In the present era, Dharunaka is a common condition mostly affecting children to middle aged people due to improper lifestyle, lack of hygiene and stress. There is no specific etiology explained in Any Ayurvedic texts. Dharunaka can be compared with seborrhoeic dermatitis / Dandruff in contemporary science. Maintenance of proper hygiene is the most important thing in this disease. Otherwise the chances of recurrence are more. With Ayurvedic formulations we can give good result in this condition. A 30-year-old male patient with chief complaint of itching on the scalp, dryness on scalp and hairfall came to our hospital and herbal medicines like Aragwadadi Kashayam (orally), Triphala Choornam (external), Tankanabasma, Manjishtadi Kashayam, Prapoundarika Tailam were given and procedures like Shiroabhyanga with Durdoorapathradi oil and Nasya with Prapoundarikadi oil also given. This medication was found very effective in controlling signs and symptoms of Dharunaka and improving the overall condition of the scalp. The improvement provided by the treatment is assessed on the basis of sign and symptoms before and after the treatment.

Acromegaly is a devastating chronic slowly progressive disease. Its early diagnosis is a challenging issue that necessitates clinical suspicion of signs and symptoms as a first step. This report introduces an unusual early sign in the oral cavity that lead to the early diagnosis of an acromegaly case. A case of a healthy 40-year-old male patient presented with progressively growing multiple hard swellings in the upper and lower jaws. Clinical examination revealed bony hard multiple small spiky exostosis-like swellings, located at the maxillary and mandibular alveolar bones. An array of investigations revealed a 2-mm diameter pituitary tumour in MRI of sella. To the best of the author’s knowledge, this is the first report of spiky exostosis-like growths in the alveolar bone as an early sign of acromegaly. In this case, thorough examination of oral signs and symptoms was the first step for early diagnosis and hence, better prognosis for acromegaly.

Abstract Background Plaque Psoriasis is very embarrassing condition of skin as it produces itchy rashes and scaly lesions which may not respond well to the treatment. It can be correlated with Kitibhakushta which is one among the 18 types of Kushta (skin diseases). Several corticosteroids for skin applications are used now a day’s which gives only temporary relief. Case presentation A 36 years old moderate built male patient complains of blackish skin rashes with itching, pain and scaling all over the body for three months. Based on the symptoms, Dosha (Biological humors) involved were assessed as Kapha (binding factors) and Vata (vital force of life) and suggestive of Kitibhakushta. Diagnosis of the Plaque psoriasis was further confirmed by biopsy. Classical Kushta treatment was adopted here. As Poorvakarma (pre-operative procedures), Rukshana (Dehydrating therapy) was done and then Snehapana (intake of medicated ghee) was administered. Then classical Vamana (emetic therapy) and Virechana (purgative therapy) were performed. This took almost one month. After Samsarjana (special diet) patient was administered Manibhadragula as Shodhana Rasayana for a period of one month. Results At the end of Manibhadragula intake, remarkable changes were observed in all the symptoms. There were significant changes seen in PASI, DLQI and photographs taken before and after treatment. Conclusions Here the special mode of administration of Manibhadragula as mentioned in Ashtanga Hridaya helps for Shodhana and the Shodhana itself has Rasayana action. Drugs also have Rasayana property. This made the authors to propose a new concept called Shodhana Rasayana. Classical treatment by considering the condition of Roga (disease) and Rogi (patient) helps for management of Kushta.

Pleomorphic sarcoma of the larynx is a rare variant of laryngeal cancer. We present the case of a 59-year-old male patient who has been smoking for 40 years. He presented with signs and symptoms of an obstructive glottic mass. The diagnostic workup pointed to a malignant pathology; the histopathology report confirmed the diagnosis of glottic undifferentiated pleomorphic sarcoma (malignant fibrous histiocytoma).

