Objective: to assess the value of the psychoemotional state of pregnant women in the development of a prolonged second period of labor.Material and methods. 76 pregnant women were examined. In 26 women, childbirth was complicated by a prolonged second period of labor, and they constituted the main group. The control group included 50 women who had physiologic birth. The psychoemotional state was analyzed using the Spielberg test and the test of a pregnant woman's attitudes.Results. There were some differences between the groups in terms of the tendency in the levels of personal anxiety and occurrence of high situational anxiety. The Spielberg test has revealed more «positive» affirmative answers than «negative» ones. 46.1 % women in the main group do not «feel a sense of inner satisfaction», which is statistically significantly higher than in the control group - 24.0 % (p = 0.04). In the main group, in contrast to the control group, the women are more often not «worried about possible failures». During the study of the types of the psychological component of the gestational dominant, mixed types were mainly observed, and the distribution in the groups did not have statistically significant differences. The women with a prolonged second period of labor had a tendency to form the predominantly euphoric type of the psychological component of the gestational dominant.Conclusion. During pregnancy, the patients with a prolonged second period formed a coping strategy in the form of a predominantly euphoric attitude to pregnancy associated with elevated situational anxiety.

Aim. To study of the influence of complex psycho-preventive preparation for childbirth in primiparous patients on some features of their psycho-emotional status. Materials and methods. A cross-sectional prospective study of 286 nulliparous women in full-term pregnancy, aged 21 to 35 years, was carried out, among them 142 pregnant women (the main group) attended courses of complex psycho-preventive preparation for childbirth in the third trimester of pregnancy. The control group consisted of 144 primiparas who did not attend antenatal training courses. To assess the psychological status of pregnant women, the method for assessing the psychological component of the gestational dominant (PCGD) was used according to I. V. Dobryakov. Anxiety was assessed according to the scale of situational and personal anxiety Ch.D. Spielberger and Yu.L. Khanina. Results. It was revealed that attending birth-giving women 4.8 times more often leads to the formation of the optimal type of psychological component of the gestational dominant. The effects of attending antenatal training classes on the level of situational anxiety in pregnant women are manifested in the form of a significant decrease in its level – OR 0.28 (95% CI 0.15-0.54). Conclusion. Comprehensive antenatal preparation for childbirth in primiparous leads to an improvement in some emotional characteristics of pregnant women, including the formation of the optimal type of psychological component of the gestational dominant and a decrease in situational anxiety.

Introduction. The presence of somatic diseases during pregnancy leaves a serious imprint on the emotional state of a woman, significantly worsens the quality life indicators and affects the clinical characteristics of pregnancy. One of these diseases is chronic pyelonephritis. There are a fairly large number of recommendations for the treatment of this pathology. However the focus is mainly on drug therapy. At the same time the existing restrictions on the number of drugs using during pregnancy, the problem of polypragmasia, and the increase in the number of allergic complications of drugs using dictate the need to search alternative methods of treatment, primarily non-drug ones. Moreover, the problems of pregnant women life quality with chronic pyelonephritis during complex drug therapy are often not given due attention.The goal of research — to assess the impact of osteopathic correction on the psycho-emotional state and life quality of pregnant women with concomitant pathology of the urinary system.Materials and methods. A prospective controlled randomized study was conducted in the period 03.2016– 01.2018 on the basis of medical clinics of LLC «Mokhov Institute of osteopathy» and LLC «Vasileostrovskaya clinic of reproduction and genetics». There were observed work 48 pregnant women with chronic pyelonephritis aged 25–45 years, with a gestation period 13–27 weeks. During the processing of the clinical material, 8 patients were eliminated. As a result of the selection process, a group of 40 people was formed. All pregnant women with chronic kidney disease, depending on the used treatment method, were divided into two groups using a simple randomization method with a random number generator. There was formed the main group (20 people), and the control group (20 people). Patients of the main group received medication and osteopathic correction (3 procedures with an interval of 7–10 days). Patients in the control group received only traditional drug therapy. All pregnant women with chronic pyelonephritis had an osteopathic examination before and after treatment with forming an osteopathic conclusion, and the assessment of the pain syndrome severity by a visual analog scale (VAS), the general psycho-emotional state (the method of rapid assessment of health, activity and mood — HAM), and the level of life quality (Questionnaire Medical Outcomes Study — Short Form).Results. Pregnant women with chronic pyelonephritis were characterized by a weak pain syndrome, a psychoemotional state violations in the categories «well-being» and «activity», and a decrease in both the physical and mental components of life quality. After the complex treatment including the additional to traditional drug therapy osteopathic correction methods, the statistically significant decrease of the pain severity degree (p=0,001), increase of psychoemotional state rates in the category of «activity» (p=0,05) and the increase of physical and mental components of life quality (p=0,02) were observed in the main group compared with the control.Conclusion. Chronic pathology of the kidneys during pregnancy affects the emotional state of a woman, significantly worsens life quality indicators. The use of osteopathic correction as a part of the complex therapy of pregnant women with chronic pyelonephritis can improve the emotional state and life quality; reduce the pain severity, and so can be used to improve medical care for this population category.

