Abstract:Objective: To find out the success rate of uterovaginal packing of Atonic Uterus in a tertiary care hospital, in low resource setting.Methods: This was a interventional prospective trial conducted in Gynae department of Lady Reading Hospital from January 2019 to December 2019. Our study included those patients with PPH not responding to medical treatment in vaginal delivery. Patients in shock with PPH and PPH after Csection were excluded from the study. The study was approved by the ethical committee of hospital.Results: In our study 250 patients with Atonic Uterus with PPH with failed medical treatment were done uterovaginal packing, the success rate after 12 hours was 86.4%, in rest of the patients PPH was controlled by second line surgical intervention in 34 cases (13.6%), B Lynch was done in 18 (7.2%) cases, subtotal hysterectomies 13 (5.2%) cases and internal Iliac ligation in 3 cases (1.2%)Conclusion: Uterovaginal packing is an effective, easy, quick and life saving method in the control of PPH in low resource setting.

Introduction: Epistaxis is a common medical emergency with 5% to 15% of patients admitted for epistaxis will require surgical management as nasal packing has high failure rates. A modern endoscopic technique like Endoscopic Sphenopalatine Artery Ligation has increased in popularity for managing intractable posterior epistaxis. It has less complication and a high success rate. The study conducted to estimate the success rate of Endoscopic Sphenopalatine Artery Ligation of refractory posterior epistaxis among admitted patients in a tertiary care hospital. Methods: This is a descriptive cross-sectional study conducted from June 2019 to June 2020 at the Department of Otorhinolaryngology, Nobel Medical College and Teaching Hospital among the patient with refractory posterior epistaxis with the help of retrospective data. A convenient sampling method was used. These patients underwent endoscopic sphenopalatine artery cauterization for recurrent/intractable posterior epistaxis. Ethical clearance was taken from the Institutional Review Board. Data were analyzed in Statistical Package for the Social Sciences. Results: Out of the total patient with refractory posterior epistaxis who underwent Endoscopic Sphenopalatine Artery Ligation, the overall success rate was 39 (95.12%). Among them, 25 (60.97%) males and 16 (39.02%) females underwent endoscopic sphenopalatine artery ligation. Twenty (48.78%) of them were unilateral whilst 21 (51.21%) were bilateral disease. About 2 (4.8%) cases had re-bleeding within 48 hours which was managed conservatively. Hypertension was found to be the most common comorbid condition followed by diabetes, chronic kidney. Conclusions: From our study, we conclude that the success rate for Endoscopic Sphenopalatine Artery Ligation in a patient with refractory posterior epistaxis was high.

Background: The decline in fertility rate based on the National Health Profile is attributed to the choice of permanent sterilisation as the preferred method of contraception among Indian women. The uptake of spacing methods has declined over the years in many parts of India related to low awareness about options other than sterilisation. Hence understanding the needs, beliefs and barriers of women regarding contraceptive use can identify the factors behind their choice of contraceptive methods. This study aims to explore the needs, beliefs and barriers of women in using different methods of contraception in a low-resource area of urban Vellore, Tamil Nadu.Methods: A qualitative study was conducted in three underserved areas of urban Vellore served by the secondary care hospital of a private academic institution. A descriptive qualitative method was chosen. Focus group discussion was carried out among study participants. Thematic analysis was used to analyse data.Results: The need for contraceptive use was felt after completing the families determining the type of method chosen. Fear of impending side-effects of commonly available spacing methods of contraception limited their use. The results highlight the forceful use of intra-uterine device among study participants reflecting the incentive-based family planning services implemented in most Indian states.Conclusions: Our study reinforces that neither the availability of contraceptives nor forceful implementation of policies will increase the utilisation of contraceptives. There is a need for community-based education on the indications, types and side-effects of reversible methods of contraception.

ABSTRACT Introduction Placenta percreta is rare but may present with life-threatening emergencies. Case Report We present a near-miss case with a placenta percreta presenting with hemoperitoneum and shock and describe how multidisciplinary multimodality management was used to save the lady in a tertiary care center of a low-resource setting. Peripartum hysterectomy with bilateral internal iliac ligation could not completely control the oozing. The general condition of the patient was poor with acidosis, coagulopathy, and hypothermia. Pelvic packing followed by embolization was done along with intensive care to treat metabolic derangements. It helped salvage the patient who later underwent a relaparotomy for pack removal. Conclusion and clinical significance: Role of individualized multimodality multidisciplinary management and intensive care is emphasized. Objective: To evaluate the success rate of endoscopic endonasal dacryocystorhinostomy at tertiary care hospital. Material and methods: Between July 2020 to December 2020, total 50 patients having age between 20-60 years, both gender, cases of epiphora, cases with evidence of obstruction were recruited from Department of ENT, DG Khan Medical Collge, DG Khan. Success of DCR was evaluated. Results: Fifty cases were selected for this study. Age range was 20-40 years with mean age 38.80 ± 8.74 years. DCR was successful in 47 (94%) patients while in 3 (6%) patients surgery was not successful. Male patients were 35 (70%) while female patients were 15 (30%). Surgical success was noted in 33 (94.29%) male patients and 14 (93.33%) female patients. Insignificant (P = 1.00) association between surgical success and gender was noted. Conclusion: In this study high success rate of DCR was noted. Males were prominent. Most of the patients were between 20-40 years and no association of success of surgery with age group and gender found. Keywords: Dacryocystorhinostomy, Epiphora, Intubation, Chronic dacryocystitis
