Every day, millions of people make decisions about menstruation. They make decisions about what sanitary products to use, about pain relief, about with whom they will discuss their experience of menstruation. They make decisions about contraception to induce amenorrhea. These decisions may be influenced by family, poverty, society, and culture, but they remain, for the most part, up to the individual. However, this right to autonomy is not extended to all people equally. Some disabled people, for example, have these decisions made by substituted decision makers, including the courts. This is in violation of their rights; nevertheless, this practice continues in various jurisdictions, including through guardianship and conservatorship laws.

Quality function deployment (QFD) is an essential tool in implementing total quality management (TQM). This study applies a Web-QFD approach using group decision-making analysis in the Web environment to reduce the complicated data collection, aggregation and analysis processes. A Web-based questionnaire is designed by using an active service pages (ASP) involving the Internet relay chat (IRC) technique and the Delphi method with Internet (E-Delphi) to determine the importance degree of the customers' requirements. However, the traditional Delphi method is time-consuming mission. This study applies the proposed Web-QFD approach to efficiently gather the individual opinions of each team member, the requirements that are critical for customers, and then enables decision makers to accurately assess the priorities of these requirements. An empirical example of an education system in Taiwan is employed to demonstrate the practicability of the proposed Web-QFD model. This real world example involves team members communicating easily and quickly with other experts in the team through the Internet to accelerate the reaching of a consensus among multiple decision makers regardless of where their location. Customers' requirements can be rapidly prioritized based on the assessment results.

Conflict between substitute decision makers (SDMs) and health care providers in the intensive care unit is commonly related to goals of treatment at the end of life. Based on recent court decisions, even medical consensus that ongoing treatment is not clinically indicated cannot justify withdrawal of mechanical ventilation without consent from the SDM. Cardiopulmonary resuscitation (CPR), similar to mechanical ventilation, is a life-sustaining therapy that can result in disagreement between SDMs and clinicians. In contrast to mechanical ventilation, in cases for which CPR is judged by the medical team to not be clinically indicated, there is no explicit or case law in Canada that dictates that withholding/not offering of CPR requires the consent of SDMs. In such cases, physicians can ethically and legally not offer CPR, even against SDM or patient wishes. To ensure that nonclinically indicated CPR is not inappropriately performed, hospitals should consider developing ‘scope of treatment’ forms that make it clear that even if CPR is desired, the individual components of resuscitation to be offered, if any, will be dictated by the medical team’s clinical assessment.

The role and the place of early use of technical means of rehabilitation in the system of complex rehabilitation of patients and disabled people owing to a military trauma is considered. Need of early providing the needing persons with technical means of rehabilitation is proved during recovery treatment, including before establishment by him of disability and formation of the individual program of rehabilitation and an abilitation of disabled people. The efficiency of the developed non-plaster technology of prosthetics manufacturing techniques of artificial limbs of the lower extremities is proved by express method, options of her execution are offered. The first option of production of products on this technology consists in use of polymeric silicone covers and the water-hardening polymeric bandage directly on the patient’s stump. The second option of prosthetics is applied at some defects and diseases of a stump excluding application of silicone covers and full contact individual reception sleeves. For this purpose at production of medical and training artificial limbs of a shin and hip adjustable demountable reception sleeves from thermolayers for right-and left-side amputating defects are used. These options of prosthetics by express method are innovative, are aimed at early rendering the prosthetic and orthopedic help to patients with amputating defects. At production of artificial limbs on these technology domestic materials, modular and not modular accessories are used. The modular complete set of medical and training artificial limbs provides fast and individual setting up the scheme of construction, the individual choice of combinations of functional elements taking into account group of physical activity of the patient, a possibility of replacement of a reception sleeve and any of product elements without withdrawal of an artificial limb at the user. In need of service of a product, for example repair, replacement of the module (artificial foot, a knee) there is no requirement of urgent production of a similar design. Adjustable reception sleeves for primary artificial limbs of a shin and hip in the form of a standard series and moisture-curing bandage can add the list of products of medical appointment in laying for expansion of medical institutions during the special period.

Purpose The purpose of this paper is to investigate low-wage retention using a survival analysis approach. Design/methodology/approach Variables explaining low-wage retention take into account the characteristics of the employee, such as education, age, tenure with the company, gender and nationality, and the characteristics of the job and the company such as industry affiliation, number of employees, age of the company and location. Findings Female workers and workers with low level of education, older ones, those with more seniority in the company and those of Asian origin remain longer in a low-wage situation. Also, workers in smaller and older companies located outside the Lisbon region are more likely to stay in a low-wage situation. Practical implications The policy implications are clear. Education plays a prominent role: the higher the level of education of the individual, the higher the probability of him/her leaving low pay. Training programs may help employees in Portugal to leave the low-wage situation. Furthermore, policies must address the different mobility rates of different nationalities and different activities. Training programs are more urgent for hotels and restaurants and transports and communication. The findings also indicate that those initially working in younger firms and larger firms have a higher probability of leaving the low-wage situation. This is a stimulus for decision makers to stimulate employment in the younger firms or in the larger firms. Originality/value Despite low-wage retention being a well-known field of research, to our knowledge this is the first research paper using survival analysis to explain the duration of a low-wage situation.

Similar to practices in top management positions worldwide, there has been an increasing tendency in recent decades to fire football managers when the team does not perform to the stakeholders' expectations. Previous research has suggested that improvements after change of manager are a statistical artefact. Based on 12 years of data from the Norwegian Premier League, we conduct a natural experiment showing what would have taken place if the manager had not been fired. In this case, the performance might have improved just as well and even quicker. Building on theories in expertise and decision making, we explore the data and argue that decision makers may be fooled by randomness and learn wrong lessons about team leadership. Our analyses support a post-heroic view of team leadership as an emergent, output variable. Exaggerated focus on the individual manager may ruin long-term performance. Practical implications are discussed.

