AbstractDiabetes mellitus is a chronic disease characterized by an increase in blood sugar exceeding 140 mg/dL. The purpose of this case study is to describe a foot care intervention with type 2 diabetes mellitus to improve blood circulation. The design of this scientific paper uses a case study method with the subject of two clients who have type 2 diabetes mellitus with blood sugar above 140 mg/dL in Sidorejo Comal Village. The intervention provided was daily foot care, and it was carried out for four days. Evaluation of this intervention has been proven to reduce blood sugar and increase blood circulation which can be measured by glucometer and Ankle Brachial Index (ABI). The results of this study showed a decrease in blood sugar and an increase in ABI in both clients, client 1 experienced a decrease in blood sugar from 256 mg/dL to 197 mg/dL, the ABI value from 0.81 increased to 0.90 and the second client experienced a decrease in blood sugar. from 318 mg/dL to 195 mg/dL, the ABI value from 0.77 increased to 0.92. The conclusion of this case study shows that foot care can reduce blood sugar and increase ABI in patients with type 2 diabetes mellitus. Suggestions for health workers are expected to provide foot care interventions to reduce blood sugar and increase ABI, in the form of foot care interventions in people with diabetes mellitus. type 2.Keywords: Ankle brachial index, Blood circulation, Diabetes mellitus, Foot care AbstrakDiabetes melitus merupakan penyakit kronis yang ditandai dengan kenaikan gula darah melebihi 140 mg/dL. Tujuan studi kasus ini adalah untuk menggambarkan tindakan perawatan kaki dengan diabetes melitus tipe 2 untuk meningkatkan sirkulasi darah. Rancangan karya tulis ilmiah ini menggunakan metode studi kasus dengan subyek dua klien yang mengalami diabetes melitus tipe 2 dengan gula darah diatas 140 mg/dL di Desa Sidorejo Comal. Intervensi yang dilakukan adalahmelakukan perawatan kaki setiap hari dan dilakukan selama empat hari Evaluasi dari tindakan tersebut terbukti dapat menurunkan gula darah dan meningkatkan sirkulasi darah yang dapat diukur dengan glukometer dan Ankle Brachial Index (ABI). Hasil studi ini menunjukan adanya penurunan gula darah dan peningkatkan ABI pada kedua klien,klien 1 mengalami penurunan gula darah dari 256 mg/dL menjadi 197 mg/dL, nilai ABI dari 0,81 meningkat menjadi 0,90 dan klien kedua mengalami penurunan gula darah dari 318 mg/dL menjadi 195 mg/dL, nilai ABI dari 0,77 meningkat menjadi 0,92. Simpulan studi kasus ini menunjukan bahwa perawatan kaki mampu menurunkan gula darah dan meningkatkan ABI pada penderita diabetes melitus tipe 2. Saran bagi tenaga kesehatan diharapkan dapat memberikan tindakan untuk menurunkan gula darah dan meningkatkan ABI berupa tindakan perawatan kaki pada penderita diabetes melitus tipe 2.Kata kunci: Ankle brachial index, diabetes melitus,perawatan kaki, sirkulasi darah

Peripheral arterial disease (PAD) merupakan salah satu komplikasi makrovaskular diabetes melitus tipe 2 (DMT2) yang dikaitkan dengan peningkatan risiko mortalitas kardiovaskular. Pemeriksaan ankle-brachial index (ABI) merupakan salah satu pemeriksaan yang sederhana dan mudah dilakukan untuk menegakkan diagnosis PAD. Oleh karena itu, penulis ingin mengetahui tingkat mortalitas kardiovaskular pasien DMT2 dengan PAD. Dari hasil penelusuran literatur, didapatkan tujuh literatur. Studi Bundo dkk melaporkan hasil HR 2,45 (interval kepercayaan [IK] 95%: 0,84-7,17). Studi Mostaza dkk melaporkan hasil HR 1,64 (IK 95%: 0,64-4,49). Studi Aboyans dkk melaporkan hasil HR 2,21 (IK 95%: 1,16-4,22). Studi Mohammedi dkk melaporkan hasil HR 1,35 (IK 95%: 1,15-1,60). Studi Quiles dkk melaporkan hasil HR 6,61 (IK 95%: 2,47-17,72). Studi Mueller dkk melaporkan hasil  RR 3,53 (IK 95%: 1,80-6,91). Studi lanjutan Mueller dkk melaporkan hasil  RR 4,06 (IK 95%: 2,67-6,18). Ankle-brachial index (ABI) is one of the simple and widely available tool to diagnose PAD. The authors aim to find out the cardiovascular mortality in T2DM patient with PAD. Moreover, an ankle-brachial index can be used as an independent stratification tool to predict the risk of cardiovascular mortality

AbstractDiabetes Mellitus is a group of metaboloc disoreders with sysptoms of increased blood sugar levels(hyperglycaemia) which results in damages to insulin secretion, insulin secretion,insulin activity, or both wrong behaviour ina diabetic diet will make blood sugar levels unstable and will have an impact in patients health problems. This study aimed to describe compliance in patients with type 2 diabetes mellitus through a literature review. The method used in the literature review wa to search for articles from 2011-2021 through NCBI searchles and the Garuda Portal. The searching results were obtained in the from of full text and pdf,rhen reviewed by using the strobe instrument,extracted then discussed, and concluded. The results of a literature review of 5 articles showed that there was an effect of dietary compliancr in patients with diabetes mellitus with results. Dietary adherence of diabetes patients was 324 respondents (58%) non-adherence and 235 respondents (42%) obedient in dientary compliance in type 2 diabetes mellitus patients. It can be concluded thet non-adherence diabetic patients can experience unstable blood sugar levels. Nurses should be able to increase the understanding of the importance of dietary compliance for patients with diabetes mellitus.Keywords: Compliance, Diet, Diabetes Mellitus. AbstrakDiabetes melitus merupakan sekumpulan hambatan metabolik dengan gejala peningkatan kadar gula darah (hiperglikemia) yang berdampak kerusakan sekresi insulin,aktivitas insulin, atau keduanya. Perilaku yang salah dalam diet diabetes akan menjadikan kadar gula darah tidak stabil dan akan berdampak pada gangguan kesehatan pada pasien. Penelitian ini bertujuan untuk mengetahui pengaruh kepatuhan diet pada penderita diabetes melitus tipe 2 melalui literatur review. Metode yang digunakan dalam literature review adalah mencari artikel tahun 2011-2021 melalui penelusuran Ncbi dan Portal Garuda. Hasil pencarian yang didapatkan berupa fulltext dan pdf, kemudian direview dengan menggunakan Intrumen strobe, diekstraksi kemudian dibahas dan disimpulkan. Hasil penelitian literature review dari 5 artikel menunjukkan bahwa adanya pengaruh terhadap kepatuhan diet pada pasien diabetes mellitus dengan hasil Kepatuhan diet pasien diabetes 324 responden (58%) tidak patuh dan 235 responden (42%) patuh dalam Kepatuhan Diet Pada Penderita Diabetes Mellitus Tipe 2. Pada penelitian ini didapatkan hasil penderita diabetes tidak patuh menjalani diet sehingga kadar gula darah dalam tubuh tidak stabil. Bagi perawat hendaknya dapat meningkatkan pemahaman tentang kepatuhan guna dapat memperbaiki diet bagi pasien Diabetes mellitus.Kata kunci: Kepatuhan , Diet, dan Diabetes Mellitus.

