In the context of the spread of coronavirus infection, the main problem of medical organizations is not an obvious increase in the cost of providing medical care associated with ensuring the sanitary and epidemiological regime (personal protective equipment, disinfection, etc.), but a decrease in income due to a drop in the volume of planned medical care, suspension of preventive measures, downtime (during the period of re-profiling, etc.). The state quickly responded to this situation by making changes to the relevant legislation and adopting a number of special regulatory legal acts. However, the implementation of these rather varied measures tied to a number of factors, such as the presence or absence of restrictive measures on the territory of the Russian Federation; participation or non-participation in the care of patients with coronavirus infection; periods within which the one or the other order advances, etc. All this causes difficulties in understanding what financial security mechanism is applied to this particular situation, what should be the actions of medical organizations. This article is devoted to the analysis of ways of financial support of medical organizations in the system of compulsory medical insurance at various stages of the fight against coronavirus infection.

Introduction. The addition of the section “Information on the cost of medical services rendered” to “Public Services”, the state information system, allowed the citizens of the Russian Federation to receive relevant information promptly. This possibility soon exacerbated the problem of unreliability of information about the medical services provided to the insured persons under compulsory medical insurance. The article defends a position based on the legal analysis of the legislation of the Russian Federation in the sphere of compulsory medical insurance. According to this position, the problem can be overcome by appropriate changes in the regulatory documents governing the control powers of the territorial funds of compulsory medical insurance, health insurance organizations. Theoretical analysis. The right to reliable information is enshrined in a number of regulatory legal acts of the Russian Federation, the analysis of which allows us to investigate the problem of unreliability of information in the system of compulsory medical insurance (“medical prescriptions”). Empirical analysis. Identification of unreliability of information about the provided medical services can be carried out both by the insured persons under compulsory medical insurance and through the control of the competent authorities. In this regard, the article analyzes the control powers of the territorial funds of compulsory medical insurance, medical insurance organizations. Results. The result of the author’s analysis of the problem of unreliability of information in the system of compulsory medical insurance (“medical attributions”) is a proposal to improve the procedure for organizing and monitoring the volume, timing, quality and conditions of providing medical care for compulsory medical insurance.

In the beginning of the COVID-19 pandemic in March 2020, by the decree of the Mayor of Moscow, the provision of dental services to the population was temporarily suspended, with the exception of diseases and conditions requiring emergency or urgent dental care. A Headquarters was established to coordinate the provision of emergency and urgent dental care for COVID-19 patients and for those who was in isolation during lockdown. The Moscow Healthcare Department has been organized an activity of mobile medical and nursing teams (MNT) to provide emergency medical care in the "dentistry" profle. All MNTs were equipped with portable dental equipment. The MNTs has been working for free within the framework of compulsory medical insurance. For the period from March 26 to June 16 – 2020, medical care was provided to 1501 patients, of which 294 with a confrmed diagnosis of COVID-19, including 57 children. The measures taken were of a temporary nature and were aimed at preserving the health of Moscow residents and preventing the further spread of the new coronavirus infection COVID-19 in Moscow.

Article is devoted to the analysis of the Order of the Russian Ministry of Health of 19.03.2020 № 198n (an edition of 29.04.2020) “About a temporary order of the organization of work of the medical organizations for implementation of measures for prevention and reduction of risk of spread of a new koronavirusny infection of COVID19” regarding formation of the temporary staff list of the division of the medical organization providing medical care to citizens with a koronavirusny infection of COVID-19 and also the requirement with the staff of this division. The criteria recommended by the Ministry of Health of the Russian Federation for determining the number of rates of medical workers in these divisions, requirements for medical workers of the division have been considered, a practical example of calculation of rates has been given.