A disabled person in a company is affected by a number of various environmental conditions. Among these the most important are such as organizational, technical, social, economic and, finally, legal working conditions. The standard and quality of the just mentioned conditions determine significantly the way in which a disabled person performs the assigned tasks and duties in the company and it also helps to create the individual sense of self-fulfillment and work satisfaction. What is more, at the same time the level of environmental conditions influences directly work efficiency and profits of the company. It must be clearly stated that working conditions do not appear immediately and spontaneously - they need careful planning and modern organization. This research paper presents the environmental conditions and discusses a model design of the work organization for the disabled people.

The conditions of passing rehabilitation routes by invalids with cardiovascular diseases are studied. The lowest inclusion of invalids by various rehabilitation measures is stated among invalids aged up to 29. They are rarely directed to rehabilitation by physicians of corresponding specialities and are often forced to pay for their treatment in full measure. The data obtained are used to develop the methodical recommendations directed to the optimization of the medicosocial examination and rehabilitation, to increase the efficary of the individual rehabilitation programs made for invalids with cardiovascular disease.

AbstractProposals to strengthen regional representation in national institutions are premised on the assumption that representation of (regional) interests and accommodation of conflicts between competing interests are compatible roles, and most likely when national decision makers represent all contending interests. The article argues that the two roles are not necessarily compatible. The behaviour of MPs on the agriculture and transport committees in debating three bills during the 32nd Parliament shows considerable efforts at representation but much less at conflict management. The emergence of specialist interest groups promotes representational efforts by MPs but often at the expense of conflict management. Other factors affecting the joint occurrence of interest representation and interest accommodation include the individual and collective characteristics of members, their partisan incentives, and the diversity of the goals of their client groups.

The Precise consistency consensus matrix (PCCM) is a consensus matrix for AHP-group decision making in which the value of each entry belongs, simultaneously, to all the individual consistency stability intervals. This new consensus matrix has shown significantly better behaviour with regards to consistency than other group consensus matrices, but it is slightly worse in terms of compatibility, understood as the discrepancy between the individual positions and the collective position that synthesises them. This paper includes an iterative algorithm for improving the compatibility of the PCCM. The sequence followed to modify the judgments of the PCCM is given by the entries that most contribute to the overall compatibility of the group. The procedure is illustrated by means of its application to a real-life situation (a local context) with three decision makers and four alternatives. The paper also offers, for the first time in the scientific literature, a detailed explanation of the process followed to solve the optimisation problem proposed for the consideration of different weights for the decision makers in the calculation of the PCCM.

Abstract. Within the last decades serious flooding events occurred in many parts of Europe and especially in 2005 the Austrian Federal Province of Tyrol was serious affected. These events in general and particularly the 2005 event have sensitised decision makers and the public. Beside discussions pertaining to protection goals and lessons learnt, the issue concerning potential consequences of extreme and severe flooding events has been raised. Additionally to the general interest of the public, decision makers of the insurance industry, public authorities, and responsible politicians are especially confronted with the question of possible consequences of extreme events. Answers thereof are necessary for the implementation of preventive appropriate risk management strategies. Thereby, property and liability losses reflect a large proportion of the direct tangible losses. These are of great interest for the insurance sector and can be understood as main indicators to interpret the severity of potential events. The natural scientific-technical risk analysis concept provides a predefined and structured framework to analyse the quantities of affected elements at risk, their corresponding damage potentials, and the potential losses. Generally, this risk concept framework follows the process steps hazard analysis, exposition analysis, and consequence analysis. Additionally to the conventional hazard analysis, the potential amount of endangered elements and their corresponding damage potentials were analysed and, thereupon, concrete losses were estimated. These took the specific vulnerability of the various individual elements at risk into consideration. The present flood risk analysis estimates firstly the general exposures of the risk indicators in the study area and secondly analyses the specific exposures and consequences of five extreme event scenarios. In order to precisely identify, localize, and characterize the relevant risk indicators of buildings, dwellings and inventory, vehicles, and individuals, a detailed geodatabase of the existing stock of elements and values was established on a single object level. Therefore, the localized and functional differentiated stock of elements was assessed monetarily on the basis of derived representative mean insurance values. Thus, well known difference factors between the analysis of the stock of elements and values on local and on regional scale could be reduced considerably. The spatial join of the results of the hazard analysis with the stock of elements and values enables the identification and quantification of the elements at risk and their corresponding damage potential. Thereupon, Extreme Scenario Losses (ESL) were analysed under consideration of different vulnerability approaches which describe the individual element's specific susceptibility. This results in scenario-specific ranges of ESL rather than in single values. The exposure analysis of the general endangerment in Tyrol identifies (i) 105 330 individuals, (ii) 20 272 buildings and 50 157 dwellings with a corresponding damage potential of approx. EUR 20 bn. and (iii) 62 494 vehicles with a corresponding damage potential of EUR 1 bn. Depending on the individual extreme event scenarios, the ESL solely to buildings and inventory vary between EUR 0.9–1.3 bn. for the scenario with the least ESL and EUR 2.2–2.5 bn. for the most serious scenarios. The correlation of the private property losses to buildings and inventory with further direct tangible loss categories on the basis of investigation after the event in 2005, results in potential direct tangible ESL of up to EUR 7.6 bn. Apart from the specific study results a general finding shows that beside the further development of modelling capabilities and scenario concepts, the key to considerably decrease uncertainties of integral flood risk analyses is the development and implementation of more precise methods. These are to determine the stock of elements and values and to evaluate the vulnerability or susceptibility of affected structures to certain flood characteristics more differentiated.
