Aim: Determination of dental students' approaches to fissure sealant applications; the widespread use and evaluation of their effectiveness of protective dental practices is extremely important. This research aims to evaluate the level of understanding and clinical attitudes of 3rd, 4th and 5th grade students of Necmettin Erbakan University Faculty of Dentistry on preventive dental treatments. Methods: The first part of the questionnaire consists of 10 questions aimed at assessing the students' technical levels of knowledge with data on age, gender and educational level. In the second section analyzing students' clinical behaviors, 11 questions were asked for answers to multiple choices (always, often, sometimes, never). This questionnaire was used on the internet in an anonymous way. Results: The questionnaire was received by 68 3rd grade, 67 4th grade and 75 5th grade students who were previously educated on fissure sealant applications. The vast majority (96.2 percent) of the students involved in the study indicated that the effectiveness of fissure sealants in preventing the development of tooth decay was confirmed by strong scientific evidence. Again, 99.5 percent of the students participating in the study indicated that the justification of the application procedures for the fissure sealant was identified and understood. With the assumption that only new teeth can be applied to fissure sealants, a statistically significant difference between the classes was observed (p<0.05); 3rd graders responded "yes" mainly. Conclusion: Although dental students have sufficient knowledge of protective dental practices and fissure sealants, variations in understanding and implementing clinical practice procedures may be found between classes. Standardization should be provided and education / lectures on preventive dental practices should be increased to obtain the equivalent of theoretical education at the clinical level.

Objectives. The present study evaluated the antimicrobial effects of fissure sealants containing chitosan nanoparticles. Materials and Methods. Antibacterial effect of Master Dent fissure sealant alone and after incorporating chitosan nanoparticles was evaluated on Streptococcus mutans, sanguis, and Lactobacillus acidophilus. Biofilm growth was evaluated by determining colony counts. Antimicrobial effect was determined on days 3, 15, and 30 by counting microbial colonies using eluted components test. One-way ANOVA, Tukey HSD tests, t test, and two-way ANOVA were used for statistical analyses (α = 0.05). Results. Biofilm inhibition test showed that fissure sealant containing 1 wt.% chitosan decreased colony counts significantly ( P < 0.05 ). Eluted components test with S. mutans and sanguis showed significant decrease in colony counts during the first 15 days in chitosan containing group; however, from day 30, antimicrobial activity decreased noticeably, with no significant difference from control group ( P > 0.05 ). Antimicrobial activity against L. acidophilus was maintained in chitosan group up to 30 days, and decrease in colony counts was significant ( P < 0.05 ). Conclusion. According to the results of this study, incorporation of 1 wt.% chitosan into fissure sealant induced an antimicrobial activity. Antibacterial effect on L. acidophilus persisted for longer time (30 days) compared to the two other bacterial species (15 days).

Objective: To compare the microleakage around resin modified glass ionomer cement (RMGIC) based sealants and flowable resin based sealants placed with or without ameloplasty in extracted human teeth. Methods: This in-vitro experimental study was conducted at the Operative Dentistry Department, Dow University of Health Sciences, Karachi, Pakistan from June 2017 to December 2018. Sixty extracted human molars and premolars were assigned to four groups (n=15) each, according to the type of fissure sealant (flowable resin based sealant or resin modified glass ionomer based sealant) used and either placed with or without ameloplasty. Specimens were thermocycled and then immersed in 1% methylene blue for 24 hours. Specimens were then sectioned and examined using stereo-microscope (50X) for microleakage that was scored on an ordinal scale. Mann-Whitney U test and Ordinal regression were applied. Level of significance kept at 0.05. Results: There was a statistically significant difference (p-value <0.001) between the two sealant types for the microleakage scores. Sealants placed with ameloplasty demonstrated significantly higher microleakage values (p-value <0.001). Conclusion: Microleakage was found to be more pronounced in RMGIC based sealants compared to the resin based sealants. Objectives: Pit and fissure sealants are recognized as an effective preventive approach in pediatric dentistry. Composite resin is the most commonly used sealant material. Adding nanoparticles to composite resin could result in production of flowable composite with higher mechanical properties and better flowability than previous sealants. This study aimed to compare the microleakage of a flowable nanocomposite and materials conventionally used as pit and fissure sealants. Materials and Methods: A total of 185 extracted mandibular third molar teeth were selected and randomly divided into 5 groups (n=36): flowable nanocomposite, flowable composite, filled sealants, nano-filled sealants, and unfilled sealants. Five teeth were reserved for examination under a scanning electron microscope (SEM). The samples were thermocycled (5-55°C, 1-minute dwell time) for 1000 cycles and immersed in 0.2% fuchsine solution for 24 hours. Teeth were sectioned buccolingually. Microleakage was assessed qualitatively and quantitatively by means of dye penetration and SEM. Data were analyzed using chi-square, Kruskal-Wallis, and Bonferroni-corrected Mann-Whitney U tests. Results: Qualitative microleakage assessment showed that flowable composite and nanofilled flowable composite had almost no microleakage (P<0.001). Regarding quantitative scores, the nanofilled flowable composite and unfilled fissure sealant showed the lowest and the highest rate of microleakage, respectively. No statistically significant difference was found between the two flowable composites (P=0.317). Filled resin-based sealant had significantly lower microleakage than unfilled resin-based sealant (P<0.001). Conclusion: Use of flowable and nanofilled flowable composites (but not unfilled resin-based fissure sealant) is recommended for sealing of pits and fissures of molars.

