Resumen. Se realiza un estudio transversal en estudiantes de medicina de 5to ciclo de Medicina Humana en la Universidad Andina del Cusco, estudiando el perfil psicológico. Se aplican, previo consentimiento informado, los test de Gaston Berger, Impulsividad de Barratt, Agresividad de Buss y Perry, Habilidades Sociales de la OMS y SQR-18 del MINSA y se realiza un análisis de correlaciones entre las distintas variables con el sexo. Los resultados obtenidos más relevantes son que hay un nivel promedio de impulsividad y agresividad. Nivel Promedio-alto y alto en habilidades sociales. Nivel de ansiedad - depresión de 22%; 54.4% de síntomas psicóticos, 10.3% de problemas graves con el consumo de alcohol. Hay escasos niveles de correlación entre las variables. Se recomienda hacer estudios con mayor población y análisis factorial e implementar medidas de soporte emocional y consejería para los estudiantes. Palabras clave:  impulsividad, agresividad, habilidades sociales, síntomas psicológicos Abstract.  A transversal study is carried out in medical students of the 5th cycle of Human Medicine at the Universidad Andina del Cusco, studying the psychological profile. The Gaston Berger, Barratt Impulsivity, Buss and Perry Aggression, WHO Social Skills and SQR-18 MINSA tests are applied with prior informed consent and a correlation analysis between the different variables with sex is performed. The most relevant results obtained are that there is an average level of impulsivity and aggressiveness. Average-high and high in social skills. Level of anxiety - depression of 22%; 54.4% of psychotic symptoms, 10.3% of serious problems with alcohol consumption. There are few levels of correlation between the variables. It is recommended to conduct studies with a larger population and factor analysis and implement measures of emotional support and counseling for students. This study aimed to clarify the role that career resilience plays in preventing inhibition of career development when individuals confront changes during their working life, such as changes in work tasks or health condition. Career resilience consists of five factors: ability to cope with problems, social skills, interest in novelty, optimism about the future, and willingness to help others. In all, 1,000 Japanese company employees completed an online survey. The results showed that optimism about the future and ability to cope with problems exhibited a negative correlation with NPC when confronting changes. The results of simple slope analysis suggested that social skills and ability to cope with problems decreased the negative influence that psychological symptoms caused by changes had on job satisfaction, which was one index of career development. This study underlines the necessity of developing the ability to cope with problems and social skills.

Parkinson's disease (PD) is traditionally regarded as a movement disorder. In recent years, however, non-motor symptoms have been considered significant factors of disability at all stages of the illness. Behavioral and psychological symptoms or neuropsychiatric syndromes associated with PD are frequent and may represent a challenge in the management of these patients. They include anxiety, depression, psychosis, sleep, sexual and impulse control disorders, apathy and cognitive dysfunction. Their pathogenesis in PD is complex, involving neurodegenerative, drug-related and psychological mechanisms. We will review the current knowledge of this growing field, also focusing on the management of theses syndromes.

Evidence of diagnostic validity of the DetectaWeb-Distress ScaleAbstract: The DetectaWeb-Distress Scale is a web-based scale for screening and detecting internalizing disorder symptoms (anxiety, depression, posttraumatic stress, obsessions and compulsions, and suicidality) in children and adolescents. Previous studies examined the validity and reliability of the scale, however, there are no data on its diagnostic validity. The objective of this study was to examine the evidence of diagnostic validity of the DetectaWeb-Distress scale in a sample of 244 children and adolescents from clinical (n = 51) and community (n = 193) settings, all of them with the diagnostic process completed. The results indicated that the DetectaWeb-Distress scale is a useful measure from a diagnostic point of view, as it discriminates between people with emotional disorders such as anxiety and depressive disorders and suicidality, and those without, presenting ROC values of approximately .80 and good sensitivity and specificity for detecting the main emotional disorders. The DetectaWeb-Distress Scale is a valid measure and diagnostically useful for detecting and identifying children and adolescents with anxiety disorders, depression and suicidality, with the advantage that it is a short measure, specifically developed for Internet use, especially relevant in the era of COVID-19.Keywords: DetectaWeb-Distress; assessment, anxiety, depression, children, adolescents.Evidencias de Validez Diagnóstica de la Escala Detectaweb-Malestar Resumen: La Escala DetectaWeb-Malestar es una escala de detección del malestar emocional a través de internet que evalúa síntomas de trastornos interiorizados (ansiedad, depresión, estrés postraumático, obsesiones y compulsiones y suicidalidad) en niños y adolescentes. Estudios anteriores han examinado la validez y fiabilidad de la escala, sin embargo, no cuenta con datos relativos a su validez diagnóstica. El objetivo de este estudio es examinar las evidencias de validez diagnóstica de la escala DetectaWeb-Malestar en una muestra de 244 niños y adolescentes procedentes de contextos clínico (n = 51) y comunitario (n = 193), todos con un proceso diagnóstico completado. Los resultados indicaron que la escala DetectaWeb-Malestar es una medida útil desde el punto de vista diagnóstico, ya que discrimina entre personas con trastornos de emocionales, como son los de ansiedad, depresión y suicidalidad y los que no lo padecen, presentando valores ROC en torno a .80 y adecuada sensibilidad y especificidad para detectar los principales trastornos emocionales. La escala DetectaWeb-Malestar es una medida válida y útil desde el punto de vista diagnóstico para detectar e identificar niños y adolescentes con problemas de ansiedad, depresión y suicidalidad, con la ventaja de que es una medida breve y desarrollada específicamente para su uso a través de Internet, especialmente relevante en la era de la COVID-19.Palabras clave: DetectaWeb-Malestar; evaluación; ansiedad; depresión; niños y adolescentes.

