BACKGROUND: Metabolic syndrome is a cluster of risk factors that is responsible for the risk of coronary heart disease and stroke. Therefore, the aim of this study was to assess the prevalence of MetS and its components among T2DM patients.METHODS: A cross-sectional study was conducted at the Diabetes Clinic of the Hospital, from June to July, 2015. Data were entered into EPI INFO software and exported to SPSS 20 for analysis. MetS prevalence was estimated using NCEP ATPIII and IDF criteria. Anthropometric measurements, investigations of serum glucose and lipid profiles were done. Logistic regression analysis was used to evaluate associated factors. A P-value ≤ 0.05 wasconsidered statistically significant.RESULT: A total of 159 participants were included in the study; 119 (59.7%) were females with mean (±SD) age of (49.8±8.7) year. The prevalence of MetS was 66.7% in NCEP-ATP III and 53.5% in IDF definitions. The most prevalent component of MetS was elevated triglyceride (56.6% in ATPIII and 62.3% in IDF criteria), followed by abdominal obesity (61%) IDF and elevated blood pressure (55.4%) NCEP-ATPIII criteria. The regression analysis showed that increased age, being female, high BMI, having diabetes for over 5 years and poor glycemic control were significantly associated with metabolic syndrome.CONCLUSION: The prevalence of MetS and its components among T2DM patients were high, suggesting that diabetic patients are at increased risk of CVD and other complications. Efforts should be geared towards addressing these abnormalities through lifestyle modification, health awareness and medications in order to reduce this complication.

Objective: To assessed the types 2 diabetic patient’s knowledge, the practice of, and attitude to lifestyle modification at University College Hospital (UCH), Ibadan. Methods: A cross-sectional descriptive study of types- 2 diabetic patients at the Medical outpatient clinic in UCH was done with a validated structured 48 item self-administered questionnaire that measured the socio-demographics, knowledge, practice of and attitude to lifestyle modifications. A purposive sampling of 99 consenting patients out of 103 determined with Yaro Tamane formulae was done.  Data were entered into SPSS v. 21 for descriptive and inferential analysis. Results: About 59.6% of respondents were within ≥60 years age range, and 68.7% of them were females. Many (51.5%) of them had tertiary education qualifications. On knowledge of symptoms, the majority 78 (78.8%) had good knowledge and 21(21.2%) had poor knowledge of diabetes mellitus. While 64 (64.6%) respondents had a good attitude and 35(35.4%) had a poor attitude towards lifestyle modification. Besides, a vast majority (94.9%) reported that lifestyle modification helps attain normal blood glucose levels, preserves life, assuage symptoms, and avert extended complications. The respondents tend to strongly agree to have a good practice of lifestyle modification with a weighted mean of 3.76 to 4.69 on a 5point scale. Conclusion: The majority of the respondents had good knowledge, the practice of and attitude to lifestyle modification which can help patients in altering long-term habits, eating or physical activity, and maintaining these practices as part of the management of the disorder.

Abstract Background Meconium is a green viscous fluid and its passage normally occurs within the first 24-48 hours after birth. However, fetuses could pass meconium before or during labor and mostly intrauterine passage was found to be associated with different feto-maternal stress factors resulting in increased risk of perinatal morbidity and mortality. In Ethiopia little is known about the proportion and associated factors for meconium stained amniotic fluid (MSAF). The aim of the study was to assess the proportion and associated factors of MSAF in women who came for labor and delivery service in a tertiary hospital in Ethiopia.Methods A cross sectional study was conducted from 1st June to 31 August, 2018 on 606 laboring mothers at Felege Hiwot Referral Hospital, northwest Ethiopia. Study participants were selected using a systematic random sampling technique. Data was collected using interviewer administered structured questionnaire. Data entry and analysis were made by using Epi-data v3.1 and SPSS v24, respectively. Both descriptive & analytical statistics were computed. Statistical significance was set at P < 0.05. Logistic regression was used to explore factors associated with MSAF.Results The proportion of MSAF was at 149 (24.6%) [95%CI: 21.3-28.2%]. Nonreassuring fetal heart rate pattern (AOR: 21.9,95% CI: 10.96-43.83), post term pregnancy (4.54,95% CI: 2.24-9.20), duration of labor more than 15 hours (AOR: 2.83,95% CI: 1.76-4.53), pregnancy induced hypertension (AOR: 2.43, 95% CI: 1.45-4.05), oligohydramnios (AOR: 2.53, 95% CI: 1.25-5.12) inter pregnancy interval less than 2 years (AOR: 2.24,95% CI: 1.12-4.51) and monthly family income less than 5000 Ethiopian Birr (185 United States Dollar) (AOR: 2.03, 95% CI:1.18-3.51) were significantly associated with MSAF.Conclusions The proportion of MSAF was at 24.6% which was higher than a previous study done in Ethiopia and elsewhere. Non-reassuring fetal heart rate pattern, post term pregnancy, duration of labor more than 15 hours, pregnancy induced hypertension, oligohydramnios, inter pregnancy interval less than 2 years and monthly family income less than 5000 Ethiopian Birr were factors associated with an increased risk for MSAF. Health professionals should assess laboring women for the presence of these factors and should stay alert to detect MSAF early and to act accordingly.