ABSTRACT Objectives: To investigate the prevalence of caries and fissure sealants on the first permanent molars (FPMs) among 6–9-year-old girls in Riyadh, Saudi Arabia. Materials and Methods: The FPMs of 17,891 school girls from 120 randomly selected public primary schools were evaluated by visual and tactile examination for the carious status and the presence of fissure sealants. Chi-square test was used to analyze the data at level of 5%. Results: A total of 58,140 FPMs were assessed in the 17,891 children. Nearly 64.6% of the children were caries free. Only 1.3% of the children had at least one fissure sealant applied. At tooth level, the decayed FPMs counted for 24.6%. There was obvious underutilization of fissure sealants on the FPMs; 0.8% (n = 478). The caries prevalence in the mandibular FPMs (33%) was significantly higher than in the maxillary FPMs, 18.2% (P < 0.01). The proportion of carious FPMs increased with age of the children significantly (P < 0.01). There was no significant difference in the proportion of the presence of fissure sealants among the three different grade/age groups (P > 0.05). Conclusions: Caries prevalence in the FPMs was high and serious among this cohort of young students. This was contrasting the very low prevalence of fissure sealants.

SUMMARYAimTo compare the retention rates of a nanofilled occlusal fissure sealant placed with the use of an etch-and-rinse or a self-etch adhesive over 48 months.Materials and MethodsThe authors enrolled 244 teeth, each with no restoration or sealant and no detectable caries, from 16 patients. The sealants were placed with Solobond M two-step etch-and-rinse adhesive or Futurabond NR one-step self-etch adhesive by four previously calibrated dentists using a table of random numbers. After completion of the adhesive application, a nanofilled sealant, Grandio Seal, was applied and light-cured. Two other calibrated examiners, who were unaware of which adhesive had been used, independently evaluated the sealants at baseline and at 12-, 24-, 36-, and 48-month recalls. Each sealant was evaluated in terms of caries formation being present or absent and retention using the following criteria: 1 = completely retained, 2 = partial loss, and 3 = total loss. The Pearson χ2 test was used to evaluate differences in retention rates among the sealants used with different adhesives for each evaluation period.ResultsThe retention rates for sealants in the Solobond M group were significantly higher than those in the Futurabond NR group in all periods of evaluation (p&lt;0.05). No statistically significant difference between the retention rates for premolars and molars was found at each evaluation period (p&gt;0.05). There was no new caries formation throughout the 48-month recall period.ConclusionFissure sealants placed with etch-and-rinse adhesive showed better retention rates than those placed with self-etch adhesive.

Introdution: The occlusal surface of the molars are more susceptible to dental caries because of their anatomy, so in some cases, the sealants are indicated. Objective: The objective of this study was to evaluate the efficacy, retention, presence of caries and marginal discoloration of resinous sealants (Fluoroshield and Prevent) in first permanent molars. Methods: Ninety - one children aged 7 to 9 years were selected from a municipal institution Regente Feijó, SP, Brazil, for the application of resinous sealants to first permanent lower first molars. The application of Fluoroshield and Prevent resin sealants was performed according to the manufacturer's recommendations. The evaluation was performed after 6 and 12 months by double-blind examiners and followed the criteria: alpha (total sealant, absence of caries and absence of pigmentation) charlie (partial sealing, superficial caries and light pigmentation) charlie complete sealing, presence of cavitation and pigmentation) after 12 months of application by two previously calibrated examiners. Results: Using Fischer's exact test, no significant difference (p 0.05) was observed between the resin-based sealant groups. Introduction: In contemporary preventive dentistry there are numerous experimental in vitro and clinical studies on the growing number of materials for sealing pits and fissures. Aim: the aim of this study was to evaluate the impact of occlusal tooth morphology on penetrating abilities of two types of fissure sealants and measure the unfilled space at the bottom of occlusal groove using scanning electron microscopy (SEM). Materials and methods: The sample consisted of 10 freshly extraced intact premolars and 10 molars randomly divided into 4 groups. In group I (5 premolars), and group II (5 molars) glass ionomer Fuji VII was used as fissure sealant, while in groups III (5 premolars) and IV (5 molars) resin-based sealant (Helioseal-F) was used. Six sections of each tooth were prepared for SEM. Sealant penetration was observed at 30x magnification and the unfilled space at the bottom of the groove at 500x. Sealant penetration in relation to the absolute depth of occlusal grooves was recorded and the unfilled space beneath the sealant was expressed through its greatest diameter. Results were statistically analyzed using ?2 test. Results: There was no statistically significant difference in penetration ability and the unfilled space between tested materials or teeth groups. The average depth of penetration for glass ionomer was 83% and for resin-based sealant 81% of fissure depth. Mean diameters of the unfilled space beneath the sealants were 95 ?m (glass ionomer) and 93 ?m (resin-based sealant). Shallower penetration of both sealing materials was observed in deep pits and fissures with greater diameter of unfilled space (p<0.05). Conclusion: Based on results of the present study it can be concluded that penetration of contemporary fissure sealants largely depends on occlusal tooth anatomy.