The objective: to improve the treatment of placental dysfunction in pregnant women with oligohydramnios regarding their psychological condition. Patients and methods. Survey was conducted in 120 pregnant women with oligohydramnios the gestational age of 27-29 weeks (main group) and 30 physiologically healthy pregnant women (control group) on the basis of the city clinical hospital of Ivano-Frankivsk. Results. During the study of psychological features we have not yet determined significant differences in the level of personal anxiety with oligohydramnios as compared with pregnant women with physiological gestation. Also, was found the division of examined for low, medium, and high levels of this indicator between the two pregnant groups. In particular, approximately half of the individuals had an average level of personal anxiety – 14 (46.67±9.11%) and 57 healthy pregnant women (47.50±4.56%) with oligohydramnios, third – highest, respectively 11 (36.67±8.80%) and 39 (32.50±4.58%), the lowest level recorded in 5 (16.67±6.80 per cent) and 24 (20.00±3.65%) patients. A comprehensive approach to correction of the condition of water shortage has been applied in women of the main group, which included: psychological support for couples with sessions with a psychologist to control emotions. Establishing emotional contact with a woman, trust relationships, discussion of physical and emotional changes is a part of pregnancy. Conclusion. In the result of the researches, the approach proposed by us to the treatment of pregnancy with oligohydramnios, allowed to reduce the stress of compensatory mechanisms of the fetoplacental unit, which contributed to the reduction of anxiety in pregnant and have improved not only obstetric but also perinatal outcomes in women of the main group.                                   The issue of preserving the reproductive health of the nation in Ukraine is very acute. The psycho-emotional state of a woman is of particular importance for the prolongation of pregnancy and the normal course of labor. Violation of adaptive mechanisms which are aimed at restructuring the functional systems of the body during pregnancy and on the eve of childbirth, as well as the state of chronic stress contribute to the increase in the of obstetric and perinatal complications frequency and have an adverse effect on the course of pregnancy. The aim of the reaserch was to study the dynamics of psycho-emotional state features in pregnant women after assisted reproductive technologies (ART) application in order to improve the tactics of antenatal surveillance. To achieve this goal, 299 pregnant women were comprehensively examined in the dynamics of prospective observation, which were distributed as follows – the main group included 249 women whose pregnancy occurred as a result of the ART application. The control group consisted of 50 pregnant women with spontaneous pregnancy and its physiological course, which were registered for pregnancy at 6-8 weeks. The women of main group whose pregnancy occurred as a result of ART application were divided into three groups, depending on the factor that caused infertility. The first group included 94 women with tubal-peritoneal factor of infertility, the second group was formed by 87 women with endocrine factor of infertility, the third group included 68 women whose infertility was caused by the male factor. Pregnant women of the study groups by age, marital and social status, place of residence were representative, which allowed further to judge the differences caused by etiological factors of infertility. In order to determine the psycho-emotional state of pregnant women in the study groups in the screening mode, a clinical interview was conducted by filling out questionnaires that contained the constituent questions of the STAI tests in the modification of Yu. L. Khanin and the "Pregnant Attitude Test" by the method of I. V. Dobryakov. The obtained test results indicate that the average score of reactive anxiety in pregnant women of the main group exceeded the same indicator of the control group and was: 49 - pregnant women with tubal-peritoneal type of infertility, 56 - pregnant women with endocrine type of infertility, 44 - pregnant women with male factor of infertility, 24 - control group. According to the results of testing, a significant difference was revealed in the indicators of personal anxiety. The values of indicators of personal anxiety were: 51 - pregnant women with tubal-peritoneal type of infertility, 54 - pregnant women with endocrine type of infertility, 31  -pregnant women with male factor of infertility, 31 - control group. Low level of personal anxiety was observed in 26 (27,6%) pregnant women of I group, 29 (33,3%) pregnant women of II group, which is significantly less in comparison with the indicator of pregnant women of the control group – 33 (66,0%) and III group - 33 (48,5%) (p < 0,05). Moderate type of personal anxiety was observed in 46 (48.9%) pregnant women of group and 40 (45.9%) pregnant women of II group compared to the indicator of the control group – 11 (22, 0%) (p < 0,05). Moderate type of personal anxiety was determined in 25 (36,7%) cases in pregnant women of III group and did not differ significantly from the indicators of I, II and control groups (p > 0,05). A high level of personal anxiety was observed in 22 (23,5%) pregnant women of group I, 18 (20,8%) pregnant women of group II, which is significantly higher in comparison with the indicator of pregnant women of the control group – 6 (12,0%) (p < 0,05). Thus, the study of the psycho-emotional state of pregnant women after the application of ART deviations in the level of personal and reactive anxiety and pathological types of gestational dominant, which are inherent in pregnant women of the main group. Taking into account the above, we consider it appropriate to involve a psychologist to work with the examined pregnant women in order to correct the identified abnormalities.

