Preparation for the first childbirth: assessment of the impact on some characteristics of psychological status

Aim. To study of the influence of complex psycho-preventive preparation for childbirth in primiparous patients on some features of their psycho-emotional status. Materials and methods. A cross-sectional prospective study of 286 nulliparous women in full-term pregnancy, aged 21 to 35 years, was carried out, among them 142 pregnant women (the main group) attended courses of complex psycho-preventive preparation for childbirth in the third trimester of pregnancy. The control group consisted of 144 primiparas who did not attend antenatal training courses. To assess the psychological status of pregnant women, the method for assessing the psychological component of the gestational dominant (PCGD) was used according to I. V. Dobryakov. Anxiety was assessed according to the scale of situational and personal anxiety Ch.D. Spielberger and Yu.L. Khanina. Results. It was revealed that attending birth-giving women 4.8 times more often leads to the formation of the optimal type of psychological component of the gestational dominant. The effects of attending antenatal training classes on the level of situational anxiety in pregnant women are manifested in the form of a significant decrease in its level – OR 0.28 (95% CI 0.15-0.54). Conclusion. Comprehensive antenatal preparation for childbirth in primiparous leads to an improvement in some emotional characteristics of pregnant women, including the formation of the optimal type of psychological component of the gestational dominant and a decrease in situational anxiety.

Objective: to assess the value of the psychoemotional state of pregnant women in the development of a prolonged second period of labor.Material and methods. 76 pregnant women were examined. In 26 women, childbirth was complicated by a prolonged second period of labor, and they constituted the main group. The control group included 50 women who had physiologic birth. The psychoemotional state was analyzed using the Spielberg test and the test of a pregnant woman's attitudes.Results. There were some differences between the groups in terms of the tendency in the levels of personal anxiety and occurrence of high situational anxiety. The Spielberg test has revealed more «positive» affirmative answers than «negative» ones. 46.1 % women in the main group do not «feel a sense of inner satisfaction», which is statistically significantly higher than in the control group - 24.0 % (p = 0.04). In the main group, in contrast to the control group, the women are more often not «worried about possible failures». During the study of the types of the psychological component of the gestational dominant, mixed types were mainly observed, and the distribution in the groups did not have statistically significant differences. The women with a prolonged second period of labor had a tendency to form the predominantly euphoric type of the psychological component of the gestational dominant.Conclusion. During pregnancy, the patients with a prolonged second period formed a coping strategy in the form of a predominantly euphoric attitude to pregnancy associated with elevated situational anxiety.

Abstract Background Although more and more attention has been paid to the psychological consequences of the lockdown policy amongst pregnant women, the underlying mechanism linking the lockdown policy to maternal depression has not been studied in the context of China. This study aimed to explore the association between the lockdown policy and maternal depressive symptoms, and whether such association was mediated by internet use and/or family support. Methods This cross-sectional study used multi-stage sampling techniques in central and western China. Data were collected from 1266 pregnant women using a structtured questionnaire that measured internet use, family support, and depressive symptoms. The Patient Health Questionnaire-9 (PHQ-9) was used to measure depressive symptoms. Internet use was measured by length of usage and varierity of purpose for internet use. Family support was measureed by spousal support and parental support. The structural equation modelling was employed to conduct mediation analysis to test the specificity of the hypothetical paths. Results Overall, 527 respondents (41.63%) presented depressive symptoms. The lockdown policy was negatively associated with depressive symptoms in pregnant women (β = − 0.925, 95% CI = −1.510, − 0.360). The impact of the lockdown policy on depressive symptoms was partially mediated by internet use (β = 1.589, 95% CI = 0.730, 2.807) and family support (β = − 0.162, 95% CI = − 0.341, − 0.017), accounting for 42.67% of the total effect. Conclusions The lockdown policy was generally associated with fewer depressive symptoms in pregnant women. The lockdown policy increased maternal depressive symptoms through increased internet use, but decreased maternal depressive symptoms through enhanced family support. The findings suggest that the psychological consequence of the lockdown policy may vary across different populations, and warrant the need to take into consideration the features of subgroups.

