Abstract Background Male condoms are effective in preventing common sexually transmitted infections (STIs) and unintended pregnancy, if used correctly and consistently. However, condom use errors and problems are common and young people report negative experiences, such as reduced pleasure. The Kinsey Institute Home-Based Exercises for Responsible Sex (KIHERS) is a novel condom promotion intervention for young women, which aims to reduce condom errors and problems, increase self-efficacy and improve attitudes towards condoms, using a pleasure-focussed approach. The study objective was to test the operability, viability and acceptability of an adapted version of the KIHERS intervention with young women aged 16–25 years in the United Kingdom (UK) (Home-Based Exercises for Responsible Sex-UK (HERS-UK). Methods A repeated-measures single-arm design was used, with a baseline (T1) and two follow-up assessments (T2 and T3), conducted 4 weeks and 8 weeks post intervention over a 3-month period. Participants were provided a condom kit containing different condoms and lubricants and were asked to experiment with condoms alone using a dildo and/or with a sexual partner. Ten process evaluation interviews were conducted post intervention. Results Fifty-five young women received the intervention; 36 (65%) completed T2 and 33 (60%) completed T3. Condom use errors and problems decreased, self-efficacy increased and attitudes towards condoms improved significantly. The proportion of participants who reported using a condom for intercourse in the past 4 weeks increased from T1 (20; 47%) to T2 (27; 87%) and T3 (23; 77%) and using lubricant with a condom for intercourse increased from T1 (6; 30%) to T2 (13; 48%)) and T3 (16; 70%). However, motivation to use condoms did not change. Cronbach’s alpha scores indicated good internal consistency of measures used. Qualitative data provided strong evidence for the acceptability of the intervention. Conclusions HERS-UK was implemented as intended and the recruitment strategy was successful within a college/university setting. This feasibility study provided an early indication of the potential effectiveness and acceptability of the intervention, and the benefits of using a pleasure-focussed approach with young women. Measures used captured change in outcome variables and were deemed fit for purpose. Future research should explore cost-effectiveness of this intervention, in a large-scale controlled trial using a diverse sample and targeting young women most at risk of STIs.

Nigella Sativa has been named as a prophetic Spice, as it has many therapeutic effects. It is found to control diabetes, reduce high cholesterol levels, and serve as an antioxidant, anti-cancer, anti – informatory and an immune booster. The compound Thymoquinone can be responsible for enhancing immune system as it acts as an anti-oxidant and has an anti-inflammatory property. As various research has showed that to protect from Coronavirus, it is important not only to avoid human contact, but also to strengthen the Immune system. This paper is a review of various studies which prove that Nigella Sativa, can be beneficial in enhancing the immune system and thus fight COVID – 19.

Background:Social isolation is a significant problem for frail older adults and the determinants ofsocial engagement are poorly understood. Objectives:This study explored the social engagement of frail eldersto identify personal attributes associated with social engagement. Design, Setting and Participants:A cross-sectional sample of seventy-three people receiving home-based care in one town on the South Island of NewZealand (mean age 82 (7.2) yrs, n=51 Females, 21 Males). Measurements:Face-to-face semi-structuredinterviews and questionnaires. Functional independence was measured using Nottingham Extended Activities ofDaily Living (EADL), self-efficacy by General Self Efficacy Scale, and 2 open-ended questions were piloted onsocial activities and helping others. Results:Regression models identified two statistically associatedcomponents of social engagement: social activities and civic involvement. Contributions to families andcommunity organizations and exercise were important social activities. Personal attributes included perceivedfunctional independence and self-efficacy. Conclusions:In frail older adults, a measurement of socialengagement should address activities older adults identify as important, including exercise. Independence, self -efficacy, and social engagement may interact in reinforcing cycles of empowerment and could play a role indeveloping interventions to retain and maintain function in frail older adults.