The paper considers a way to represent the relationship between indicators in the form of copulas. Copulas are popular mathematical tools. This is due to the fact that, on the one hand, the marginal distributions of indicators are divided in the copulas, and on the other hand, the structure of the relationship between these marginal distributions is divided, which makes it  possible to very effectively study the connections that arise in real  populations. Special attention in the work is paid to extremal dependence coefficients - important numerical characteristics of the connection in conditions of extreme small or extremely large values of indicators. It is shown that even under conditions of close correlation between the indices for a two-dimensional Gaussian distribution, the lower and upper coefficients of the extreme dependence take zero values. This indicates the impossibility of predicting the values of one indicator when fixing too small or too large values of another indicator. This work shows that the relationship between the number of COVID-19 coronavirus infections per 100,000 people and the number of deaths from COVID-19 coronavirus infection per 100,000 people in the regions of the Russian Federation can be represented in the form of a Gaussian copula.

The subject of the study is the inter-regional differences in the state of public health and the demographic situation in the Russian Federation regions. The theoretical aspect of the subject is determined by the development of priorities of the Russian healthcare development strategy aimed at alignment of regional differences in the levels of healthcare development in the Russian Federation regions. The empirical aspect of the subject is to identify interregional differences in the state of public health and the demographic situation in the Russian Federation regions by using the EM cluster analysis method (Expectation Maximization). The method was implemented in the integrated development environment RStudio. The official statistics from Rosstat for the period 2014–2018 were used as the initial dataset. The purpose of the study is justifying the regulation of inter-regional differences of the Russian Federation regions. As a result of clustering, nine homogeneous clusters of the Russian Federation regions were identified. The main characteristics of the formed clusters are determined. Among the priorities of the RF healthcare development strategy are as follows: implementation of a unified tariff policy in the system of compulsory medical insurance; ensuring the balance of territorial compulsory medical insurance programs within the framework of the basic programme of compulsory medical insurance through financial security based on a single per capita standard; development of telemedicine, providing prompt remote consultation of leading experts in the provision of medical care, regardless of the territorial location of the patient and the doctor; ensuring the implementation of distance education courses and continuing education programs for medical workers; rationalization of the distribution of resources and capacities of medical organizations based on a three-tier system of medical care; development of regional public health centres. The results of this study can be used to develop federal and territorial programs for socioeconomic development, formulate a strategy for the development of healthcare at macro- and meso- levels, and optimize decisions of regional authorities regarding population policy.

Objective was to analyze the epidemiological situation on Ixodidae tick-borne borreliosis (ITBB) in the constituent entities of the Russian Federation in the dynamics over the period of 2010–2020, taking into account the features of the year 2020, associated with the spread of COVID-19, and to forecast the development of the epidemic process for 2021. In 2020, 4180 cases of ITBB were registered in the Russian Federation, which is 1.93 times less than in 2019 and 1.73 times less than the long-term average annual figure for the previous 10 years. A significant decrease was noted in all federal districts (FD), with the exception of the Central FD, where more than half of all cases occurred. Compared to 2019, 75 out of 78 subjects experienced a decrease in the incidence rate in 2020, while in 56 entities that decrease was statistically significant. The multiplicity of the decrease in the incidence of ITBB in 2020 for all FD was significantly greater than the multiplicity of the decrease in the frequency of contacts of the population with ticks. Infection of Ixodidae ticks of various species with borrelia in natural foci in 2020 was statistically significantly higher than in 2019. Probably, the decrease in the registered incidence of ITBB against the background of the COVID-19 pandemic is due not only to a decrease in the intensity of contacts of the population with natural foci, but also to a sharp redistribution of the volume of inpatient and outpatient medical care in favor of COVID-19 patients. Given the high degree of significance of the identified trends in the dynamics of the epidemic ITBB process during 2010–2020, there is reason to expect an increase in the incidence in the Bryansk, Voronezh, Ryazan, Tula and Saratov regions in 2021, which requires special attention in terms of strengthening preventive measures and monitoring of the activity and structure of natural foci. In the rest of the entities of the Russian Federation, provided that the volumes of preventive measures are maintained on the same level and in the absence of abnormal natural and climatic phenomena, preservation or a slight reduction in the incidence of ITBB is likely to occur in the short term. At the same time, despite the tense situation regarding the new coronavirus infection, there is an obvious need to draw close attention to the problems of diagnosis and prevention of natural focal infections.

