Periodontal disease is a chronic inflammatory disease of periodontium characterized by increased pocket depth, clinical attachment loss, sulcular bleeding, and bone loss. Bacterial plaque bio-film stimulates host derived enzymes and cytokines like TNF , IL-1 and matrix metalloproteases that lead to destruction of periodontium, collagenolytic activity, decreased bone mineral density, intra-bony defects and ultimately bone loss. Chronic periodontitis is managed by conventional and systemic approach, where conventional therapy comprises of scaling and root planning. Mechanical debridement of plaque also requires an adjunct to eradicate the root cause of progressing disease. Hence, worldwide paradigm has shifted towards novel therapies; therefore, local delivery of drug is now preferred due to direct access to target sites with considerably less adverse effects and a better approach to deal with chronic periodontitis. The nano-particle technology to treat periodontitis is still an emerging and promising strategy for the management of disease with the provision of minimal dose, less invasive procedure and clinical efficacy.

Bone loss is a feature of both periodontitis and osteoporosis, and periodontal destruction may be influenced by systemic bone loss. This study evaluated the association between periodontal disease and bone mineral density (BMD) in a cohort of 1347 (137 edentulous) older men followed for an average of 2.7 years. Participants were recruited from the Osteoporotic Fractures in Men Study. Random half-mouth dental measures included clinical attachment loss (CAL), pocket depth (PD), calculus, plaque, and bleeding. BMD was measured at the hip, spine, and whole-body, by dual-energy x-ray absorptiometry, and at the heel by ultrasound. After adjustment for age, smoking, race, education, body mass index, and calculus, there was no association between number of teeth, periodontitis, periodontal disease progression, and either BMD or annualized rate of BMD change. We found little evidence of an association between periodontitis and skeletal BMD among older men.

Relevance: to indentify if periodontal treatment which is presented by photoactivated disinfection (PAD) adjunctively to scaling and root planing (SRP) yield better outcomes than ozone therapy as an adjunct to SRP in periodontitis.Materials and methods: we examined 57 (mean age 49,3 ± 1,02) patients with chronic periodontitis, divided into groups, SRP + PAD, SRP + ozone therapy and SRP alone. Subgingival plaque samples were subjected to DNA extraction and real time PCR amplifcation for detection Porphyromonas gingivalis (Pg), Prevotella intermedia (Pi), Tannerella forsythensis (Tf), Treponema denticola (Td), Aggregatibacter actinomycetemcomitans (Aa). The amount of periodontopathogens and clinical parameters including plaque index, clinical attachment loss, pocket depth, bleeding on probing were measured at baseline, after 40, 90 and 180 days.Results: the results in groups of PAD+SRP and ozone therapy+SRP showed an improvement in all clinical parameters PI, BOP, PD, CAL and the quantity of Pg, Td and Tf compared to the control group during an observation period.Conclusion: the results showed additional benefts from PAD and ozone therapy as an adjunctive treatment to SRP for patients of chronic periodontitis.

The aim of the present study was to evaluate the periodontal conditions of anterior teeth that presented pathologic migration in patients with chronic periodontitis and to compare periodontal destruction in migrated versus non-migrated teeth. The sample included 32 patients of both sexes (mean age: 46.0 ± 11.6 years) diagnosed with generalized chronic periodontitis and selected on the basis of the presence of pathologic migration in one or more anterior teeth. This migration was classified according to the following categories: facial flaring, diastema, proximal tilting, rotation or extrusion. The periodontal parameters recorded were clinical attachment loss (CAL) and percentage of radiographic bone loss (BL). Mean CAL of 5.50 ± 2.20 mm and mean BL of 41.90 ± 15.40% were found in 115 teeth assessed. The most frequent type of migration was facial flaring (34.80%), followed by diastema (27.00%). Extrusion was hardly observed in the sample (4.30%). However, greater severity of BL and CAL were observed in teeth with this type of migration (59.44% and 8.42 mm, respectively), and in teeth with facial flaring (45.17% of BL and 6.07 mm of CAL). Kruskal-Wallis test indicated that BL presented by teeth with extrusion or facial flaring was greater than that observed in rotated or tilted teeth (p < 0.05), while there was no difference between groups regarding CAL (p = 0.11). It was observed that anterior teeth with pathologic migration presented greater CAL and BL (5.1 mm and 40%) than non-migrated teeth (4.1 and 31%). The study indicated that the most prevalent kind of pathologic migration is facial flaring, which was associated to higher level of bone loss.