Introducción: La bioética en la práctica médica, es una disciplina que aporta fundamentación teórica para contribuir a la toma de decisiones considerando los principios de beneficencia, no maleficencia, autonomía y justicia. El presente estudio tiene como objetivo determinar el nivel de conocimientos teóricos, aplicados, y percepciones generales hacia la bioética en los estudiantes de Medicina de escuelas de América Latina. Materiales y métodos: Estudio observacional descriptivo de corte transversal desarrollado en el marco de la videoconferencia “Actualización en conocimiento bioéticos en Investigación en estudiantes de Medicina de Latinoamérica” a cargo del Comité de Educación Médica de la Federación Latinoamericana de Sociedades Científicas de Estudiantes de Medicina. Que incluye como herramienta de recolecciónun formulario de Google Docs auto aplicado vía on-line y aprobado por expertos. Resultados: El 82,7% de los estudiantes han sido contactados por la cátedra de bioética médica en su universidad, mayormente (66,67%) durante un semestre. Un 17,3% indicó que la cátedra de bioética no figura en la malla curricular. Los conocimientos teóricos en bioética fueron adecuados en el 59,48% de los estudiantes encuestados y los conocimientos aplicados fueron deficientes en el 62,07%. El análisis de los estudiantes que recibieron la materia versus los que no, no presentó una diferencia significativa. Conclusiones: La mayoría de los estudiantes encuestados reciben la cátedra de bioética. No se encontró diferencia entre impartir la cátedra o no hacerlo. La correcta aplicación de principios bioéticos a la práctica fue deficiente entre los estudiantes. Palabras clave: Bioética, Estudiantes de medicina, Universidades de América Latina.

ABSTRACTThe present study allowed describing the characteristics and identifying the aspects of permanence and migration after graduating of Medicine students from Federal University of Acre. Transversal study with a convenience sample of 89 graduates of the five already graduated classes until 2011. The data were collected through questionnaires submitted online, in the period of June to August 2012. The study results show that 71% of graduates are coming from other states of Brazil, 80% aged below 30 years, 61% male, 81.8% rated the medicine course of UFAC as good, 66.7 % reported being prepared for professional practice, with 64% approved on residency and / or internship specialty. The vast majority of doctors are residing and working in the capitals of Brazil (79%), working in the public sector (61.4%) satisfied with their chosen career (87.4%). About 58% stayed in Acre and pointed family (26%) the main cause of permanence. Of the 42% who migrated to another state, better job offers (29.7%) was the main cause of change. The present study demonstrates that in places like Amazon, the best working conditions and remuneration can be measures to be taken by the competent bodies for fixing physicians in areas with lack of professionals and should be prioritized.RESUMOO presente estudo permitiu descrever as características e identificar os aspectos relacionados à fixação ou migração dos egressos do curso de medicina da Universidade Federal do Acre após a graduação. Estudo transversal, com uma amostra de conveniência de 89 egressos das cinco turmas já graduadas até 2011. Os dados foram coletados por meio de questionários encaminhados via eletrônica, no período de Junho a Agosto de 2012. Os resultados do estudo demonstraram que 71% dos egressos são procedentes de outros estados do Brasil, 80% com idade abaixo de 30 anos, 61% do sexo masculino, 81,8% avaliaram como bom o curso de medicina da UFAC, 66,7% referiram estarem preparados para o exercício profissional, com 64% aprovados em curso de residência médica e/ou estágio de especialidade. A grande maioria dos médicos está residindo e trabalhando nas capitais do Brasil (79%), em setor público (61,4%), satisfeitos com carreira que escolheram (87,4%). Aproximadamente 58% ficaram no Acre e apontaram os laços familiares (26%) como causas principais de permanência. Dos 42% que migraram para outro estado, as melhores propostas de trabalho (29,7%) foram as principais causas de mudança. O presente estudo demonstra que a em locais como a Amazônia, as melhores condições de trabalho e remuneração, podem ser medidas a serem adotadas pelos órgãos competentes para a fixação de médicos em áreas com poucos profissionais e devem ser priorizadas.