Premature labour (PL) is one of the most actual and unsolved problem of obstetrics. There are many data in the li terature about the role of the disturbed ratio of pro- and antiinflammatory cytokines, neopterin in the implementation of PL. Many authors support the theory of the predominant fetus factor in the initiation of PL. The goal of our research is studying of the characteristics of the local immune status, neopterin and cortisol levels in various variants of preterm labour. Materials and methods. 77 patients were divided into 2 groups. The main group (52 pregnant women with PL) was also divided into 2 subgroups: the first (1A) subgroup consisted of 28 pregnant women with premature rupture of membranes (PROM) without regular labour contractions. Another subgroup (1B) was presented by 24 women with labour activity and unruptured amniotic sac (true PL). In the control group we included 25 women with term labour. The level of the expression of mRNA genes of a congenital immunity in a cervical canal by the test-system ImmunoQuantex (IL1B, IL10, IL18, TNF2, TLR4, GATA3, CD68, B2M), as well as serum levels of neopterin and cortisol. The results were analyzed by standard statistical methods. Results. There were no significant difference in the expression levels of most of the genes of innate immunity between the study groups (p > 0.05). The investigation of expression of TLR4, GATA3 genes in different types of PL revealed a significant decrease (p < 0.05) in patients with PROM (1A subgroup). In addition in the 1A subgroup the inflammation index was higher (Me = 99,5%, p < 0.01). There were no fundamental difference in the state of the local immune status between groups of true preterm and term labour. Analysis of neopterin content among the subjects showed a significant increase in the main group (PL) compared with the control group (p = 0.0064). The comparison of the neopterin level between different variants of PL had a higher index (p < 0.025). The concentration of cortisol in the study groups had no principle difference from the main (PL) group and control group (p > 0.05). The maximum cortisol level was found in the subgroup 1B (true PL), which is significantly higher than in the subgroup 1A (with PROM) and in the control group (p < 0.01). Conclusion. The difference in the state of local immune status in different variants of premature labour demonstrates different mechanisms of initiation of preterm labour. It is likely that the ratio of TLR4/GATA3 and index of inflammation determins the onset of preterm labour. The level of neopterin can be used as a marker of the onset of PL. The increase of cortisol determines the role of the fetus factor in the onset of labour.