Combination of the classical subgingival instrumentation (scaling and root planing procedure, SRP) with an antibiotic administered to periodontal pockets in a suitable medium is a promising alternative protocol of nonsurgical periodontal treatment. It enables obtaining the long-term minimum drug concentration inhibiting the development of periopathogens. Objectives: Clinical and microbiological evaluation of periodontal pockets two months after single application of a gel containing piperacillin and tazobactam (Gelcide)® in relation to the nonsurgical treatment procedure (SRP). Materials and methods: Ten patients aged 24–56 years (mean 39.5) with chronic periodontitis, nonsmokers with acceptable oral hygiene and no classical exclusion criteria were qualified for treatment. In the maxilla area, SRP was performed and the assessed gel was inserted to two randomly selected adjacent periodontal pockets. Clinical evaluation included the assessment of bleeding on probing (BoP), pocket depth (PD), and clinical attachment loss (CAL) at six measurement points. A microbiological examination with the use of PET deluxe diagnostic kit in the drug-administered pockets and symmetrically in two pockets on the other side of the dental arch was performed. The examination was conducted before the treatment and two months later. Results: Two months after the treatment, a significant improvement in all analyzed clinical parameters was observed. However, the extent of this improvement did not differ significantly between the compared treatment methods. No statistically significant differences were found in the number of bacteria before and after the treatment, except for a significant decrease in the number of Micromonas micros (2957 vs. 589, p = 0.028) and a higher number of the green complex bacteria Capnocytophaga gingivalis (5439 vs. 2050, p = 0.041) after antibiotic had been used. Conclusion: No significant clinical and microbiological differences were found after additional administration of gel with piperacillin and tazobactam in relation to SRP in the preliminary study.

Abstract The aim of this case control study was to assess the association between the extent and severity of chronic periodontitis and oral cavity and/or oropharyngeal cancer. The case group comprised 35 patients (mean age 56.1±8.4), diagnosed for oral and/or oropharyngeal cancer. The control group comprised 40 individuals (mean age 55.4±9.4) without diagnostic of cancer. All individuals were subjected to a periodontal examination, including bleeding on probing, plaque index, gingival index, probing pocket depth (PPD), clinical attachment loss (CAL), and decayed, extracted and filled teeth index (DMFT). The case group had significantly more sites with plaque. GI and BOP had similar values in both groups. The median PPD and CAL values were significantly higher for the case group. Chronic generalized periodontitis was predominant in 80% of patients with oral and/or oropharyngeal cancer. Eighty nine percent of the patients in the case group presented severe chronic periodontitis. There was no significant difference between groups for median values of DMFT. The extent and severity of chronic periodontitis remained as risk indicators for oral cavity and/or oropharyngeal cancer even after the adjustments for traditional confound factors, i.e. smoking and alcohol consumption.

Periodontitis is a chronic inflammatory disease, highly prevalent almost around 10-15% of adults. Periodontitis is a group of chronic, progressive bacterial infections causing inflammation and destruction of supporting structures of teeth and has multiple factors affecting the quality of an individual's life. Diabetes is also a chronic inflammatory disease. Both diseases share a common platform in disease pathogenesis, a periodontal abscess is the sixth complication of diabetes, and there is clear evidence showing the relationship between periodontitis and diabetes Overall no clear evidence and studies which correlate diabetes and periodontal parameters. This study aims to compare blood sugar levels with demographic data age, gender and to assess the association between periodontal severity with diabetic status. Results: Periodontal severity was found to be greater in both male and female with diabetic Mellitus. 39 out of 52 subjects have generalized chronic periodontitis, and 13 was found to have localized chronic periodontitis. The age group of (50-70) years had high severity of clinical attachment loss. No significant difference between male and female on periodontal severity was found. Conclusion: This study concludes the age group of 50-60 years more prevalent among diabetes with periodontitis. The male is more affected by diabetes and periodontitis. The association between periodontal severity (CAL, PD) increases with an increase in blood sugar level was statistically significant.