Grahani as an organ is considered as the seat of Agni which is supported and nourished by Agni with Aadhara Aadheya Sambandha. This Agni is vitiated by Ama formed because of improper diet and regimen. Among them, Grahani dosha is the prime disease and is seen often in day-to-day practice. Material And Method: A 32-years- old male patient was coming to the OPD of Kaya Chiktsa,Quadra Institute of Ayurveda with complaints of defecation just after taking a meal, Bowelfrequency 4-5 times/day, irregular bowel habit, loss of appetite, gen- eralized weakness, occasional pain in the abdomen, slow weight gain. These complaints were persisting for the past1 year. This condition can be understood as Grahani Dosha, treatments including with somemedicine course of 2 months. Result: There were significant improvements in the condition of the patient. Discussion: while treat- ing Grahani, the drugs alleviating Vata, Pitta, Doshas, which are having Deepan, Pachana, Grahi, Stambhana as well possessing tikta, katu rasa and ushna veerya should be selected. there was a significant reduction in all the signs and symptoms ofthe disease are observed. Conclusion: A meticulous planning of treatment considering the Bala, and Agni of patient accompanied with proper diet will provide encouraging results in treating Grahani Dosha. Gridhrasi is one among Vatajananatmaja Vikara characterized by Sthabdhata, Ruk, Toda which radiates from buttock region, lumbar region, thigh, knee, calf muscles and legs. Gridhrasi is of two types viz Vataja and Vata Kaphaja Gridhrasi. Vataja Gridhrasi is characterized by severe pain and Vata Kaphaja Gridhrasi has symptoms viz Tandra, Gaurava and Aruchi. The signs and symptoms of Gridhrasi can be correlated to sciatica of modern medicine. Sciatica is characterized by constant aching pain felt in lumbar region which may radiate to the buttock, thigh, calf and foot and pain is experienced along the sciatic nerve pathway. It is a common condition with lifetime incidence varying from 13% to 40%. Matra Basti is indicated in dis-eases caused by vitiated Vata Doshas and thus considered as one of the treatments for Gridhrasi. It was a comparative clinical study with a pre and post design in 40 patients who were diagnosed with Gridhrasi, were assigned into 2 groups of 20 patients each randomly. After examination MatraBasti had given for group A with Vyoshadi Taila and group B with Sahacharadi Taila after Sthanika Abhyanga and Nad-isweda with respective oil for 7 days. The assessment criteria were noted before and after treatment and on followup. Among the subjective and objective parameters, Group A showed better reduction 51.47% in Ruk, Sthambha, Toda, Gaurava, active and passive SLR test, Bragard’s test and Lumbar movements. Group B does not have statistically significant over Gaurava, left lateral flexion and rotation to leave. Showed reduction 40.49% in Ruk, Sthambha, Toda active and passive SLR test and lumbar movement. Hence it can be concluded that MatraBasti with Vyosadi Tailam is having more effect on symptoms of Gridhrasi and shows long lasting result.

Introduction: Dental implants gained popularity as the treatment to replace grossly decayed teeth. However, with the increasing evidence and the difficulty to manage complications associated with dental implants, clinicians are pushed to opt for a more conservative approach. Case report: A male patient with a chief complaint of frequent food impaction in a severely decayed mandibular molar wished to retain his tooth. Upon thorough examination, the carious lesion extended to furcal area that rendered the distal root unsalvageable. However the mesial root can be retained; thus, hemisection was proposed. Management: Root canal treatment was carried out in the mesial canals. Then, the tooth was split mesio-distally.The distal root was extracted, and the mesial root was retained. Subsequently, the tooth was restored with PFM crown.Conclusion: Hemisection with subsequent prosthetic rehabilitation can be a viable alternative to retain severely decayed mandibular molar.

Takayasu arteritis (TA) is a type of primary systemic vasculitis mainly affecting the medium and large arteries. The signs and symptoms are due to systemic inflammation or ischemia of an organ or limb and include angiodynia, claudication, peripheral pulselessness, murmurs, ischemic stroke, myocardial infarction and severe systemic arterial hypertension. The disease tends to affect more women than men. Here we present a case of TA in a 22-year-old male patient. Our patient presented with complaints of aphasia and right-sided weakness, with on-and-off symptoms of malaise, generalized weakness, unilateral headache, fatigue and shortness of breath lasting two years. Color Doppler ultrasound was sufficient for a diagnosis of TA, after which we started the patient on medical treatment and also consulted the department of vascular surgery. Overall, this case report highlights the importance of screening for TA in male patients so that the diagnosis is not overlooked, and also adds more data to the limited literature on male patients.

42 years old male patient referred from neurology department, working as a teacher, educated up to MSc, premorbidly anxious personality, family history mental illness (first degree relatives), comes from MSES with presenting complaints of difficulty in writing or copying since seven years. Disability progressed and he was unable to write even a few words legibly and could not hold object which leads to anxiety and dependency. When the patient was examined at Neurology OPD, find out that he has normal sensory and motor nerve functions. The present treatment involved the use of Bahaviour therapy. The findings in this case is very encouraging and studies with large sample sizes can be considered for further conclusive evidence on the treatment of writer’s cramp.