Background Home-based noninvasive ventilation has proven cost-effective. But, adherence to therapy still constitutes a common clinical problem. We hypothesized that a behavioral intervention supported by a mobile health (mHealth) app could enhance patient self-efficacy. It is widely accepted that mHealth-supported services can enhance productive interactions among the stakeholders involved in home-based respiratory therapies. Objective This study aimed to measure changes in self-efficacy in patients with chronic respiratory failure due to diverse etiologies during a 3-month follow-up period after the intervention. Ancillary objectives were assessment of usability and acceptability of the mobile app as well as its potential contribution to collaborative work among stakeholders. Methods A single-blind, single-center, randomized controlled trial was conducted between February 2019 and June 2019 with 67 adult patients with chronic respiratory failure undergoing home-based noninvasive ventilation. In the intervention group, a psychologist delivered a face-to-face motivational intervention. Follow-up was supported by a mobile app that allowed patients to report the number of hours of daily noninvasive ventilation use and problems with the therapy. Advice was automatically delivered by the mobile app in case of a reported problem. The control group received usual care. The primary outcome was the change in the Self Efficacy in Sleep Apnea questionnaire score. Secondary outcomes included app usability, app acceptability, continuity of care, person-centered care, and ventilatory parameters. Results Self-efficacy was not significantly different in the intervention group after the intervention (before: mean 3.4, SD 0.6; after: mean 3.4, SD 0.5, P=.51). No changes were observed in adherence to therapy nor quality of life. Overall, the mHealth tool had a good usability score (mean 78 points) and high acceptance rate (mean score of 7.5/10 on a Likert scale). It was considered user-friendly (mean score of 8.2/10 on a Likert scale) and easy to use without assistance (mean score of 8.5/10 on a Likert scale). Patients also scored the perception of continuity of care and person-centered care as high. Conclusions The integrated care intervention supported by the mobile app did not improve patient self-management. However, the high acceptance of the mobile app might indicate potential for enhanced communication among stakeholders. The study identified key elements required for mHealth tools to provide effective support to collaborative work and personalized care. Parents play a key role in children’s home-based online learning. This study constructed a mediating model to explore the mechanism of parents’ attitudes toward online learning (PATOL) and the perceived online learning ineffectiveness (POLI) of their children and to investigate the mediating effect of parents’ self-efficacy (PSE) on PATOL and POLI. Valid questionnaire data from 18,170 middle school parents were collected by snowball sampling. The hypotheses proposed in this study were verified by using Model 4 of PROCESS. The results showed that: when controlling parents’ gender, age, and children’s length of online learning in regression equations, (1) both PATOL and PSE were negatively related to POLI, while PATOL was positively related to PSE; (2) PSE played a mediating role in the relationship between PATOL and POLI. This study also discusses how to support parents to assist children’s home-based online learning. Schools should carry out some necessary training for parents. Parents can get guidance and advice on how to create an environment conducive to children’s online learning.

BACKGROUND Post-transplant metabolic syndrome (PTMS) is a common contributor to morbidity and mortality in solid organ transplant recipients in the late post-transplant period (≥ 1-year). Patients diagnosed with PTMS are at higher risk of cardiovascular disease and frequently experience decreased physical function and health-related quality of life (HRQL). Studies in the early post-transplant period (< 1-year) have shown the benefits of facility-based exercise training on physical function and HRQL, but have not evaluated the effects on metabolic risk factors. It remains unclear whether home-based exercise programs are feasible and can be sustained with sufficient adherence and exercise dose to have effects on PTMS. This protocol outlines the methodology of a randomized controlled trial of a partly-supervised home-based exercise program in lung (LTx) and liver (OLT) transplant recipients. OBJECTIVE 1) To evaluate the feasibility (i.e. recruitment rate, program adherence, attrition, safety, and participant satisfaction) of a 12-week individualized, home-based aerobic and resistance training program in LTx and OLT recipients initiated 12 to 18 months post-transplant; and 2) to assess estimates of intervention efficacy on metabolic risk factors, self-efficacy for exercise, and HRQL. METHODS 20 LTx and 20 OLT recipients with two or more cardio-metabolic risk factors at 12-18 months post-transplant will be randomized to an intervention group (home-based exercise training) or a control group. The intervention group will receive an individualized exercise prescription comprising aerobic and resistance training 3-5 times per week for 12 weeks. Participants will meet with a qualified exercise professional weekly (via videoconference) to guide exercise progression, provide support, and promote exercise self-efficacy. Participants in both study groups will receive one counselling session on healthy eating with a dietitian at the beginning of the intervention. For the primary aim, feasibility will be assessed through recruitment rate, program adherence, satisfaction, attrition, and safety. Secondary outcomes will be measured at baseline and 12-weeks, and include assessments of metabolic risk factors (i.e. insulin resistance, abdominal obesity, blood pressure, and cholesterol), HRQL, and exercise self-efficacy. Descriptive statistics will be used to summarize program feasibility and effect estimates (means and 95% confidence interval) for sample size calculations in future trials. RESULTS Enrollment for this study started in July 2021. It is estimated that the study period will be 18 months with data collection completed by December 2022. CONCLUSIONS A partly-supervised home-based, individually tailored exercise program that promotes aerobic and resistance training and exercise self-efficacy may prove to be an important intervention for improving the metabolic profile of LTx and OLT recipients with cardio-metabolic risk factors. Thus, characterizing the feasibility and effect estimates of home-based exercise constitutes the first step in the development of future clinical trials designed to reduce the high morbidity associated with PTMS.