Objective. Diabetic peripheral neuropathy (DPN) is a common microvascular complication of diabetes mellitus (DM) and may progress to diabetic foot, which frequently leads to amputation and/or disability and death. Data is scanty on the burden of diabetic peripheral neuropathy in Tanzania. The aim of this study was to assess the burden of peripheral neuropathy, its severity, and the associated factors. Methods. The study was a cross-sectional hospital-based study and was carried out from October 2017 to March 2018 among adolescent and adult patients attending Kilimanjaro Christian Medical Center (KCMC) diabetes clinic. Results. A total of 327 diabetic patients, females n=215 (65.7%) and males n=121 (34.3%), were included in the study. The mean age was 57.2 yrs. A total of 238 (72%) had type 2 and 89 (27.2%) had type1 DM. The prevalence of peripheral neuropathy was 72.2% of whom 55% were severe, 19% were moderate, and 26% were mild. The severity of neuropathy increased with the increase in age >40 years (p < 0.001) and increase in body mass index (p<0.001) and duration of diabetes; duration >7 years (p <0.006). The main associated factors were age >40 years, OR 2.8 (1.0-7.7), >60 years, OR 6.4 (2.3-18.2), obesity, OR 6.7 (0.9-27.7), and hypertension, OR 4.3 (2.2-8.2). Conclusion. More than half of the patients included in this study were found to have neuropathy, nearly half of whom presented with the severe form. The main risk factors were increasing age, increasing duration of diabetes, obesity, and hypertension. Diabetic peripheral neuropathy is underdiagnosed in northern Tanzania where screening for neuropathy is not routinely done.

Background: Sleep disturbances and stress may intervene with the physiological processes in human body and act as a modulator of metabolic homeostasis. The aim and objectives of the study were to estimate the person’s average sleep propensity; to estimate the stress level in the study subjects; and to determine the association of sleep and stress with metabolic syndrome.Methods: A community based cross sectional study was conducted in an urban area of Rishikesh. Sample size was calculated to be 478. Data was entered and analyzed using SPSS Version 20.0. Chi-square test was used to examine the association between categorical variables. A p value of <0.05 was considered significant.Results: It was observed that different levels of stress did not have any significant association with metabolic syndrome (MetS) though the odds among individuals with moderate stress were 1.1 times (95% CI 0.6-2.2) higher than those with low stress. The overall sleepiness grading was not significantly associated with Metabolic Syndrome. However, people with mild excessive daytime sleepiness have 4.7 times (95% CI 1.2-18) higher odds of MetS as compared to those with low normal daytime sleepiness. Those with moderate excessive day sleepiness had 1.59 times (95% CI 0.3-8) higher odds of MetS, however it was not statistically significant.Conclusions: Mild excessive daytime sleepiness was associated with an increased risk of metabolic syndrome. People with perceived stress are having higher chance of getting MetS.

Objective: To determine the association between metabolic syndrome and alcohol consumption, including drinking patterns and risk of harm. Materials and Methods: A cross-sectional study was conducted on 400 participants recruited from the Outpatient Unit of the Family Medicine Department, Faculty of Medicine, Chiang Mai University. Physical examination and blood tests were carried out to evaluate the presence of metabolic syndrome. The drinking patterns were stratified into four levels, which were abstinence, occasional drinking, light–moderate drinking, and heavy drinking (>40 gm/day). The Alcohol Use Disorders Identification Test was used to evaluate the risk of harm from alcohol consumption. Results: The percentage of current drinkers was 27.3%, and most of them were men (87.2%). Waist circumference, triglyceride levels, and diastolic blood pressure were significantly higher among those with a heavy drinking habit and harmful alcohol use/dependence. High density lipoprotein cholesterol (HDL-C) showed different results. A J-shaped association was found between HDL-C and drinking pattern, but an inverse relationship was indicated with the risk of harm. Metabolic syndrome was found to be significantly associated with heavy drinking (OR=4.4, 95% Confidence Interval (CI) 1.2–15.6, p-value 0.021) and the harmful use/dependent categories (OR=5.0, 95% CI 1.1–23.1, p-value 0.039). Conclusion: The results suggest that alcohol drinking tends to be associated with an increased risk of metabolic syndrome. This condition should be screened regularly especially in those with heavy drinking or at-risk drinking habits.