ABSTRACT Objective The identification of new uncultured species and viruses supports the possibility of combination of the herpesvirus- bacterial periodontal infection for periodontitis. The paucity of data and studies with larger sample size in Indian subjects provides an unclear picture of the presence of the herpesvirus in this population. Materials and methods This was a cross-sectional study consisting of 100 each in the healthy group and chronic periodontitis (CP) group. The subgingival plaque was collected and polymerase chain reaction was performed post deoxyribonucleic acid (DNA) extraction by using specific primers for human cytomegalovirus (HCMV) and Epstein-Barr virus (EBV). The data were analyzed using Fisher's exact test, Mann-Whitney U test and Spearman's coefficient correlation. Results Human cytomegalovirus and EBV viruses were significantly higher in the CP group as compare to the healthy group. A higher percentage of those with CMV positive had EBV also positive (28.3%) compared to only 9.1% of CMV negative being EBV positive in the CP group. When both the healthy and CP group in total was compared, there was a significant correlation with all clinical parameters. Conclusion Both the viruses dominated in disease as compared to health were similar to the earlier findings. The CP group had higher pocket depth and clinical attachment loss in the virus positive subjects. These findings could suggest that virus serves as a prelude to the disease and the combination of the two viruses could play a role in the pathogenesis. Purpose: To evaluate the efficacy of pluronic F-127 gel containing green tea catechin extract as a local drug delivery system in the treatment of chronic periodontitis. Methods: A total of 20 chronic periodontitis patients participated as per the set inclusion and exclusion criteria. Complete scaling and root planing (SRP) was done for all subjects and pluronic F-127 gel containing green tea catechin was applied on one site. The contralateral site received SRP alone. The plaque index (PI), gingival index (GI), and probing pocket depth (PPD) were recorded at baseline and on the 28th day. Results: At the 28th-day follow-up, green tea catechin tooth sites showed significantly lower mean scores (GI = 0.55, p = 0.30 and PPD = 3.35 mm) than the corresponding SRP tooth sites (GI = 1.25, PI = 1.15, and PPD = 4.40 mm) (p < 0.05). Conclusion: When compared to scaling and root planing alone, the local drug delivery gel containing green tea catechin as an adjuvant was more effective in reducing the clinical parameters of periodontitis. Prostaglandin E2 (PGE2) is a key mediator associated with periodontal bone loss. We recently demonstrated (Yen, et al, J Periodontology 79:104, 2008) that adjunctive use of Celecoxib (a COX-2 selective inhibitor) in conjunction with Scaling and Root Planning (SRP) resulted in significant reduction in Pocket Depth (PD) and gain in Clinical Attachment (CAL) in Chronic Periodontitis (CP) patients.

Background. This study aimed to assess the association of haptoglobin (HP) and natural resistance-associated macrophage protein 1 (NRAMP1) alleles with the presence of heme-consuming periodontal pathogens in a group of Iranian patients with chronic periodontitis and peri-implantitis. Methods. This cross-sectional study evaluated 69 eligible chronic periodontitis and peri-implantitis patients selected from Shahid Beheshti Dental School. The periodontally diseased individuals had at least three teeth with clinical attachment loss of ≥3 mm and a probing pocket depth (PPD) of ≥3 mm in at least two quadrants. Peri-implant PPD of at least one site was ≥5 mm with or without suppuration and bleeding on probing. A plaque index of >20% and a radiographic crestal bone loss was present in at least one site around the implant. The paper point method was used for sampling from the deepest periodontal/peri-implant pocket of each tooth or implant for the DNA checkerboard hybridization technique. Statistical analyses were performed with PASW Statistics 18.0. The variables were presented as absolute and relative frequencies (%). Results. An Aggregatibacter actinomycetemcomitans (A. actinomycetemcomitans) score of 1–2 was 5.8 times more frequent in HP 2, rs1723540 G, and rs2276631 G alleles. A Porphyromonas gingivalis (P. gingivalis) score of 1–2 was 4.8 times more common in the subjects carrying HP 2, rs1723540 G, and rs2276631 G alleles compared with HP 1, rs1723540 A, and rs2276631 A alleles. Conclusion. Within the limitations of this study, it seems that there was a relationship between HP and NRAMP1 allele frequencies and the presence of heme-consuming periodontal pathogens in the Iranian patients with chronic periodontitis and peri-implantitis evaluated in the present study.
