Among well-known side effects such as dizziness, nausea, headache and diplopia medical treatment of epilepsy may cause side effects on cognition, mood and behavior. In special constellations this can profoundly affect compliance with the medication as well as quality of life. Some patients are more vulnerable to side effects than others. Side effects can have profound impact on the development and future life of a patient. Some antiepileptic drugs (e.g. topiramate, zonisamide) show a more severe side effect profile than others (e.g. lamotrigine, levetiracetam). Thus, in the treatment of epilepsy, it is crucial to consider such possible side effects – especially in the beginning of or while changing the medical treatment. Specific neuropsychological examinations can monitor side effects on cognitive functions like concentration, memory or speech function. If this is not possible in an ambulant setting, specific screening instruments and repeated and precise interviews of patients and/or relatives can help to discover potential side effects. Because most side effects can be reversible, dosage modification or drug replacement is required as soon as incompatibilities are discovered.

AbstractChromosomal diseases are heterogeneous conditions with complex phenotypes, which include also epileptic seizures. Each chromosomal syndrome has a range of specific characteristics regarding the type of seizures, EEG findings and specific response to antiepileptic drugs, significant in the context of the respective genetic etiology. Therefore, it is very important to know these particularities, in order to avoid an exacerbation of seizures or some side effects. In this paper we will present a review of the epileptic seizures and antiepileptic treatment in some of the most common chromosomal syndromes.

ABSTRACTEpilepsy is a chronic neurological disorder that affects many people worldwide. Temporal lobe epilepsy is the most common and most studied type of epilepsy, but the pathological mechanisms underlying this condition are poorly understood. More than 20 antiepileptic drugs (AEDs) have been developed and used for the treatment of epilepsy; however, 30% of patients still experience uncontrolled epilepsy and associated comorbidities, which impair their quality of life. In addition, various side effects have been reported for AEDs, such as drowsiness, unsteadiness, dizziness, blurred or double vision, tremor (shakiness), greater risk of infections, bruising, and bleeding. Thus, critical medical needs remain unmet for patients with uncontrolled epilepsy. Flavonoids belong to a subclass of polyphenols that are widely present in fruits, vegetables, and certain beverages. Recently, many studies have reported that some flavonoids elicit various beneficial effects in patients with epilepsy without causing the side effects associated with conventional medical therapies. Moreover, flavonoids may have a property of regulating microRNA expression associated with inflammation and cell survival. These findings suggest that flavonoids, which are more effective but impose fewer adverse effects than conventional AEDs, could be used in the treatment of epilepsy.

Neurotoxicity is a common problem in oncology practice and neurologists who care for cancer patients encounter a wide range of symptoms attributable to the side effects of radiation and/or chemotherapy. Complications involving the nervous system may be debilitating. Though generally improved by dose reduction or cessation of an offending agent, such symptoms can be irreversible and frequently have a profound impact on quality of life. The appropriate balance between therapeutic efficacy and drug or radiation toxicity requires close attention to the patient’s complaints as well as a thorough understanding of long-term consequences of both the disease and a given treatment’s side effects.

Overactive bladder is a common and bothersome condition. Antimuscarinic agents, as a class, are the cornerstone of medical treatment of overactive bladder. They offer significant improvements in symptoms and patients’ quality of life. Antimuscarinics are generally well tolerated with mild and predictable side effects. Available antimuscarinics have small, yet statistically significant, differences in their efficacy and tolerability profiles. In clinical practice, finding the agent that offers the optimum balance of efficacy and side effects for an individual patient remains the major challenge.

Substantial intensity of side effects and low tolerance of drugs are reflected directly on the quality of patients' lives. In common with positive therapeutic results, antiepileptic drugs have a toxic impact on an organism of human including drug dependency and they can lead to fatality in different conditions (in case of overdose, drugs abuse, heightened sensitivity of organism).