 Background: Diabetes Mellitus is one of the degenerative diseases which is the highest morbidity by increasing 30,2 % from 2012 to 2013 in Surabaya. Low physical activity is suspected to be one factor of uncontrolled fasting blood sugar level of Diabetes Melitus patients. Objectives: Based on the problem, this study was conducted to analyze the relationship of physical activity with fasting blood sugar level of type 2 Diabetes Mellitus patient in MulyorejoPublic Health Center Surabaya. Methods: This research used cross sectional design with a sample of 62 people. Data were collected using structured interview guide and IPAQ. Blood sampling using spectrophotometer method performed by medical analyst of laboratory of Mulyorejo Public Health Center. Analysis used was Spearman's Rho statistical test. Results: The results showed as many as 62.9% of respondents had low physical activity and as much as 58.0% of respondents had fasting blood sugar levels in the high category. The results showed there was a correlation between physical activity with fasting blood sugar level of type 2 Diabetes Mellitus patient. Conclusion: Theconclusion of this research there was relationship between physical activity with fasting blood glucose level of type 2 Diabetes Mellitus patient. We recommend that people with type 2 Diabetes Mellitus apply good physical activity such as cycling  or walking routinely 3-4 days a week for 20 minutes each day and reduce sitting activity so that fasting blood sugar levels can be controlled.ABSTRAK Latar belakang: Diabetes Mellitus merupakan salah satu penyakit degeneratif yang masih tinggi di kota Surabaya dengan peningkatan kasus dari tahun 2012 ke tahun 2013 sebesar 30,2 %. ABSTRACT            Diabetes Mellitus is one of the most popular degenerative disease in the medical worlds. Every years, the incidence rate of  this disease always shows an increase. In 2010 reported that 1,9  million of 35 years old or more diabetics also suffering the stroke complications. In addition there is an increase of 1.2% every month in people with diabetes mellitus with the incidence of ischemic stroke, so there is a need for preventive action by recognizing risk factors that can be changed. The purpose of this research is to determine the relations between fasting blood sugar levels and lipid profiles in patients with type 2 diabetes mellitus and ischemic stroke in RSUD R.A Basoeni Mojokerto from January to March 2020. This research using observational method. Research involving 34 sample of 40 years old and above patients with type 2 diabetes mellitus and ischemic stroke. Thereseach of the relations between fasting blood sugar levels and lipid profiles from the total respondents (34 patients) shows as follows, The average of the cholesterol total level is 189 mg/dl, and 35% from them (12 respondents) have >200 mg/dl of the cholesterol total level. The average of the cholesterol HDL level is 36 mg/dl, and 65% from them (22 respondents) have <40 mg/dl of the HDL cholesterol level. The average of the LDL cholesterol level is 112 mg/dl, and 24% from them (8 respondents) have >150 mg/dl of the LDL cholesterol level. The average of the triglyceride level is 211 mg/dl, and 47% from them (16 respondents) have >200 mg/dl of the triglyceride level. This shows that there is no correlation between fasting blood sugar level with the cholesterol total level, the cholesterol HDL level, and cholesterol LDL level. Tingkat insidensi dari tahun ketahun menunjukan peningkatan. Pada tahun 2010 jumlah penderita diabetes melitus yang berusia 35 tahun atau lebih dilaporkan 1,9 juta mengalami komplikasi stroke Selain itu terdapat kenaikan 1,2% setiap bulannya pada penderita diabetes melitus dengan kejadian stroke iskemik sehingga perlu adanya tindakan preventif dengan mengenal faktor-faktor risiko yang dapat diubah. Tujuan penelitian ini untuk mengetahui hubungan antara kadar gula darah puasa dan profil lipid pada penderita diabetes melitus tipe 2 dengan kejadian stroke iskemik di RSUD R.A Basoeni Mojokerto pada bulan Januari sampai Maret 2020. Penelitian ini menggunakan metode observasional. Pada penelitian ini diperoleh 34 sampel penderita diabetes melitus tipe 2 dengan kejadian stroke iskemik yang merupakan pasien awal dengan kriteria usia diatas 40 tahun dan penderita stroke iskemik yang telah dinyatakan positif stroke.Hasil pemeriksaan terhadap hubungan kadar gula darah puasa dengan profil lipid menunjukan