Operators must be able to rely on their own “sea sense,” developed through experience and mentoring, and use technology to accomplish specific objectives rather than defer to automation as the default decision-maker. Maintaining the competitive warfighting edge requires cultivating skilled mariners who know how to fight a well-equipped ship; adding complexity to the system without accounting for the human element creates added risk and cutting-edge failure modes. Technology alone cannot make the ship safe, but when the operator lacks fundamental knowledge and experience, it can make the ship unsafe.

Structural reliability thus is a property of a structure or structural member, depending on the relevant limit states, failure modes and basic variables. This is the basis for the determination of partial safety factors which are, for sake of a simpler design, applied within deterministic design procedures. In addition to the basic variables in terms of material and loads, further basic variables representing the structural model have to be considered. These depend strongly on the experience of the design engineer and the level of detailing of the model. However, in the clear majority of cases [1] failure does not occur due to unexpectedly high or low values of loads or material strength. The most common reasons for failure are human errors in design and execution. This paper will provide practical examples of original designs affected by human error and will assess the impact on structural reliability.

When faced with a decision between several options, people rarely fully consider every alternative. Instead, we direct our attention to the most promising candidates, focusing our limited cognitive resources on evaluating the options that we are most likely to choose. A growing body of empirical work has shown that attention plays an important role in human decision making, but it is still unclear how people choose with option to attend to at each moment in the decision making process. In this paper, we present an analysis of how a rational decision maker should allocate her attention. We cast attention allocation in decision making as a sequential sampling problem, in which the decision maker iteratively selects from which distribution to sample in order to update her beliefs about the values of the available alternatives. By approximating the optimal solution to this problem, we derive a model in which both the selection and integration of evidence are rational. This model predicts choices and reaction times, as well as sequences of visual fixations. Applying the model to a ternary-choice dataset, we find that its predictions align well with human data.

Chemical vapor deposition (CVD)-grown diamond films have found applications as a hard coating for cutting tools. Even though the use of conventional diamond coatings seems to be accepted in the cutting tool industry, selections of proper coating thickness for different machining operations have not been often studied. Coating thickness affects the characteristics of diamond coated cutting tools in different perspectives that may mutually impact the tool performance in machining in a complex way. In this study, coating thickness effects on the deposition residual stresses, particularly around a cutting edge, and on coating failure modes were numerically investigated. On the other hand, coating thickness effects on tool surface smoothness and cutting edge radii were experimentally investigated. In addition, machining Al matrix composites using diamond coated tools with varied coating thicknesses was conducted to evaluate the effects on cutting forces, part surface finish and tool wear. The results are summarized as follows. (1) Increasing coating thickness will increase the residual stresses at the coating-substrate interface. (2) On the other hand, increasing coating thickness will generally increase the resistance of coating cracking and delamination. (3) Thicker coatings will result in larger edge radii; however, the extent of the effect on cutting forces also depends upon the machining condition. (4) For the thickness range tested, the life of diamond coated tools increases with the coating thickness because of delay of delaminations.

<p><strong>INTRODUCTION:</strong> In any complex human system, human error is inevitable and shows that can’t be eliminated by blaming wrong doers. So with the aim of improving Intensive Care Units (ICU) reliability in hospitals, this research tries to identify and analyze ICU’s process failure modes at the point of systematic approach to errors.</p><p><strong>METHODS:</strong> In this descriptive research, data was gathered qualitatively by observations, document reviews, and Focus Group Discussions (FGDs) with the process owners in two selected ICUs in Tehran in 2014. But, data analysis was quantitative, based on failures’ Risk Priority Number (RPN) at the base of Failure Modes and Effects Analysis (FMEA) method used.<strong> </strong>Besides, some causes of failures were analyzed by qualitative Eindhoven Classification Model (ECM).</p><p><strong>RESULTS:</strong> Through<strong> </strong>FMEA methodology, 378 potential failure modes from 180 ICU activities in hospital A and 184 potential failures from 99 ICU activities in hospital B were identified and evaluated. Then with 90% reliability (RPN≥100), totally 18 failures in hospital A and 42<strong> </strong>ones in hospital B were identified as non-acceptable risks and then their causes were analyzed by ECM.</p><p><strong>CONCLUSIONS</strong>: Applying of modified PFMEA for improving two selected ICUs’ processes reliability in two different kinds of hospitals shows that this method empowers staff to identify, evaluate, prioritize and analyze all potential failure modes and also make them eager to identify their causes, recommend corrective actions and even participate in improving process without feeling blamed by top management. Moreover, by combining FMEA and ECM, team members can easily identify failure causes at the point of health care perspectives.</p>

Dry hobbing, for friendliness on the environment, for increasing productivity and for decreasing manufacturing cost, often suffers failure of cutting edge of hob or problems about the surface quality of tooth flank of manufactured gear. In the previous report, a simulation method to calculate the clearance between cutting edge of hob and tooth flank of work gear during hobbing process is developed. In this report, this simulation is applied to some practical industrial problems of dry hobbing, i.e. chipping failure of cutting edge of carbide hob and bad coarse scratched surface finish of hobbed tooth flank of manufactured gears, perhaps due to chip crush. The simulation explains the mechanism of such actual failures in detail. On this fact, the “Distance of Single Edge Cutting” of a hob tooth is proposed to be an index of pinching and crushing of chips that could intrude into the clearance between hob cutting edge and tooth flank of work gear. Influencing factors on it are investigated and the bases for finding countermeasures are shown. This method has been applied to some practical cases of problem in mass-production of automotive gears, and good result was confirmed.

Niche and proprietary data formats used in cutting-edge research and technology have specific curation considerations and challenges. The increased demand for subject liaisons, library archivists, and digital curators to curate this variety of data types created locally at an institution or organization poses difficulties. Subject liaisons possess discipline knowledge and expertise for a given domain or discipline and digital curation experts know how to properly steward data assets generally. Yet, a gap often exists between the expertise available within the organization and local curation needs. While many institutions and organizations have expertise in certain domains and areas, oftentimes the heterogeneous data types received for deposit extend beyond this expertise. Additionally, evolving research methods and new, cutting-edge technology used in research often result in unfamiliar and niche data formats received for deposit. Knowing how to ‘get-started’ in curating these file types and formats can be a particular challenge. To address this need, the data curation community have been developing a new set of tools - data curation primers. These primers are evolving documents that detail a specific subject, disciplinary area or curation task, and that can be used as a reference or jump-start to curating research data. This paper will provide background on the data curation primers and their content detail the process of their development, highlight the data curation primers published to date, emphasize how curators can incorporate these resources into workflows, and show curators how they can get involved and share their own expertise.