Background . The effectiveness of fissure sealants in caries prevention depends on their long-term retention and ability to stop caries progression. This randomized controlled clinical trial compared the retention rate and cariostatic properties of a contemporary glass-ionomer-based sealant (GIS) versus a resin-based sealant (RS) placed on fully erupted permanent molars in a split-mouth design. Methods. The sealants were placed on fully erupted permanent teeth (8‒12 years of age) in 45 children. The evaluation was conducted after one week and three and six months. Results. There was a statistically significant difference in the retention rate and caries transition between the two groups over a six-month clinical evaluation period. The resin-based sealant group showed a better retention rate than the GIS group (75.56% and 48.88%, respectively). The resin-based sealant was superior to GIS in preventing caries progression. Conclusion. Resin-based fissure sealant with fluoride releasing properties might be preferable in preventing caries progression of incipient non-cavitated carious lesions in fully-erupted teeth.

Fissure sealants are applied to teeth to prevent caries development. The presence of the fissure sealant creates a protective barrier, which prevents plaque accumulation to the pits and fissure. They have a significant role in preventing pit and fissure caries. The aim of this study was to evaluate the time-dependent fluoride ion release of giomer- and resin-based fissure sealants. Fissure sealants were divided into 4 groups: BeautiSealant (Shofu, Japan), Clinpro Sealant (3M ESPE, USA), Helioseal F (Ivoclar Vivadent, Liechtenstein), Fissurit F (Voco, Germany). Disk shaped samples were prepared for each sealant material tested, and they were transferred into polyethylene vials containing 5 ml of deionized water. Fluoride ion diffusion was determined on days 1,2,3,7,14,21, and 28 (n=7). The fluoride concentration in these samples was analyzed using a pre-calibrated spectrophotometer in parts per million (ppm). These experiments were performed in triplicates at room temperature. ANOVA and Tukey’s HSD test were used for statistical analysis. BeautiSealant was released the highest amount of fluoride on day 1 (5.33 ppm), followed by a sudden decrease on the day 2 (2.17ppm). The burst effect was observed only in this group. Clinpro Sealant, Fissurit F and HelioSeal F groups presented fluoride release of 2.69, 2.94 and 2.91 ppm on the first day, respectively, without a significant difference (p > 0.05). After the first week, a constant fluoride release level has been reached. The fluoride release for the three resin-based fissure sealants was slightly lower than that for the giomer-based. After the first week, materials exhibited no significant difference and reached a plateau. The usage of a high and prolonged fluoride-releasing fissure sealant material should be considered by clinicians, particularly in patients with caries risk.

SummaryThe plasma tissue factor (TF) concentration was correlated to factor VII concentration (FVIIag) and factor VII activity (FVIIc) in 498 healthy volunteers ranging in age from 17 to 64 years. Immunoassays using monoclonal antibodies (mAbs) were developed for the determination of TF and FVIIag in plasma. The mAbs and the test systems were characterized. The mean value of the TF concentration was 172 ± 135 pg/ml. TF showed no age- and gender-related differences. For the total population, FVIIc, determined by a clotting test, was 110 ± 15% and the factor VIlag was 0.77 ± 0.19 μg/ml. FVII activity was significantly increased with age, whereas the concentration demonstrated no correlation to age in this population. FVII concentration is highly correlated with the activity as measured by clotting assay using rabbit thromboplastin. The ratio between FVIIc and FVIIag was not age-dependent, but demonstrated a significant difference between men and women. Between TF and FVII we could not detect a correlation.