Abstract Background Understanding the relationship between breastfeeding (BF) and metabolic syndrome (Mets) is important for maternal long-term health benefits and disease prevention. This study aimed to examine the association between BF and Mets and its components among postmenopausal parous Korean women. Methods This cross-sectional study on 10,356 Korean women used nationally representative data from the KNHANES from 2010 to 2016. Anthropometric, laboratory data and manual BP were measured. A multivariate logistic regression analysis was conducted to examine the association of BF with Mets and its components after adjusting for potential confounding variables. A p-value < 0.05 was to be considered statistically significant. Results Mets was present in 42% of the study participants. The BF group had low household income and education level. The prevalence of Mets in the BF group was higher than that in the non-BF group (42.69% vs. 34.76%, p <  0.001). BF was associated with increased risk of Mets (odds ratio [OR]: 1.4, 95% confidence interval [CI]: 1.18–1.65, p <  0.001). The BF group was at higher risks for diabetes (OR: 1.5, 95%CI: 1.14–1.98), hypertension (OR: 1.32, 95%CI: 1.03–1.68), hypertriglyceridemia (OR: 1.42, 95%CI: 1.02–1.99) and low high-density lipoprotein cholesterol (OR: 1.32, 95%CI: 1.06–1.65). Conclusion In this study, BF did not affect decreasing the prevalence of Mets and its components.

ObjectivesThe aim of these analyses was to examine the association of cortisol, DHEAS and the cortisol:DHEAS ratio with the metabolic syndrome (MetS) and its components.DesignThe analyses were cross-sectional.MethodsParticipants were 4255 Vietnam era US army veterans. From military service files, telephone interviews and a medical examination, occupational, socio-demographic and health data were collected. MetS was ascertained from data on body mass index; fasting blood glucose or a diagnosis of diabetes; blood pressure or a diagnosis of hypertension; high-density lipoprotein cholesterol; and triglyceride levels. Contemporary morning fasted cortisol and DHEAS concentrations were determined. The outcomes were MetS and its components. Analysis was by logistic regression, first adjusting for age and then additionally for an array of candidate confounders.ResultsCortisol, although not in the fully adjusted analysis, and DHEAS were both related to MetS. Whereas high cortisol concentrations were associated with an increased risk of MetS, high DHEAS concentrations appeared protective. By far, the strongest associations with MetS were observed for the cortisol:DHEAS ratio; the higher the ratio, the greater the risk of having MetS. The ratio was also significantly related to four of the five MetS components.ConclusionsThe cortisol:DHEAS ratio is positively associated with MetS. Prospective analyses are needed to help untangle direction of causality, but this study suggests that the cortisol:DHEAS ratio is worthy of further study in this and other health contexts.

We aimed to determine cut-points for muscle strength based on metabolic syndrome diagnosis. This cross-sectional analysis comprised data from 2 cohorts in Brazil (EpiFloripa Adult, n = 626, 44.0 ± 11.1 years; EpiFloripa Aging, n = 365, 71.6 ± 6.1 years). Metabolic syndrome was assessed by relative handgrip strength (kgf/kg). Metabolic syndrome was defined as including ≥3 of the 5 metabolic abnormalities according to the Joint Interim Statement. Optimal cut-points from Receiver Operating Characteristic (ROC) curves were determined. Adjusted logistic regression was used to test the association between metabolic syndrome and the cut-points created. The cut-point identified for muscle strength was 1.07 kgf/kg (Youden index = 0.310; area under the curve (AUC)) = 0.693, 95% CI 0.614–0.764) for men and 0.73 kgf/kg (Youden index = 0.481; AUC = 0.768, 95% confidence interval (CI) = 0.709–0.821) for women (age group 25 to < 50 years). The best cut-points for men and women aged 50+ years were 0.99 kgf/kg (Youden index = 0.312; AUC = 0.651; 95% CI = 0.583–0.714) and 0.58 kgf/kg (Youden index = 0.378; AUC = 0.743; 95% CI = 0.696–0.786), respectively. Cut-points derived from ROC analysis have good discriminatory power for metabolic syndrome among adults aged 25 to <50 years but not for adults aged 50+ years. Novelty: First-line management recommendation for metabolic syndrome is lifestyle modification, including improvement of muscle strength. Cut-points for muscle strength levels according to sex and age range based on metabolic syndrome were created. Cut-points for muscle strength can assist in the identification of adults at risk for cardiometabolic disease.

Background: Diabetes mellitus is one of the important causes of morbidity and mortality. With a rise in non communicable diseases in India, diabetes has become a modern epidemic showing a rising trend in West Bengal also. A large number of diabetes patients come to the diabetes clinic of our tertiary care hospital in North Bengal Medical College and Hospital, Darjeeling, West Bengal. The large proportion of patients presenting with this condition prompted us to study the Sociodemographic and Anthropometric Prole of such patients. Methods: This was a descriptive hospital based cross sectional study involving a total of 344 diabetic patients enrolled in the diabetes clinic of a tertiary care teaching hospital from December 2019 to February 2020. A self-administered questionnaire was used to collect data and was analyzed in Microsoft excel sheet and using SPSS 20.0 software. Results: Among the 344 study participants, we found that 220 (63.95%) were males and 124 (36.05%) were females. Diabetes was more common among those aged 51-60 years 108 (31.40%). Family history was present among 188 (54.65%) of the diabetic patients. Almost 160 (46.51%) patients have diabetes 1.1 – 5 years. Hypertension was present among 216 (62.79%) patients. Almost 218 (63.37%) of the patients were overweight and 84 (24.42%) were obese. Conclusions: The diabetic patients presenting to this tertiary care hospital belong lower socioeconomic strata and having limited education in their age group. There is high proportion of obesity and hypertension among them.