Pregnant women have an increased risk of vulvovaginal candidosis. Recurrent candidosis is under debate as a contributor to preterm birth, and vertical transmission may cause diaper dermatitis and oral thrush in the newborn. Apart from cultural methods, the gold standard for diagnosing candidosis is Gram staining, which is time-consuming and requires laboratory facilities. The objective of this prospective study was to validate a point-of-care vaginal yeast detection assay (SavvyCheckÔ Vaginal Yeast Test) and to evaluate it in asymptomatic pregnant women. We enrolled 200 participants, 100 of whom had vulvovaginal candidosis according to Gram stain (study group) and 100 were healthy pregnant controls (control group). Of these, 22 participants (11%) had invalid test results. The point-of-care test of the remaining 85 and 93 study participants in the study and control groups, respectively, showed a sensitivity of 94.1%, specificity of 98.9%, positive predictive value of 90.3%, and negative predictive value of 99.4% when compared with Gram stain. In conclusion, we found a high correlation between the SavvyCheckÔ Vaginal Yeast Test and Gram-stained smears during pregnancy. This suggests a potential role of this point-of-care test as a screening tool for asymptomatic pregnant women in early gestation.

Stress in women, before and during pregnancy, may result in a negative impact on the mother and fetus. In Indonesia, the anxiety rate in primigravida pregnant women when facing labor is higher than multigravida. Every pregnant woman makes an attempt to overcome or manage stress in her pregnancy in order to adapt and cope with stress. To find out the coping ability, pregnant women should find the source of stress prior to labor. This study was conducted to determine the direct and indirect factors affecting the coping ability in primigravida pregnant women. The study used a cross sectional study method through PLS analysis. The study population consisted of primigravida pregnant women, and a sample of 200 of them was taken. The measurement results of Path Coefficients and TStatistics on the influence of variables in the structural model and overall variables showed a positive and significant effect. The T statistic value of all variables was above the critical value (1.96). The results of the PLS test indicated that the empowerment and social support variables, directly and indirectly, influenced the coping abilities. Trust, personality, lifestyle, perceptions, and attitudes had impacts directly on the coping abilities. The percentage of the direct and indirect influence between variables was 97.92%. Empowerment, social support, personality, lifestyle, perceptions, and attitudes variables had an influence on the coping abilities of primigravida pregnant women. A controlled trial study should be done to see the impact of this model on reducing the risks during pregnancy.

Background : Pregnancy is a state of physiological adaptations to accommodate the needs of the developing fetus. Elevated blood glucose during pregnancy could lead to gestational diabetes and anemia could cause intercurrent infection.Objective: To assess the serum glucose level & hemoglobin concentration in third trimester of pregnancy in order to find out the risk of gestational diabetes and physiological anemia.Method: This cross-sectional study was carried out in the Department of Physiology Mymensingh Medical College, Mymensingh, between the period of July, 2013 to June,2014. One hundred pregnant women in their third trimester of pregnancy aged 18-35years were enrolled in study group and age matched 100 healthy non-pregnant women were control group. Random serum glucose was estimated by GOD-PAP method and hemoglobin concentration was measured by cyanmethemoglobin (CMG) method. Data were analyzed by student’s un paired t test and chi square test.Result: Mean serum glucose level (6.76±1.72 mmol/L) was significantly higher and hemoglobin concentration (8.21±1.23 g/dl) was significantly lower in pregnant women than non-pregnant women. Increased frequency of high glucose (38%) and low H b(88%) was found in pregnant women in third trimester.Conclusion: This study concludes altered glucose metabolism may lead to gestational diabetes as well as physiological anemia may be exaggerated in third trimester of pregnancyBangladesh Soc Physiol. 2015, December; 10(2): 67-70

Abstract Background The hepatitis B virus (HBV) status of pregnant women affects HBV vaccine failure in their offspring. This study is aimed to investigate the impact of the universal infant HBV vaccination program on the long-term hepatitis B surface antigen (HBsAg) rate in pregnant women. Methods Using the National Immunization Information System, we examined a 32-year period of cross-sectional data on a maternal HBsAg and hepatitis B e antigen (HBeAg) screening program launched in July 1984. An age-period-cohort model analysis of 940 180 pregnant women screened for July 1996–June 1997 and the years 2001, 2006, 2011, and 2016 was applied. Results The annual HBsAg- and HBeAg-seropositive rates decreased from 13.4% and 6.4%, respectively, for the period 1984–1985 to 5.9% and 1.0% in 2016 (P for both trends < .0001). Pregnant women with birth years after July 1986 (the HBV vaccination cohort) had the lowest relative risk (0.27 [95% confidence interval, .26–.28]) of HBsAg positivity compared with birth years before June 1984. Conclusions The birth cohort effect in relation to the universal infant HBV immunization program has effectively reduced the HBV carrier rate in pregnant women and the burden of perinatal HBV infection on the next generation.