Background A number of community based surveys have identified an increase in psychological symptoms and distress but there has been no examination of symptoms at the more severe end of the mental health spectrum. Aims We aimed to analyse numbers and types of psychiatric presentations to inform planning for future demand on mental health services in light of the COVID-19 pandemic. Method We analysed electronic data between January and April 2020 for 2534 patients referred to acute psychiatric services, and tested for differences in patient demographics, symptom severity and use of the Mental Health Act 1983 (MHA), before and after lockdown. We used interrupted time-series analyses to compare trends in emergency department and psychiatric presentations until December 2020. Results There were 22% fewer psychiatric presentations the first week and 48% fewer emergency department presentations in the first month after lockdown initiated. A higher proportion of patients were detained under the MHA (22.2 v. 16.1%) and Mental Capacity Act 2005 (2.2 v. 1.1%) (χ2(2) = 16.3, P < 0.0001), and they experienced a longer duration of symptoms before seeking help from mental health services (χ2(3) = 18.6, P < 0.0001). A higher proportion of patients presented with psychotic symptoms (23.3 v. 17.0%) or delirium (7.0 v. 3.6%), and fewer had self-harm behaviour (43.8 v. 52.0%, χ2(7) = 28.7, P < 0.0001). A higher proportion were admitted to psychiatric in-patient units (22.2 v. 18.3%) (χ2(6) = 42.8, P < 0.0001) after lockdown. Conclusions UK lockdown resulted in fewer psychiatric presentations, but those who presented were more likely to have severe symptoms, be detained under the MHA and be admitted to hospital. Psychiatric services should ensure provision of care for these patients as well as planning for those affected by future COVID-19 waves.

Abstract PurposeInsomnia symptoms are commonly experienced by men after prostate cancer (PCa) treatment. Here we explored how sleep hygiene behaviours and psychological symptoms are associated with insomnia symptoms in PCa patients.MethodsAn online survey was posted on social media and sent to mailing lists of PCa and general cancer organisations. The survey collected information on demographic, sleep hygiene and psychological symptoms using validated questionnaires. ResultsData from 142 participants were compared based on the absence (age = 68.3 ± 8.9 years) and presence (age = 66.6 ± 9.0 years) of insomnia symptoms. Participants with insomnia symptoms had significantly higher levels of anxiety, depression, fatigue, and daytime sleepiness as well as poorer sleep hygiene than those without insomnia symptoms. Control variables (age, comorbidities, ADT experience and BMI) accounted for 12.5% of the variance in insomnia symptoms. Adding sleepiness, fatigue, anxiety, depressive symptoms to the model explained an additional 45.1% of the variance in insomnia symptoms. Further, including the sleep hygiene item “I think, plan, or worry when I am in bed” and “I sleep in an uncomfortable bedroom” explained an additional 3.6% of the variance in insomnia symptoms. ConclusionsPoor sleep hygiene, fatigue, daytime sleepiness, anxiety, depressive symptoms were all associated with worse insomnia symptoms in PCa patients. Improving sleep hygiene and treating psychological conditions may potentially help prevent and/or alleviate insomnia symptoms in PCa patients.

Objetivos: Evaluar nuestra experiencia en la aplicación del algoritmo de la Fundación de Medicina Fetal (FMF) en el tamizaje prenatal de aneuploidías en el primer trimestre en una población no seleccionada. Diseño: Estudio descriptivo, retrospectivo, transversal. Institución:Servicio de Medicina Fetal, Clínica Santa Isabel, Lima, Perú. Participantes: Gestantes y sus fetos. Intervenciones: Entre el 1 de marzo de 2012 y el 24 de setiembre de 2012, en 324 pacientes con 11 a 13+6 semanas de edad gestacional y fetos con longitud corona nalga (LCC) de 45 a 84 mm, se logró medir la translucencia nucal (TN) mediante ecografía transabdominal. Principales medidas de resultados: Factibilidad de la aplicación del algoritmo de la FMF en el tamizaje de 11 a 13+6 semanas. Resultados: La curva observada de los valores de la TN en relación a la LCC se ajustó a la ecuación cuadrática descrita por la FMF, y la distribución de las medidas presentó una disposición normal, teniendo la mediana una desviación de 0,2 mm respecto al esperado. El porcentaje de valores por encima de la mediana fue 43,3%. Se observó una TN >percentil 95 en 4,6% de casos y TN ≥3,5 mm en 1,2%. Se encontró un valor de riesgo por encima del punto de corte de 1:100 en 10 casos para T21 y en un caso para T13. Los resultados del estudio citogenético en tres pacientes fueron un cariotipo femenino normal, una trisomía 18 y una trisomía extra-estructural (47XY+mar). Conclusiones: Nuestros resultados concuerdan con lo comunicado en la literatura y muestran que es factible la aplicación del algoritmo de la FMF en el tamizaje de 11 a 13+6 semanas en un entorno de práctica clínica, en nuestro medio.
