In this article, we discuss how moral hazard affects health care insurance, the social benefit and the harmfulness of moral hazard in health care insurance. We also discuss the relationship between copayment rate and decease risks and indicate that it is necessary to reduce copayment ratio for serious decease in social health care insurance in order to generate social gain. Finally, we carry out sensitivity analysis to illustrate the effect of the change of important parameters on optimal copayment rate.

Abstract This chapter focuses on the link between migration and social protection in Ireland. The chapter has two main goals. First, it presents the general legal framework regulating the social protection system in Ireland, paying particular attention to any potential differences in terms of conditions of access to social benefits between national residents, non-national residents, and non-resident nationals. Secondly, the chapter discusses how these different groups of individuals access social benefits across five policy areas: unemployment, health care, family benefits, pensions, and guaranteed minimum resources. The chapter concludes with a discussion of the relationship between migration and social protection policy.

Health insurance according to the Law No. 3/1992 obout the social employment guarantee which consist of responsiver and the addressee and health service provider is the health social insurance which is aimed to give the health care guarantee toward the addressee that is enterprenaur and worker.  The law relationship which is consisted between: Responssiver-Addressee is the insurance relationship; Responssiver-Health services provider is the user of health service belong to the health service provider toward to the addressee; Health services provider-Addressee is gives the health service to the addresse is patient.  The responsibility of the responssiver toward the addressee is to give the health care insurance to the addressee gives the health service provider for the importance of the addressee. Kata Kunci:  Asuransi Kesehatan

Between 1950 and 1980, the United States developed a welfare state that in many ways was comparable to those of other advanced industrial nations. Building on its New Deal roots, the Social Security system came to provide a “social wage” to older Americans, people with disability, and the dependents of deceased workers. It created a health-care insurance system for the elderly, the disabled, and the poor. Using the tax system in innovative ways, the government encouraged the expansion of pension and health-care protection for a majority of workers and their families. By 1980, some Americans could argue that their identification as a “laggard” in the field of social provision was no longer justified.

Kendari city had serious issues related to emerging a hundred of fast food sellers in 2017, including in Mokoau Health Centre area. However, there were only two health environmental staffs in that health care centre. Therefore, involving cadres in the fast food control become necessary. This study aimed to analyse the relationship between cadre’s capacity and Assessing to the food seller’s performance in food hygiene and sanitation management in Mokoau Health Centre in Kendari City since March 2019. There were 15 active cadres who controlled 35 fast food sellers. This study used the One-Group Pretest- Posttest Design. Data analysis used Paired T-test, Mann-Whitney test and Chi-Square test. This study found that 93.3% cadres had a senior high school diploma and only 6.7% of them had a Bachelor degree. There were 60.6% active cadres who had been in social health work for more than 5 years. The majority of cadres have got training about less than 15 times (86.7%). Involving cadres in the fast food control affected to seller’s performance change in fast food hygiene and sanitation management (P=0.006). Cadre with bachelor degree (P=0.006), were active in the social health work for less than 5 years (P=0.003), and got training less than 15 times (Pvalue= 0.026) influenced significantly to cadre’s assessing for the seller’s performance in food hygiene and sanitation management. This study suggested that improving cadre’s or community volunteer’s knowledge and skill related to fast food control technique becomes necessarily in fast food control.

Globally, the World Health Organization has championed the introduction of various forms of health insurance as a means of improving the utilization of quality healthcare service which is targeted at achieving universal health coverage. Unfortunately, the operations of the Social Health Insurance Programme (SHIP) have witnessed inefficiencies in the demand for healthcare services as a result of moral hazard and this is evidenced by the non-achievement of set-out objective of the programme in Nigeria. Design/Methodology: The study adopted a qualitative approach which comprised of 3 focus group discussions (N=24). Key informants were purposely selected for the focus group discussion from three (3) purposively selected hospitals, one from each categorized type of facility. Content analysis was adopted and further analysis was achieved with the aid of Nvivo 11 software, which coded and categorized nodes into themes. Results: The focus group participants relayed their experiences in the programme which includes poor identification system, choice of only one provider except on emergency, benefit package not being comprehensive which has enabled moral hazard and suggested ways to improve it. Conclusions: Most of the enrollees do not show diligence in areas of demand for services and these were shown to emanate principally from economic issues around the enrollees desire to utilize the premium paid thereby leading to moral abuse. Therefore, certain measures need to be put in place so as to curtail observed market failures in the SHIP programme.