Abstract Background: Delivering high-quality cancer care requires meeting the needs of cancer patients and their families. The Committee on Improving the Quality of Cancer Care has stated that systems should support all patients and families in making informed health care decisions that are consistent with their needs, values, and preferences (http://www.ncbi.nlm.nih.gov/books/NBK202146/). Fundamental to this is the need to foster good communication, developing and disseminating evidence-based information to inform patients, caregivers, and the cancer care team about treatment options. We created a patient education activity to empower patients with information and resources focused on: general facts about AML, testing and results interpretation, treatment options, and resources for self-care and support. Methods: A one-hour online video-based program targeting patients with AML and their caregivers was developed in July 2017 to address the identified audience needs. The program was video-based, and broadcast an interactive panel discussions with a physician specialist in AML, a nurse practitioner specialist in AML, and a patient advocate from the Leukemia & Lymphoma Society. The program integrated educational slides, live polling and Q&A with audience participants, and patient video vignettes that touched on real patient experiences regarding life with AML. Programs were initially broadcast live and were later "chapterized" into 4 smaller segments and made available on-demand at CancerCoachLive.com, LLS.org, and on social media platforms (e.g., LLS and Cancercoach Facebook pages, YouTube) for 6 months. Knowledge-focused test questions were administered at 3 time points (pre-activity, immediate post-activity, and 8-week [follow-up] post), in addition to behavioral and communication-focused questions being asked at the follow up point. Data from these questions, live polling responses, and learner-submitted questions during live Q&A were analyzed to determine engagement, lessons learned and continuing patient-education gaps. Results: In total, 8,935 learners took part in the program (262 on cancercoach platform/8,673 via social media platforms). 69% of respondents reported that communication and collaboration with their healthcare provider(s) improved as a result of participating in the educational program; 27% reported improvements in care for their health. Respondents reported examples of improved communication with their HCPs specific to: their disease, traditional and alternative forms of treatment, and side-effects. Reported changes in behavior and self-care included: being more active, altering self-care behaviors when undergoing chemotherapy, and keeping informed of research and treatments. Learners showed improvements on 4 knowledge questions, including questions pertaining to: 1) Common long-term or late side effects of treatment (45 and 74% relative increases); 2) Factors related to treatment for AML (26 and 33% relative increases); 3) Factors related to testing for AML (5 and 7% relative increases); and the relationship between AML symptoms and low blood count (1 and 3% relative increases). Although this study is limited to patient and caregiver self-report, educational evaluation results and participant questions indicate that patients and caregivers benefit from online education on AML. Conclusions: Education on AML specifically catered to patients and their families can yield immediate and sustained gains in knowledge and behavior, which may improve their adherence to treatment, quality of life, and patient outcomes. Disclosures No relevant conflicts of interest to declare.

After a short overview of the history of probiotics, the author presents the development of human intestinal microflora based on the newest genetic data and the microbiological features of main probiotics. The indications of probiotic administration have been defined and extended in recent years. The author reviews significant results of probiotic treatment in some gastrointestinal diseases based on meta-analytical data. Probiotics are useful in preventing and treating diarrhoea caused by antibiotics and Clostridium difficile caused diarrhoea. In the treatment of Helicobacter pylori infection, preparations containing certain Lactobacillus,Bifidobacterium strains or Saccaromyces boulardii could enhance by 5–10% the rate of successful eradication and reduce the incidence and severity of the side effects. Some symptoms of irritable bowel syndrome and thus the quality of life can be improved by probiotics. Their beneficial effect in ulcerative colitis was proven, while in Crohn’s disease has not yet been defined. The use of probiotics is not included in guidelines, with the exception of the Maastricht IV/Florence consensus. For each disease it is advisable to use probiotics containing strains only with proven beneficial effect. The efficiency of preparations containing mixed strains has not yet been properly investigated. The author reviews the rare but potentially serious side effects of probiotics. In Hungary, there are many probiotic preparations available which can be purchased in pharmacies without prescription: their use is more empirical than evidence-based. The European Food Safety Authority has recently rejected claims for probiotics to be classed as medicines given the lack of convincing evidence on the effects of probiotics on human health and well-being. Clearly, further research is needed to collect evidence which could be incorporated into the international guidelines. Orv. Hetil., 2013, 154, 294–304.