rerata kadar total kolesterol sebesar 189 mg/dl dengan kadar kolesterol >200 mg/dl sebanyak 12 responden (35%). Rerata kadar HDL kolesterol sebesar 36 mg/dl, dengan kadar kolesterol HDL <40 mg/dl sebanyak 22 responden (65%). Rerata kadar LDL kolesterol sebesar 112 mg/dl dengan kadar kolesterol LDL >150 mg/dl sebanyak 8 responden (24%). Rerata kadar trigliserida sebesar 211 mg/dl dengan kadar trigliserida >200 mg/dl sebanyak 16 responden (47%). Hal ini menunjukan bahwa tidak ada korelasi antara kadar gula darah puasa dengan kadar total kolesterol, HDL kolesterol, LDL kolesterol dan ada korelasi atau hubungan antara kadar gula darah puasa dengan kadar trigliserida. Kata kunci : Diabetes Melitus tipe 2, Stroke Iskemik (CVA Infark), Gula Darah Puasa, Profil Lipid

Diabetes melitus (DM) tipe 2 adalah gangguan metabolik karena tubuh mampu menghasilkan insulin, namun tidak dalam kadar normal atau tidak dapat berespons terhadap efek insulin (resistensi insulin) sehingga menyebabkan hiperglikemia. Jika kadar gula darah tidak dikontrol dengan baik maka akan dapat menyebabkan komplikasi kronis berupa mikrovaskular seperti penyakit mata, neuropati dan nefropati, serta makrovaskular seperti penyakit arteri koroner, arteri perifer, dan serebrovaskular. Pada tahun 2013, riset menunjukkan prevalensi penderita DM di Indonesia tinggi, terutama di Provinsi Sulawesi Tengah. Tujuan mengetahui komplikasi makrovaskular pada penderita diabetes melitus tipe 2 dengan kadar gula darah tidak terkontrol. Metode penelitian: Scoping review dengan mengidentifikasi, menganalisis, dan mengevaluasi suatu karya tulis ilmiah yang diterbitkan dalam rentang waktu 10 tahun terakhir yang dilakukan selama September—Desember 2020. Pencarian sistematis dilakukan melalui situs database yaitu Pubmed, Springer Link, dan Proquest. Sesuai dengan kriteria inklusi dan tidak termasuk eksklusi serta dilakukan skrining menggunakan kriteria kelayakan pada PRISMA dan disajikan dalam bentuk tabel hasil penelitian berisi uraian artikel penelitian yang telah di-review (9 artikel). Simpulan: Sembilan artikel menyatakan terdapat pengaruh kadar gula darah tidak terkontrol terhadap kemunculan komplikasi makrovaskular berupa penyakit arteri koroner, arteri perifer dan serebrovaskular pada pasien diabetes melitus tipe 2. Kadar gula darah tidak terkontrol berkaitan dengan pembentukan plak aterosklerosis serta gangguan fungsi endotel sehingga pasien dengan miokardiak infark, gangren, iskemik strok merupakan presentasi yang umum pada pasien dengan kadar gula darah tidak terkontrol. The Effect of Uncontrolled Blood Sugar Levels on Macrovascular Complications in Type 2 Diabetes Mellitus Patients: Scoping ReviewDiabetes mellitus (DM) type 2 is a metabolic disorder in which the body is able to produce insulin but not in normal levels or cannot respond to the effects of insulin (insulin resistance), causing hyperglycemia. If blood sugar levels are not properly controlled, it can lead to chronic complications in the form of microvascular diseases such as eye diseases, neuropathy and nephropathy and macrovascular diseases such as coronary artery disease, peripheral arteries and cerebrovascular. In 2013, research showed the high prevalence of DM sufferers in Indonesia, especially in the province of Central Sulawesi. Objective: To see the emergence of macrovascular complications in type 2 diabetes mellitus patietns with uncontrolled blood sugar levels. Research method: Scoping review by identifying, analyzing and evaluating a scientific paper published within the last 10 years, conducted in September—December 2020. Systematic searches were carried out through database sites, i.e Pubmed, Springer Link and Proquest. In accordance with inclusion and exclusion criteria, and screening was carried out using the PRISMA eligibility criteria and presented in the form of a research table containing a reviewed descriptions of research articles (9 articles). Conclusion: Nine articles stated that there was an effect of uncontrolled blood sugar levels on the emergence of macrovascular complications in the form of coronary artery disease, peripheral arteries and cerebrovascular disease in patients with type 2 diabetes mellitus. Uncontrolled blood sugar levels were associated with atherosclerotic plaque formation and impaired endothelial function. Myocardiac infarction, gangrene, ischemic stroke are common presentations in patients with uncontrolled blood sugar levels.

