A method of bread production without using baker’s yeast by using whole-ground wheat flour according to fundamentally new technology through the mechanical loosening of dough was developed. To produce the product, the work was carried out in three stages: at the first stage - the quality of grain was studied at separate stages of its preparation for grinding; on the second - the quality of whole-grain flour obtained by the disintegrating-wave method with a weak microwave information effect was analyzed; in the third - the quality of bread was assessed. Thorough cleaning of the grain and it’s repeated passing through scouring machines, grinding by the wave-disintegrating method with the energy of the electromagnetic field of ultra-high frequency has been proven to provide effective disinfection of grain from various pollutants and reduce the microbiological seeding of whole flour and bread obtained by a mechanical method of dough loosening. To improve the taste characteristics, nutritional value and microbiological purity of bread the concentrated apple juice was added to the bread’s recipe. The obtained indices of microbiological contamination of the cooked bread, which packed in a transparent polypropylene film “BIAXPLEN”, after 7 days of storage, show that in the bread from the whole-grain wheat flour the contamination is lower by 32.6% compared to the bread from entire wheat flour. Improving the microbiological purity of whole-wheat flour became possible by using traditional methods of grain preparing for milling, using the wave-disintegrating method of grinding grain into flour by disinfecting the product with electromagnetic energy of ultra-high frequency, adding concentrated apple juice to the bread’s recipe. Compared with the traditional bread made from wheat flour, the developed bread has increased nutritional value and reduced calorie content.

The recent increase in the incidence of diphtheria in our area stimulated the search for a reliable laboratory test for the immediate differential diagnosis of diphtheritic and non-diphtheritic membranous tonsillitis. Fluorescein-conjugated diphtheria antitoxin proved to be highly specific for staining toxigenic C. diphtheriae. Eight of nine patients with proved diphtheria had fluorescent organisms on a smear of exudate. The failure in the one case is attributed to the poor quality of the specimen obtained. An early case of diphtheria was detected by examination of family contacts of one of the patients in this series. The conjugated antitoxin does not result in fluorescent staining of nontoxigenic diphtheria bacilli on of other bacteria commonly found in the nasopharyngeal tract. Fluorescent staining of cultures suggests that only a small proportion of the potentially toxigenic diphtheria organisms are actually producing toxin at a given moment. The fluorescent antitoxin test can be performed within 1 hour. A positive result of the test is an absolute indication for antitoxin therapy. With a negative result the physician must still rely on his clinical judgment and cultural and virulence tests.

Currently, Diponegoro University has been composting dry leaves with conventional methods through a facility called TPST. Even so, the processing which have been carried out has not been able to process all the organic waste in the location. Seeing these conditions, it is necessary to look for alternatives that are better and more economical to apply. Bioactivators are used from the leaves of Pterocarpus indicus which serve to accelerate the composting process. The bioactivators was made by making a solution of the leaves of Pterocarpus indicus which has been mashed, then mixed with molasses and rice flour and then left for 15 days. By using experimental method, the composting process is observed. This study aims to analyze the quality of compost with applicable standards. The quality of compost is assessed by looking at the pH, temperature, and macro nutrient percentage such as C-organic, N-total, and C / N ratio. During the composting process, variations in the stirring frequency are carried out, which is once every 2 days, 4 days and 7 days. After doing laboratory test, not all reactors meet the standards for pH, temperature, C-organic, N-total, and C / N ratio according to SNI 19-7030-2004.

The effects of the provision of cereal (barley) and legume (lupin) grain supplements when 50 adult wether cashmere goats were individually penned and fed on senescent temperate summer pasture (DDM 46.9%, N 0.67%, ME 6.2 MJ/kg DM) were investigated using a factorial experiment. The goats grazed senescent temperate pasture during summer before pen feeding for 9 weeks and were then grazed on germinating autumn pastures for 3 months until shorn in mid June. The production and quality of cashmere was affected by: (i) provision of whole grain supplements, (ii) type of supplementary grain fed, and (iii) level of feeding of supplementary grain. Unsupplemented goats grew 95 g cashmere with mean fibre diameter 17.01 m and mean cashmere fibre length 52 mm. During the period of pen feeding unsupplemented goats lost 95 g liveweight/day. Provision of whole grain supplements significantly increased total yearly cashmere growth to 179 g (+88%) which was 26 mm longer (50%) and 2.12 m coarser (26%) than cashmere grown by unsupplemented goats. Increasing the level of supplementary grain resulted in linear increases in feed intake, cashmere growth, fibre diameter and length. For each 100 g/day of lupin or barley grain fed during the pen-feeding period the regression coefficients indicated an extra 33 g of cashmere was grown, which was 0.6 m coarser and for barley 10 mm longer and for lupins 17 mm longer. Cashmere (fibre length/fibre diameter2) ratio was increased by supplementary feeding, increased feed level and liveweight gain during the experimental period. Relatively longer cashmere was produced as the level of nutrition increased. It is clear that a base diet of senescent temperate pasture that is low in digestible nutrients and low in nitrogen results in suppressed cashmere growth. Strategic grain supplementation for at least 9 weeks during late summer and autumn has the potential to significantly increase cashmere production and cashmere fibre length when adult cashmere goats are grazing pastures deficient in digestible energy and nitrogen. However feeding high levels of lupin and barley grain to adult goats is contra indicated as cashmere mean fibre diameter may exceed market limits. The practical application of the results is discussed in light of previous research.

Abstract Background: Studies have revealed inappropriate laboratory testing as a source of waste. This review aimed at evaluating the effects and features of CDSSs on physicians' appropriate laboratory test ordering in inpatient hospitals.Method: Medline through PubMed, SCOPUS, Web of Science, and Cochrane were queried without any time period restriction. Studies using CDSSs as an intervention to improve laboratory test ordering as the primary aim were included. The study populations in the included studies were laboratory tests, physicians ordering laboratory tests, or the patients for whom laboratory tests were ordered. The included papers were evaluated for their outcomes related to the effect of CDSSs which were categorized based on the outcomes related to tests, physician, and patients. The primary outcome measures were the number and cost of the ordered laboratory tests. The instrument from The National Heart Lung and Blood Institute (NIH) was used to assess the quality of the included studies. Moreover, we applied a checklist for assessing the quality and features of the CDSSs presented in the included studies. A narrative synthesis was used to describe and compare the designs and the results of included studies.Result: Sixteen studies met the inclusion criteria. Most studies were conducted based on a quasi-experimental design. The results showed improvement in laboratory test-related outcomes (e.g. proportion and cost of tests) and also physician-related outcomes (e.g. guideline adherence and orders cancellation). Patient-related outcomes (e.g. length of stay and mortality rate) were not well investigated in the included studies. In addition, the evidence about applying CDSS as a decision aid for interpreting laboratory results was rare. Conclusion: CDSSs increase appropriate test ordering in hospitals through eliminating redundant test orders and enhancing evidence-based practice. Appropriate testing and cost saving were both affected by the CDSSs. However, the evidence is limited about the effects of laboratory test CDSSs on patient-related outcomes.
