma vitamin D levels and prevalence of hypertension and smoking. Whereas, no significant association between low plasma vitamin D levels and prevalence of diabetes mellitus (DM) was found. There was a statistically significant correlation between vitamin D deficiency and acute coronary syndrome, even after controlling for traditional cardiovascular risk factors (P = 0.028).Conclusion: Vitamin D deficiency is independently associated with acute coronary syndromes, and could be an independent risk factor for the acute coronary syndrome (ACS).

Aim: Relationship between Vitamin D deficiency and pulmonary tuberculosis. Methodology: Study duration: November 2020 to April 2021 Sampling technique: A case-control study Setting: Hayatabad Medical Complex, Peshawar It included 30 adult newly identified sputum-quality pulmonary tuberculosis patients and 30 age and sex matched healthy cases as controls. All cases had undergone a thorough medical examination and repeated laboratory tests, including vitamin D, calcium, and sputum for AFB and X-ray chest. Results: Majority were males (88%). In the Study group, BMI decreased significantly i.e. 19.0 vs 23.5. Serum vitamin D concentrations were significantly lower in the tuberculosis group i.e. 17.9ng/dl than in the control group 24.8ng/dl. Mean of serum albumin in the control group was 3.9 ng/dl whereas 2.9 ng/dl in the study group. Conclusion: Hypovitaminosis D was associated with more severe medical symptoms, increased sputum smear positivity and large lesions on chest radiographs in patients with pulmonary tuberculosis. Psoriasis is an immuno-mediated chronic systemic disease involving cytokines of helper Th1 pathway. Vitamin D has an effect on keratinocyte proliferation, differentiation and immune modulation of immune system especially Th1 pathway, which is altered in psoriatic skin suggesting that Vitamin D may have a role in pathogenesis of psoriasis. To study correlation between psoriasis vulgaris and serum vitamin D. To evaluate serum vitamin D level in psoriasis cases and in control group and correlating vitamin D level with severity and duration of the psoriasis. 57 cases (&#62;15years of age) with psoriasis and 57 healthy subjects were recruited. Psoriasis was clinically diagnosed and severity evaluated by PASI scale. Vitamin D was analysed by enhanced chemilumine scence on vitrus Eci autoanalyzer of Orth clinical diagnostic. Vitamin D deficiency defined as &#60;20ng/ml, insufficiency 20-30ng/ml and sufficient 30-100ng/ml. Vitamin D deficiency in the study was 22.8% in patients and 14% in control group. Vitamin D insufficiency was found in 42.1% of cases and 19.3% of control. According to chi-square the p-value is 0.003 showing significant association. There was a tendency towards decrease in vitamin D level with increase in disease duration. There was negative correlation between vitamin D and PASI score. The study found a significant relationship between vitamin D and psoriasis. Further metanalysis involving larger study population will be required to establish whether vitamin D levels benefits patient with psoriasis vulgaris.

Vitamin D deficiency actives renin-angiotensin-aldosterone system (RAAS) which affects cardiovascular system. Activation of RAAS is associated with pulmonary hypertension (PHT). Relation between vitamin D deficiency and PHT could be therefore suggested. In  our study we compared pulmonary artery pressure between vitamin D deficiency and control groups. 115 consecutive patients (average age: 61.86 ± 5.86) who have detected very low vitamin D (vitamin D levels < 10 ng/ml) were enrolled. 117 age matched persons (average age: 61.74 ± 5.99) were selected as the control group. All groups underwent transthoracic echocardiography. Routine biochemical measurement of 25-OH vitamin D and parathormon (PTH) levels were performed. Baseline characteristics of the study groups were comparable. Systolic pulmonary artery pressure (SPAP) of patients in  the low vitamin D group was higher than the control groups. As a  result our study, a  relation between vitamin D deficiency and pulmonary artery hypertension was revealed.

Abstract Background: Previous cross-sectional studies have shown that Parkinson’s disease (PD) patients have lower serum 25- hydroxy vitamin D (25(OH)D) concentrations than controls. whether the research in other regions findings are generalizable to China populations remains untested in other studies. In this case-control study we examined the Correlation between 25-hydroxy-vitamin D and Parkinson's disease.Methods:We establish an association between deficiency of 25-hydroxy-vitamin D and PD in a case-control study of 100 PD patients and 100 control subjects free of neurologic disease in the First Affliated Hospital of Xinjiang Medical University.Results:Total 25-hydroxy-vitamin D levels, were deficient in 21% of patients with PD compared with 4% of controls. In univariate analyses Plasma levels of 25-hydroxy-vitamin D were associated with PD(p<0.001), respectively. In multivariate analyses, Vitamin D deficiency(25(OH)D <20 ng/mL) were significant associated with PD (p=0.008,OR=17.13,95% CI, 2.082-141.075). Individuals with levels in the lowest quartile of 25(OH)D values had the highest prevalence of PD(p =0.026,OR=11.786,95%CI,1.342-103.51)compared with individuals with values in the highest quartile.Conclusions:Our study reveals an association between 25-hydroxy-vitamin D and PD.Patients with incident PD had significantly lower serum 25(OH)D concentrations than age-matched controls, High-risk PD patients with vitamin D deficiency who have not yet developed exercise impairment, these populations should undergo vitamin D measurement and vitamin D treatment as soon as possible.