AbstractDiabetes mellitus is a condition of increasing blood sugar levels that exceed 140 mg/dl. The purpose of this case study is to describe the application of giving diabetic foot exercises to reduce blood sugar levels in the body in patients with type 2 diabetes mellitus. The design of this scientific paper uses a case study method with 2 clients suffering from type 2 diabetes mellitus with blood sugar above 140 mg. /dl in Poncol Village and in Karangsari Village, Batang Regency. The intervention carried out was giving diabetic foot exercises for 4 days and once a day. Evaluation was carried out to determine the decrease in blood glucose using a glucometer. The results of the study showed a decrease in blood sugar in both clients, for client 1 there was a decrease in blood sugar from 220mg/dl to 135mg/dl and for client 2 there was a decrease in blood sugar from 250mg/dl to 137mg/dl. The conclusion of this case study shows that diabetic foot exercises can reduce blood sugar levels in patients with type 2 diabetes mellitus. Tujuan dari studi kasus ini adalah menggambarkan penerapan pemberian senam kaki diabetik untuk menurunkan kadar gula darah dalam tubuh pada penderita diabetes mellitus tipe 2. Rancangan karya tulis ilmiah ini menggunakan metode studi kasus dengan subjek 2 klien yang menderita diabetes mellitus tipe 2 dengan gula darah diatas 140mg/dl di Desa Poncol dan di Desa Karangsari Kabupaten Batang. Intervensi yang dilakukan adalah pemberian tindakan senam kaki diabetik selama 4 hari dan dilakukan satu kali sehari. Evaluasi dilakukan untuk mengetahui penurunan glukosa darah menggunakan alat ukur glukometer Hasil studi menunjukan adanya penurunan gula darah dalam tubuh pada kedua klien, untuk klien 1 mengalami penurunan gula darah dari 220mg/dl menjadi 135mg/dl dan untuk klien 2 mengalamai penurunan gula darah dari 250mg/dl menjadi 137mg/dl. Simpulan studi kasus ini menunjukan bahwa senam kaki diabetik mampu menurunkan kadar gula darah pada penderita diabetes mellitus tipe 2. Saran bagi tenaga kesehatan diharapkan mengajarkan senam kaki diabetik untuk menurunkan kadar gula darah pada penderita diabetes mellitus tipe 2. This study aims to gain an in-depth understanding of the meaning and significance of the experience of type 2 diabetes mellitus patients in taking preventive measures for injury at PERSADIA Islamic Hospital Jakarta Pondok Kopi. This study uses a qualitative design and data collection is done by in-depth interview techniques. The results obtained were four) themes produced, namely the picture of maintaining diet, diverse blood sugar control activities, the challenges of therapeutic adherence and the ability to properly care for the feet. Conclusion, the experience of type 2 diabetes mellitus patients in preventing injuries, should adhere to the treatment and maintain the food according to diet so that blood sugar levels remain stable. This study aimed to determine the effectiveness of diabetic foot exercises on the sensitivity of the feet of Type 2 Diabetes Mellitus patients. The method used in this study was a descriptive research method with a case study approach. The results showed when measuring the feet' sensitivity after doing leg exercises 2x / day for three days, and there was a change. The change in sensitivity value increases 1-2 points from the maximum point 3. Conclusions The implementation of foot exercises can increase the sensitivity of the foot in type 2 diabetes mellitus patients.   Keywords: Diabetes Mellitus Type 2, Foot Gymnastics, Foot Sensitivity

Background: Serum ferritin is known as an index for body iron stores also as an inflammatory marker and it is influenced by several disease. We were looking for a correlation between HbA1c and S. Ferritin in type 2 DM. Methodology: The present study a total of 150 participants were enrolled of which 100 were confirmed cases of Type 2 Diabetes Mellitus and rest 50 age and sex matched healthy subjects constituted the control group. All were screened for HbA1c, Fasting blood sugar, Post prandial blood sugar and S.Ferritin. Results: A highly significant variation and positive correlation was observed with respect to S.Ferritin and HbA1c levels. Mean S.Ferritin was high in the subgroup with poor glycemic control. Conclusion: The fasting, post prandial sugar levels, HbA1c and S.Ferritin were significantly higher in the diabetic subjects. This study shows a positive correlation between HbA1c and S. Ferritin levels. So we can conclude that in diabetic patients S. Ferritin may serve as an independent marker of poor glycemic and metabolic control.