One of the most important options of a favorable course of pregnancy, fetal development and physiological delivery is a balanced and preserving psycho-emotional condition of a woman during pregnancy. In order to study peculiarities of psycho-emotional condition of pregnant women-displaced people, dynamic prospective examination of 96 pregnant women in term of gestation more than 22 weeks (the main group) and 34 pregnant women from the local area (the control group) was conducted. According to STAI data, the mean score of the state anxiety in the main group was 1,6 times higher, personal anxiety — 1,4 times. According to results of the testing by I.V. Dobrjakov, the optimal PCGD type was characterized for 36 (37,5%) pregnant women-displaced people, 24 (25,0%) — hypogestognostic, 9 (9, 4%) — an euphoric, 18 (18,7%) — an anxious, 9 (9,4%) — a depressive. However, every second pregnant woman from the control group had optimal PCGD type – 20 (58,8%; p<0, 05), 12 (35,3%) — a hypogestognostic, 2 (5,9%) — an euphoric. The results of the testing by L. M. Rabovaluk confirmed, that almost all pregnant women of the control group had shown a low anxiety rate, at the same time, choosing the statements of the test, every fifth pregnant women of the main group (21–21,9%) obtained high diagnostic points, which give evidence of high anxious, depression presence. The study of indicators of anxiety, depression and violations of the attitude to pregnancy among pregnant women-displaced person will allow to individualize the approaches to antenatal observation, improve the quality indicators of a mother’s and newborn’s health.

Habitual miscarriage is a stressful condition and a significant factor of frustration for married couples. The psycho-emotional state of the pregnant woman affects the course of the subsequent pregnancy and its outcome. Correction of the psychological component of habitual miscarriage is a promising direction for preventing reproductive losses. The aim of the work is to study the psychological components of miscarriage and determine the ways of their correction. Materials and methods. The level of chronic stress was studied in pregnant women with symptoms of miscarriage and a history of habitual reproductive losses (main group) based on indicators of psychoemotional state, the condition of the autonomic nervous system, and the degree of activation of stress-implementing systems. The results were evaluated with the scale PSS, PSM-25 as adapted by N. Ye. Vodopianova, Spielberger–Khanin inventory and the scale by L. D. Malkova, Baevsky index, cortisol content in the blood and urine. The pregnant women with a physiological course of pregnancy and uncomplicated history were included in the control group. Based on the fact that the etiology of habitual miscarriage is multifactorial, and the psychological component is amenable to correction, stress-protective therapy was included in the treatment complex of habitual miscarriage. To assess the effectiveness of treatment, the main group was divided into subgroups: 1 – with psychotherapy, 2 – without psychotherapy. Results. A high level of personal perception of stress, anxiety and asthenia determine the psycho-emotional state of the main group pregnant women, among whom two dominant psycho types were identified: A – a high level of anxiety and low neuropsychic resistance; B – poise and good adaptive ability against the background of stress. Non-drug stress-protective therapy in the complex of miscarriage treatment contributes to: a decrease in psycho-emotional stress in pregnant women with psycho type A – 2.6 times, anxiety level – 1.7 times, asthenia level – 2.3 times; and also: a decrease in similar indicators in pregnant women with psycho type B – 1.6 times, 3.9 times and 2.5 times, respectively (P < 0.05); a decrease in the frequency of premature termination of pregnancy by more than 11 %. Conclusions. The psychological component of miscarriage is a high level of personal perception of stress, anxiety and asthenia. Stress-protective therapy in combination with pathogenetic therapy of miscarriage improves pregnancy outcomes for the mother and the fetus. It is advisable to conduct pregravid training with the participation of a psychologist in women with a history of reproductive losses in order to overcome the psychological component of stress perception, expand adaptive potential, increase neuropsychic stability, and form a positive orientation toward pregnancy and motherhood.