Abstract Objective: To obtain the profile of D-dimer in uncomplicated pregnancy. Methods: A cross sectional study was done on 90 uncomplicated pregnant women consisted of 30 women in each trimester and 30 healthy, nonpregnant women as control group from July to August 2012. D-dimer level was measured by particle enhanced immunoturbidimetry method using Innovance D-dimer and Sysmex CA 1500 in the Department of Clinical Pathology, Dr. Cipto Mangunkusumo Hospital, Jakarta. Results: All women in the control group showed normal D-dimer level (<0.,5 mg/L FEU). The median and range of D-dimer level in the 1st trimester, 2nd trimester, and 3rd trimester were 0.42 mg/L FEU and 0.1-1.07 mg/L FEU, 0.97 mg/L FEU and  0.6-3.34 mg/L FEU, and 1.56 mg/L FEU and  0.69-3.75 mg/L FEU, respectively.  Increased D-dimer level was found in 27% of pregnant women in 1st trimester, 87% in 2nd trimester, and 100% in 3rd trimester. Conclusion: Increased D-dimer level was found in  27% of pregnant women in 1st trimester, 87% in 2nd trimester, and  100% in 3rd trimester. The range of D-dimer level in the 1st trimester was 0.1-1.07 mg/L FEU, in the 2nd trimester was 0.6-3.34 mg/L FEU, and in the 3rd trimester was 0.69-3.75 mg/L FEU. Introduction: Vitiligo is a chronic pigmentary cutaneous disorder. Although vitiligo is not a life-threatening disease, it seriously affects patients' psychological status and Quality-of-Life (QoL). Moreover, the effect of vitiligo on genital image severely affects sexual function, especially in females. Patients & Methods: This cross-sectional questionnaire-based study had targeted patients with vitiligo of both genders. 173 patients who had responded to the given questionnaires were included and their responses were statistically analyzed. Patients’ files were revised for extraction of demographic and clinical data and the previous evaluation of the Vitiligo Area Scoring Index (VASI) score. Results and Discussion: Median VIS-22 score was in the range of 21-49, but 109 patients had a median score of 28. Seventy-eight patients had mild and 15 patients had moderate depression. Regression analysis defined the sense of being unattractive, disappointed in self and discouragement about the future as the most significant underlying causes of depression. Ninety-two patients had sexual dysfunction and scoring of satisfaction by the sexual act was the highest while scoring for the desire was the lowest. Regression analysis revealed that female gender, high ASEX and VASI scores are the significant predictors for high BDI-II. ROC curve analysis defined a high ASEX score as a significant predictor for a high BDI-II score. Conclusion: The obtained results indicated a deleterious effect of vitiligo on patients’ psychological status that may progress to depression. The effects of the disease on body image, especially the genital area, induced sexual dissatisfaction with an impact on sexual function. Altogether, raise a suggested adjuvant role for psychotherapy in patients with vitiligo which might even indirectly lead to improvement of vitiligo.

Background: Asthma and its medications have been linked to oral diseases in asthmatic children. Aim: Assessment of the dental caries status, salivary Streptococcus mutans count and S. mutans colony score in children receiving inhaled anti asthmatic medications and their comparison in healthy children Material and Method: A cross-sectional study was performed on 40 asthmatic children and 40 healthy children in the age group of 6-14 years. DMFT/deft indices were calculated and saliva samples were collected. Diluted saliva was inoculated on MSB agar plates. S. mutans count and colony score were analysed after 24-48 hours of inoculation. Results: Statistically significant difference was observed in the mean DMFT/deft index, salivary S. mutans load and S. mutans colony score in children of the asthmatic group and control group. Conclusion: Prevalence of dental caries and cariogenic bacteria is higher in asthmatic children.

Background: The prevalence of sleep disorders increases as pregnancy progresses, which affects the health of pregnant women, fetal health, and the outcomes of pregnancy. The aim of this study is to evaluate the quality of sleep and life in pregnant women in Korea.Methods: This study is a prospective cross-sectional, case-control study of pregnant women and age-matched controls. From July to September 2019, all participants completed Korean-language versions of the sleep questionnaires including Pittsburgh sleep quality index (K-PSQI), Insomnia severity index, Epworth sleepiness scale, Back’s depression inventory-2(K-BDI), STOP (Snoring, Tiredness, Observed apneas, and high blood Pressure), and short-form 36 (K-SF-36).Results: A total of 422 participants consisted of 385 pregnant women and 137 controls. Second and third trimester were 200 and 185, respectively. K-PSQI scores were higher in pregnant women compared with the controls (7.87±3.49 and 8.50±3.55 vs. 5.79±2.76, <i>p</i><0.001). Total score of K-SF-36 was lowest in third trimester (62.07±17.72) and highest in the control group(79.41±13.36). There was no statistical difference between groups in K-BDI.Conclusions: This study demonstrated worsening of sleep quality as well as life quality during pregnancy. More attention to sleep of pregnant women is needed.