AbstractThe paper presents results of research related to familiarity and understanding of home hospice term and shows how the social media discourse of palliative care looks like. Answers and conclusions are crucial for palliative care organisations as their existence depends on donors financial support which engagement is strongly related to communication activities performed by those organisations.In the paper there has been presented opinions about public discussion about terminally ill children and its potential need for being treated as a taboo. The data whether futile medical care should be performed whatever the cost is also shown in the paper. Researchers asked also who should be responsible for executing management of hospices in Poland. The main conclusions focus on the necessity to intensify communication activities, especially by the professionals.

BACKGROUND: In March 2020, the coronavirus disease (COVID-19) infection was assigned the status of a pandemic. As of the beginning of 2021, the Russian Federation ranks fourth in terms of the prevalence of coronavirus infection. Over the period from March 2020 to February 2021, more than 84,000 fatal cases of the disease were recorded in Russia. AIM: However, at the moment, there are no medications with proven effectiveness and safety against the novel coronavirus infection. In this regard, the purpose of our study was to conduct a pharmacoeconomic analysis of medications for etiotropic therapy of all forms of COVID-19 recommended by the Ministry of Health of the Russian Federation (clinical guidelines, version 10 dated February 8, 2021) to identify the best treatment option. MATERIALS AND METHODS: In the course of the study, the “cost of illness” was determined for all forms of the disease in an outpatient and inpatient setting. The authors took into account the direct medical costs of medication therapy and diagnostic and treatment procedures. In terms of direct non-medical costs, they calculated the cost of a bed-day excluding medication treatment, and indirect costs included payments for temporary disability sheets. Costs for medications were calculated based on the active ingredient (AI) and the packages for treatment on an outpatient basis and in the case of the hospital setting based on the AI only. The cost of medical and diagnostic procedures was determined based on the Tariff Agreement for 2020 dated December 30, 2019. Next, a cost-effectiveness analysis was performed. Effectiveness criteria were selected based on published clinical trial results for the medications in question. Then, they performed a calculation of the cost-effectiveness coefficients and an incremental analysis. RESULTS: Thus, in the course of the analysis of the cost of illness, the most economically profitable treatment regimens were the ones with hydroxychloroquine both for outpatient treatment (13,150.31 rubles: Mild form, 22,326.44 rubles: Moderate form excluding antibiotic therapy, and 21,513.76 rubles: Moderate form, taking into account antibacterial therapy) and for inpatient treatment (34,441.53 rubles). CONCLUSION: As a result of the cost-effectiveness analysis, the use of favipiravir can be considered optimal (comparative effectiveness research = 17,607.14 rubles), and for the mild form, the optimal medication is umifenovir, since during the incremental analysis, it was found that for therapy with favipiravir, 100 people would need an additional allocation of 96.291 rubles, which, given the form of the disease, is not entirely appropriate.

Aim. To analyze the dynamics of the development of voluntary medical insurance in the Russian Federation. To identify the factors hindering the development of this insurance sector in modern conditions. Methods. In the course of the study, analysis was conducted of the legislative framework for organizing medical care for the population of the Russian Federation in the system of voluntary medical insurance. The problem-chronological, systematic, and analytical research methods were used. Results. It has been established that, despite the development of voluntary medical insurance system over the past 25 years, the availability of this type of insurance for citizens of the Russian Federation still remains very low. The policy of voluntary medical insurance is mainly available only to working citizens, and only in those large enterprises where the employer is interested in preserving and protecting the health of its employees. For most citizens of the Russian Federation, the voluntary health insurance policy remains inaccessible due to the high cost of the policy, as well as due to relatively low incomes. At the same time, a voluntary health insurance policy is required by law for labor migrants to obtain a patent for employment in the Russian Federation. However due to the absence of legislative framework for voluntary health insurance, organization of medical care for labor migrants is not always standardized. Conclusion. For the further development of voluntary medical insurance, it is necessary to develop the measures for decreasing the cost and increasing the availability of a voluntary medical insurance policy for citizens of the Russian Federation; to optimize organization of health care for labor migrants it is necessary to primarily develop regulatory framework of emergency health care.