Oligohydramnios is an obstetric pathology being one of the most common perinatal diseases. In 70% of cases, its causes are unclear which requires further improvement of diagnostics and treatment tactics. The objective of the research was to examine psychoemotional state in pregnant women with oligohydramnios.Materials and methods. 120 pregnant women with oligohydramnios diagnosed at 27-29 weeks of gestation were examined. The control group included 30 somatically healthy women with physiological course of pregnancy. Psychoemotional state of women was evaluated using the survey and the scale proposed by S.D. Spilberg and adapted by Yu.A. Khanin. Results. When studying psychological peculiarities, we did not reveal realiable differences in the level of trait anxiety between women with oligohydramnios and pregnant women without it. Moreover, there was observed a similar division of patients by the low, medium and high levels of this indicator among patients of both groups. However, the analysis of the obtained data showed the increase in the level of state anxiety in the main group (43.28±1.36 points) compared to the control one (35.30±2.22 points, p<0.01). Among women with oligohydramnios, an increase in the proportion of individuals with a high level of this parameter (p<0.01) as well as a decrease in the proportion of individuals with a low level of this parameter was observed.Conclusions. The level of trait anxiety in pregnant women with oligohydramnios did not differfrom that in healthy women and corresponded to the average value, while the level of state anxiety which corresponded to the average values as well, significantly increased compared to pregnant women with physiological gestation (p<0.01). Among this category of patients, a statistically significant increase in the proportion of individuals with a high level of this parameter was observed (p<0.01).

A functional cervical insufficiency plays an important role in the etiology of miscarriage and premature birth. The aim of research was to estimate the hormonal balance indexes during the course of pregnancy in pregnant women with functional cervical insufficiency and to correct the revealed violations in order to prevent obstetric complications. 101 pregnant women with confirmed diagnosis of a functional cervical insufficiency in term of gestation 22–32 weeks of pregnancy (the main group) and 34 pregnant women and women in labor with physiological condition of the cervix (the control group) were examined. The content of estradiol, progesterone, cortisol and prolactin was examined in the blood serum. On the second stage of examination 63 pregnant women obtained general basic therapy (I main group) and 38 pregnant women (II main group) received a developed treatment complex which included progesterone support by the micronized progesterone 200–400 mg twice a day up to 34–35 weeks of pregnancy, magnesium support by the magnesium orotate dehydrate in the therapeutic dosage and arginine glutamate in the therapeutic dosage. The correction of cervical insufficiency was performed with the help of a cervical pessary (according to indications). The examination of hormonal balance revealed an accurate decease of progesterone concentration on the background of high concentration of estradiol and increased levels of stress-associated hormones. Propitious influence of the developed therapy is proved by increase of progesterone concentration in the blood serum, normalization in stress-associated hormones balance according to gestational age. All this provideded the decrease of specific weight of threatened preterm labors, placental dysfunction, vaginitis, bacterial vaginosis and delay the labors up to 38–39 weeks of gestation in 97,4% of pregnant women (82,5% in the I main group). Prescription of micronized progesterone, magnesium orotate dehydrate, arginine glutamate to pregnant women with a cervical insufficiency is reasonable and effective one comparing with general and traditional therapy.

Aim: to evaluate the efficacy and safety of the L-lysine escinate venotropic drug in the treatment of cephalgic, atactic and asthenic syndromes in patients with chronic cerebral ischemia, mainly caused by venous disorders. Patients and Methods: the study involved 60 patients with a diagnosis of chronic cerebral ischemia with signs of chronic venous insufficiency. All patients were randomly divided into two groups consisting of 30 people. Patients in the main group received L-lysine escinate, and the control group received Vinpocetine. The drugs were administered by intravenous drip for 10 days. A dynamic assessment was performed using a modified subjective scale of asthenia evaluation (MFI-20), 100 mm long visual analogue scale (VAS) for headache, subjective neurological impairment scale (SNIS), and Tinetti test. An ophthalmic examination with an assessment of the fundus vessels and an ultrasound duplex scanning of head vessels (veins) were also conducted. Results: it was shown that L-lysine escinate therapy showed a statistically more significant reduction in the severity of headache (according to VAS) and an improved stability measured by Tinetti test versus during Vinpocetine treatment. There were no significant differences in the level of asthenia (MFI-20), the severity of emotional disorders, and the SNIS score. Also, the number of patients with dilated fundus veins significantly decreased during treatment in the main group (p<0.05), which was not the case in the control group. According to ultrasound duplex scanning data, the venous flow velocity through the veins of Rosenthal and angular veins during L-lysine escinate therapy was significantly higher (p<0.05) than in patients receiving Vinpocetine. Conclusion: the inclusion of vasotropic drugs (in particular, the L-lysine escinate venotonic drug) in the treatment regimen of patients with chronic brain ischemia seems appropriate, especially if there are signs of venous insufficiency.
